Abstract:Objective To evaluate the clinical effective of radical retropubic prostatectomy and study the methods of decreasing its postoperative complications. Methods Clinical data of 53 cases who had undergone radical retropubic prostatectomy from June 2000 to April 2006 were retrospectively analyzed.neurovascular bundles were preserved during the prostatectomy in 9 of the 53 cases. Results Mean operating time was 140 minutes,and the average blood loss was 700ml.There were no operative deaths.3 patients were complicated with stress incontinence and no patients were with totally incontinence. Conclusion Radical retropubic prostatectomy is a safe and effective method for treating prostatic carcinoma,and the nerve-sparing operation can preserve the erectile function of the patients satisfactorily.

Abstract:Ductal adenocarcinoma of the prostate (DAP) is a rare variant of prostate cancer, and has poorly-identified clinical characteristics. Few cases have been previously reported in Chinese males and DAP is more commonly reported in Caucasian males over 70-years of age. In the present study, a 55-year-old Chinese male patient that demonstrated typical lower urinary tract symptoms and normal prostatic-specific antigen levels underwent a transurethral resection of the prostate. The patient was subsequently diagnosed with DAP. A total pelvis exenteration was then conducted successfully. During the 40-month follow-up, the patient remained progression-free. A review of the literature was conducted in order to assess the clinical course, diagnosis, prognosis and optimal management for DAP. The review indicated that DAP is more likely to demonstrate an aggressive clinical course and an unfavorable prognosis; therefore, once the diagnosis has been confirmed, an aggressive management strategy is recommended for the patient, even in the case of metastatic disease.

Abstract:Objective To determine the chinical characteristics,pathologic features,principles of diagnosis and treatment of ductal adenocarcinoma of the prostate( DAP).Methods 8 patients diagnosed with ductal adenocarcinoma of the prostate between July 2008 and July 2012 in Nanjing Drum Tower Hospital were retrospectively analyzed.Results Eight patients' age ranged from 53 to 86-year-old with a median age at 70.25year-old.All cases were confirmed by pathological examination.Among the eight patients,three underwent radical prostatectomy,three received transurethral resection of the prostate associated with androgen deprivation,one received hermone therapy solely,one underwent prostate brachytherapy.Seven of eight patients had been followed up which lasted from 14 months to 4 years.One patient developed multiple liver and bone metastases and died shortly after.The other 6 patients didn't show any evidence of local recurrence or distant metastasis.Conclusion DAP is very difficult to diagnosis during the early stage,and tends to pursue more aggressive clinical course and poorer prognosis.Once the diagnosis is determined,local radical therapies should be recommended.

Abstract:Objective: To evaluate the clinical and pathological characteristics and treatment of ductal adenocarcinoma of the prostate. Methods: A case of ductal adenocarcinoma of the prostate gland was reported, and the relevant literature was reviewed to investigated the diagnosis and treatment of this disease. A 59-year-old male suffered from hemospermia and hematuria. A soft nodule in the right lobe of the prostate was found by digital rectal examination (DRE). His serum prostatic specific antigen (PSA) level was 0.7ng/ml. The prostate biopsy result indicated prostate cancer and radical prostatectomy was performed on him. Results: The postoperative pathology showed a primary ductal adenocarcinoma in the right lobe of prostate, which infiltrated the right vesicle. Gleason score was 5+4 (Sum=9) and the pathologic stage was pT3b. With immunohistochemical staining, the tumor cells were positive for PSA and prostatic acid phosphatase, but negative for androgen receptor. Postoperatively, the patient recovered well with no hemospermia or hematuria. During a follow-up of 7 years, the patient was alive. Conclusion: Ductal adenocarcinoma of the prostate is a clinically rare subtype of prostate cancer and difficult to be found in early stage. The diagnosis depends on histopathologic and immunohistochemical studies. Radical prostatectomy is recommended for this disease.

Abstract:Objective To discuss the pathological and clinical features, diagnosis and treatment of prostatic duct adenocareinoma. Methods The clinical data of nine cases of prostatic duct adeno-carcinoma were retrospectively analyzed, with the average age of 76 (59-106) years. Six cases were presented with dysuresia and/or nocturia, and two of them had the painless gross hematuria. Two pa-tients presented painless gross hematuria as the first symptom. One case was detected the elevated ser-um PSA in a routine healthy examination. Radical prostatectomy (RP) was performed in 1 case;RP and bilateral orchidectomy and external beam radiotherapy (EBRT) were performed in 1 case;5 cases underwent transurethral resection of the prostate (TURP) combined with photoselective vaporization of the prostate (PVP) by green laser and bilateral orchidectomy;1 case underwent TURP combined with PVP;1 case underwent bilateral orchidectomy combined with EBRT. Eight cases took flutamide for 3-45 months. All patients were followed-up according to the scheduled time. Results The op-erations were successfully performed in all 9 patients. The papillary or cauliflower-like tumors infiltra-ted colli culus seminalis and prostate duct nearby. The glands were coated with tall pseudostratified columnar cells. The nuclei were large, dark stained with more frequent mitoses. The positive rates of immunolabelling antibody PSA, AR, PAP were found to be 89%(8/9), 100%(5/5), 100%(5/5) re-spectively. The distribution of Gleason score was 6-7(3 cases), and≥8(6 cases), and a coexisting acinar carcinoma component was identified in 5 cases of the group. Nine cases had a mean follow up for 20(3-48) months. Five cases have developed biochemical recurrence, of whom 3 died of bone metas-tasis and multiple organ failure, and 1 developed lung and bone metastasis. Three cases remained alive without recurrence. The remaining 1 case survives during the follow-up survey for 6 months until now, without examinations due to the old age. Conclusions Duct adenocarcinoma of the prostate presents the low incidence and lacks of typical symptoms in the early stage. Diagnosis was confirmed mainly on the basis of pathology. The tumors tend to have a more advanced stage and a very short term survival rate. The treatment options and management are similar to that of high-grade adenocar-cinoma of the prostate;meanwhile, close follow-up survey should be performed.
Adjuvant Hormone Therapy after Radical Prostatectomy in High-Risk Localized and Locally Advanced Prostate Cancer: First Multicenter, Observational Study in China

Abstract:Objective Potential of combined androgen blockade (CAB) has not been explored extensively in Chinese males with prostate cancer (PCa). Therefore, this study evaluated the 2-year prostate-specific antigen (PSA) recurrence rate and quality of life (QoL) in patients with high-risk localized and locally advanced PCa receiving adjuvant hormone therapy (HT) after radical prostatectomy (RP). Methods This prospective, multicenter, observational study conducted in 18 centers across China enrolled patients with high-risk factor (preoperative PSA>20 ng/mL or Gleason score >7) or locally advanced PCa. Different adjuvant HT were administered after RP according to investigator’s decision in routine clinical practice. Relationship of baseline and postoperative characteristics was assessed with recurrence rate. PSA recurrence rate and Functional Assessment of Cancer Therapy-Prostate (FACT-P) QoL scores were recorded at 12 months and 24 months. Kaplan-Meier analysis was used to construct the PSA recurrence rate during follow-up. Results A total of 189 patients (mean age: 66.9±6.5 years) were recruited, among which 112 (59.3%) patients showed serum PSA>20 ng/mL preoperatively. The highest postoperative pathological advancement noticed was from clinical T2 (cT2) to pathological T3 (pT3) (43.9%) stage. The majority of the patients (66.1%) received CAB as adjuvant HT, for a median duration of 20.0 months. The least recurrence (15.2%) was noticed in patients treated with CAB, followed by those treated with luteinizing hormone-releasing hormone agonist (LHRHa) (16.1%), and antiandrogen (19.0%), with non-significant difference noted among the groups. None of the baseline or postoperative characteristics was related with PSA recurrence in our study. The 24-month FACT-P QoL score of 119 patients treated for >12 months showed significant improvement above baseline compared with those treated for ≤12 months. Conclusions Adjuvant CAB therapy after RP showed reduction trend in 2-year PSA recurrence rate in high-risk Chinese patients with localized and locally advanced PCa, compared with adjuvant anti-androgens (AA) or LHRHa therapy. Further long-term therapy (>12 months) significantly improved QoL compared to short-term HT therapy, suggesting the beneficial effect of long-term CAB therapy in improving QoL.

Abstract:Objective To improve the diagnosis and treatment of advanced prostatic cancer.Methods Eighty-four cases of late-stage prostatic cancer were reviewed in this study.All the patients underwent bilateral orchiectomy and endocrinal therapy(flutamide);22 cases with severe ureteric obstruction were treated by transurethral resection of prostate;3 cases underwent radical protatetomy;3 cases were treated by interventional chemotherapy.Results 78 cases were followed up for 3-24 months.46 cases were followed up for 5 years.The subjective symptoms and objective signs were improved;the obstruction of bladder outlet and bone pain were relieved to different extent;PSA decreased and the life quality was improved apparently.Conclusion Bilateral orchiectomy and endocrinal therapy were effective to inhibit the growth of cancer.TURP can relieve bladder outlet obstruction.
Experience of the Robot-Assisted Laparoscopic Radical Prostatectomy via Completely Posterior and Intrafascial Approach: A Report of 5 Cases

Abstract:Objective To briefly introduce the initial experience of the robot-assisted laparoscopic radical prostatectomy (RALP) by completely posterior and intrafascial approach.Methods By retrospectively reviewing the clinical data of the 5 cases who received RALP by completely posterior and intrafascial approach between August 2016 and October 2016.Results All these 5 RALP by completely posterior and intrafascial approach were successfully completed.The operation time was between 124 and 155 minutes [(139.8±13.9) minutes on average].The estimated intraoperative blood loss was between 30 to 80 ml [(52.0±19.2) ml on average].No case with positive surgical margin was found.The serum PSA levels decreased to the criteria of radical prostatectomy 6 weeks postoperatively.Three patients were able to fully control their urination, with no need for diapers, and the other two regained full urinary continence within 1 month after surgery.Two patients recovered their erectile function within 1 month after surgery, while the other 3 patients didn't report erection function recover.Conclusions The tumor control, the postoperative urinary continence, and the erectile function recovery of the RALP by completely posterior and intrafascial approach are relatively satisfactory.It is feasible to perform this approach for selected suitable patients.

Abstract:Objective To evaluate laparoscopic radical prostatectomy for prostate cancer. Methods 8 patients presented clinical stages pT 1b to pT 2 prostate cancer.Laparoscopic radical prostatecomy was carried out transperitoneally with combining posterior and anterior approachs to the prostate,transecting the bladder neck,lateral dissection of the prostate and urethrovesical anastomosis. Results The operation time was 5 to 11 h with an average of 7.3 h and the blood loss 200 to 1 100 ml,averaged 620 ml.All the patients recovered well and uneventful with no complications such as urethral stricture or incontinence. Conclusions Laparoscopic radical prostatectomy is a better approach and least invasive.The procedure provides clear anatomic vision that facilitates operative performance and quicker recovery.
Radical Prostatectomy Plus Extended Lymph Node Dissection for Locally Advanced Prostate Cancer: 12-Year Experience of a Single Centre

Abstract:Objective To evaluate the functional and oncological outcomes of patients with locally advanced prostate cancer (PCa) treated by hormone therapy combined with extra-fascia1 laparoscopic radical prostatectomy (LRP) plus extended lymph node dissection (ePLND).Methods From January 2004 to June 2016,a total of 255 PCa cases (pT3-4NxM0) who received LRP plus ePLND were enrolled into our study.The mean age of the patients was 67 (range 44-88) years,and median PSA level was 21.2 (range 0.6-454.0) ng/ml.The patients were divided into earlier group (from January 2004 to December 2011,160 cases) and later group (from January 2012 to June 2016,95 cases) according to different treatment periods.The baseline demographics between the two groups were similar.All patients routinely received adjuvant hormone therapy (AHT) postoperatively.The patients in the later group underwent collapsin response mediator protein 4 (CRMP4) methylation study on the prostatic biopsy preoperatively.Those with a CRMP4 methylation level ＞ 15％ or rectum/bladder neck invasion,were treated by neoadjuvant hormone therapy (NHT) for 3-6 months.Positive surgical margin (PSM),progression-free survival (PFS),cancer-specific survival (CSS),overall survival (OS) and postoperative continence rates between the two groups were analyzed and compared.Results The mean operative time of the earlier and later group were (239 ±65) min and (203±51) min,mean blood loss were (109-±65) ml and (96-±44) ml,mean dissected nodes were (19 ± 5) and (21 ± 7),respectively (all P ＞ 0.05).The total PSM rate was 19.2％,and PSM rates of the two groups were 23.1％ and 12.6％ (P =0.04).All the 255 cases received AHT and 25 cases in the later group underwent NHT.The median follow-up time was 73 months (range 10-152 months).The total 5-year PFS,CSS and OS rates were 77.7％,94.3％ and 87.1％,respectively,and the rates between groups were 73.8％ vs.86.1％ (P=0.03),93.1％ vs.98.6％ (P=0.07),and 85.0％ vs.92.8％ (P =0.11),respectively.The 1-year postoperative continence rates were 91.9％ vs.97.9％ (P =0.09).Conclusions Hormone therapy combined with LRP plus ePLND represents an oncological and functional effective option in patients with locally advanced PCa,and improved PFS might be acquired by preoperative tumor staging.

Abstract:In the western world, adenocarcinoma of the prostate is the most common malignant neoplasm of human males. In recent years, the incidence of the disease has increased dramatically in China. Surgery is an important treatment for prostate cancer. This article reviews the progress in radical retropubic and perineal prostatectomy, standard laparoscopic and robot-assisted laparoscopic radical prostatectomy. It covers the necessity, techniques and experience of surgery, nerve preserving techniques and erectile dysfunction, complications and outcomes, advantages and disadvantages of and comparison between various surgical approaches.

Abstract:167 Background: Previous studies have indicated that ductal adenocarcinoma of the prostate (DA) is associated with adverse prognosis in patients with localized prostate cancer (PCa). However, the clinical significance of DA in advanced PCa remains largely ambiguous. This study endeavors to investigate the relationship between the existence of DA in prostate biopsy specimens and treatment outcomes among patients with advanced PCa receiving abiraterone acetate (AA) therapy. Methods: We retrospectively analyzed data from 440 patients with advanced PCa who received AA at either the metastatic hormone-sensitive (mHSPC, N=123) or castration-resistant PCa (mCRPC, N=317) stage. The presence of DA and its proportion was evaluated based on prostate biopsy specimens. Kaplan-Meier curves and COX regression were used to evaluate the predictive significance of DA on AA efficacy, including PSA response, PSA progression-free survival (PSA-PFS), and radiographic progression-free survival (rPFS). Results: In aggregate, DA was detected in 35/440 (8.0%) patients, with 13/440 (3.0%) and 22/440 (5.0%) men harboring DA<5% and ≥5%, respectively, in the total tumors. The overall PSA response rate in the entire cohort was 302/440 (68.6%), and it was comparable for people with and without DA, with both groups having a response rate of 68.6%. Of note, compared with patients without DA, men with DA have significantly longer median PSA-PFS (mPSA-PFS, 12.1- vs. 25.0-Mo, P=0.005) and median rPFS (mrPFS, 19.8 vs. 38.2-Mo, P=0.034). Subgroup analysis based on DA proportion further revealed that in comparison with PCa without DA, DA≥5% was an indicator of favorable PSA-PFS (HR, 95%CI: 0.28, 0.14-0.56, p<0.001) and rPFS (HR, 95%CI: 0.35, 0.18-0.72, p=0.004), while DA<5% was not. In multivariate COX regression analysis, after adjusting by clinicopathological factors, including mCRPC/mHSPC stage, ISUP grading, metastatic burden, pain score and pretreatment hemoglobin, alkaline phosphatase, and lactate dehydrogenase, DA≥5% was still independently associated with better therapeutic efficacy of AA treatment (PSA-PFS: HR, 95%CI: 0.36, 0.17-0.74, p<0.001; rPFS: HR, 95%CI: 0.47, 0.23-0.96, p=0.040). Conclusions: A DA proportion ≥5% in the tumor is related to improved treatment efficacy for individuals with advanced PCa receiving AA. Thus, in the pathological diagnosis of PCa, it is necessary to routinely report the presence and proportion of DA in order to aid with treatment decision-making.

Experience of laparoscopic radical prostatectomy for high risk protate cancer

Abstract:Objectives To evaluate the clinical efficiency of laparoscopic radical prostatectomy for high risk prostate cancer.Methods The clinical data of 26 patients with high risk prostate cancer who underwent laparoscopic radical prostatectomy from March 3, 2014 to November 2015 were analyzed retrospectively, with the average age of 65. 2 .In preoperative examination, there were 9 cases with PSA≥20ng/ml only, and 17 cases with PSA≥20ng/ml and Gleason score≥8,meanwhile there is one case in both the T3b phase and T4 phase.Three cases were treated with neo-adjuvant hormone therapy for 3 ~6 months.All patients underwent extraperitoneal laparoscopic radical prostatectomy and pelvic lymph node dissection.Results All operation were successful.The mean operation time was 152min, mean blood loss was 85ml, mean catheterization time were two weeks.Postoperative temporary urinary incontinence oc-curred in 8 cases, and all recoved within 1 week to 3 months.After the operation, there were 7 cases in T2a~T2b phase and Gleason score≤7, 16 cases in T2c~T4 phase and Gleason score≥8.The mean number of lymph node dis-sected was 5.5.Three cases had lymph positive ,four cases hadpositive surgical margins and all underwent radiation therapy after continence was regained.19 cases were followed up for 3~30 months, no true incontinence was observed, and PSA≤0.2ng/ml in all patients.Conclusions As the core of comprehensive treatment strategies, radical pros-tatectomy is safe and effective for high risk prostate cancer .
Clinical Characteristics and Treatment Strategies of Prostatic Mucinous Adenocarcinoma:A Report of 10 Cases and Literature Review

Abstract:OBJECTIVE:To investigate the clinical characteristics and treatment strategies of prostatic mucinous adenocarcinoma (PMAC).METHODS:We retrospectively analyzed the clinical data on 10 cases of PMAC treated in the First Affiliated Hospital of Nanjing Medical University from January 2014 to June 2018. The patients were aged 51－79 (65 ± 14) years, with a medium PSA level of 89 (14.63－128.05) μg/L and Gleason scores of 3 + 3 in 1 case, 3 + 4 in 2, 4 + 3 in 1 and 8 in 6 cases preoperatively, 1 treated by robot-assisted radical prostatectomy and the other 9 by laparoscopic radical prostatectomy. We conducted pelvic cavity lymph node dissection for all the patients and analyzed their prognosis and survival.RESULTS:Operations were successfully completed in all the cases. Pathological examination revealed 2 cases of mucinous adenocarcinoma with signet ring cell carcinoma in the 10 PMAC patients, 2 at stage ≤T2b, 5 at stage ≥T2c, 3 positive at pelvic lymph node dissection and 5 positive at the incision margin. The patients were followed up for 6－48 (median 26) months. Four of the patients were found with biochemical recurrence within 2 years after operation and treated by androgen-deprivation therapy, radiotherapy and chemotherapy, which reduced the PSA level to <1.0 μg/ml in all the 4 cases.CONCLUSIONS:PMAC has a good prognosis. Radical surgery is recommended for moderate and low-risk PMAC and the patients with postoperative biochemical recurrence can benefit from comprehensive treatment of total androgen blockade.

Abstract:OBJECTIVE:To study the technique and clinical outcomes of laparoscopic radical prostatectomy for high risk prostate cancer.METHODS:A total of 65 patients with high risk prostate cancer were treated with surgery in the First Affiliated Hospital of Nanjing Medical University from January 2011 to June 2013. The mean age was 67 years (range 45-75 years). The mean preoperative prostate specific antigen (PSA) level was 26.7 µg/L (range 11.2-65.5 µg/L). The transrectal biopsy revealed Gleason score of 3+3 in 4 patients, Gleason 3+4 in 27 patients, Gleason 4+3 in 11 patients, Gleason 4+4 in 21 patients and Gleason 4+5 in 2 patients. The bone metastasis was excluded by scintigraphy examination. The surgical procedures were performed through transperitoneal approach. Extended pelvic lymph nodes dissection was performed after the removal of the prostate. Adjuvant radiotherapy or hormonal therapy was administrated according to the pathological results. Serum PSA was detected every 1 to 2 month and urinary continence was evaluated every 3 month in the first year, and then serum PSA was detected every 2 to 3 month.RESULTS:The mean operative time was (134±21) minutes and the median blood loss was (300±146) ml. Bladder neck reconstruction was performed in 15 cases. The drainage was removed on postoperative day 4 and the catheter was removed on day 7. Pathologic results demonstrated pT2 in 25 patients, pT3a in 28 patients, pT3b in 9 patients and pT4 in 3 patients. Positive surgical margin was presented in 15 patients. A median of 19 lymph nodes (range 11-24 nodes) were retrieved during lymphadenectomy and 11 patients had lymph nodes metastasis with a total of 19 positive nodes. Forty-three patients recovered continence after the removal of catheter. Eleven patients received adjuvant hormonal therapy and 19 patients received adjuvant radiation therapy. With the median of 20 months follow-up (range 12-30 months), 5 patients got biochemical recurrence.CONCLUSIONS:Laparoscopic radical prostatectomy with extended lymph nodes dissection for high risk prostate cancer is safe and technical feasible. It provides accurate information on tumor stage and grade. It is an important component of multimodality for the treatment of high risk prostate cancer.

Abstract:Introduction: To investigate the clinical features, diagnosis, treatment, and prognosis of ductal adenocarcinoma of the prostate.Methods: The clinicopathological and immunohistochemical data of seven patients with ductal adenocarcinoma of the prostate were retrospectively analyzed. All patients underwent physical examination, magnetic resonance imaging (MRI), bone scan, cystoscopy, and computed tomography (CT) scan. The level of prostate-specific antigen (PSA) before and after surgery was assessed. Different prostate cancer markers were used for immunohistochemical staining.Results: The mean age of the seven patients diagnosed with prostatic ductal adenocarcinoma in this study was 76.2 years (range 57-88). Five patients presented with intermittent and painless gross hematuria, one patient with progressive dysuria, and one patient with elevated serum PSA on routine health examination. The level of PSA before surgery ranged from 1.3 to 45.0 ng/mL. Immunohistochemical staining results of the prostatic ductal adenocarcinoma confirmed positivity for PSA, prostatic acid phosphatase, androgen receptor, and alpha-methyacyl co-enzyme A (CoA)-reductase markers. Two of the patients underwent bilateral orchiectomy combined with anti-androgen therapy, three underwent transurethral resection of prostate, one received radical prostatectomy, and one received medical castration therapy. The clinical outcomes of all patients were satisfactory, based on follow-up data. The symptoms of hematuria and dysuria were ameliorated well, and the postoperative PSA level decreased below 4.0 ng/mL. Recurrence or metastasis of disease was not detected on MRI and bone scan.Conclusion: Ductal adenocarcinoma of the prostate is a rare subtype of prostate carcinoma, the diagnosis of which could be based on pathological and immunohistochemical examination. Earlier management promises better prognosis.

Abstract:Background: Robot-assisted radical prostatectomy (RARP) is increasingly used worldwide, but comparisons of perioperative, functional, and oncologic outcomes among RARP, laparoscopic radical prostatectomy (LRP), and open radical prostatectomy (ORP) remain inconsistent. Material/Methods: Systematic literature searches were conducted using EMBASE, PubMed, the Cochrane Library, CNKI, and Science Direct/Elsevier up to April 2017. A meta-analysis was conducted using Review Manager and Stata software. Results: We included 33 studies. Meta-analysis revealed that blood loss, transfusion rate, and positive surgical margin (PSM) rate were significantly lower following RARP compared with LRP (SMD (95% confidence interval [CI]) 0.31 [0.01, 0.61]; combined ORs (95% CI) 5.32 [1.29, 21.98]; 1.27 [1.10, 1.46]) and ORP (SMD (95% CI) 0.75 [0.30, 1.21]; and combined ORs (95% CI) 3.44 [1.21, 9.79]); positive surgical margin (PSM) rates were significantly lower following RARP compared with LRP (combined ORs (95% CI) 1.27 [1.10, 1.46]), but not ORP. Operation time was also shorter for RARP than for LRP. The rates of nerve-sparing, recovery of complete urinary continence, and recovery of erectile function were significantly higher following RARP compared with LRP (combined ORs (95% CI) 0.55 [0.31, 0.95]; 0.66 [0.55, 0.78]; 0.46 [0.30, 0.71]) and ORP (combined ORs (95% CI) 0.36 [0.21, 0.63]; 0.33 [0.15, 0.74]; 0.65 [0.37, 1.14]). Conclusions: This meta-analysis demonstrates that RARP results in better overall outcomes than LRP and ORP in terms of blood loss, transfusion rate, nerve sparing, urinary continence and erectile dysfunction recovery, and suggests that RARP offers better results than LRP and ORP in treatment of prostate cancer. However, studies with larger sample sizes and long-term results are needed.

Abstract:Prostate cancer (PCa) is a prevalent malignant tumor affecting the male reproductive system and there are mainly three widely accepted PCa surgery types in current clinical treatment: open radical prostatectomy (ORP), laparoscopic radical prostatectomy (LRP) and robot-assisted radical prostatectomy (RARP). Here, we aimed to evaluate the clinical effect of RARP for PCa patients compared with ORP and LRP based on the context of PCa encompass two dimensions: oncological outcomes (biochemical recurrence (BCR) and positive surgical margin (PSM)) and functional outcomes (urinary continence and recovery of erectile function) in this network meta-analysis (NMA). PubMed, Embase and Cochrane databases were systematically searched in January 7, 2024. 4 randomized controlled trials (RCTs) and 72 non-RCTs were included. RARP displayed significant positive effect on lower BCR and better recovery of erectile function but no significant differences existed among three surgery types for PSM and urinary continence.

Abstract:OBJECTIVE:To present the clinical results of 51 patients of clinically localized prostate cancer treated by laparoscopic radical prostatectomy, especially recovery of urinary continence. To analysis how to improve recovery of urinary continence.METHODS:From February 2004 to March 2008, we performed LRP on 51 patients. All patients were diagnosed by pathological result preoperatively. T1a-1b 4 cases (8%), T1c 15 cases (29%), T2a 7 cases (14%), T2b 5 cases (10%), T2c 20 cases (39%).RESULTS:Forty-nine cases were treated by LRP successfully. There were two cases converted to open surgery. Urine leaking happened in 3 cases, but recovered without intervention. Urinary catheter duration was 14 - 45 days (mean 16 days). Six months and 12 months after LRP, incontinence happened 7/39 cases (17.9%) and 5/20 cases (25%) respectively. Complete incontinence happened in one case. The ratio of incontinence happened in the first 20 cases and the following 31 cases were 6/20 (30%) and 7/31 (22%) respectively. There were two cases had rectal injury. Both cases underwent colostomy. Duration of follow up were from 3 - 53 months (mean 17 months). Two cases recurred. One of them was treated with hormone therapy, the other one died of operation for lung metastasis. Other patients' serum total PSA were less than 0.2 microg/L.CONCLUSIONS:Extraperitoneal laparoscopic radical prostatectomy is efficient and safe approach for localized prostate cancer. The correct handling of pubic prostatic ligaments, external urethral sphincter and neurovascular bundle are important for recovery of urinary continence. And so does the experience of LRP.
