Abstract:Objectives: To find out the problems on household spare drugs(HSD),and ensure the safety and effectivity using drugs by analyzing the storage of HSD.Method: Employ stratified sampling method to choose samples and,statistical analyzed indicators with SPSS.Results: Most of the households had Chinese medicines,antibiotics,antipyretic and analgesics.Of all the household spare drugs,45.2% and 43.5% were prescription medicines in Shandong and Ningxia province.3.7% and 1.0% of the residents always lend the HSD to the others,12% and 32.7% lend the HSD to the others occationally in Shandong and Ningxia province respectively.Conclusion: Chinese medicines and antibiotics near half of the HSD were prescription medicine and there were high storage rate.The management of HSD should be further improved.

Abstract:Objectives To analyze the medical knowledge,drug purchases and using behavior of rural residents and estimate the problems and influencing factors of drug use by residents.Methods Stratified sampling method was employed to choose samples and SPSS,was used forstatistical analysis.Results Residents in investigated rural areas were lack of drug knowledge.Of the self-medicators,more than 40% used antibiotics,while 38%(Shandong) and 47.8%(Ningxia) used Chinese medicines.Of the patients who used drugs in the four weeks preceding the survey,5.1% and 7.8% always forgot their medication,25.3% and 36.1% occasionally forgot their medication in Shandong and Ningxia respectively.Conclusions The survey suggested the existence of irrational drug use in rural area.Measure should be made to improve the medical knowledge and drug use behavior of rural residents.

Abstract:Objective To learn the status of drug application in village health posts in thirty-eight poverty areas of western china.Methods Five townships were sampled randomly from each county,three villages from each county.In each village health post a survey on sixty prescriptions was conducted.Results The range category of western medicines in each province was 83-202;The proportion of medicine prescribed from the Essential Medicines List in Innore mongolia,Guangxi,Jiangxi were upper,that's 95.24%,87.71% and 80.73%,and that in Ningxia was 34.73%,Qinghai was 53.95%;The proportion of the Essential Medicines List for traditional Chinese medicine in Sichuan and Chongqing were 41.45% and 51.77%;The application rates of anti-infection drugs was the highest of all;Average number of medicines prescribed per patient encounter was 2.52;The rate of three kinds of drugs using together was 25.75%;The use rate of mainline was 3.50%.Conclusion In order to deal with these problems,some suggestions should be made to consolidate and perfect village health posts'function of health care.

Abstract:It is a great challenge to ensure medication safety in healthcare for older adults, since polypharmacy in this group may increase the risk of potentially inappropriate medications, leading to adverse drug reactions, drug-drug interactions, hospitalizations, increased medical costs, and even deaths. Deprescribing improves patient prognosis and reduces adverse drug events through patient participation, and shared decision-making. The prevalence of multimorbidity and polypharmacy in older adults is notably in community care, the frontline for the management of use of multiple drugs. As primary care physicians have a deep understanding of patients' past and current diagnoses and treatments, they play a vital role in reviewing and managing patients' medications. However, there are few reports on deprescribing in primary care in China. We described the definition of deprescribing, and reviewed its recent advances, with a focus on the implementation process of deprescribing in the community, and the impact of deprescribing on chronic disease management by primary care physicians. This review may provide a reference for further study and policy making regarding deprescribing in the community in China.

Abstract:OBJECTIVE: Drug shortages were a complex global problem. The aim of this study was to analyze, characterize, and assess the drug shortages, and identify possible solutions in Shaanxi Province, western China.METHODS: A qualitative methodological approach was conducted during May-June 2015 and December 2015-January 2016. Semi-structured interviews were performed to gather information from representatives of hospital pharmacists, wholesalers, pharmaceutical producers, and local health authorities.RESULTS: Thirty participants took part in the study. Eight traditional Chinese medicines and 87 types of biologicals and chemicals were reported to be in short supply. Most were essential medicines. Five main determinants of drug shortages were detected: too low prices, too low market demands, Good Manufacturing Practice (GMP) issues, materials issues, and approval issues for imported drugs. Five different solutions were proposed by the participants: 1) let the market decide the drug price; 2) establish an information platform; 3) establish a reserve system; 4) enhance the communication among the three parties in the supply chain; and 5) improve hospital inventory management.CONCLUSIONS: Western China was currently experiencing a serious drug shortage. Numerous reasons for the shortage were identified. Most drug shortages in China were currently because of "too low prices." To solve this problem, all of the stakeholders, especially the government, needed to participate in managing the drug shortages.

Abstract:OBJECTIVE: To investigate the utilization status of antibiotics in rural area of Ningbo and to put forward improvement measures.METHODS: The data regarding the utilization of antibiotics were obtained by questionnaire survey and the on-the-spot investigation among the randomly selected 52 township hospitals,451 village clinics,and 117 drugstores in Ningbo.RESULTS: The irrational drug utilization phenomena were widespread in township hospitals,village clinics,and drugstores in Ningbo,which involved technical problems as well as administrative loophole,and manifested as insufficient etiology evidence,nonstandard dosage regimen,unclear indications for preventive medication or combined uses of antibiotics,and lack of effective drug control measures etc.CONCLUSION: The irrational drug utilization phenomena are widespread in rural area of Ningbo,which should be given proper intervention.

Abstract:The current situation of household storage drugs in China was investigated with 6278 questionnaires by random sampling.It was found that 56.2％of surveyed families had reserved drugs,and traditional Chinese medicine,over-the-counter medicine,and drugstore-purchased drug accounted for 64.2％,53.8％,and 79.1％,respectively.Therefore,it might be necessary to strengthen the propaganda and education on rational prescription,preparation,and rational drug use.

Abstract:Objective To provide reference for solving the problem of antitumor drugs shortage by the investigation of medical insti-tutions in Shaanxi province.Methods The status of the antitumor drugs shortage for the medical institutions in Shaanxi province was analyzed by using the questionnaire method,mainly including the different varieties and reasons.Results There were 24 kinds of antitumor drugs which have been in short supply,and the most serious shortage of antitumor drugs was 4 kinds.The shortage of frequency from high to low was:mitomycin,pingyangmycin,fluorouracil and vincristine.There were at least 27 medical institu-tions which were suffering from mitomycin shortage,and 10 medical institutions which were suffering from pingyangmycin short-age.Conclusion In order to alleviate the shortage of antitumor drugs,the government departments should play the guiding role in policy,and avoid fluctuations in the prices of raw materials,and encourage enterprises to develop more alternative medicines.

Abstract:Objective: To probe into the status quo of rural clinics in Shaanxi province,so as to provide reference to strengthen the construction of rural clinics and improve the service ability of rural physicians.Methods: Using multistage sampling processes,a field survey including 60 rural clinics,1386 prescriptions and 74 rural physicians in Shaanxi in 2008 is carried out,analyzing indexes include the condition of rural clinics and physicians,the structure of patients,diagnoses of disease and drug use by descriptive statistical method.Results: Low quality and the imbalance in sex and age structure in rural physicians are significant.Patients who visited rural clinics were mainly young adults.Respiratory diseases and digestive disease are main dieases among those patients in rural clinics.Some prescriptions were nonstandard written out.Conclusions:the measures that should be taken into action are as follows: strengthen the qualities of rural physicians by regular training;pay attention to the preventive and health protection of common diseases and frequent diseases;improve and perfect diagnostic and treatment norms.

Abstract:Objective To analyze the current situation of drug use in primary health care institutions in Shaanxi Province,and to promote the rational drug use in primary health care institutions. Methods 2 340 outpatient prescriptions were sampled from 27 prima-ry health care institutions of Shaanxi Province from April 2013 to May 2013. The average number of drugs per prescription,the average prescription fee,the percentage of injections and antimicrobial agents were used to analyze the rationality of the prescription. Results 2 319 valid prescriptions were analyzed. The average number of drugs per prescription was 3. 01,the average prescription fee was 34. 03 yuan, the percentage of injections and antimicrobial agents were 28. 76% and 43. 94%,respectively. All the evaluating indexes were statisti-cally different among different regions and different types of health care institutions ( P ﹤ 0. 01). Conclusion Irrational use of drugs extensively existed in primary health care institution and the abuse of injections and antimicrobial agents were extremely seri-ous. Strengthen the supervision of injections and antimicrobial agents would be helpful to improve rational drug use. We also suggested establishing laws and strengthening the education of rural residents and implementing regular training among medical staff in primary health care institutions.

Abstract:Medicines play an important role in protecting people's health, but drug shortage is a long-standing and complex problem in the world. As the world's second largest pharmaceutical market, China's drug shortage problem occurs frequently and requires a lot of research to be solved. This study uses the method of literature analysis to summarize the statistics, research and reports mainly from China on the domestic drug shortage problem. It is found that the main reasons for China's drug shortage are low drug profits, flawed centralized procurement policies, drug production capacity and insufficient development and inadequate supplement of APIs. According to this, this study also puts forward related suggestions including strengthening system construction, guaranteeing Supply and Strengthen Monitoring and improving the supply security of active pharmaceutical ingredients. This study enables other scholars to understand the situation of drug shortages in China, and also provide theoretical support for policy improvement and subsequent research.

Abstract:Background: Drug shortages significantly threaten public health and medical service provision worldwide. Research evidence on the complete picture of drug shortages is currently scant in China. This study aimed to provide a descriptive overview and a reference for alleviating of drug shortages in China. Methods: National and provincial lists of drug shortages issued in China from 2018 to 2021 were collected and summarized. The information on essential medicines, medical insurance drugs, emergency drugs, and volume-based purchasing drugs was then matched with a drug shortage list to analyse the characteristics, proportion and incidence of drug shortage on each list based on the analysis of information such as dosage form, shortage frequency, and Anatomical Therapeutic Chemical (ATC) classification of the drugs in shortage. Results: A total of 24 provinces issued drug shortages lists involving 408 drugs from 2018 to 2021. All 58 drugs in the national drug list were included on the provincial drug shortage list. Among all the drugs in shortage, the most significant shortage involved injections, accounting for 45.3% (185/408). Ninety-five drugs (23.3%) were in shortage 5 times (annual shortage > 1 time) or more in the provincial lists, and 199 drugs (48.8%) were on the shortage list only once. In terms of therapeutic property, nearly all categories of drugs had been reported in shortage, among which cardiovascular drugs, nervous system drugs, anti-tumor and immunomodulatory drugs, and blood and hematopoietic organ drugs accounted for more than 10%. There is no significant difference in drug shortage among economic regions. Comparing drugs in shortage and various lists, 81.9% (334/408), 51.0% (208/408) and 67.9% (277/408) fell on the National Medical Insurance Drug List, National Essential Medicines List, and WHO Model List of Essential Medicines, respectively, while the volume-based purchasing drugs accounted for 3.4% (14 drugs). The incidence of drug shortages on NEML, WHO Model List of Essential Medicines and medical insurance category A was significantly higher than that of medical insurance category B and volume-based purchasing drugs (P < 0.05). Of the Emergency Drugs List, 72.0% (36/50) also experienced shortages, significantly higher than all the above categories (P < 0.05). Conclusions: In China, drug shortages were severe and complicated. Drug shortages vary among economic regions but are not significant. In comparison, the national procurement pattern of volume-based drug purchasing may be conducive to alleviating the drug shortage problem. Collaboration of all partners was recommended to ensure the supply of clinically necessary drugs.

Abstract:Objective To investigate the status of drug use in new rural cooperative medical institutions and promote the level of rational drug use. Methods Randomly selected the out-patient prescriptions in county and township new rural cooperative medical institutions in a county in Henan province,analyzed the average kinds of drugs prescribed in each prescription,the average cost,and the percentage of outpatients that received antibacterial drugs,injections. Results Maximum numbers of drugs per prescription were 5 for county hospital and 9 for township hospital,and the mean numbers were 1.97 and 2.68,respectively.There were 52 prescriptions including antibacterial drugs in county hospitals,and 67.3% contained one kind of antibacterial drugs,28.8% with two kinds,1.9% of them consisted of three or four kinds of antibacterial drugs.While in township hospital,the number of prescriptions including antibacterial drugs was 35,91.4% with one,5.7% and 2.9% contained two and three kinds of those drugs respectively.The number of prescriptions with injections was 5 for county hospital and 32 for township hospital,meanwhile,the utilization rates of injections were 3.4 and 23.5,respectively.The maximum of per prescription expense of county hospitals was 380.2 yuan,minimum of that was 0.2 yuan,and the mean was 84.86 yuan,while the numbers of the township hospitals were 434,0.4 and 50.58 yuan,respectively. Conclusion The status of drug use in sample medical institutions is general good,reduce prescription costs and quantities of prescription drugs in township level medical institutions should be strengthened.Improve the level of rational drug use in rural areas needs many efforts,which including the followings,improve the internal management of medical institutions,enhance knowledge and skills training of the rural doctors,cultivate awareness of rational use of drugs of the farmers and so on.

Abstract:Objective:To summarize the research status of drug shortages and to provide references for. Methods:The databases such as CNKI, Wan Fang Data and VIP were electronically searched to collect studies related to drug shortages and related issues such as the varieties, the reasons, strategies and so on. The retrieval was from January, 1989 to April, 2015.The literature was screened independently according to the inclusion and exclusion criteria, thendata were extracted and ifnallythe related studies were analyzed.Results:The varieties of drugs in-short in China added up to 148, including 126 kinds of chemical drugs and 22 kinds of traditional Chinese medicines. The chemical medicines in-short were mostly biological products, cardiovascular drugs, hematological system drugs, antimicrobial agents, antiphrastic drugs and so on. While the shortages of traditional Chinese medicineswere mostly internalmedicines. Moreover, reasons for the drug shortages in China were complicated, such as the unreasonable pricing policy, the unsound supporting measurements and bidding systems, loose supervision in relateddepartments and some institutional issues in hospital systems and lack of medical knowledge in patients. The strategies to deal with drug shortage includedestablishing special department and platform to manage drug shortages,formulatingguidelines to handle the problems, setting reasonable drug price and bidding policy, strengthening drugs registration,normalizing reservation of drugs in shortage and so on. Conclusion: Up to now, the status of drug shortages in China was not so optimistic. Drug shortages in China were varied, andthe analysis of the reasons was not profound enough.Furthermore, the strategies for solving related problems were superifcial and not in detail.What’s more, therewere few strategies toimprove this situation and few concerns about the impact of drug shortages. More studies about drug shortages were needed to provide support and advice.

Abstract:This study used a field research of basic medical institutions,distribution companies and drug retailers on the national essential medicines of Ningbo pilot counties(cities).We interviewed medical staffs,medicines managers and primary patients and made a questionnaire investigation.We used the quantitative analysis,qualitative interviews and empirical methods to analyze the operating conditions of the basic medical institutions and enterprises of supply and circulation after implementing the essential medicines system in pilot areas.We propose to add drugs according with local realities into the essential medicine list,explore the multi-channels assistance,improve grass-roots health care sector performance appraisal system,regulate circulation order,strengthen health insurance policies linkage and try to implement the pilot of essential medicines and other countermeasures.
Survey and Analysis on Drug Shortage in China

Abstract:OBJECTIVE:To provide reference for dealing with drug shortage in China. METHODS:The situation of drug shortage in China was analyzed comprehensively by following methods. Literature research and questionnaire survey were conducted in 177 medical institutions of 10 provinces or cities. Fieldwork was performed in 3 drug distributing enterprises. RESULTS CONCLUSIONS:Results indicated 284 kinds of drugs were in short,in which 198 drugs were involved in National Essential Medicine List,222 drugs were contained in Drug List for National Basic Medical Insurance and Employment Injury Insurance and 104 drugs were included in Medication Reference List for Community Health Service Institution. It covered all treatment fields mostly. The analysis shows drug shortage does exist in China,which is mostly caused by local drug shortage. The direct reason presents as many,diverse and messy situation in drug production and distributing section and industrial cut-throat competition.

Abstract:OBJECTIVE:To provide reference for the adjustment of national essential drug list and the improvement of national essential drug system.METHODS:The cross section survey was used.The utilization information of essential drugs in 68 community healthcare centers in Futian district,were investigated from Jul.1st 2010 to Mar.31th 2011.A total of 601 drugs included in the list of essential drugs in community healthcare facilities of Shenzhen Futian District,were analyzed.RESULTS:A total of 428 drugs(548 drug preparations),which included 248 national essential drugs,130 supplemental essential drugs by Guangdong province and 50 supplemental essential drugs by Futian District Authorization,were used in 68 community healthcare centers in Futian district and reached 71.21% of all essential drugs in the list.Traditional Chinese drugs for internal medicine therapy,antimicrobials and cardiovascular drugs ranked top according the total consumed sum.The immune modulating agents,anesthetics and detoxications were seldom utilized.Among ranking in sum top 20 drugs,9 belonged to national essential drugs,7 belonged to provincial supplemental essential drugs and 4 belonged to Futian District supplemental essential drugs.CONCLUSIONS:The absolute majority of drugs in the national essential drug list are suitable for the community healthcare facilities.All supplemental drugs by Futian District authorization and only a few part of supplemental essential drugs by Guangdong Province are suitable for the community healthcare centers in Futian District.

Abstract:Background: Drug shortage is a worldwide problem that seriously threatens public health. China released the most comprehensive list of key drug shortage monitoring varieties ever in 2022. We aim to analyze the attributes and characteristics of the medicines within the list to provide a reference for improving China's supply security of shortage drugs. Methods: We used public data to extract information on drug types, dosage forms, indications, classification of clinical uses, whether they were included in medical catalogs such as the National Essential Drugs, and the number of drug and active pharmaceutical ingredient (API) manufacturers. A descriptive statistical analysis was used. Results: Of the 980 drugs on the list, 99.59% were chemicals and 92.65% were injectables. Drugs for blood and hematopoietic organs, the cardiovascular system, and the digestive tract and metabolism ranked among the top three shortages. Verification of the medical catalogs showed that 90.41% of the drugs belonged to the national essential drugs, 95.10% were medicare drugs, 2.55% were volume-based procurement drugs, and 14.70% were for rare diseases, and 42.04% were for children. In terms of drug supply capacity, 21.33% of drug approvals are less than 10, and there were even 26 drugs for exclusive production, close to 90% of manufacturers need to purchase APIs from outside. Among the 256 APIs included in the list, 152 APIs had less than 10 manufacturers, and there were even 5 APIs produced by only one enterprise nationwide. Conclusions: The situation of drug shortages in China was severe and complex, with serious shortages of medicines adapted to basic medical and healthcare needs and clinically necessary medicines, and a need to improve the production capacity of drugs and the ability to supply APIs. We recommend strengthening drug monitoring and stockpiling and accelerating the approval of shortage drugs to improve drug supply security.
