Abstract:Freud's ambivalently negative attitude toward countertransference discouraged systematic study until some psychoanalysts, predominantly Kleinians, began to treat patients with narcissistic neuroses. Recognizing the need to understand the unconscious and conscious contribution of the analyst to the therapeutic process, Heimann, Rosenfeld, Balint, and Racker pioneered in serious study of countertransference. Racker and Boyer found that unresolved countertransference problems contributed significantly to unfavorable responses to psychoanalysis in seriously disturbed patients. Searles, Giovacchini, Ogden, and Volkan have like-wise furthered countertransference research. Following a historical review, the author delineates his personal approach to understanding patients, especially seriously disturbed ones, in terms of the ongoing introjection of patient and analyst of each other's projections. This approach stems from Rosenfeld's initial propositions.
Can neuroscience help to understand narcissism? A systematic review of an emerging field

Abstract:Abstract Narcissism is a Janusian personality construct, associated with both grandiose self-assuredness and dominance, as well as vulnerable insecurity and reactivity. Central questions of intra- and interpersonal functioning in narcissism are still a matter of debate. Neuroscience could help to understand the paradoxical patterns of experience and behavior beyond the limitations of self-reports. We provide a systematic review of 34 neuroscience studies on grandiose, vulnerable, pathological narcissism, and Narcissistic Personality Disorder (NPD), spanning experimental investigations of intra- and interpersonal mechanisms, research on neurophysiological and neuroendocrine aspects of baseline function, and brain structural correlates. While neuroscience has scarcely directly studied vulnerable narcissism, grandiose narcissism is associated with heightened vigilance to ego threat and stress responses following ego threat, as well as heightened stress indicators in baseline measures. Such responses are not commonly observed in self-reports, highlighting the potential of neuroscience to augment our understanding of self-regulatory dynamics in narcissism. Interpersonal functioning is characterized by deficits in social–affective processes. Both involve altered activity within the salience network, pointing to a double dissociation regarding the expression of narcissism and self/other oriented situational focus. Findings are summarized in an integrative model providing testable hypotheses for future research along with methodological recommendations.

Abstract:Sigmund Freud asserted that a part of the child's narcissism is transferred upon his superego, and thus narcissistic tensions occur in the ego as it strives to live up to the ego ideal. The antithesis to narcissism is not the object relation but object love. An individual's profusion of object relations, in the sense of the observer of the social field, may conceal his narcissistic experience of the object world; and a person's seeming isolation and loneliness may be the setting for a wealth of current object investments. The concept of primary narcissism is a good case in point. Although it is extrapolated from empirical observations, it does not refer to the social field but to the psychological state of the infant. The balance of primary narcissism is disturbed by maturational pressures and painful psychic tensions which occur because the mother's ministrations are of necessity imperfect and traumatic delays cannot be prevented.
Identification and its vicissitudes

Abstract:This paper attempts to understand the vicissitudes of identification within the co-ordinates of narcissism and the object relation. Firstly the dialectic pair primary identification/secondary identification are studied, and primary narcissism is suggested as the hypothesis which best explains them. The complex identification processes in the primary scene are considered next and the importance of the introjection of the oedipal parents for the formation of the superego is underlined. The importance of the structuring function of the introjection and projection mechanisms becomes embodied in the concept of projective identification, which comes to question the postulate of primary narcissism. The theory of projective-introjective identification is an extremely powerful instrument for explaining phenomena, however it obliges one to accept that the first introjections are radically different from the others. They have nothing to do with mourning but rather with primitive mechanisms which question the subject/object polarity and, so this author believes, spring basically from envy. Lastly, it is maintained that envy and libido are factors of a dialectic from which the object relation and the earliest processes of identification, previous to the Oedipus complex, proceed at one and the same time.

Abstract:Aim: Narcissism is a direct result of the lack of self-expression. Thus, this trait is enshrined in cycles of strategies to protect self-worth in interpersonal relationships. The aim of the present study was to understand in what way narcissistic individuals understand their interactions with themselves and others. Method: The study included two groups comprising high grandiose narcissism (GN, n = 43) and high vulnerable narcissism (VN, n = 44). The participants who received scores that were one standard deviation (SD) above the mean, either on the PNI-grandiosity or on the PNI-vulnerable dimensions, constituted the GN and VN groups among 518 students (Age: M = 23.57, Sd = 2.13). Results: The chi-square test was performed to evaluate the dimensions of narcissism with respect to responses of the participants and their partners (behavioral, cognitive, and emotional). The Pathological Narcissism Inventory (PNI) was administered to evaluate narcissistic characteristics. The "Criticism Story" of the Story Completion Inventory in Romantic Relationships (SCIRR) was used for criticism. The circular pattern between the responses of the participants and their partners was examined using the Interpersonal Schemas Scale (ISS). The results revealed that the vulnerable group gave more complementary responses emotionally and made more complementary predictions in terms of the expected reactions from the romantic partner than the grandiose group. Discussion: The results were discussed in reference to the basic self-esteem-protecting motivations of the groups.

Abstract:Anti-TNFalpha strategies can result in significant clinical benefits in rheumatoid arthritis (RA), but with an increased rate of opportunistic infections. Visceral leishmaniasis (VL) is a severe disease that can develop in immunocompromised hosts, principally in HIV patients. VL in RA patients treated with TNFalpha antagonists is an extremely rare event, and only one case has been described. Here we report a case of VL, occurring after 9 infusions of infliximab in association with azathioprine, in a patient who developed blood cytopenia, fluctuant fever, and splenomegaly.

Abstract:Using detailed clinical vignettes, the author illustrates and compares several North American approaches to the analysis of transference, tracing their origins in Freud's works and in various post-Freudian conceptualizations, including the writings of Anna Freud and Charles Brenner. Particular attention is paid to the work of Merton Gill, Evelyne Schwaber, Paul Gray, and the British analyst, Betty Joseph. Discussed and illustrated are controversies over the broader and narrower views of transference, the interpretation of action in the analytic setting, earlier and later interpretations of transference with particular emphasis on the contrast between contemporary Kleinian and ego psychological perspectives, the role of extra-transference interpretation, and the concept of the transference neurosis. An argument is made for an integrative approach, drawing upon different emphases, depending on the clinical circumstances and the point of affective immediacy for the patient, which may or may not coincide with the point of affective immediacy for the analyst.

Abstract:Individual canine synovial villi were used to establish short-term synovial organ cultures. These villi incorporated 3H-glucosamine into highly-polymerized 3H-hyaluronic acid (3H-HA), which was the only 3H-glycosaminoglycan identified in the culture medium. Some 3H-HA, and larger amounts of other 3H-glycosaminoglycans, were recovered from cultured tissues. Culture medium 3H-HA content was proportional to the surface area of cultured villi. Organ cultures of nonvillous synovium were compared with villi; nonvillous cultures synthesized less 3H-HA per mm2 of their synovial intimal surface than villi. These cultures complement cell culture techniques for in vitro studies of synovial lining cell function.

Abstract:To understand the affect-laden psychological processes of patients in a fully informed way, analysts must be able to tolerate a regression in which their unconscious or primary-process modes of functioning are given full sway. With the aid of impressions acquired in this way, cognitive functions in the analytic task are provided with a kind of authenticity not otherwise achievable. There are potential problems that can interfere with the analyst's functioning in the desired manner, and an awareness of these problems is essential. Basic issues of theory and technique that are involved are considered herein.

Abstract:In this work, three-dimensional (3D) film-based proton beam measurements were used for the first time to verify the patient-specific radiation dose distribution, beam range and compensator shape. Three passively scattered proton beams and one uniform scanning proton beam were directed onto an acrylic phantom with inserted Gafchromic EBT films. The average gamma index for a comparison of the dose distributions was less than one for 97.2 % of all pixels from the passively scattered proton beams and 98.1 % of all pixels for the uniform scanning proton beams, with a 3 % dose and a 3 mm distance-to-dose agreement tolerance limit. The results also showed that the average percentage of points within the acceptance criteria for proton beam ranges was 94.6 % for the passively scattered proton beams. Both the dose distribution and the proton beam range determined by the 3D EBT film measurement agreed well with the planning system values.

Abstract:Pathological Narcissism (PN) is a challenge to clinicians, who have difficulties dealing with clients relationally and forming and agreeing on a therapy contract. PN sufferers easily fuel relational conflict or withdraw from relationships. In spite of its severity and prevalence, there is no empirically supported treatment for this condition. Given this, integrative therapists need to be offered a series of principles of good clinical practice, that they can adopt irrespective of their preferred orientation. This article focuses on 5 domains of PN, that is: (a) maladaptive self–other schemas, (b) poor self-reflection and intellectualizing, (c) disturbed agency, (d) maladaptive coping and defenses, and (e) poor theory of mind and empathy. With this background, I offer specific treatment suggestions that can be applied in an integrative spirit and are formulated in a way that lends them to empirical investigation. With this and other recent efforts, the hope is to increase clinicians' and researchers' awareness of how PN can be treated and possibly increase the amount of empirical studies aimed at showing what principles of change are actually effective. Pathological Narcissism and narcissistic personality disorder are prevalent and present with significant comorbidity and create problems to self and others, but there is no empirically supported treatment to date for these conditions. Abstract:Central to a Kleinian view of the person and the analytic process is the notion that narcissism is mainly pathological, involving the denial of the object, of his existence as independent of oneself or of his having value. In this paper I will argue that the study of how this view is grounded in Freud's thinking, especially his thinking on the death instinct, and particularly in his <i>Beyond the Pleasure Principle</i>, deepens the understanding of this Kleinian view and points to its complexity. Also, through this study what essentially characterizes Kleinian practice, distinguishing it from other kinds of analytic practice is highlighted and Freud's ongoing relevance and contribution to contemporary Kleinian practice is made apparent.

Narcissism and female gender identity: a reformulation

Abstract:The theory of narcissism and the study of the psychological development of the female child have both been subjects of considerable controversy within psychoanalysis. Some issues central to these controversies have been discussed and evaluated with attention to the contributions made by advances in each area upon the other. From a historical perspective, Freud's illuminating introduction of the concept of narcissism opened the door toward explaining a variety of puzzling clinical phenomena. Deutsch then elaborated upon Freud's contribution as well as upon his specific bias in her description of the psychology of women. Later, Jacobson addressed herself to a reformulation of then existing views of female psychological development. Kohut's innovative clinical observations and Stolorow's functional definition of narcissism provided a unique vantage point from which to offer a critical restatement of narcissistic factors in the development of the female child. Specifically, narcissism was freed from its singular relationship to female development (the emphasis on castration shock and penis envy as rock bottom factors in female character formation) and related to structuralization of the representational world--self- and object-representations. Sexual differences can then be understood as most relevant for the content of these structures. In this context, the work of Stoller was cited. Finally, three topics that grew out of these considerations and elaborations were considered: the role of the little girl's attachment to and separation from her mother, the role of the psychosexual phases and an evaluation of penis envy, and the role of the father as an ideal. The reciprocal relationship between psychosexual development and the differentiation, integration and consolidation of self and object images has specific relevance for the manner in which femininity as the content of the self-representation of the developing girl enhances and is enhanced by the structuralization of the representations. The unfolding of the psychosexual phases with their associated imagery can be viewed as the gradual unfolding of more complex, differentiated and articulated self and object representations. Conversely, the gradual separation, integration and consolidation of self- and object-representations makes possible the development of increasingly more advanced forms of psychosexual experiences. Thus, the original question of the relationship between narcissism and female development has led to much broader issues. Narcissism concerns the structuralization and maintenance of the self. However, while narcissistic activities may promote structuralization, they may also serve to shore up a precarious sense of gender identity.
On the nature of transference interpretation and why only it can bring about analytic change

Abstract:Transference interpretation has always been regarded as very important to psychoanalytic practice. However, analysts differ on its centrality relative to other forms of intervention. This paper argues that transference interpretation as introduced by Freud and then taken up and developed by Klein ("transference interpretation proper") is, in fact, the only form of intervention that could bring about essentially analytic change. To understand why, a taxonomy of different forms of intervention commonly practiced within the analytic situation is presented, including interventions that relate to transference, but do not constitute transference interpretation proper. The latter kind is then described in detail. Next, the paper defines analytic change. It relies on a particular perspective on what it is to come to know psychic truth; one that sees such knowing as a lived state of mind, rather than a state of having knowledge about one's dynamics. This foundational Freudian perspective has been especially advanced through Klein's notion of phantasy. Given this view of analytic change it becomes clear that it can only be brought about through transference interpretation proper. The paper also addresses reasons why it seems especially difficult to embrace this view in contemporary psychoanalytic culture, while stressing how crucial it is to do so.
Full and empty spaces in the analytic process

Abstract:In this paper the author discusses two categories of patients which differ in terms of the impact they have in the countertransference. On the one hand, there are patients who create an empty space in the analyst's mind. The response they provoke is a kind of depressive feeling that remains after they leave. The patient may bring dreams and associations, but they do not reverberate in the analyst's mind. The experience is of dryness, a dearth of memory, which may--at times--leave the analyst with a sense of exclusion from the patient's internal world. At the other extreme, there are patients who fill the consulting room. They do that with their words, dreams and associations but also with their emotions and their actions. The experience is that the analyst is over-included in the patient's world. They have dreams that directly refer to the analyst and the analyst feels consistently involved in the patient's analysis. The pathway through which the analyst can understand both these types of patients is via the countertransference or, to put it another way, the analyst's passion. In 'Analysis terminable and interminable' Freud suggested that the bedrock of any analysis is the repudiation of femininity. The author believes this statement may be viewed as lying at the crossroads of the discussion about the limits of the theoretical and clinical psychoanalytic formulations which she refers to. In the examples presented the author relates the repudiation of femininity in its connections to the gaps implicit in psychoanalytic understanding.

Abstract:Based on the results of a technology assessment study we show that pervasive computing could amplify already existing problems related to the environment, human health, and society. Power consumption for digital networks, e-waste streams, and exposure to nonionizing radiation may all increase. Furthermore, social sustainability could be threatened by the technology if it is applied in a way that restricts consumers' privacy and freedom of choice. We refer to the precautionary principle as an analytical framework for discussing the opportunities and risks of pervasive computing for sustainable development.

Abstract:There has been a surge in interest in and research on narcissism and narcissistic personality disorder (NPD). Despite or because of this increased attention, there are several areas of substantial debate that surround the construct, including descriptions of grandiose and vulnerable dimensions or variants, questions regarding the existence of a consensual description, central versus peripheral features of narcissism, distinctions between normal and pathological narcissism, possible etiological factors, the role of self-esteem in narcissism, where narcissism should be studied, how it can be assessed, and its representation in diagnostic nosologies. We suggest that a failure to distinguish between grandiose (i.e., overtly immodest, self-centered, entitled, domineering) and vulnerable (e.g., self-centered, distrustful, neurotic, introverted) presentations of narcissism has led to a less cohesive and coherent literature and that trait-based models of personality and personality disorder can bring greater clarity to many of these important debates.

Abstract:Journal of the American Psychoanalytic Association, Volume 71, Issue 6, Page 1175-1210, December 2023. Interpretation of the latent meaning of manifest content is the core of the traditional approach to psychoanalytic treatment. The main purpose of such interpretation is to enhance the patient's self-knowledge, in particular his or her awareness of unconscious wishes and their embeddedness in inner conflicts. An assumption of classical psychoanalysis is that veridical interpretations—as Freud put it, interpretations that tally with what is real in the patient—will be especially effective therapeutically. These basic assumptions have been called into question, as reflected in such concepts as "narrative truth" and the overriding importance of the patient's "assured conviction" regarding interpretations. Also called into question is the therapeutic value of "deep" interpretations intended to uncover repressed impulses. To an important extent, these have been replaced by interpretations of defensive processes just below the surface of consciousness, and interpretations that make connections among different experiences, both of which are intended to help the patient understand how his or her mind works. There is also an increased emphasis on nonsemantic aspects of interpretation, as well as some degree of skepticism toward the therapeutic value of interpretation itself, along with an increased emphasis on the implicit interpretive aspects of the therapeutic relationship. Finally, representative research is presented on the relation between transference interpretation and therapeutic outcome.
