Abstract:Background . The relationships between adipose tissue distribution and peak bone mineral density (BMD) in young adults are still unclear. The aim of this study was to investigate the body composition associations between fat mass (FM), lean mass (LM), regional adipose tissue distribution, and peak BMD across a cohort of young Chinese adults. Methods . Dual-energy X-ray absorptiometry (DXA) scans were performed on 786 men and 825 women aged from 20 to 40 years old to measure the markers for whole-body LM, FM, and BMD in the lumbar spine (LS), femoral neck (FN), and total hip (TH) areas. The android/gynoid FM ratio (A/G FMR) based on the DXA scans was calculated as an indicator of adipose tissue distribution. Pearson’s correlation and multivariable linear regression analyses were conducted to determine the body composition relationships between adipose tissue distribution and BMD of each skeletal site. Results . The body composition characteristics were different in young males and females: a higher body FM percentage was found in females, while males had higher LM and A/G FMR. The markers for WBLM and WBFM had significant positive correlations with BMD among the linear regression analyses in both genders, while the relationships between A/G FMR and BMD were different in males and females; significant inverse associations were showed in all skeletal sites for women (standard β ranged from -0.266 to -0.170, P < 0.001 for all), but no significant relationships were found in men except for an inverse association in the LS skeletal site (standard β with -0.115, P = 0.016 ). Conclusions . In this sample of young Chinese adults, both whole-body lean mass and fat mass had significant positive relationships with BMD in both genders. The A/G FMR, as an indicator of central adipose tissue distribution, was inversely associated with BMD, especially in females.

Abstract:Objective: Many studies have examined the relationships between body composition and bone mineral density (BMD), but little attention has been given to how these relationships vary by age and sex. The aim of this study was to investigate the distributions of lean mass (LM), fat mass (FM), and BMD and the correlation between body composition and BMD in Chinese men and women of different ages. Methods: In total, the body compositions of 1,475 men and 1,534 women aged 20 to 96 years were analyzed. Using dual-energy x-ray absorptiometry, we measured the BMD of the spine, femur, and total body and the LM, FM, and percentage of body fat (Fat %). The population was divided into groups based on age and sex: young, premenopausal, and postmenopausal women and young, middle-aged, and older men. The correlations between BMD and variables of body composition were investigated using the Pearson correlation test and multiple regression analysis. Results: The peak BMD values of the spine, femur, and total body are observed in women aged 30 to 39, 20 to 29, and 30 to 39 years, respectively, and in men aged 20 to 29 years at all sites. The peak LM, FM, and Fat % values were observed at age 40 to 49, 60 to 69, and 70 to 79 years in women, respectively, and at 40 to 49, 70 to 79, and 70 to 79 years in men, respectively. A statistically significant correlation was observed between LM and BMD of all sites (r = 0.253-0.591, P < 0.01) in all groups. However, FM was significantly correlated to BMD only in postmenopausal women and older men (r = 0.089-0.336, P < 0.01). Fat % negatively correlated to BMD in young people (r = -0.169 to -0.366, P < 0.05). When stepwise regression models were analyzed, LM remained the strongest predictor of total body, spine, and femur BMD (standardized coefficients = 0.264-0.637, P < 0.001) in Chinese men and women of different ages. Conclusions: We believe that LM is the strongest predictor of BMD at all ages for Chinese men and women, even though positive correlations between FM and BMD existed in old people.
Correlational study between regional fat content and the bone mineral density in 12285 healthy subjects

Abstract:Objective To investigate the correlation between the fat percentage around the lumbar vertebrae and the hip and bone mineral density ( BMD) , and to discuss the effect of regional fat content on BMD.Methods A total of 12285 subjects of normal people receiving physical examination were collected.BMD of the lumbar vertebrae and the hip was detected using DEXA.The fat percentage of the trunk and the legs was also detected using local model of DEXA.All subjects were grouped by gender and BMI. One-way analysis of variance and multiple linear regression analysis were performed to analyze the correlation between regional fat percentage and BMD.Results The results of one-way analysis of variance revealed that in groups with higher hip fat percentage, the average BMD was lower.The average BMD of the lumbar vertebrae and the hip in groups with higher trunk fat percentage in males was higher.The average BMD of the lumbar vertebrae in groups with higher trunk fat percentage in females was lower, while no significant changes of the hip BMD were observed.The results of multiple linear regression analysis showed that, in female groups, BMD was positively correlated with height ( r =0.276 -0.497 ) and weight ( r =0.216 -0.526 ) , while negatively correlated with age (r=-0.7300 to -0.454), the trunk fat percentage (r=-0.400to -0.245), and the leg fat percentage (r=-0.369 to -0.352 ) .After controling other variables, the lumbar BMD showed no statistical correlation with the trunk fat percentage.However, remarkable negative correlation existed between the lumbar BMD and the trunk fat percentage, as well as the hip BMD and regional fat percentage, which was even more remarkable as BMI rising.Therefore, the negative correlation between the lumbar BMD and the trunk fat percentage was most notable.In male groups, BMD was positively correlated with height ( r=0.139-0.388) and weight (r=0.139-0.388), while negatively correlated with age (r=-0.494-0.077), trunk fat percentage ( r=-0.329 to -0.223 ) , and the leg fat percentage ( r=-0.400 to -0.216 ) .In multiple linear regression models, after controling other variables, the lumbar BMD presented notable positive correlation with the trunk fat percentage only when BMI<20kg/m2(r=0.294).And remarkable negative correlation also existed between the lumbar BMD and the trunk fat percentage, as well as the hip BMD and the regional fat percentage.The negative correlation between the lumbar BMD and the trunk fat percentage was most notable (-0.207 to -0.108) .The negative correlation was decreased along with the increase of BMI.Conclusion Fat content exerts notable influence in BMD.Along with the increase of local fat proportion, BMD decreases significantly.And the correlation is more notable in women with higher BMI index.Fat distribution also has obvious effect on BMD.The negative correlation between the hip fat proportion and the lumbar BMD is most remarkable.
Gender-specific associations between abdominal adipose mass and bone mineral density in the middle-aged US population

Abstract:Objectives: The relationship between abdominal adipose tissue and osteoporosis is poorly understood. The purpose of this study was to examine the associations of abdominal adipose tissue with bone mineral density (BMD) among a nationally representative sample of US middle-aged adults. Material and methods: This study included 1498 participants from the National Health and Nutrition Examination Survey 2013-2014 and 2017-2018. Dual-energy x-ray absorptiometry was used to measure BMD at the lumbar spine and femoral neck, as well as to assess abdominal adipose mass by categorizing total adipose tissue (TAT) into visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT). Linear regression was used to assess the relationship between abdominal adipose tissue and BMD, and logistic regression and generalized additive model were used to assess the associations of abdominal adipose tissue with the development of low BMD. Results: In our study, men accounted for 51.3%, and the mean age and body mass index for men and women were 49.3 and 49.6 years, and 23.9 and 28.3 kg/m2, respectively. In the univariate model, we found that abdominal adipose mass was positively associated with BMD at femoral neck and spine in both genders. In the multivariate model, among men, a negative correlation was observed between TAT and SAT and BMD at the femoral neck. Additionally, higher masses of TAT, SAT, and VAT were found to significantly increase the risk of low BMD at both the femoral neck and lumbar spine. In contrast, there was no significant association between abdominal adipose mass and BMD in middle-aged women, regardless of menopausal status. Conclusions: Our finding suggested that abdominal adipose tissue, regardless of its location (SAT or VAT), may have a negative impact on BMD in middle-aged men independently of body weight, but this relationship was not observed in women. Further research is needed to confirm these findings and investigate potential mechanisms underlying these associations.
Associations of Fat Mass and Fat Distribution With Bone Mineral Density in Non-Obese Postmenopausal Chinese Women Over 60 Years Old

Abstract:Background Bone mineral density (BMD) loss is a major complication of menopause, and this loss is closely associated with Fat mass (FM). The relationship between FM, fat distribution (FD), and BMD in postmenopausal women, however, remains incompletely understood. The present study was thus developed to explore these associations between body fat accumulation, FD, and BMD among non-obese postmenopausal women over the age of 60. Methods This was a cross-sectional analysis of 357 healthy postmenopausal women between the ages of 60.2 and 86.7 years. Dual-energy X-ray absorptiometry (DXA) was utilized to measure total and regional BMD as well as fat-related parameters including total FM, android and gynoid fat, body fat percentage (BF%), and total lean mass (LM) for all subjects. The android-to-gynoid fat ratio (AOI) was used to assess FD. Pearson’s correlation testing and multiple regression analyses were used to explore relationships among AOI, LM, FM, and BMD. Results Both LM and FM were positively correlated with total and regional BMD in univariate analysis (all P < 0.01), whereas BMD was not significantly associated with AOI in any analyzed site other than the head. Multivariate linear regression models corrected for age, height, and years post-menopause, revealed a sustained independent positive relationship between FM and BMD (standard β range: 0.141 – 0.343, P < 0.01). The relationship between FM and BMD was unaffected by adjustment for LM (standard β range: 0.132 – 0.258, P < 0.01), whereas AOI had an adverse impact on BMD at most analyzed skeletal sites (total body, hip, femoral neck, arm, leg, and head) (standard β range: −0.093 to −0.232, P < 0.05). These findings were unaffected by using BF% in place of FM (standard β range: −0.100 to −0.232, P < 0.05). Conclusions In this cohort of non-obese postmenopausal women over the age of 60 from China, total FM was positively associated with BMD, while AOI was negatively correlated with BMD. As such, a combination of proper weight gain and the control of central obesity may benefit the overall bone health of women after menopause.

Abstract:BACKGROUND The widely observed association between birth size and risk of later coronary heart disease (CHD) has not been examined in an impoverished pre-20th century birth cohort. METHODS Birth weights and maternal characteristics, for births between 1857 and 1900 in a charity hospital, were recorded from preserved ledgers. Names were linked to death certificates to determine age and cause of death. Death with CHD was coded using specific criteria, and survival analysis methods were used to relate risk of CHD to birth weight, allowing for competing causes of death and adjusting for potentially confounding maternal factors. RESULTS Death certificates were traced for 8,584 (53%) of 16,272 registered live-births. Survival analyses were confined to 2,938 subjects (1,572 male, 1,366 female) who survived beyond age 40, since none of the 486 CHD cases was recorded earlier. CHD risk increased with time, but there was no evidence that it was related to birth weight, in men or women. CONCLUSIONS We did not replicate findings in more recent cohorts. This may represent a true lack of association in a historical cohort who we believe remained impoverished through their early life. However, we acknowledge the strong possibility of misclassification of cause of death by the person filling in the death certificate and/or our coding criteria, and temporal change in diagnostic criteria for CHD. We cannot exclude the possibility that low birth weight babies 'programmed' in utero for later CHD were more likely to die in infancy, in this cohort with a high infant mortality rate.

Abstract:We examined ethnic difference in the association of body fat and trunk fat with bone mineral density (BMD) among Chinese, white, and black subjects. We found that, with greater body and trunk fat, both white and black subjects were more likely to have a low BMD than Chinese subjects.

Abstract:BACKGROUND CONTEXT: Previous studies have reported conflicting results for the relationships between anthropometric adiposity indexes and bone mineral density, based on dual-energy X-ray absorptiometry (DXA). However, few studies were published based on quantitative computed tomography (QCT), especially for Chinese population. PURPOSE: To evaluate the associations of spine bone mineral density (BMD) with body mass index (BMI), waist circumstance (WC), waist-to-hip ratio (WHR), waist-to-height ratio (WHtR) and a body shape index (ABSI) using QCT. STUDY DESIGN/SETTING: A Cross-sectional study. PATIENT SAMPLE: Around 3,457 participants in multiple communities across 7 administrative regions of China. OUTCOME MEASURES: Spine BMD was measured using QCT, and the classification of osteoporosis was defined as follows: 1) osteoporosis if BMD <80mg/cm(3), 2) osteopenia if BMD 80 -119 mg/cm(3), and 3) normal bone mass if BMD >= 120 mg/cm(3). METHODS: This study was conducted using convenient sampling between 2013 and 2017. Multivariable linear regression model and logistic regression models were used for the associations of continuous and categorical BMD, respectively. RESULTS: Around 3,405 participants were included in the final analyses, including 1,272 males and 2,133 females, with spine BMD of 111.00 +/- 35.47 mg/cm(3) and 99.38 +/- 40.60 mg/cm(3 ), respectively. Spine BMD decreased significantly with the increase of ABSI in females (adjusted beta, -5.74; 95% confidence interval [CI], -8.50 to -2.98), and this trend also was kept in females aged at less than 60 years (adjusted beta, -14.54; 95% CI, -20.40 to -8.68), and females with age >60 years (adjusted beta, -7.59; 95% CI, -10.91 to -4.28). However, this inverse association was observed only in males with age >= 60 years (adjusted beta, -5.19; 95% CI, -10.08 to -0.29). Except ABSI, negative associations of Spine BMD with WC (adjusted beta, -0.46; 95% CI, -0.77 to -0.15), WHR (adjusted beta, -6.25; 95% CI, -10.63 to -1.86), WHtR (adjusted beta, -6.80; 95% CI, -11.63 to -1.97) were shown in females aged at <60 years, and positive association with BMI in males with age >= 60 years (adjusted beta, 0.92; 95% CI, 0.29-1.55). Age and Gender Differences in Vertebral Bone Marrow Adipose Tissue and Bone Mineral Density, Based on MRI and Quantitative CT

Abstract:Purpose: To investigate the age and gender differences in vertebral bone marrow adipose tissue (BMAT) and volumetric bone mineral density (vBMD).Method: A total of 427 healthy adults, including 175 males (41 %) and 252 females (59 %) with an age range of 21-82 years, underwent MRI and quantitative CT examinations of the lumbar spine (L2-L4), and the corresponding BMAT and vBMD values were measured. The age-related progressions of BMAT and vBMD in men and women were evaluated and compared.Results: In males, vertebral BMAT rose gradually throughout life, while in females, BMAT increased sharply between 41 and 60 years of age. In participants aged < 40 years, BMAT was greater in males compared to females (p <= 0.01), while after the age of 60, BMAT was higher in females (p < 0.05). In males, vBMD decreased gradually with age, while in females, there was a sharp decrease in vBMD after the age of 40 years. At age of 31-40 years, vBMD was higher in females (P < 0.002), while at age > 60 years, vBMD was higher in males (61-70 years, P < 0.01; > 70 years, P = 0.02).Conclusions: We found significant age and gender differences in lumbar BMAT and vBMD. These findings will help to improve our understanding of the interaction between bone marrow fat content and bone mineral density in the ageing process.

Abstract:Background: Bone mineral acquisition during adolescence is crucial for maximizing peak bone mass. Fat mass (FM) and bone mass are closely related. This study investigated the association of FM distribution with bone mass in Chinese male adolescents. Method: A total of 693 male adolescents aged 10–18 years were recruited from a secondary school in Jiangmen, China. Their bone mass and body composition were measured by quantitative ultrasound and bioelectrical impedance analysis, respectively. The associations of the measures of fat distribution with bone parameters, i.e., broadband ultrasound attenuation, speed of sound (SOS), and stiffness index (SI), were analyzed using multiple linear regression. Age, height, body mass index, stage of puberty, physical activity, sedentary behavior, dietary energy intake, and dietary calcium and vitamin D intake were adjusted in the model. Further subgroup analyses of prepubertal and pubertal participants were conducted. Results: The measures of fat distribution showed negative associations with SOS and SI in total subjects (p < 0.010). In prepubertal boys, the measures of fat distribution were only associated with SOS (β = −0.377 to −0.393, p < 0.050). In pubertal boys, the measures of fat distribution had associations with all bone parameters (β = −0.205 to −0.584, p < 0.050). The strongest association was between trunk FM and SOS (β = −0.584, p < 0.001). Conclusion: This study supported that the measures of fat distribution were negatively associated with bone parameters in Chinese male adolescents. Trunk FM had the strongest association with bone parameter. These associations appear to be stronger in pubertal boys than in prepubertal boys.

Contributions of fat mass and fat distribution to hip bone strength in healthy postmenopausal Chinese women

Abstract:The fat and bone connection is complicated, and the effect of adipose tissue on hip bone strength remains unclear. The aim of this study was to clarify the relative contribution of body fat accumulation and fat distribution to the determination of proximal femur strength in healthy postmenopausal Chinese women. This cross-sectional study enrolled 528 healthy postmenopausal women without medication history or known diseases. Total lean mass (LM), appendicular LM (ALM), percentage of lean mass (PLM), total fat mass (FM), appendicular FM (AFM), percentage of body fat (PBF), android and gynoid fat amount, android-to-gynoid fat ratio (AOI), bone mineral density (BMD), and proximal femur geometry were measured by dual energy X-ray absorptiometry. Hip structure analysis was used to compute some variables as geometric strength-related parameters by analyzing the images of the hip generated from DXA scans. Correlation analyses among anthropometrics, variables of body composition and bone mass, and geometric indices of hip bone strength were performed with stepwise linear regression analyses as well as Pearson's correlation analysis. In univariate analysis, there were significantly inverse correlations between age, years since menopause (YSM), hip BMD, and hip geometric parameters. Bone data were positively related to height, body weight, LM, ALM, FM, AFM, and PBF but negatively related to AOI and amount of android fat (all P < 0.05). AFM and AOI were significantly related to most anthropometric parameters. AFM was positively associated with height, body weight, and BMI. AFM was negatively associated with age and YSM. AOI was negatively associated with height, body weight, and BMI. AOI positively associated with age and YSM. LM, ALM, and FM had a positive relationship with anthropometric parameters (P < 0.05 for all). PLM had a negative relationship with those parameters. The correlation between LM, ALM, FM, PLM, ALM, age, and YSM was not significant. In multivariate linear regression analysis, the hip bone strength was observed to have a consistent and unchanged positive association with AFM and a negative association with AOI, whereas its association with other variables of body composition was not significant after adjusting for age, years since menopause, height, body weight, and BMI. AFM may be a positively protective effect for hip bone strength while AOI, rather than android fat, shows a strong negative association with hip bone strength after making an adjustment for confounders (age, YSM, height, body weight, and BMI) in healthy postmenopausal Chinese women. Rational weight control and AOI reduction during menopause may have vital clinical significance in decreasing postmenopausal osteoporosis.

Abstract:Objective To investigate the effect of the contents of body fat mass and lean mass on bone mineral density(BMD) in healthy women.Methods Four hundred and fourteen premenopausal women and 1020 postmenopausal women were enrolled in this study.BMDs of the lumbar spine,the left proximal femur,and whole bones were measured using a dual-energy X-ray absorptiometry(DEXA).Fat mass and lean mass were also determined.Results ①Fat mass and lean mass were significantly negative-correlated in all sites.② Lean mass was a significant determinant of BMDs of the lumbar spine,the left proximal femur,and whole bones in premenopausal women.③Fat mass contributed most to BMD in postmenopausal women.Conclusion Fat mass and lean mass have different roles in BMD.

Abstract:Objective To investigate the effects of abdominal obesity on bone quality and bone mass. Methods A total of 466 participants were enrolled from July 2013 to April 2016. They were divided into 4 groups:male abdominal obesity group ( n=136, waist circumference≥90 cm, aged≥40 years) , male control group ( n=129) , female abdomi-nal obesity group (n=95, waist circumference≥85 cm, postmenopausal women) and female control group (n=106). Secondary osteoporosis was excluded. Bone mineral density ( BMD) of the femoral neck, total hip, and lumbar spine was measured by dual-energy x-ray absorptiometry. Lumbar spine trabecular bone score ( TBS ) was measured by the TBS iNsight software. Height, weight, waist circumference, hip circumference, waist-to-hip ratio, and body mass index were calculated. The differences of TBS and BMD between abdominal obesity groups and their control groups were analyzed. Results 1. BMD of the femoral neck and total hip was higher in male abdominal obesity group than in male control group [ (0. 79±0. 11) g/cm2 vs (0. 76±0. 12) g/cm2, (0. 94±0. 14) g/cm2 vs. (0. 90±0. 12) g/cm2, t=-2. 021,-2. 087, P=0. 004, 0. 038] , while the lumbar spine BMD was unchanged ( t=-0. 703, P=0. 483) . The lumbar spine TBS was significantly lower in male abdominal obesity group, compared with male control group ( 1. 27 ± 0. 09 vs 1. 33 ± 0. 07, t=5. 675, P<0. 001) . 2. Compared with female control group, the lumbar spine TBS was significantly lower in fe-male abdominal obesity group (1. 22±0. 09 vs 1. 27±0. 09, t=4. 154, P<0. 001), whereas BMD of the femoral neck, total hip and lumbar spine was unchanged (t=-1. 209, -1. 367, -0. 550, P=0. 228, 0. 173, 0. 583). Conclusion The effect of abdominal obesity on bone mass varies by sex and the body parts, whereas abdominal obesity has a negative effect on bone quality in men aged ≥40 years and postmenopausal women.

Abstract:I will speak of Sam for his remarkable intellectual contribution to what, I believe, was one of the rst successful interdisciplinary long-term studies conducted at the National Institutes of Health (NIH) between 1955 and 1966 and I will speak about Sam the man. In 1955 we began to plan the National Institute of Mental Health (NIMH) Human Aging Study which, along with similar work at Duke University, helped shape modern gerontology. Sam’s role was critical and he served as one of the ve principal investigators representing di erent elds. The use of statistical methods is essential to a multidisciplinary study of this kind. If the statisticians are an integral part of the research team, they are also in the position to contribute to the project beyond mere experimental design and analysis of data. This Sam most certainly did as an active collaborator. Since the major objective of the study was to determine relationships among variables studied by di erent disciplines, the fact that investigators should not have access to, or discuss, each other’s data when they were being collected and encoded after collection, Sam had one clerk in his statistics section assigned the task of keeping a central data le. Over 600 characteristics were measured and observed on each of the subjects who volunteered for the study. The IBM 650 computer library program available in the Fall of 1957, dictated that the largest block contain no more than 49 variables. Sixteen blocks representing experimental psychiatry, clinical psychology, social and environmental measures and cerebral circulation and metabolism were submitted for computation of their intrablock correlations. Block size ranged from 5 to 49 variables. A problem arises in the analysis of categorical data, as commonly found in the clinical and behavioural sciences, in relating such data to each other and to continuous variables. This is a problem to which Sam certainly did not present any clear-cut solution. No one could. But he provided innovative solutions. Sam noted, ‘This study was initiated by de nite questions and hypotheses both interand intra-disciplinary in nature.’ This was not a ‘ shing expedition.’
Relationship of Body Surface Area with Bone Density and Its Risk of Osteoporosis at Various Skeletal Regions in Women of Mainland China

Abstract:The aim of this study was to investigate the relationship between body surface area (BS) and bone mineral density (BMD) and the associated osteoporosis risk at various skeletal regions in women from mainland China. BMD was measured at the posteroanterior (PA) spine (L1–L4), supine lateral spine (L2–L4) including volumetric BMD (vBMD), hip including femoral neck, trochanter and total hip, and forearm, including radius+ulna ultradistal (R+UUD), 1/3 site (R+U1/3) and total region (R+UT) using a dual-energy X-ray absorptiometry (DXA) fan-beam bone densitometer (Hologic QDR 4500A) in 3418 females aged from 18 to 75 years. Data analysis revealed a positive correlation between BS and BMD at the various skeletal regions (r=0.114–0.373, all P=0.000), but no correlation with vBMD (r=0.000, P=0.934). Using the stepwise regression model, BMDs at various skeletal regions were dependent variables while height, weight, body mass index (BMI), BS and projective bone area (BA) were independent variables; BS was determined to be the most important variable that affected the PA spine, hip and forearm BMDs. Subjects were divided into three groups according to size: large BS group (LBSG), intermediate BS group (IBSG) and small BS group (SBSG). The BMD at different skeletal regions of subjects between groups exhibited a significant gradient difference, with LBSG>IBSG>SBSG, but this was not seen for vBMD. On the fitting curves where BMD varied with age at the PA spine, femoral neck, total hip and R+UUD, BMDs of LBSG were 6.93–9.29% higher than those of IBSG and 12.1–16.9 % higher than those of SBSG, whereas those of SBSG were 6.12–9.59% lower than those of IBSG at various skeletal regions, respectively. The prevalence rates and risks of osteoporosis of LBSG were significantly lower than those of SBSG and IBSG, whereas those of IBSG were obviously lower than those of SBSG at various skeletal regions, respectively, presenting a gradient difference among the three study groups, LBSG<IBSG<SBSG. Our study shows that the relationship between BS and BMD exceeds that between BMD and height or weight in women in mainland China. When areal BMD is employed, those with a larger BS have higher areal BMD and lower risks of osteoporosis while, conversely, those with a smaller BS have lower areal BMD, and therefore higher risk for osteoporosis. However, when vBMD is used, these differences diminish or even disappear.
Association between Body Composition and Bone Mineral Density in Children and Adolescents: A Systematic Review and Meta-Analysis

Abstract:Background: Bone mass acquisition during growth is a major determinant of the risk of developing osteoporosis later in life. Body composition is an anthropometric determinant of bone mineral density (BMD) and significantly influences its development during childhood and adolescence. Objective: This study aimed to systematically examine the association between body composition and bone mineral density in children and adolescents. Methods: Observational studies addressing this association were identified from PubMed (MEDLINE), Embase, Scopus and the Cochrane Library (up to January 2021). The study populations consisted of healthy children and adolescents. The DerSimonian and Laird method was used to compute pooled estimates of effect size and the respective 95% confidence intervals for upper limbs, femoral neck (FN), lumbar spine (LS) and total body, respectively. Subgroup analyses were further performed based on age, sex and ethnicity. Results: Thirty-one published studies were eligible for inclusion in this systematic review and meta-analysis, including three longitudinal studies. The combined population from all the studies amounted to 21,393 (11,205 males and 10,188 females). The pooled estimates of the correlation coefficients for lean mass (LM) and BMD ranged from 0.53 to 0.74 (p < 0.050), and the pooled regression coefficients ranged from 0.23 to 0.79 for FN, LS and total body (p < 0.050). For fat mass (FM), the pooled correlation coefficients ranged from 0.10 to 0.50 (p < 0.050) and the pooled regression coefficient was only significant for FN BMD with a weak strength (pooled β = 0.07, p < 0.050). The pooled regression coefficients for body fat percentage (BF%) were between −0.54 and −0.04 (p < 0.050). The subgroup analysis revealed a stronger association in Asians than in Caucasians for LM and in males compared to females for BF% (p < 0.050). Conclusions: This systematic review and meta-analysis supports a positive association between LM and BMD. BF% appears to have a deleterious effect on bone acquisition in children and adolescents.

Abstract:BACKGROUND AND OBJECTIVES:To examine the associations of regional body fat distribution with metabolic risk factors among Chinese.METHODS AND STUDY DESIGN:Truncal fat (TF) and leg fat (LF) were measured by dualenergy X-ray absorptiometry (DXA) among 947 adults, and abdominal visceral fat (VAT) and subcutaneous fat (SAT), upper leg SAT were measured by magnetic resonance image (MRI) among 103 adults during 2008-2013. Metabolic risk factors included fasting blood glucose, total triglyceride, total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, and metabolic syndrome (MetS).RESULTS:TF showed adverse effect while LF showed beneficial effect on metabolic risk factors, and all these effects were independent of body mass index (BMI) (mostly p<0.01). Individuals with higher TF and lower LF experienced the highest risk of MetS compared to other subgroups of combination of TF and LF (all p<0.05). Abdominal VAT was positively associated with risk of MetS (men: odds ratio (OR)=4.45, 95% confidence interval (CI): 1.18, 16.8; women: OR=6.54, 95% CI: 1.08, 39.6) and serum triglyceride (men: beta (β)=0.379, 95% CI: 0.090, 0.667; women: β=0.700, 95% CI: 0.327, 1.07). Upper leg SAT showed an opposite association with most metabolic factors compared to abdominal SAT and VAT, however, the association was not statistically significant.CONCLUSION:TF and LF showed opposite effects on metabolic risk factors among Chinese adults. Abdominal VAT, but not abdominal SAT, was positively associated with serum triglyceride and risk of MetS. Future studies are warranted to examine the potential mechanism of the opposite effects between TF and LF on metabolic risk factors among Chinese.

Abstract:Low bone mineral density (BMD) is an important risk factor for osteoporotic fractures. The impact of gynecological history on BMD is of great concern, but the results are largely inconsistent. In this study, we investigated the association of gynecological history with BMD in 214 postmenopausal women (60.4 +/- 5.7 yr), as well as with peak bone density (PBD) in 428 premenopausal women (30.8 +/- 5.3 yr) from Shanghai City in China. BMD was measured at lumbar spine (L1-4) and total hip by dual energy X-ray absorptiometry. Raw BMD values were adjusted by age, age2, height, and weight. In the postmenopausal group, more parity had significantly detrimental effects on BMD at both the spine and hip (p < 0.01). The age of the first delivery, the duration of lactation, and the age at menarche did not show significant impacts on BMD (p > 0.05). More years since menopause only had marginally significant decreasing effects at the spine (p = 0.09), but not at the hip (p > 0.10). In the premenopausal group, none of the three reproductive factors had significant impact on PBD (p > 0.05); the age of menarche inversely affected PBD at both the spine (p < 0.01) and hip (p < 0.05). Our results suggest that some gynecological events might influence BMD variation in healthy Chinese women.
Nonlinear relationship between cardiometabolic index and bone mineral density in U.S. adults: the mediating role of percent body fat

Abstract:Background: Both lipid metabolism and obesity are crucial factors in osteoporosis, influencing the relevance of the cardiometabolic index (CMI), a new body fat index incorporating obesity and lipid metrics. Our study aims to explore the relationship between CMI and lumbar spine bone mineral density (BMD) and the mediating role of body fat percentage. Methods: Utilizing the National Health and Nutrition Examination Survey (NHANES) data from 2011 to 2018, we conducted a cross-sectional analysis. We employed multiple linear regression models, subgroup analyses, generalized additive models (GAM), smooth curve fitting, and mediation analysis to evaluate the linear and nonlinear relationships between CMI and lumbar spine BMD. Results: The study involved 5,124 participants with an average lumbar spine BMD of 1.03 ± 0.15 g/cm 2 . We identified a negative correlation between CMI and lumbar spine BMD (β = -0.015; 95% CI: -0.023, -0.008). Nonlinear associations were evident, with inflection points at CMI values of 1.12 and 2.86. Subgroup analyses showed consistent negative correlations across all categories without significant differences (p for interaction > 0.05). Moreover, body fat percentage negatively correlated with BMD (β = -0.005; 95% CI: -0.006, -0.004) and mediated 9.41% of the relationship between CMI and BMD. Conclusion: Increased CMI levels are associated with lower lumbar spine BMD, with body fat percentage significantly mediating this relationship. This underscores the importance of managing body composition in the context of bone health, highlighting CMI's potential utility in osteoporosis risk assessment.
