Abstract:Objective: Current studies on body fat distribution and bone metabolism in adolescents are very limited. This study aimed to examine the relationship between the body fat distribution and serum osteocalcin levels in adolescents. Design and method: A total of 1068 subjects (564 boys,52.8%) aged 12–18 years from Ningxia were randomly selected through a stratified cluster sampling. Serum osteocalcin levels were measured by ELISA. An automatic bioelectric impedance analysis (BIA) was used to measure body compositions, which included body fat mass(BFM),trunk fat mass(TFM),limbs fat mass(LFM),visceral fat level(VFL). Results: In the total population, serum osteocalcin levels were negatively correlated with BFM, TFM, LFM, and fat mass percentage (FMP) (r = -0.076, -0.066, -0.094 and -0.117, respectively, all P < 0.05); Serum osteocalcin levels were positively correlated with limb fat mass percentage (LFMP) (r = 0.167, P < 0.001). Logistic regression analysis showed that the odds ratios (ORs) of FMP in the third and second quintiles suggesting high levels of osteocalcin is 0.28 (95% confidence interval [CI],0.13—0.57) and 0.44(95% CI, 0.25—0.76) compared with the lowest quartile level among total population, respectively. Additionally, the corresponding ORs for high levels of osteocalcin were 0.2 (95%CI, 0.05–0.72) in the fourth quantile of FMP,0.21 (95%CI, 0.08–0.55) in the third quantile of FMP and 0.45(95% CI, 0.25–0.82) in the second quantile of FMP in boys, respectively. Compared with the reference subgroup, the fourth quantile level of LFMP could better predict high osteocalcin levels between boys and girls, and the ORs were 7.46 (95%CI, 2.79—19.96) and 2.60(95%CI: 1.36—4.96), respectively. Conclusions: FMP have a negative impact on bone resorption in in Chinese adolescents, especially in boys. However, LFMP has a protective effect on bone metabolism between Chinese boys and girls.
Analysis of the Association between Fat Mass Distribution and Bone Mass in Chinese Male Adolescents at Different Stages of Puberty

Abstract:Background: Bone mineral acquisition during adolescence is crucial for maximizing peak bone mass. Fat mass (FM) and bone mass are closely related. This study investigated the association of FM distribution with bone mass in Chinese male adolescents. Method: A total of 693 male adolescents aged 10–18 years were recruited from a secondary school in Jiangmen, China. Their bone mass and body composition were measured by quantitative ultrasound and bioelectrical impedance analysis, respectively. The associations of the measures of fat distribution with bone parameters, i.e., broadband ultrasound attenuation, speed of sound (SOS), and stiffness index (SI), were analyzed using multiple linear regression. Age, height, body mass index, stage of puberty, physical activity, sedentary behavior, dietary energy intake, and dietary calcium and vitamin D intake were adjusted in the model. Further subgroup analyses of prepubertal and pubertal participants were conducted. Results: The measures of fat distribution showed negative associations with SOS and SI in total subjects (p < 0.010). In prepubertal boys, the measures of fat distribution were only associated with SOS (β = −0.377 to −0.393, p < 0.050). In pubertal boys, the measures of fat distribution had associations with all bone parameters (β = −0.205 to −0.584, p < 0.050). The strongest association was between trunk FM and SOS (β = −0.584, p < 0.001). Conclusion: This study supported that the measures of fat distribution were negatively associated with bone parameters in Chinese male adolescents. Trunk FM had the strongest association with bone parameter. These associations appear to be stronger in pubertal boys than in prepubertal boys.

Association Of Weight-Adjusted Body Fat And Fat Distribution With Bone Mineral Density In Middle-Aged Chinese Adults: A Cross-Sectional Study

Abstract:Background: Although it is well established that a higher body weight is protective against osteoporosis, the effects of body fat and fat distribution on bone mineral density (BMD) after adjustment for body weight remains uncertain. To examine the relationship between body fat and fat distribution and BMD beyond its weight-bearing effect in middle-aged Chinese adults.Method: The study had a community-based cross-sectional design and involved 1,767 women and 698 men aged 50-75 years. The BMD of the lumbar spine, total hip, and whole body, and the fat mass (FM) and percentage fat mass (% FM) of the total body and segments of the body were measured by dual-energy X-ray absorptiometry. General information on the participants was collected using structured questionnaire interviews.Result: After adjusting for potential confounders, an analysis of covariance showed the weight-adjusted (WA-) total FM (or % FM) to be negatively associated with BMD in all of the studied sites (P < 0.05) in both women and men. The unfavorable effects of WA-total FM were generally more substantial in men than in women, and the whole body was the most sensitive site related to FM, followed by the total hip and the lumbar spine, in both genders. The mean BMD of the lumbar spine, total hip, and whole body was 3.93%, 3.01%, and 3.65% (in women) and 5.02%, 5.57%, 6.03% (in men) lower in the highest quartile (vs. lowest quartile) according to the WA-total FM (all p < 0.05). Similar results were noted among the groups for WA-total FM%. In women, abdominal fat had the most unfavorable association with BMD, whereas in men it was limb fat.Conclusion: FM (or % FM) is inversely associated with BMD beyond its weight-bearing effect. Abdominal fat in women and limb fat in men seems to have the greatest effect on BMD.
Association of Body Compositions and Bone Mineral Density in Chinese Children and Adolescents: Compositional Data Analysis

Abstract:The purpose of this study was to investigate the relationship between body compositions and bone mineral density (BMD) and the effect of composition substitution among Chinese children and adolescents without the influence of multicollinearity. A dual-energy X-ray absorptiometry scan was used to determine the amount of truncal fat (TF), nontruncal fat (NTF), fat-free mass (FFM), and BMD. The compositional data analysis and the compositional proportional substitution analysis were conducted to determine the effect of each part of body compositions on BMD and its substitution effects. Four hundred sixty-six (466) (boys: 51.9%) participants completed this cross-sectional study. For girls, in the overweight group, the relationship between TF and the BMD was positive (β=2.943e−01, p=0.006) while the NTF showed the opposite trend (β=−2.358e−01, p=0.009). When 4% NTF or FFM was substituted by TF, the BMD increased by about 0.1 and 0.05 units (p<0.05), respectively. For boys, the association between FFM and BMD was statistically positive (β=4.091e−02, p=0.0001). There was a positive correlation between TF and BMD (β=7.963e−02, p=0.036). But with the increase of BMI, this correlation shifted in the opposite direction. In conclusion, compared to TF and NTF, FFM had a better protective effect on BMD, especially for boys. The risk of NTF accumulation on BMD was greater than that of TF accumulation. Compared with girls, boys were more sensitive to the amount of TF.
The influence of body fat content and distribution on bone mass in healthy Chinese adults

Abstract:Background: Previous studies have reported a close relationship between body mass index (BMI) and bone mineral density (BMD). However, the effects of fat on bone mass remain controversial, particularly for fat tissue distribution. The aim of this study was to evaluate the association between regional fat percentage and BMD using a population-based database. Methods: This study included participants who were referred to the Department of Radio Diagnosis for dual-energy X-ray absorptiometry (DEXA) scan from January 2018 to December 2020. The relationships between BMI and regional fat percentage with BMD were assessed using multiple linear regression and generalized additive models. The risk of low bone mass was determined using logistic regression. Results: There was a negative relationship between the regional fat percentage and femoral neck BMD (FN BMD) or lumbar spine BMD (LS BMD) in both genders (p < 0.05). In females, an inverted U-shaped relationship was observed between regional fat percentage and BMD at both the femoral neck and lumbar spine. The impact of trunk fat percentage on LS BMD was associated with the highest OR of low bone mass in females (OR 3.1, 95% CI 2.6 to 3.7, p for trend <0.001), while the impact of abdomen fat percentage on FN BMD was associated with the highest OR of low bone mass in males (OR 2.2, 95% CI 1.8 to 2.7, p for trend <0.001). Conclusion: There was an inverted U-shaped relationship between regional fat percentage and BMD. Excessive regional fat percentage may be harmful to bone health in both genders. To promote bone health, males should restrict their abdomen circumference and avoid abdominal adiposity, while females should control their trunk circumference.

Abstract:Objective: To investigate the relationship between bone mineral density (BMD) and age, body mass index (BMI), fat mass (FM), fat-free mass (FFM), abdominal fat mass(ABD FM) in middle-aged and aged men and women in Tianjin area of China. Methods:BMD at lumber spine2-4(L2-4),total(TOT) bone and body composition were measured by dual energy X-ray absorptiometry(DXA) in eighty-two middle-aged and aged men and compared with one hundred and six middle-aged and aged women who were controlled. Results: (1)FM, FFM,ABD FM and BMI were all positively correlated with BMD, age was negatively correlated with BMD in middle-aged and aged men and women. (2)Multiple regression analysis revealed that FFM was a main determinant of BMD in men while FM was a main determinant of BMD in women and ABD FM might influence on L2-4 BMD in senior woman. Conclusion: FM and FFM are positively correlated with BMD and there are gender differences in the middle-aged and aged persons. Abdominal obesity is related with BMD and has protective effects on lumber spine L2-4 BMD.

Abstract:The aim of the present study was to determine association between body composition, muscle strength, diet and physical exercise with bone mineral content (BMC) and bone area (BA) in 283 Chinese adolescent girls aged 15 years in Beijing, China. Body composition, pubertal stage, physical activity and dietary intakes were assessed using standard validated protocols. Total body and forearm bone, lean body mass (LBM) and fat body mass (FBM) were determined by dual X-ray absorptiometry. Multivariate linear regression analyses were carried out to examine the predictors of BMC and BA, after controlling for potential confounders. The subjects had a mean age of 15.0 (SD 0.9) years and 99.6% of them had reached menarche. Multivariate analyses showed that LBM, FBM, handgrip muscle strength and milk intake were significant independent determinants of BMC and BA of the total body and/or forearm sites. LBM was found to be a stronger independent determinant than FBM of BMC and BA, whereas handgrip muscle strength was only found as significant determinant of BMC and BA at the forearm sites than in total body BMC and BA. Further, total physical activity level had a significant positive association with handgrip and LBM. This suggested that greater muscle strength and higher LBM may reflect higher levels of physical activity. Therefore, continuous healthy lifestyle practices such as adequate intake of milk and continuous participation in physical activity should be encouraged throughout adolescence to optimise bone growth during this period.
Age and sex effects on the association between body composition and bone mineral density in healthy Chinese men and women.

Abstract:Objective: Many studies have examined the relationships between body composition and bone mineral density (BMD), but little attention has been given to how these relationships vary by age and sex. The aim of this study was to investigate the distributions of lean mass (LM), fat mass (FM), and BMD and the correlation between body composition and BMD in Chinese men and women of different ages. Methods: In total, the body compositions of 1,475 men and 1,534 women aged 20 to 96 years were analyzed. Using dual-energy x-ray absorptiometry, we measured the BMD of the spine, femur, and total body and the LM, FM, and percentage of body fat (Fat %). The population was divided into groups based on age and sex: young, premenopausal, and postmenopausal women and young, middle-aged, and older men. The correlations between BMD and variables of body composition were investigated using the Pearson correlation test and multiple regression analysis. Results: The peak BMD values of the spine, femur, and total body are observed in women aged 30 to 39, 20 to 29, and 30 to 39 years, respectively, and in men aged 20 to 29 years at all sites. The peak LM, FM, and Fat % values were observed at age 40 to 49, 60 to 69, and 70 to 79 years in women, respectively, and at 40 to 49, 70 to 79, and 70 to 79 years in men, respectively. A statistically significant correlation was observed between LM and BMD of all sites (r = 0.253-0.591, P < 0.01) in all groups. However, FM was significantly correlated to BMD only in postmenopausal women and older men (r = 0.089-0.336, P < 0.01). Fat % negatively correlated to BMD in young people (r = -0.169 to -0.366, P < 0.05). When stepwise regression models were analyzed, LM remained the strongest predictor of total body, spine, and femur BMD (standardized coefficients = 0.264-0.637, P < 0.001) in Chinese men and women of different ages. Conclusions: We believe that LM is the strongest predictor of BMD at all ages for Chinese men and women, even though positive correlations between FM and BMD existed in old people.

Abstract:This study investigated the relative contribution of fat mass and lean mass to bone mineral density (BMD) in young and premenopausal healthy Chinese women. The study was performed in 282 young and premenopausal healthy women with regular menstrual cycles. The BMD at lumbar spine (L2–L4), total hip and total body, together with fat mass and lean mass were assessed by dual-energy X-ray absorptiometry (DXA); body height, weight, waist and hip circumference were also measured, and body mass index (BMI) and waist-hip ratio were calculated. Fat mass was a major determinant for BMI, BMI and lean mass were positively related to L2–L4, total hip and total body bone density (P<0.001 for all), lean mass was the only independent factor contributing to BMD at L2–L4 (standardized coefficient β=0.282, P<0.001), total hip (β=0.336, P<0.001) and total body (β=0.361, P<0.001) in multiple stepwise regression analysis. The correlation between BMI and BMD was improved after adjustment for fat mass, while decreased or even lost when lean mass was adjusted. These data suggested that in the Chinese population, lean mass is an important factor determining BMD in young and premenopausal women.
Impact of high fat and low lean mass phenotype on bone mineral content: A cross-sectional study of Chinese adolescent population

Abstract:Background and objective: Research on body composition phenotypes and bone health in adolescents is limited. Hence, this study aimed to analyze the relationship between different body composition phenotypes, bone mineral content, and bone metabolism markers in Chinese adolescents. Methods: In this cross-sectional study, 1852 adolescents aged 12 to 18 years were selected from six schools in Yinchuan City between 2017 and 2020 using stratified cluster random sampling. The participant's body composition and bone mineral content (BMC) were measured using bioelectrical impedance analysis (BIA). Serum bone metabolic markers (OC, CTX, and Ca) were measured. Based on their FMI and LMI, individuals were categorized into four body composition phenotypes: low fat mass-low lean mass (LFMI-LLMI), low fat mass-high lean mass (LFMI-HLMI), high fat mass-high lean mass (HFMI-HLMI), high fat mass-low lean mass (HFMI-LLMI). Results: There was a statistically significant difference between the four different body composition phenotypes with BMC, CTX and Ca in boys (all P < 0.05), similar conclusions were found in girls, except the OC and CTX. After adjusting for age, gender, smoking, drinking, and others, compared with the LFMI-HLMI reference group, the two high FMI groups (HFMI-LLMI and HFMI-HLMI) had a greater negative correlation with BMC, while the low BMC risk of the HFMI-LLMI group was the highest (OR = 33.28; 95%CI: 11.12-99.63; P < 0.001). The correlation between BMC of different body composition phenotypes in boys was greater than that in girls. HFMI-HLMI is a risk phenotype negatively associated with Ca content (β = -0.12; 95%CI: -0.19 to -0.04; P < 0.05). Regardless of body composition level, BMC was always negatively correlated with fat mass (LLMI: β = -0.27; 95%CI: -0.32-0.21; HLMI: β = -0.52, 95%CI: -0.65-0.40) and positively correlated with lean mass (LFMI: β =0.24; 95%CI: 0.20-0.28; HFMI: β =0.23, 95%CI:0.13-0.33) (all P < 0.001). The fat mass showed different correlations with OC and CTX in girls and boys based on LLMI or HLMI (all P < 0.05). Conclusion: HFMI-LLMI is a risk phenotype of low BMC in Chinese adolescents, and the relationship between fat mass and bone metabolism markers is affected by lean body mass and gender.

Abstract:Identifying modifiable factors that influence bone status during adulthood to maximize bone quality is a potential primary strategy in the prevention of osteoporosis in later life. We investigated the impact of body height, body weight, body mass index, and body composition on calcaneal bone characteristics as measured with quantitative ultrasound in 441 Chinese adults (238 women) aged 20-55 yr from the middle areas of China. Body composition, including fat-free mass (FFM), muscle mass, and fat mass were obtained by bioelectrical impedance analysis. Bivariate correlation analysis demonstrated a significant negative correlation between age and broadband ultrasound attenuation (BUA), speed of sound (SOS), and stiffness index (SI) both in men (r = -0.177, p < 0.05; r = -0.499, p < 0.001; r = -0.530, p < 0.001, respectively) and women (r = -0.344, p < 0.001; r = -0.336, p < 0.001; r = -0.369, p < 0.001, respectively). Body height, body weight, FFM, and muscle mass had positive correlations with BUA, SOS, and SI in both genders, with FFM having the strongest correlation with BUA, SOS, and SI in men (r = 0.351, p < 0.001; r = 0.391, p < 0.001; r = 0.406, p < 0.001, respectively) and women (r = 0.331, p < 0.001; r = 0.288, p < 0.001; r = 0.324, p < 0.001, respectively). Fat mass had a positive correlation with BUA (r = 0.331, p < 0.001), SOS (r = 0.288, p < 0.001), and SI (r = 0.324, p < 0.001) in women, which was not found in men. Multivariate regression analysis revealed that, in both genders, FFM was a positive predictor for all 3 quantitative ultrasound variables.
Sex-specific and Age-Specific Characteristics of Body Composition and Its Effect on Bone Mineral Density in Adults in Southern China: a Cross-Sectional Study.

Abstract:ObjectivesThis study was an attempt to investigate the variation trend of body composition with ageing and explore the association between regional body composition and bone mineral density (BMD).DesignCross-sectional study.Setting and participantsA total of 5749 healthy adults aged 20–95 years was recruited from 2004 to 2017.Primary outcome measuresWhole-body lean mass (LM), fat mass (FM), android FM, gynoid FM, appendicular lean mass (ALM) and BMD in the lumbar spine, femoral neck and total hip were obtained by dual-energy X-ray absorptiometry (DXA). The android/gynoid fat mass ratio (A/G FMR) based on DXA scan was calculated as an indicator of adipose distribution. Pearson correlation and multiple linear regression analyses were used to determine the associations between body composition, adipose distribution, and BMD of each skeletal site.ResultsWhole-body FM, percentage of whole-body FM, Android FM and A/G FMR consistently increased with age in both genders, especially in women, and ALM began to decrease in the fifth decade for both men and women. In multivariable linear regression models with age, body mass index, A/G FMR and ALM as predictor variables, ALM was associated with the most BMD variance of all skeletal sites in men (standard β ranged from 0.207 to 0.405, p<0.001), although not the largest but still a positive predictor of BMD in women (standard β ranged from 0.074 to 0.186, p<0.05). A/G FMR was an inverse predictor of BMD at all skeletal sites for women (standard β ranged from −249 to −0.052, p<0.01) but not in men.ConclusionsIn this large cohort of Chinese adults, ALM had a strong positive association with BMD in both genders. A/G FMR as an indicator of central adipose accumulation was inversely associated with BMD in women but not in men.

Abstract:Background . The relationships between adipose tissue distribution and peak bone mineral density (BMD) in young adults are still unclear. The aim of this study was to investigate the body composition associations between fat mass (FM), lean mass (LM), regional adipose tissue distribution, and peak BMD across a cohort of young Chinese adults. Methods . Dual-energy X-ray absorptiometry (DXA) scans were performed on 786 men and 825 women aged from 20 to 40 years old to measure the markers for whole-body LM, FM, and BMD in the lumbar spine (LS), femoral neck (FN), and total hip (TH) areas. The android/gynoid FM ratio (A/G FMR) based on the DXA scans was calculated as an indicator of adipose tissue distribution. Pearson’s correlation and multivariable linear regression analyses were conducted to determine the body composition relationships between adipose tissue distribution and BMD of each skeletal site. Results . The body composition characteristics were different in young males and females: a higher body FM percentage was found in females, while males had higher LM and A/G FMR. The markers for WBLM and WBFM had significant positive correlations with BMD among the linear regression analyses in both genders, while the relationships between A/G FMR and BMD were different in males and females; significant inverse associations were showed in all skeletal sites for women (standard β ranged from -0.266 to -0.170, P < 0.001 for all), but no significant relationships were found in men except for an inverse association in the LS skeletal site (standard β with -0.115, P = 0.016 ). Conclusions . In this sample of young Chinese adults, both whole-body lean mass and fat mass had significant positive relationships with BMD in both genders. The A/G FMR, as an indicator of central adipose tissue distribution, was inversely associated with BMD, especially in females.

Abstract:Measurements of body composition by dual-energy X-ray absorptiometry (DXA) have evident value in evaluating skeletal and muscular status in growing children and adolescents. This study aimed to generate age-related trends for body composition in Chinese children and adolescents, and to establish gender-specific reference percentile curves for the assessment of muscle-bone status. A total of 1541 Chinese children and adolescents aged from 5 to 19 years were recruited from southern China. Bone mineral content (BMC), lean mass (LM) and fat mass (FM) were measured for total body and total body less head (TBLH). After 14 years, total body LM was significantly higher in boys than girls (p < 0.001). However, total body FM was significantly higher in girls than boys in age groups 13–19 years (p < 0.01). Both LM and FM were consistent independent predictors of total body and subcranial bone mass in both sexes, even after adjustment for the well-known predictors of BMC. The results of multiple linear regression identified LM as the stronger predictor of total body and subcranial skeleton BMC while the fat mass contributed less. For all the subjects, significant positive correlations were observed between total body LM, height, total body BMC and subcranial BMC (p < 0.01). Subcranial BMC had a better correlation with LM than total body BMC. We have also presented gender-specific percentile curves for LM-for-height and BMC-for-LM which could be used to evaluate and follow various pediatric disorders with skeletal manifestations in this population.
Relationship Of Body Composition With Prevalence Of Osteoporosis In Central South Chinese Postmenopausal Women

Abstract:P>ObjectivesTo elucidate the relationship between body composition and bone mineral density (BMD) and the prevalence of osteoporosis in central south Chinese postmenopausal women.MethodsA cross-sectional study was conducted on 954 healthy central southern Chinese postmenopausal women, aged 50-82. Total body, lumbar spine and left femur BMD and total body soft tissue composition were measured by dual X-ray absorptiometry.ResultsAmong the study population, 578 (60 center dot 5%) subjects were without osteoporosis and 376 (39 center dot 4%) subjects were osteoporotic. The osteoporotic women were older, shorter and thinner, had an earlier age at menopause, a lower BMD and bone mineral content (BMC) of the total body and at different sites, and had lower body mass and body mass components than the women without osteoporosis. Both fat mass and lean mass were positively correlated with age at menopause, height, weight, body mass index (BMI) and BMD at all sites. Fat mass and lean mass were also inversely correlated with age and years since menopause (P < 0 center dot 05). After controlling for age, age at menopause and height, both fat mass and lean mass were positively correlated with BMD at the lumbar(1-4) spine, the femoral neck and the total hip. Fat mass was the most significant determinant of BMD at the lumbar(1-4) spine with a higher R2 change and a partial R2 compared with that of lean mass, while lean mass had more impact on the total hip values. Either a fat mass below 18 center dot 4 kg or a lean mass below 33 center dot 9 kg was correlated with a higher prevalence of osteoporosis at the lumbar spine or total hip.ConclusionsIn central south Chinese postmenopausal women, both fat mass and lean mass are correlated with BMD at the lumbar spine and hip. Fat mass was the most significant determinant of BMD at the lumbar spine, while lean mass had more impact on the total hip value. Both lower values of fat mass and lean mass are related to a higher prevalence of osteoporosis at either the lumbar spine or the total hip. Thus, it is important to maintain a reasonable body weight to balance bone health and other metabolic disorders.

Abstract:We examined ethnic difference in the association of body fat and trunk fat with bone mineral density (BMD) among Chinese, white, and black subjects. We found that, with greater body and trunk fat, both white and black subjects were more likely to have a low BMD than Chinese subjects.

Abstract:Accumulation of body fat is known to be beneficial to bone mass through increased body weight. However, not all the skeleton is loaded by body weight. Therefore, we assume that fat stores would exert different effects on bone mass at different skeletal sites. In this study, 84 perimenopausal Chinese women were recruited. Using dual-energy X-ray absorptiometry, total body fat mass (TBFM), total body lean mass (TBLM), percent body fat (PBF), and total body and regional bone mineral density (BMD) were measured. Correlation analysis indicated that PBF correlated negatively with BMD at ribs and both arms (all P < 0.05). After adjusting for TBLM, PBF had a significantly negative correlation with BMD at head, ribs, both arms, and whole body (all P < 0.05). With adjustment for body weight and height, a significantly negative correlation between PBF and BMD was present, not only at ribs and arms but also at legs and whole body (all P < 0.05, except right leg, at P = 0.094). There was a significantly positive correlation between body weight and leg BMD (all P < 0.001). Body weight was positively correlated with TBFM (r (2) = 0.783, P < 0.001) and TBLM (r (2) = 0.770, P < 0.001). Based on the results, we conclude that increased body fat stores would exert a detrimental effect on BMD, but this effect is more prominent on non-weight-bearing bone. On weight-bearing bone, the detrimental effect of increased body fat could be offset or outweighed by the beneficial effect of increased body weight.
