Abstract:Abstract Background: Currently, the surgical technique and design of surgical instruments for lateral unicompartmental knee arthroplasty (UKA) in treating lateral knee compartment-specific anatomical structures cannot be replicated. Moreover, few articles have reported on the surgical techniques and clinical effects.These have caused most joint surgeons to dare not to carry out this operationinChina.Here, we report on how we perform this procedure in a replicable manner using redesigned tools, and evaluate subsequent American Knee Society scores, range of motion (ROM), and revision rates.Methods: In this retrospective review (January 2014 through January 2016) we identified 158 patients (35 males and 123 females) who had 160 lateral unicompartmental arthroplasties with a minimum of 36 months follow-up (mean 54, range 36–60 months). The initial symptoms of painful osteoarthritis or osteonecrosis were localized to the lateral compartment. The standard of X-slices is mainly limited to the lateral compartment (Ahlbäck grade II OA or worse) while the articular cartilage of the medial compartment shows no obvious clinical or imaging signs of wear. Any valgus deformity must be correctable to reduce varus stress, and any form of inflammatory arthritis is an absolute contra-indication for lateral UKA.Results: At follow-up, American Knee Society scores (0–100 points) averaged 47.3 ± 1.5 for pain, 97.0 ± 1.7 for clinical outcome, and 97.1 ± 4.1 for function, and ROM averaged 125.5 ± 5.3°. None of the patients had reoperations or revisions.Conclusion: These standard surgical techniques and tools are useful to beginners because they can effectively avoid impingement of the femur against the tibial spine eminences and provide good results.
Unicondylar Knee Arthroplasty Versus Total Knee Arthroplasty in Adults with Isolated Medial Osteoarthritis: A Matched Study Protocol.

Abstract:Background: The choice between unicondylar knee arthroplasty (UKA) and total knee arthroplasty (TKA) is likely to have long-term implications for patient-reported health outcomes. However, high-quality studies that compare the outcomes of TKA and UKA and their effects are still lacking in the literature. Thus, the aim of the present study was to compare the UKA and TKA techniques with regard to functional outcomes and perioperative complications in patients who had isolated medial osteoarthritis. Methods: This was a retrospective, single-center, matched-controlled study performed with approval of our hospital (Kunshan hospital of Traditional Chinese Medicine affiliated to Nanjing University of Traditional Chinese Medicine), with the ethics number KZY2020-37. To reduce the effect of selection bias and potential confounding in this observational study, a 1:1 matching algorithm was applied. The groups were split by sex, age to within 6 years, and body mass index within 5 kg/m(2). Thus, we retrospectively reviewed the records of 240 consecutively enrolled patients who underwent UKA and 240 patients who underwent TKA from January 2013 to June 2015 from the database of our institution. Written informed consent was obtained from all subjects participating in the trial. Clinical outcomes included range of motion, Short Form 12 score, new Knee Society Score, Western Ontario and McMaster Universities Arthritis Index, and the complications. The outcome measures were evaluated by a physiotherapist and were assessed preoperatively and postoperatively at 6 months and 2 years. The mean follow-up time was 3 years. Conclusion: We hypothesized that there was no significant difference between the 2 groups in terms of postoperative outcomes.

Abstract:There is a low prevalence of osteoarthritis in the lateral compartment of the knee, but the overall number of domestic patients is large, and lateral unicompartmental arthroplasty (UKA) has good prospects.The unique anatomical structure and kinematic characteristics of the lateral compartment make the surgical operation more challenging.Traditional UKA patients have a high incidence of lower limb mal-alignment and poor prosthetic position, which leads to limit of their promotion and application.In recent years, with the development of treatment concepts, surgical techniques and materials, the survival time of UKA prosthesis has been continuously extended, and the clinical effect has been continuously optimized.Strictly grasp the surgical indications in radiology, anatomy and clinical manifestations, familiarize with the lateral compartment anatomy and biomechanical features, and master the technical details are the prerequisites and guarantees for the success of the lateral UKA.With the advancement of technology, minimal invasion, precision and individuation should be the goal pursued for lateral UKA surgery.
[Early efficacy of lateral unicondylar replacement for lateral compartment osteoarthritis of the knee]

Abstract:Objective: To discuss the surgical technique of lateral unicondylar knee replacement (UKR) for patients with lateral compartment osteoarthritis and evaluate its recent clinical efficacy. Methods: A retrospective analysis was conducted on 22 cases of lateral compartment osteoarthritis patients receiving lateral UKR at Orthopedic Department, Beijing Chao Yang Hospital, Capital Medical University from January 2016 to January 2019.There were 8 males (8 knees) and 14 females (14 knees), aged 65.7 years(range: 54 to 83 years). All patients completed preoperative imaging examination and excluded extra-articular deformity. Lateral parapelatellar approach was used for the operation and the tibial osteotomy was performed by extramedullary positioning.The residual cartilage of the external condyle was removed by pendulum saw to subchondral bone.Appropriate prosthesis were selected, and the tightness of the gap and the stability of the joints were repeatedly confirmed during the operation. The postoperative complications and the results of physical and imaging examinations were collected. The preoperative and postoperative Oxford knee Score (OKS) and knee society score (KSS) of patients were recorded and compared by paired t test. Results: All patients were followed up for 24.6 months (range: 8 to 40 months). No complications such as incision exudation, skin necrosis and deep venous thrombosis of lower limbs were found after the operation.No cases of prosthesis loosening, excessive wear of pads, dislocation and postoperative infection were found at the last follow-up.The OKS decreased from 46.5±3.1 preoperative to 22.4±3.3 postoperative(t=13.4, P=0.00) and KSS improved from 69.5±8.7 preoperative to 90.2±7.4 postoperative (t=16.9, P=0.00). Conclusions: For the treatment of lateral compartment osteoarthritis, UKR has the advantages of rapid recovery of knee function, less complications and less trauma.The short-term efficacy is satisfactory, but the long-term efficacy remains to be observed.
Does Unicompartmental Knee Replacement Offer Improved Clinical Advantages Over Total Knee Replacement in the Treatment of Isolated Lateral Osteoarthritis? A Matched Cohort Analysis From an Independent Center

Abstract:Background: The purpose of this study is to compare the functional and radiographic results, perioperative complications, satisfaction rate, and mid-term survivorship after unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) for the treatment of lateral compartmental knee osteoarthritis (LCKO). Methods: Between March 2007 and September 2017, we identified 35 patients with primary TKAs and 121 patients with lateral UKAs (LUKAS) for LCKO with a minimum follow-up of 2 years (mean 5.3 years, range 2-12.4). The matched variables were age, gender, operation side, body mass index, American Society of Anesthesiologist grade, initial diagnosis, osteoarthritis grade in lateral compartment, and followup time. All patients were assessed using the Oxford Knee Score, Hospital for Special Surgery score, range of motion, length of hospital stay, satisfaction, and complications. Survivorship of UKA and TKA implants was also compared. Results: At last follow-up, LUKA had a significantly better postoperative Oxford Knee Score, Hospital for Special Surgery score, range of motion, shorter length of hospital time, and higher satisfaction rate than matched TKA group. There were significant differences regarding patellar tendon injury (P = .043), superficial wound infection (P = .028), patellar snapping or impingement (P = .047), and stiffness (P < .001). Five-year survivorships free from revision were similar in both groups (99.2% vs 97.1%, P = .347). Conclusion: LUKA for LCKO demonstrated more favorable 5-year results in comparison with TKA. Furthermore, LUKA achieved comparable mid-term survivorship and was less likely to suffer from wound infection and knee stiffness, although not overall surgical complications. (C) 2020 Elsevier Inc. Abstract:Purpose: We aimed to investigate the technical advantages and clinical outcomes of lateral unicompartmental knee arthroplasty (LUKA) through the medial parapatellar approach. Methods: From August 2022 to June 2024, 104 patients who underwent LUKA via the medial parapatellar approach were enrolled. The operation time, hospital stays, surgical complications and follow-up period were collected. Pre- and postoperative Range of movement (ROM), Oxford knee score (OKS) and Hospital for special surgery knee score (HSS) were recorded and further investigated by paired-samples t-test analysis. Results: Primary demographic data of the retrospective case series collected include gender (30 males and 74 females), age (63.1 ± 8.37 years, range 49-84 years), operation time (86 ± 17 min, range 52-135 min), hospital stays (4.0 ± 1.1 days, range 3-8 days) and follow-up period (9.9 ± 5.7 months, range 1-23 months). There was a significant improvement in ROM (P < 0.001), OKS (P < 0.001) and HSS (P < 0.001) compared to preoperative values. Patient satisfaction at 1-month postoperative follow-up was up to 95%. The incidence of deep venous thrombosis was 16.3%. There was no incidence of postoperative infection. Conclusion: The medial parapatellar approach is a understandable, easy to master and credible surgical approach for LUKA, showing a striking improvement in clinical outcomes without adverse events in the short-term follow-up.

Abstract:Background: Approximately 5% to 10% of patients with knee arthritis have isolated lateral compartment arthritis; however, lateral unicompartmental knee arthroplasty (UKA) comprises just 1% of all knee arthroplasties1. This low proportion is partly because of the perceived complexity of lateral UKA and concerns over implant longevity and survivorship compared with total knee arthroplasty (TKA)2,3. With an improved understanding of knee kinematics alongside advances in implant design and tools to aid in appropriate restoration of limb alignment, lateral UKA can be an appealing surgical alternative to TKA for certain patients with lateral knee arthritis4,5. In appropriately selected patients, lateral UKA has been associated with reduced osseous and soft-tissue resection, more natural knee kinematics, less pain, shorter hospitalization, decreased blood loss and infection rates, and excellent survivorship and patient-reported outcomes6-9. Description: This surgical approach and technique described for lateral UKA utilizes robotic-arm assistance and modern fixed-bearing implants10. The specific steps involve appropriate patient evaluation and selection, extensive radiographic and computed-tomography-based preoperative templating, a lateral parapatellar approach, intraoperative confirmation of component position and alignment, and robotic-arm assistance to perform osseous resections to achieve limb alignment and kinematic targets10. Final implants are cemented in place, and patients typically are discharged home on the day of surgery10. Alternatives: Nonoperative treatment for end-stage knee arthritis includes weight loss, activity modification, assistive devices, bracing, nonsteroidal anti-inflammatory medications, and various injections11. Alternative surgical treatments include TKA4 and, in certain patients, an offloading periarticular osteotomy12. Rationale: Lateral UKA is an appealing surgical option for nonobese patients who have disabling knee pain isolated to the lateral compartment, good preoperative range of motion, and a passively correctable valgus limb deformity10,13. Expected outcomes: Patients are typically discharged home on the day of surgery, or occasionally on postoperative day 1 if medical comorbidities dictate hospital monitoring overnight10. Patients return to light activities, including walking, immediately postoperatively. By 3 months postoperatively, patients will generally have returned to all desired activities9. The mid-term outcomes of this procedure, as performed by the corresponding author, have been published recently14,15. The 5-year survivorship of 171 lateral UKAs was 97.7%, with 72.8% of patients reporting that they were very satisfied with their procedure and 19.8%, that they were satisfied14. Only 3.8% of patients reported dissatisfaction with their lateral UKA14. The mean Knee Injury and Osteoarthritis Outcome Score (KOOS) and standard deviation were 85.6 ± 14.314. These outcomes did not differ from those observed in 802 medial UKAs, which showed a survivorship of 97.8% and KOOS of 84.3 ± 15.914. These findings are generally in line with previously published studies, which have demonstrated excellent survivorship and patient-reported outcomes with fixed-bearing lateral UKA16-19. Important tips: Component position and alignment are critical to achieve target knee kinematics.Target postoperative alignment is 1° to 4° of valgus.A meticulous cementation technique is required for optimal fixation and avoidance of excess residual cement in the posterior knee. Abstract:Improved prosthetic design and application of strict criteria in selection of patients have resulted in wide use of unicompartmental knee arthroplasty (UKA) in surgical treatment of damaged joints. Here, we review the contemporary indications, outcomes, and complications of procedures such as lateral and medial UKA and total knee arthroplasty (TKA), and compare the severity of these complications in UKA and TKA. Patients who received unicompartmental femorotibial osteoarthritis and patients who underwent UKA and ACL reconstruction for deficiency in anterior cruciate ligament (ACL), they all showed good clinical outcomes and survival. Reliable and successful options in the treatment of patients with isolated tibiofemoral osteoarthritis include lateral and medial UKA.

Abstract:OBJECTIVETo explore the feasibility and short-term effectiveness of lateral unicompartmental knee arthroplasty (LUKA) through a lateral parapatellar approach for lateral compartmental osteoarthritis (LCOA).METHODSBetween November 2010 and August 2012, 15 consecutive patients (15 knees) with LCOA were treated with LUKA. There were 7 men and 8 women with a mean age of 67.3 years (range, 51-82 years). The mean duration of disease was 5.4 years (range, 3-15 years). The left knee was involved in 6 cases and the right knee in 9 cases. According to Ahlback rating, there were 2 cases (2 knees) of grade I, 8 cases (8 knees) of grade II, and 5 cases (5 knees) of grade III. Theincision length, operation time, blood loss, drainage, and complication were recorded. The pre- and post-operative knee function was evaluated by Hospital for Special Surgery (HSS) score system. The pre- and post-operative range of motion (ROM) and alignment of the lower limbs (hip-knee-ankle angle) were measured and compared.RESULTSACL rupture or medial compartmental osteoarthritis occurred in 2 patients (2 knees) who changed to total knee arthroplasty (TKA); 1 case (1 knee) failed to follow up. The other 12 cases (12 knees) were followed up 32.5 months on average (range, 26- 45 months). The mean length of incision was 6.9 cm (range, 6-8 cm); the mean operation time was 115.8 minutes (range, 90-155 minutes); the mean blood loss volume during operation was 152.2 mL (range, 105-250 mL); and mean drainage was 145.6 mL (range, 50-300 mL). At last follow-up, the average HSS score was significantly improved from 73.4 ± 4.6 preoperatively to 94.6 ± 2.1 postoperatively (t = 14.240, P = 0.000). The results were excellent in 9 cases, good in 2 cases, and fair in 1 case, with an excellent and good rate of 91.7%. The hip-knee-ankle angle was significantly decreased from valgus angle of (10.08 ± 1.38)° preoperatively to valgus angle of (5.17 ± 0.94)° postoperatively (t = 14.626, P = 0.000). Postoperative ROM was significantly improved to (123.75 ± 4.09)° from (108.67 ± 5.10)° preoperatively (t = 8.998, P = 0.000). Two patients developed superficial skin infection, which was managed with anti-inflammatory therapy and dressing. No patient had complication of deep vein thrombosis, prosthesis dislocation and loosing, or development of medial osteoarthritis.CONCLUSIONLUKA through a lateral approach has the advantages of rapid recovery of joint function, less complication, and small trauma in the treatment of LCOA. Correct patient selection and further mid- and long-term studies, however, are essential.

Abstract:Objective To investigate the early and medium term result of unicompartmental knee arthroplasty (UKA) versus totle knee arthroplasty (TKA) for the treatment of isolated compartmental osteoarthritis of knee. Methods The UKA group contained 23 patients (23 knees) and 50 patients were in TKA group (56knees). All the patients accepted arthroplasty during Feb 2002-Dec 2008. 12 at The Second Hospital of Xiang Ya Medical College by the same operator. 22 patients of UKA group were followed up for 4 months-7 years, average18.6 months, and the other one died 2 years after the operation. All patients of TKA group were followed up for 2 months-7 years,average 22.9 months. Patients evaluation focused on questionnair (HSS), blood loss, pain relief, the term of knee flexing to 90°, the number of knee being able to flex ≥ 120°. All the results were compared and analyzed. Results Therapeutic effects of both two groups appeared excellent. There were no component loosening and revision. HSS knee score improved significantly in both two groups: UKA group was promoted from (64±5.75) to (86±7.85) (t=11.53, P<0.001) and TKA group from (61±6.53) to (84±7.92) (t=18.64, P<0.001). Compared with the TKA group, the UKA had less blood loss (t=12.47, P<0.001), the same relief from pain(X2=0.007, P>0.05). The dates of knees being able to flex ≥ 90° afte operation were less (t=3.97, P<0.05) while the number of knees being able to flex ≥ 120° were higher in the UKA group and there were more excellent and good cases in this group. Conclusions Our study indicated that, with the strict indications selection, UKA for the treatment of isolated compartmental osteoarthritis of knee shows as well as TKA, and it is less trauma, less blood loss, more rapid postoperative recovery than TKA, at least in the early and medium term.
Predictors of Satisfactory Outcomes With Fixed-Bearing Lateral Unicompartmental Knee Arthroplasty: Up to 7-year Follow-Up

Abstract:Background: There is little literature available examining factors that may predict functional recovery after lateral unicompartmental knee arthroplasty (UKA). The purpose of this study was to report short to mid-term effectiveness and evaluate predictors of better outcome following lateral UKA.Methods: We retrospectively reviewed 248 patients (260 knees) who underwent lateral UKA from January 2013, with a mean 5-year follow-up. The primary outcome measures comprised the Hospital for Special Surgery (HSS) score and patient satisfaction. Multivariate regression analyses were implemented to investigate associations between these factors with a satisfactory outcome. Implant survival was estimated by Kaplan-Meier analysis.Results: Complete follow-up was available for 186 patients (198 knees). At last follow-up, the HSS scores were changed from 52.1 (range, 38-80) preoperatively to 85.6 (range, 61-98) (P <.001), The OKS improved from 22.8 (range, 16-32) preoperatively to 42.7 (range, 30-47) postoperatively (P <.01). The 5year survival was 99.5%. The multivariate analysis showed that the following factors tended to obtain a satisfactory outcome: higher proportion of ASA class I (P <.001), diagnosis of primary OA (P = .007), postoperative limb alignment (P = .007), and higher preoperative HSS score (P = .019). Patients with valgus 9 degrees-12 degrees reported the highest HSS scores among different subgroups (P <.001).Conclusion: Following lateral UKA, postoperative outcomes were satisfactory in patients with lower ASA scores, diagnosis with primary OA, higher preoperative HSS scores and those with postoperative valgus alignment. It is important to understand these correlations to help appropriate patient selection to obtain optimal function after lateral UKA. (C) 2020 Elsevier Inc. All rights reserved.

Abstract:[Objective]To explore the outcome and surgical technique of minimal invasive unicompartmental knee arthroplasty(UKA) for osteoarthritis of the knee. [Method]Ninety-four knees of 87 cases with medial compartmental osteoarthritis of the knee treated by minimal invasive UKA were reviewed retrospectively.The pain,range of motion(ROM) and HSS score of the knees were evaluated before and after UKA.Pre-and post-operative alignment of the low limbs were measured and compared respectively.The surgical skills of UKA were also summarized. [Result]All of the patients were followed up for a mean time of 3.4 years,2 knees were revised to total knee arthroplasty.HSS score was increased from 61.05 to 92.67.VAS score was reduced from 6.46 to 2.80.Pain ralief rate was 94%.The mean post-operative ROM of the knees was 127.53°,and alignment was 2°varus on the average.The rate of excellent and good results was 92%. [Conclusion]Minimal invasive UKA is an effective method for unicompartmental knee osteoarthritis with less trauma,quick recovery.The early term outcome of UKA is encouraging.
Preliminary Clinical Results of Robotic-Assisted Unicompartmental Knee Arthroplasty

Abstract:Objective To investigate the accuracy of robotic-assisted UKA and its safety and short term functional outcomes.Methods The clinical data of 20 patients who received robotic-assisted UKA in Shanghai 6th People's Hospital and Chinese PLA General Hospital from September 2016 to December 2016 were retrospectively reviewed, with 10 patients in each hospital.All patients had a preoperative diagnosis of osteoarthritis.Five patients received simultaneous bilateral UKA.Of all the 20 patients, there were 3 male patients and 17 female patients, with an average age of 65 years old (range from 51 to 79 years old) and an average BMI of (26.9±3.4) kg/m2.All the procedures were performed with MAKO RIO robotic-assisted surgical system and RESTORIS MCK unicompartmental knee prosthesis (STRYKER MAKO Surgical, USA).Each patient's operation time, blood loss and post-operative pain scores were recorded.Pre-operative and post-operative knee society scores were recorded;lower limb alignment and the position of the tibial component in coronal plane were measured.Results For patients who received unilateral procedure, the mean operating time of tourniquet was (27±10) minutes.The average total blood loss on post-op day 2 was (476±290) mL, and no transfusion was needed for any patient.The mean visual analogue scale pain score at 8, 16 and 24 hours after surgery was 3.2±1.2, 1.8±0.8 and 1.1±0.3, respectively.All 20 patients (25 knees) were followed-up at a minimal time of 8 weeks (average 14.1 weeks, range from 8 to 20 weeks).On latest follow-up visit, the mean Knee Society Score was improved from a pre-operative value of 57±13 to 90±6, the mean function score was improved from a preoperative value of 48±18 to 79±12, both changes were statistically significant (t=14.941 and 10.014 respectively, all P values<0.01).The position of the tibial component relative to the mechanical axis of the tibial in coronal plane was at a mean varus of 1.7°±1.2°(range, 0°-3.9°), the mean deviation from the target position (1°of varus) was 1.1°±0.9°, none of these cases had a deviation of more than 3°.The mean deviation from the target position of cases from the 2 centers was 1.2°±1.1° and 1.0°±0.6°, respectively.There was no statistic significant difference between the 2 centers (t=0.421, P>0.05).No severe complications, such as surgical site infection, deep venous thrombosis or vascular and nerve injury, were observed during the follow-up period.Conclusions Robotic-assisted UKA can provide us with excellent precision and safety through a minimally invasive approach.Further long-term controlled study is required to identify the long term clinical outcomes and the role of robotic technique on the improvement of post-operative knee function warrant.

Abstract:Unicompartmental knee arthroplasty (UKA), an alternative technology that has been prevailed upon Orthope?dics which was proven by time that it's not only an effective treatment for patients with anterior medial knee osteoarthritis, addi?tionally, it plays an important role in the staged treatment of knee osteoarthritis. The advantage of UKA is that patients would re?ceive minimal invasion and faster recovery and it is more suitable for elderly patients. Nevertheless, UKA was also an advisable choice of young patients due to its maximum retention of the knee joint sensation, with a reflect on higher post operation score on FJS (forgetting joint score). The ideal clinical efficacy of unicompartmental knee arthroplasty is relied on the improvements on the latest artificial joint design and surgical skill approaches, and as a consequence, indications and contraindications for UKA have renovated. A subset of patients who were previously thought to be contraindicated to perform UKA (such as medial patellofemoral joint mild arthritis) can be reassessed and performed UKA today which called relative contraindications or Non?essential contrain?dications. The main objective of this article is to provide a new concept of UKA to surgeons, and to provide the best treatment op?tions for patients with different complications according to the latest preoperative evaluation criteria. According to our experience, it is recommended that surgeons should accumulate a certain amount of total joint replacement experience and then to carry out UKA surgery, which will achieve better results.

Abstract:Journal of Orthopaedic Surgery, Volume 32, Issue 1, January-April 2024. BackgroundUnicompartmental knee arthroplasty (UKA) is an effective treatment method for knee osteoarthritis. With the development and implementation of enhanced recovery after surgery, UKA is now increasingly performed in outpatient surgical centers. However, there is ongoing debate regarding the safety and effectiveness of performing UKA in outpatient settings.MethodsThe search was performed to retrieve randomized controlled trials and cohort studies on outpatient UKA from PubMed, Cochrane Library, EMbase, CNKI, and WanFangData databases. The search was conducted from the inception of the databases until August 31, 2023. After independent screening, data extraction, and risk of bias evaluation by two researchers, meta-analysis was performed using RevMan 5.4 software.ResultsA total of eight studies involving 18,411 patients were included. The results showed that the postoperative transfusion rate in the outpatient group was lower than that in the inpatient group [OR = 0.36, 95%CI (0.24, 0.54), p < 0.00001], and the difference was statistically significant. However, there was no significant difference between the two groups in terms of readmission rate, reoperation rate, surgical site infection, and periprosthetic fracture. The differences were not statistically significant.ConclusionCompared to the traditional inpatient route, the blood transfusion rate for single-condyle replacement in the outpatient operation center is lower, and there is no significant difference in readmission rate, reoperation rate, surgical site infection, and periprosthesis fracture. The outpatient approach to UKA is safe, feasible, and highly satisfactory for patients. However, the results have certain limitations, and a rigorous preoperative complication risk assessment can minimize the risk of UKA in outpatient surgery centers.Trial registrationPROSPERO number CRD42023405373.

Abstract:Background: Osteoarthritis has become the predominant manifestation of arthritic conditions on a worldwide scale and serves as a significant instigator of pain, impairment, and increasing socio-economic strain on a global level. The ongoing discourse on the choice between total knee arthroplasty (TKA) and unicompartmental knee arthroplasty (UKA) for patients suffering from anterior medial osteoarthritis continues to ignite scholarly controversy. Our objective was to assess and compare the clinical outcomes of UKA and TKA within the same patient, hereby offering a novel perspective on this topic. Materials and methods: Fifty-seven individuals who underwent TKA on one knee and UKA on the other knee at the Department of Orthopaedics, First Hospital of Hebei Medical University between March 2019 and March 2024 were analysed for this retrospective study. We conducted a comprehensive examination and evaluation of perioperative laboratory assessments, radiological examinations, knee functionality, contentment levels, and postoperative complications within the two groups. Results: Following surgical procedures, levels of hemoglobin, red blood cells, and albumin were found to be elevated in the UKA group when compared to the TKA group (hemoglobin: 121.2 ± 12.54 vs. 110.1 ± 13.21 g/L; red blood cells: 4.0 ± 0.47 vs. 3.6 ± 0.42 *1012/L; albumin: 37.7 ± 5.66 vs. 35.3 ± 5.23 g/L). There is a significant difference in the hip-knee-ankle angles between the postoperative UKA group and the TKA group (5.3 ± 3.46° vs. 4.1 ± 2.86°, p < 0.05). There existed no notable disparity in postoperative visual analog scale, knee society score, and forgotten joint score between the two groups. However, a remarkable variance was observed in postoperative range of motion between the two groups (116.4 ± 5.96° vs. 108.4 ± 5.32°). Conclusion: We found that UKA resulted in less physical strain, less postoperative inflammatory response, improved joint mobility, although with less effective lower limb force line correction compared to TKA. Many patients have shown a preference for UKA and express higher levels of satisfaction with the procedure.
Lateral unicompartmental knee replacement for the treatment of arthritis progression after medial unicompartmental replacement

Abstract:PurposeLateral progression of arthritis following medial unicompartmental knee arthroplasty (UKA), although infrequent, is still the most common reason for revision surgery. Treatment options normally include conversion to total knee arthroplasty. An alternative strategy for some patients may be addition of a lateral UKA. We report the first results of staged bi-compartmental UKA (Bi-UKA) strategy.MethodsWe retrospectively selected from our UKA database patients who underwent a lateral UKA to treat a symptomatic lateral osteoarthritis progression after a medial UKA. The analysis included a clinical and radiological assessment of each patient.ResultsTwenty-five patients for a total of 27 knees of staged Bi-UKA were carried out in a single centre. The mean time interval between primary medial UKA and the subsequent lateral UKA was 8.1 years (SD ± 4.6 years). The mean age at the time of the Bi-UKA was 77.1 years (SD ± 6.5 years). The median hospital stay was 3 (range 2–9 days) days, and the mean follow-up after Bi-UKA was 4 years (SD ± 1.9 years). The functional scores showed a significant improvement as compared to the pre-operative status (paired t test, p = 0.003). There were no radiological evidences of failure. None of the patients needed blood transfusion, and there was no significant complications related to the surgical procedure without further surgeries or revisions at final follow-up.ConclusionsThese results suggest that addition of a lateral UKA for arthritis progression following medial UKA is a good option in appropriately selected patients.Level of evidenceObservational study without controls, Level IV.

Abstract:Knee osteoarthritis is one of the most common joint diseases worldwide, which is characterized by knee pain and structural changes, leading to resulting in disability, diminished quality of life, and financial burden. [ 1 ] Joint replacement surgery is the primary surgical treatment for knee osteoarthritis at present. Compared to traditional total knee arthroplasty, unicompartmental knee arthroplasty (UKA) offers advantages such as faster postoperative recovery, improved postoperative range of motion, preservation of bone stock, lower mortality rate, and fewer complications. [ 2 , 3 ] Despite various advantages, higher revision surgery rates continues to challenge patients who treated with UKA. [ 4 ] Therefore, decrease the risk of revision surgery for patients who underwent UKA has become an important issue to be solved urgently. [ 5 ]
