Abstract:To the Editor: Patent foramen ovale (PFO) is present in 20% to 35% of the population, and its prevalence varies with age.1 In an old but still valid echocardiographic study PFO incidence in patients aged 70 to 79 was approximately half that of patients aged 40 to 49 but still important (6.15%).2 Moreover, the risk of adverse cerebral events in patients aged 65 and older was significantly higher in the patients with PFO (P=.01, hazard ratio=3.21, 95% confidence interfal (CI)=1.33–7.75; 2-year event rates 37.9% vs 14.5%).3 The increase in life expectancy makes the diagnosis of PFO a possible and not easily manageable event in patients aged 60 and older; the presence of different comorbidities and in particular of diastolic dysfunction could be a serious contraindication to PFO closure. We propose a simple algorythm for managing elderly patients with PFO based on a few considerations. First, elderly patients cannot be excluded from PFO closure a priori based on age. Second, medium-to-large PFOs, especially if associated with a large persistent Eustachian valve or Chiari network, can frequently embolize and cause recurrent stroke in patients aged 60 and older; moreover, in elderly people, PFO may be the cause of other syndromes, including unexplained hypoxemia, hypoxemia after lung surgery, and hypoxemia associated with sleep apnea syndrome, which can all benefit from transcathter closure.4,5 Third, careful evaluation of carotid and aortic atherosclerotic plaques, and especially of diastolic dysfunction, should be employed before any closure attempt; soft carotid plaques or Montgomery Class III or greater aortic plaques or Class III or greater diastolic dysfunction should be considered as absolute contraindications. Occlusion test with a sizing balloon without worsening of left ventricle function suggests that closure in patients with mild diastolic dysfunction can be performed safely. Fourth, elderly patients should be evaluated using an integrated protocol including a stress test to evaluate coronary artery disease. Finally, only selected elderly patients evaluated with similar protocols can proceed safely to PFO closure (Figure 1).
