Clinical Analysis on the Resection of Pineal Region Lesions Through Infratentorial-Supracerebellar Approach at Sitting Position

Abstract:Objective To research the indications,problems and experiences of the resection of pineal region lesions through infratentorial-supracerebellar approach at sitting position.Methods Twenty-two cases of pineal region lesions removed via infratentorial-supracerebellar approach at sitting position were analyzed retrospectively,and compared with other 17 cases undergoing the resection at three-quarter-prone or concorde position.Results Among the 39 cases,24 were male and 15 were female,aging 7~52 years(average 22.6 years).Pathological diagnosis:germinoma 17 cases,embryonal carcinoma 4 cases,endodermal sinus tumor 2 cases,choriocarcinoma 1 case,benign teratoma 8 cases,malignant teratoma 2 cases,pinealocytoma 2 cases,meningioma 1 case,and dermoid cyst 2 cases.All 39 cases were suited for infratentorial-supracerebellar approach for their lesions were located mainly in the middle and beneath the extended line of straight sinus,and their boarders over the line were less than 1 cm and from the middle less than 3 cm.The 17 cases at three-quarter-prone or concorde position had the craniotomy with 7 cm×8 cm skull window;the total resection was achieved in 11 cases,subtotal resection in 2,partial resection 4 cases;postoperative ventricular hematoma occurred in 11 cases,cerebellar edema in 5,comatose state in 3 and death in 3.The 22 cases at sitting position had their craniotomy with 4 cm×5 cm ellipse-like skull window;total resection was achieved in 21 cases and subtotal resection in 1;cerebellar edema happened in only 1 case,pneumocephalus in 2,air embolus in 1 case that was successfully ameliorated by rapid management;no postoperative ventricular hematoma or serious disabilities or death were seen.The living patients had their radiotherapy and/or chemotherapy after operation according to theirpathological diagnosis.Conclusion There is obvious superiority with sitting position for the pineal lesions which are suited to infratentorial-supracerebellar approach.The air embolus would be decreased obviously or relieved rapidly by means of correct position,assisted strategies and appropriate anesthesia.The sitting position would be favor for minimal invasion with small craniotomy.
