Abstract:The outcome for children with high-grade gliomas (HGG) remains dismal, with a 2-year survival rate of only 10% to 30%. Diffuse intrinsic pontine glioma (DIPG) comprise asubset of HGG that arise in the brainstem almost exclusively in children. Genome-wide analyses of copy number imbalances previously showed that plateletderived growth factor receptor a (PDGFRA) is the most frequent target of focal amplification in pediatric HGGs, including DIPGs. To determine whether PDGFRA is also targeted by more subtle mutations missed by copy number analysis, we sequenced all PDGFRA coding exons from a cohort of pediatric HGGs. Somatic-activating mutations were identified in 14.4% (13 of 90) of nonbrainstem pediatric HGGs and 4.7% (2 of 43) of DIPGs, including missense mutations and in-frame deletions and insertions not previously described. Forty percent of tumors with mutation showed concurrent amplification, whereas 60% carried heterozygous mutations. Six different mutations impacting different domains all resulted in ligand-independent receptor activation that was blocked by small molecule inhibitors of PDGFR. Expression of mutants in p53-null primary mouse astrocytes conferred a proliferative advantage in vitro and generated HGGs in vivo with complete penetrance when implanted into brain. The gene expression signatures of these murine HGGs reflected the spectrum of human diffuse HGGs. PDGFRA intragenic deletion of exons 8 and 9 were previously shown in adult HGG, but were not detected in 83 nonbrainstem pediatric HGG and 57 DIPGs. Abstract:In the 2016, WHO classification of tumors of the central nervous system, isocitrate dehydrogenase (IDH) mutation is a main classifier for lower grade astrocytomas and IDH-mutated astrocytomas is now regarded as a single group with longer survival. However, the molecular and clinical heterogeneity among IDH mutant lower grade (WHO Grades II/III) astrocytomas have only rarely been investigated. In this study, we recruited 160 IDH mutant lower grade (WHO Grades II/III) astrocytomas, and examined PDGFRA amplification, CDKN2A deletion and CDK4 amplification by FISH analysis, TERT promoter mutation by Sanger sequencing and ATRX loss and p53 expression by immunohistochemistry. We identified PDGFRA amplification, CDKN2A homozygous deletion and CDK4 amplification in 18.8%, 15.0% and 18.1% of our cohort respectively, and these alterations occurred in a mutually exclusive fashion. PDGFRA amplification was associated with shorter PFS (P = 0.0003) and OS (P < 0.0001). In tumors without PDGFRA amplification, CDKN2A homozygous deletion or CDK4 amplification was associated with a shorter OS (P = 0.035). Tumors were divided into three risk groups based on the presence of molecular alterations: high risk (PDGFRA amplification), intermediate risk (CDKN2A deletion or CDK4 amplification) and low risk (neither CDKN2A deletion and CDK4 amplification nor PDGFRA amplification). These three risk groups were significantly different in overall survival with mean survivals of 40.5, 62.9 and 71.5 months. The high-risk group also demonstrated a shorter PFS compared to intermediate- (P = 0.036) and low-risk (P < 0.0001) groups. One limitation of this study is the relatively short follow-up period, a common confounding factor for studies on low-grade tumors. Our data illustrate that IDH mutant lower grade astrocytomas is not a homogeneous group and should be molecularly stratified for risk.

Abstract:Background and purpose:Malignant astrocytomas are the most common brain tumors.The activation,over-expression or amplification of some oncogenes as well as the deletion or inactivation of tumor suppressor genes may be involved in the formation of glioma and malignant transformation.p53 plays a key role in preventing cancer progression as it is often mutated in many tumors,including brain tumors.Wild-type p53-induced phosphatase 1 (Wip1) is overexpressed in many cancers.This study investigated the expression of the Wip1 gene in human astrocytomas and its relationship with p53 mutation.Methods:Fifty-two cases of human astrocytomas and 8 cases of normal brain tissues were collected in our hospital.All samples were evaluated for the mutation at exon 5-8 of p53 gene with the use of DNA sequencing of genomic DNA.The Wip1 mRNA expression was examined through real-time quantitative polymerase chain reaction.Protein expression of Wip1 of human astrocytomas was determined through western blot analysis.The statistical analysis was conducted in order to show significant differences between the expression level of Wip1 based on pathological grade and the mutational status of the p53 gene.Results:p53 gene mutations were found in 22 of the 52 astrocytoma samples (42.3%).The mRNA expression of Wip1 in high-grade pathology (WHO Ⅲ-Ⅳ) showed slightly elevated when compared with that in low-grade astrocytomas (WHO Ⅰ-Ⅱ) and normal brain tissues,but the difference did not reach statistical significance (P=0.305).However,the expression of Wip1 tumor samples with wild-type p53 showed significantly elevated mRNA expression in comparison to the samples with mutant p53 (P=0.009).The Wip1 gene was also found to be selectively overexpressed in human astrocytomas with wild-type p53.This means that the overexpression of Wip1 in astrocytomas was not exactly relevant to the pathological grade,but associated with the status of p53.The protein expression of Wip1 detected by western blot showed that it is in concordance with the mRNA level.Conclusion:Wip1 is selectively overexpressed in astrocytomas with wild-type p53.Overexpressed Wip1 may be another key factor to the malignant progression of astrocytomas besides the mutation of p53 gene.
