DRGKB: A Knowledge Base and Platform for Comparison, Evaluation and Worldwide Practices of Diagnosis-Related-Groups

Abstract:Objective: As a prospective payment method, the effects of Diagnosis-Related-Groups(DRGs) implementation in different regions are not the same. Our goal is to build a structured open access knowledge base describing the worldwide practice of DRGs, which includes quantitative performance indicators for comparison, evaluation and worldwide practices of DRGs.Materials and methods: We manually collected the qualified literature from PUBMED and constructed DRGKB website. Then we carried out descriptive statistics and analysis on outcome measurements performance, improvement strategy and specialty performance. The evaluation indicators are divided into four categories: (1) average length of stay & cost per case & medical quality, (2) resource utilization efficiency, including hospitalization rate and outpatient visits, (3) reimbursement rate & cost-benefit & risk adjustment ability, (4) DRG intra-group differences & complexity-based classification ability & explainability for costs.Results: The DRGKB finally contains 297 entries, including the research conducted on DRGs in all countries from 1980 to the present. It was found that DRGs generally has a considerable impact on hospital operation, including average length of stay, medical quality, and use of medical resources. At the same time, the current DRGs also have many deficiencies, including insufficient reimbursement rates and the ability to classify complex cases. Then we analyzed these underperforming parts by domain. Finally, we discussed the artificial intelligence trends and made analyses of the most concerned areas: cardiovascular disease, intensive care units, and arthroplasty.Discussion: After analyzing the pros and cons of current DRGs practices through DRGKB, lessons can be learned for countries seeking to pilot and further reform DRGs in the future. With the establishment of large databases, data-driven grouping strategies will become an important trend for DRGs.Conclusion: This paper constructed a knowledgebase to quantify the practice effects of DRGs, analyzed and visualized the development trends and area performance from a comprehensive perspective. This study provides a data-driven research paradigm for following DRGs-related work along with a proposed DRGs evolution model.Availability and implementationDRGKB is freely available at http://www.sysbio.org.cn/drgkb/.
