Abstract:Pathological Narcissism (PN) is a challenge to clinicians, who have difficulties dealing with clients relationally and forming and agreeing on a therapy contract. PN sufferers easily fuel relational conflict or withdraw from relationships. In spite of its severity and prevalence, there is no empirically supported treatment for this condition. Given this, integrative therapists need to be offered a series of principles of good clinical practice, that they can adopt irrespective of their preferred orientation. This article focuses on 5 domains of PN, that is: (a) maladaptive self–other schemas, (b) poor self-reflection and intellectualizing, (c) disturbed agency, (d) maladaptive coping and defenses, and (e) poor theory of mind and empathy. With this background, I offer specific treatment suggestions that can be applied in an integrative spirit and are formulated in a way that lends them to empirical investigation. With this and other recent efforts, the hope is to increase clinicians' and researchers' awareness of how PN can be treated and possibly increase the amount of empirical studies aimed at showing what principles of change are actually effective. Pathological Narcissism and narcissistic personality disorder are prevalent and present with significant comorbidity and create problems to self and others, but there is no empirically supported treatment to date for these conditions. Abstract:This review summarizes current knowledge about narcissistic personality disorder (NPD). Each section brings the reader up to date on advances in our knowledge during the last decade. In terms of NPD diagnosis, this review describes the addition of the dimensional model to the categorical model. The accumulating knowledge has led to the description of grandiose and vulnerable narcissism as well as their complex interrelationship. Strong support exists for co-occurrence of these presentations among people with high levels of grandiose narcissism. Studies have identified mechanisms, in domains such as self-esteem dysregulation, emotion dysregulation, cognitive style, interpersonal relations, and empathy, and possible developmental and temperamental antecedents of the disorder. Thus, it appears that NPD has a multifactorial etiology and pathogenesis, with numerous mechanisms associated with each area of dysfunction. Longitudinal studies support the view that these patients can improve, but such improvement is gradual and slow. Several treatments have been developed for the disorder, and a majority share commonalities, including clear goals, attention to treatment frame, attention to relationships and self-esteem, alliance building, and monitoring of countertransference.

Abstract:Persons with pathological narcissism (PN) or narcissistic personality disorder present with a wide array of symptoms and social problems. They need effective psychotherapies but empirical evidence for what works with them is mostly lacking. In this issue of the Journal and Clinical Psychology: In Session four authors from different orientations described single cases of patients suffering from different manifestations of PN. In this commentary we reviewed in detail the four cases and tried to distill (a) the treatment principles adopted; (b) the actual mechanisms of therapeutic change. We hope that the list of principles and mechanisms and change we identified here can help both clinicians and researchers develop and test effective options for persons with PN who are difficult to treat but do have chances to change in a meaningful way in treatment.

Abstract:Treatment of patients with pathological narcissism presents several challenges and there is paucity of published case reports that document meaningful and durable change in patients suffering from this condition. Using descriptive and atheoretical language, this paper presents a treatment of a young man in his transition from young adulthood to middle adulthood while he was negotiating complex residues of his experiences of growing up along with developmental challenges related to work and love. Against the backdrop of these transitions, the patient was working through various aspects of functioning related to pathological narcissism. Initially, given academic pressures and past romantic disappointments, he was confronting issues related to perfectionism, self-criticism, and avoidance. While he was able to move past some of these dynamics and function academically, later challenges related to becoming an independent adult led to a retreat into an avoidant state of futility and pessimism. Working through painful family dynamics related to not being seen and controlled, along with a deepening attachment in therapy as well as confrontation with realities of his life, led him to take steps toward greater independence. Thereafter, his treatment focused on learning from life experiences such as a newly developed career and romantic life, accepting the complexity of self and others, and tolerating disillusionments.

Abstract:Narcissistic personality disorder is a severance mental health issue, and affect person from both biological and social aspects. The article reviewed the latest international researches about the etiology, subtypes of narcissistic personality disorder, and related treatment strategies of narcissistic personality disorder. Narcissistic personality disorder is defined by an inflated self-importance, eagerness for admiration, and lack of compassion. It encompasses two subtypes, grandiose and vulnerable. Impact of narcissism on individuals' self-esteem, Self Control was examined through reviewing prior research on Chinese population. Moreover, correlation between parenting style, narcissistic personality and parent child relationship was also discussed. The result revealed that the formation of narcissistic personality disorder was influenced by environmental and social factors. Long-term psychological therapy is the main treatment for narcissistic personality disorder. Medication is not a primary treatment option, and certain antidepressants may exacerbate symptoms. The challenge and study directions have been identified in the end, abundant clinical trails on biological study of narcissistic personality disorder are needed, and the further researches on subtypes of narcissistic personality disorder are expected

Abstract:The following case study provides a description of the transference‐focused psychotherapy (TFP) treatment of a young man diagnosed with narcissistic personality disorder (NPD). TFP is an individual, psychodynamic therapy developed to treat a range of personality disorders. TFP is evidence‐based for the treatment of borderline personality disorder and has been adapted for the treatment of NPD. This case illustrates the application of the strategies and techniques of TFP to treatment of NPD as well as challenges clinicians may face in arriving at timely diagnosis of the disorder. Although no specific treatment for NPD has been empirically validated, TFP utilizes the therapeutic techniques identified across modalities for successful treatment of pathological narcissism. This report describes how treatment interventions such as goal setting, developing a therapeutic alliance, using a treatment contract and addressing treatment interfering behaviors contributed to this patient's improvement in self‐reflection, formation of healthier and more flexible ideas about self and other, increased self‐agency, tolerance of normative disillusionments and increase in empathy.

Abstract:Aim: Narcissism is a direct result of the lack of self-expression. Thus, this trait is enshrined in cycles of strategies to protect self-worth in interpersonal relationships. The aim of the present study was to understand in what way narcissistic individuals understand their interactions with themselves and others. Method: The study included two groups comprising high grandiose narcissism (GN, n = 43) and high vulnerable narcissism (VN, n = 44). The participants who received scores that were one standard deviation (SD) above the mean, either on the PNI-grandiosity or on the PNI-vulnerable dimensions, constituted the GN and VN groups among 518 students (Age: M = 23.57, Sd = 2.13). Results: The chi-square test was performed to evaluate the dimensions of narcissism with respect to responses of the participants and their partners (behavioral, cognitive, and emotional). The Pathological Narcissism Inventory (PNI) was administered to evaluate narcissistic characteristics. The "Criticism Story" of the Story Completion Inventory in Romantic Relationships (SCIRR) was used for criticism. The circular pattern between the responses of the participants and their partners was examined using the Interpersonal Schemas Scale (ISS). The results revealed that the vulnerable group gave more complementary responses emotionally and made more complementary predictions in terms of the expected reactions from the romantic partner than the grandiose group. Discussion: The results were discussed in reference to the basic self-esteem-protecting motivations of the groups.

Abstract:The narcissism spectrum model synthesizes extensive personality, social–psychological, and clinical evidence, building on existing knowledge about narcissistic grandiosity and vulnerability to reveal a view of narcissism that respects its clinical origins, embraces the diversity and complexity of its expression, and reflects extensive scientific evidence about the continuity between normal and abnormal personality expression. Critically, the proposed model addresses three key, inter-related problems that have plagued narcissism scholarship for more than a century. These problems can be summarized as follows: (a) What are the key features of narcissism? (b) How are they organized and related to each other? and (c) Why are they organized that way, that is, what accounts for their relationships? By conceptualizing narcissistic traits as manifested in transactional processes between individuals and their social environments, the model enables integration of existing theories of narcissism and thus provides a compelling perspective for future examination of narcissism and its developmental pathways.

Abstract:Several decades of theory suggest that pathological narcissism (PN) may limit psychotherapy success, but empirical evidence for such theories is limited and mixed. In addition, it has been proposed that individuals with high levels of PN may benefit more from supportive compared to interpretive psychodynamic therapies, but no studies thus far have investigated this question empirically. As such, our study aimed to extend past research by investigating (a) whether higher levels of pretreatment PN predict poorer treatment outcome and (b) whether the type of psychodynamic therapy (supportive or interpretive therapy) moderates these findings, in a sample of patients undergoing group psychodynamic psychotherapy for perfectionism. The sample was drawn from the University of British Columbia Perfectionism Treatment Study II (Hewitt et al., 2023) and consisted of 80 treatment-seeking adults with elevated perfectionism. Contrary to expectations, multilevel and multiple regression analyses showed that pretreatment PN did not significantly predict posttreatment changes in symptom severity, life satisfaction, or work and social impairment. We also did not find that either grandiose or vulnerable narcissism predicted likelihood of patient dropout. Finally, treatment type did not moderate the relationship between pretreatment PN and treatment outcome, suggesting that, contrary to our hypotheses, PN does not impact treatment outcome regardless of the interpretive nature of the psychodynamic group therapy. These results, taken together with past findings, suggest that PN may not be associated with poorer psychotherapy outcomes in certain contexts, such as in the case of supportive or interpretive psychodynamic group psychotherapy for perfectionism. Abstract:Narcissistic personality disorder is characterized by self-absorption, grandiosity, exploitation of others and lack of empathy. People with that disorder may switch from an overt form, mainly with grandiosity, to a covert presentation, with fears, hypersensitivity and dependence from others. Empathy represents a key point in detecting people affected by narcissistic personality disorder because, even if it is described as reduced, it plays a fundamental role in exploitation and manipulation. A systematic search of Literature without any language or time restriction, was performed combining thesaurus and free-search indexing terms related to Narcissistic personality disorder and empathy and produced 531 results. Fifty-two papers that analyzed possible issues in the empathic attitude of people with narcissistic personality disorder were included in this narrative review. Empathy is the capability of understating and feeling others emotions. It is not a unitary construct and can be distinguished in cognitive and affective. It might be channeled into prosocial and antisocial behaviors. A crucial trait identified in narcissistic empathy is affective dissonance that is closely related to rivalry as part of the dark tetrad (narcissism, machiavellianism, psychopathy, and sadism). Subjects affected by narcissistic personality disorder show greater impairment in affective aspects while their cognitive part of empathy appears preserved. Saving at least the cognitive aspects of empathy may contribute to therapeutic improvement of affective aspects.

Abstract:Sigmund Freud asserted that a part of the child's narcissism is transferred upon his superego, and thus narcissistic tensions occur in the ego as it strives to live up to the ego ideal. The antithesis to narcissism is not the object relation but object love. An individual's profusion of object relations, in the sense of the observer of the social field, may conceal his narcissistic experience of the object world; and a person's seeming isolation and loneliness may be the setting for a wealth of current object investments. The concept of primary narcissism is a good case in point. Although it is extrapolated from empirical observations, it does not refer to the social field but to the psychological state of the infant. The balance of primary narcissism is disturbed by maturational pressures and painful psychic tensions which occur because the mother's ministrations are of necessity imperfect and traumatic delays cannot be prevented.

Abstract:Modern diagnostic and classification frameworks such as the ICD‐11 and DSM‐5‐AMPD have adopted a dimensional approach to diagnosing personality disorder using a dual "severity" and "trait" model. As narcissistic personality has historically struggled to be adequately captured in dominant diagnostic systems, this study investigated the utility of the new ICD‐11 framework in capturing diverse narcissistic expressions. Participants were mental health clinicians (N = 180, 67% female, age = 38.9), who completed ratings of ICD‐11 personality severity, trait domains and a clinical reflection for two hypothetical case vignettes reflecting either prototypical "grandiose" or "vulnerable" narcissism. The majority of clinicians (82%) endorsed a diagnosis of personality disorder for both grandiose and vulnerable vignettes. Discriminant elements of personality impairment included rigid, unrealistically positive self‐view, low empathy and high conflict with others for grandiosity, and incoherent identity, low self‐esteem and hypervigilant, avoidant relations with others for vulnerability. Regarding trait profile, grandiose narcissism was predominately dissocial whereas vulnerable narcissism was primarily associated with negative affectivity and detachment. Qualitative responses highlight distinct clinical themes for each presentation. These findings suggest that clinicians using the ICD‐11 framework are able to identify common core elements of personality dysfunction in grandiose and vulnerable narcissism while also recognizing their distinctive differences.

Abstract:Narcissism is of great interest to behavioral scientists and the lay public. Research across the last 20 years has led to substantial progress in the conceptualization, measurement, and study of narcissism. The present paper reviews the current state of the field, identifying recent advances and outlining future directions. Advances include hierarchical conceptualizations of narcissism across one (narcissism), two (grandiose vs. vulnerable narcissism), and three factor levels (agentic extraversion, antagonism, narcissistic neuroticism), the development of measures to assess the components of narcissism, clarification of the relations between narcissism and self-esteem, an understanding of the behavioral and motivational dynamics underlying narcissistic actions and social outcomes, and insight regarding potential fluctuations between narcissistic states. Future directions point in general to increased research using the lower levels of the narcissism hierarchy, especially the three-factor level. At this level, more research is required on the etiology, heritability, stability, and centrality of the three components.
Distinguishing Between Grandiose Narcissism, Vulnerable Narcissism, and Narcissistic Personality Disorder

Abstract:This chapter draws upon the empirical literature to delineate the distinguishing characteristics of grandiose and vulnerable narcissism and narcissistic personality disorder (NPD). We find that these constructs can be well described using models of general personality such as the five-factor model (FFM) and, in particular, three primary traits including (low) agreeableness (or antagonism, entitlement, and self-involvement), agentic extraversion (or boldness, behavioral approach orientation), and neuroticism (or reactivity, behavioral avoidance orientation). Our review led to three primary conclusions. First, the FFM trait correlates of NPD and grandiose narcissism overlap quite substantially. Second, the two differ to some degree with regard to the role of extraversion, with stronger relations found for grandiose narcissism than NPD. Third, extant data suggest that vulnerable narcissism represents a construct that is largely divergent from NPD and grandiose narcissism, composed of the tendency to experience a wide array of negative emotions such as depression, self-consciousness, stress, anxiety, and urgency. Nevertheless, vulnerable narcissism shares a common core of interpersonal antagonism, though the traits associated with grandiose and vulnerable narcissism are not identical. Finally, our chapter concludes with recommendations for aligning the alternative model of personality disorders (PDs) in Section III of DSM-5 with the substantial and long-standing empirical research literature that documents the improved validity of dimensional, trait-based models of PDs.

Abstract:Anti-TNFalpha strategies can result in significant clinical benefits in rheumatoid arthritis (RA), but with an increased rate of opportunistic infections. Visceral leishmaniasis (VL) is a severe disease that can develop in immunocompromised hosts, principally in HIV patients. VL in RA patients treated with TNFalpha antagonists is an extremely rare event, and only one case has been described. Here we report a case of VL, occurring after 9 infusions of infliximab in association with azathioprine, in a patient who developed blood cytopenia, fluctuant fever, and splenomegaly.

Abstract:Abstract Narcissism is a Janusian personality construct, associated with both grandiose self-assuredness and dominance, as well as vulnerable insecurity and reactivity. Central questions of intra- and interpersonal functioning in narcissism are still a matter of debate. Neuroscience could help to understand the paradoxical patterns of experience and behavior beyond the limitations of self-reports. We provide a systematic review of 34 neuroscience studies on grandiose, vulnerable, pathological narcissism, and Narcissistic Personality Disorder (NPD), spanning experimental investigations of intra- and interpersonal mechanisms, research on neurophysiological and neuroendocrine aspects of baseline function, and brain structural correlates. While neuroscience has scarcely directly studied vulnerable narcissism, grandiose narcissism is associated with heightened vigilance to ego threat and stress responses following ego threat, as well as heightened stress indicators in baseline measures. Such responses are not commonly observed in self-reports, highlighting the potential of neuroscience to augment our understanding of self-regulatory dynamics in narcissism. Interpersonal functioning is characterized by deficits in social–affective processes. Both involve altered activity within the salience network, pointing to a double dissociation regarding the expression of narcissism and self/other oriented situational focus. Findings are summarized in an integrative model providing testable hypotheses for future research along with methodological recommendations.

Abstract:There has been a surge in interest in and research on narcissism and narcissistic personality disorder (NPD). Despite or because of this increased attention, there are several areas of substantial debate that surround the construct, including descriptions of grandiose and vulnerable dimensions or variants, questions regarding the existence of a consensual description, central versus peripheral features of narcissism, distinctions between normal and pathological narcissism, possible etiological factors, the role of self-esteem in narcissism, where narcissism should be studied, how it can be assessed, and its representation in diagnostic nosologies. We suggest that a failure to distinguish between grandiose (i.e., overtly immodest, self-centered, entitled, domineering) and vulnerable (e.g., self-centered, distrustful, neurotic, introverted) presentations of narcissism has led to a less cohesive and coherent literature and that trait-based models of personality and personality disorder can bring greater clarity to many of these important debates.
