Effects of Maximal Urethral Length Preservation on Recovery of Urinary Continence After Laparoscopic Radical Prostatectomy

Abstract:Objective To evaluate effects of maximal urethral length preservation technique on recovery of urinary continence after laparoscopic radical prostatectomy . Methods From January 2011 to June 2012, laparoscopic radical prostatectomy was performed by a same surgical team in 63 cases.Of them, 33 patients underwent maximal urethral length preservation (MULP group) and 30 patients underwent traditional operation ( non-MULP group ) .Urinary incontinence was evaluated by using patients ’ self-assessment questionnaire based on the International Consultation on Incontinence Questionnaire .The preoperative PSA , positive surgical margins, and urinary continence at 1, 3, 6, 12 months after operation were compared between the two groups retrospectively . Results The operation was successful in all the cases .The follow-up lasted 12-48 months.The MULP Group showed better urinary continence than the non-MULP group at 1 and 3 months after operation .At 1 month after operation , the urinary continence degree of grade 0, 1, 2, and 3 included 12, 10, 8, and 3 cases in the MULP group and 7, 5, 11, and 7 cases in the non-MULP group ( Z=-1.979, P=0.048).At 3 months after operation, the urinary continence degree of grade 0, 1, 2, and 3 included 20, 7, 5, and 1 cases in the MULP group and 12, 6, 7, and 5 cases in the non-MULP group (Z=-2.012, P=0.044).There were no statistical differences between the two groups in urinary continence at 6 months and 12 months after operation (P >0.05).No significant difference in positive surgical margins was found in the two groups . Conclusion Maximal urethral length preservation technique can improve the early recovery of urinary continence after laparoscopic radical prostatectomy without increasing the risk of positive surgical margins.
