Abstract:Objective Oncogenic Kras-activated robust Mek/Erk signals phosphorylate to the tuberous sclerosis complex (Tsc) and deactivates mammalian target of rapamycin (mTOR) suppression in pancreatic ductal adenocarcinoma (PDAC); however, Mek and mTOR inhibitors alone have demonstrated minimal clinical antitumor activity.Design We generated transgenic mouse models in which mTOR was hyperactivated either through the Kras/Mek/Erk cascade, by loss of Pten or through Tsc1 haploinsufficiency. Primary cancer cells were isolated from mouse tumours. Oncogenic signalling was assessed in vitro and in vivo, with and without single or multiple targeted molecule inhibition. Transcriptional profiling was used to identify biomarkers predictive of the underlying pathway alterations and of therapeutic response. Results from the preclinical models were confirmed on human material.Results Reduction of Tsc1 function facilitated activation of Kras/Mek/Erk-mediated mTOR signalling, which promoted the development of metastatic PDACs. Single inhibition of mTOR or Mek elicited strong feedback activation of Erk or Akt, respectively. Only dual inhibition of Mek and PI3K reduced mTOR activity and effectively induced cancer cell apoptosis. Analysis of downstream targets demonstrated that oncogenic activity of the Mek/Erk/Tsc/mTOR axis relied on Aldh1a3 function. Moreover, in clinical PDAC samples, ALDH1A3 specifically labelled an aggressive subtype.Conclusions These results advance our understanding of Mek/Erk-driven mTOR activation and its downstream targets in PDAC, and provide a mechanistic rationale for effective therapeutic matching for Aldh1a3-positive PDACs.

Abstract:1101 Pancreatic Ductal Adenocarcinoma (PDA) is the fourth most common cause of cancer related deaths annually and has a dismal five-year survival of only 5%. Presently little is known about PDA pathogenesis, though activated K-Ras is found in greater than 90% of cases. It is known that K-Ras can activate PI3K by binding to the p110 catalytic subunit and 32% of human pancreatic tumor samples have been shown to have increased Akt2 activity, a downstream signaling molecule of the PI3K pathway. More recently, Pten expression was shown to be absent or significantly decreased in 71% of human PDA samples. In order to further delineate the pathogenetic pathways involved in PDA, we are studying the involvement of the PI3K-Akt pathway utilizing pancreatic cancer cell lines and tissue specific mouse models. We demonstrated reduced PTEN levels in poorly differentiated pancreatic cancer cell lines by western blot analysis. Moreover, in vivo studies reveal that Pdx1-Cre, PtenLox/Lox mice develop Pancreatic Intraepithelial Neoplastic (PanIN) lesions as early as 8 weeks of age. We plan to further evaluate the differential PTEN levels in various cells lines and evaluate downstream effectors that are mediating increased Akt activity. Lastly, we are evaluating the role of PTEN in PDA development and the cellular origin of PanIN lesions using a pancreas specific PTEN knockout containing a LacZ reporter.

Abstract:Pancreatic ductal adenocarcinoma (PDAC) is a devastating malignant disease with a dismal prognosis. In the past decades, a plethora of genetically engineered mouse models (GEMMs) with autochthonous pancreatic tumor development have greatly facilitated studies of pancreatic cancer. Commonly used GEMMs of PDAC often harbor the oncogenic KRAS driver mutation (KrasG12D), in combination with either p53 mutation by knock-in strategy (Trp53R172H) or p53 loss by conditional knockout (Trp53cKO) strategy, in pancreatic cell lineages. However, the systematic comparison of the tumor microenvironment between KrasG12D; Trp53R172H (KPmut) mouse models and KrasG12D; Trp53cKO (KPloss) mouse models is still lacking. In this study, we conducted cross-dataset single-cell RNA-sequencing (scRNA-seq) analyses to compare the pancreatic tumor microenvironment from KPmut mouse models and KPloss mouse models, especially focusing on the cell compositions and transcriptomic phenotypes of major cell types including cancer cells, B cells, T cells, granulocytes, myeloid cells, cancer-associated fibroblasts, and endothelial cells. We identified the similarities and differences between KPmut and KPloss mouse models, revealing the effects of p53 mutation and p53 loss on oncogenic KRAS-driven pancreatic tumor progression.

Abstract:Mutational activation of the KRAS proto-oncogene is the initiating mutational event in pancreatic ductal adenocarcinoma (PDAC) and ~90% of patients harbor KRAS mutations. PDAC is marred by a 12% five-year overall survival rate, and developing effective therapeutic strategies remains a priority. Accumulating evidence suggests that all KRAS mutations are not created equal. While the KRASG12R mutation is rare in lung and colorectal cancers (<1%), it is the third most common KRAS mutation in PDAC, accounting for approximately 20% of all cases. However, KRASG12R cannot interact with the lipid kinase PI3Ka, a well-characterized RAS effector necessary for KRAS-driven tumorigenesis. Previous studies in mice models of cancer have demonstrated that ablation of the KRAS:PI3Ka interaction limits tumorigenesis. Despite the inability of KRASG12R to activate PI3Ka directly, AKT signaling is robustly activated in KRASG12R-mutant PDAC. The mechanisms that allow the KRASG12R mutant to overcome the inability to activate PI3K and promote PDAC are unclear. We recently developed a Ptf1a-CREERT2;KrasLSL-G12R genetically engineered mouse model to study KRASG12R in an in vivo context. This model does not develop pancreas lesions or tumors. We have uncovered two unique characteristics of human PDAC that we hypothesize allows for the KRASG12R mutation to produce tumors only in human pancreatic tissue. First, we have found that all four PI3K isoforms are overexpressed in human PDAC, and the PI3Kd and PI3Kg isoforms are specifically upregulated in KRASG12R-mutant PDAC. Second, PTEN is oxidized in PDAC. PTEN oxidation results in an intramolecular disulfide bond, which inhibits the phosphatase activity of PTEN and leads to hyperactivated PI3K signaling. Critically, mouse pancreas tissue only expresses the PI3Ka/b isoforms and PTEN is in the reduced state, demonstrating two significant differences between mouse models and human disease. Additionally, we have determined that PTEN becomes fully oxidized in nutrient-restricted medium (low glucose/glutamine), a common strategy employed to mimic the pancreatic tumor microenvironment in cell culture. Using PTEN oxidation-resistant variants, we demonstrate that KRASG12R-mutant PDAC cell lines are reliant on oxidized (inactivated) PTEN for proliferation. To confirm that PTEN inactivation can aid KRASG12R-mediated tumorigenesis in mouse models, we generated at KrasG12R/+;Ptenfl/fl genetically engineered mouse model, and the results of this model will be presented herein. Thus, our data demonstrate that increased PI3K isoform expression, coupled with PTEN oxidation, creates a unique environment that allows KRASG12R to initiate and promote pancreatic tumorigenesis. As PTEN oxidation overcomes the need for KRAS to activate PI3K signaling in PDAC, these results indicate that directly targeting KRAS alone will be insufficient at reducing tumor growth in humans and clinically successful therapeutic strategies will have to develop alternative methods to reduce PI3K signaling in addition to directly targeting KRAS activity. Abstract:Background: Mutated Kirsten rat sarcoma viral oncogene homolog (KRAS) is the most common oncogene alteration in pancreatic ductal adenocarcinoma, and KRAS glycine to cystine substitution at codon 12 (G12C) mutations (KRAS G12Cmut) are observed in 1%-2%. Several inhibitors of KRAS G12C have recently demonstrated promise in solid tumors, including pancreatic cancer. Little is known regarding clinical, genomics, and outcome data of this population. Methods: Patients with pancreatic cancer and KRAS G12Cmut were identified at Memorial Sloan Kettering Cancer Center and via the American Association of Cancer Research Project Genomics, Evidence, Neoplasia, Information, Exchange database. Clinical, treatment, genomic, and outcomes data were analyzed. A cohort of patients at Memorial Sloan Kettering Cancer Center with non-G12C KRAS pancreatic cancer was included for comparison. Results: Among 3571 patients with pancreatic ductal adenocarcinoma, 39 (1.1%) with KRAS G12Cmut were identified. Median age was 67 years, and 56% were female. Median body mass index was 29.2 kg/m2, and 67% had a smoking history. Median overall survival was 13 months (95% CI: 9.4 months, not reached) for stage IV and 26 months (95% CI: 23 months, not reached) for stage I-III. Complete genomic data (via American Association of Cancer Research Project Genomics, Evidence, Neoplasia, Information, Exchange database) was available for 74 patients. Most common co-alterations included TP53 (73%), CDKN2A (33%), SMAD4 (28%), and ARID1A (21%). Compared with a large cohort (n = 2931) of non-G12C KRAS-mutated pancreatic ductal adenocarcinoma, ARID1A co-mutations were more frequent in KRAS G12Cmut (P < .05). Overall survival did not differ between KRAS G12Cmut and non-G12C KRAS pancreatic ductal adenocarcinoma. Germline pathogenic variants were identified in 17% of patients; 2 patients received KRAS G12C-directed therapy. Conclusion: Pancreatic cancer and KRAS G12Cmut may be associated with a distinct clinical phenotype. Genomic features are similar to non-G12C KRAS-mutated pancreatic cancer, although enrichment of ARID1A co-mutations was observed. Targeting of KRAS G12C in pancreatic cancer provides a precedent for broader KRAS targeting in pancreatic cancer.
