Abstract:OBJECTIVE: To investigate the antibiotic use in Shanghai Second and Third Class Hospitals. METHODS: The use of antibiotics in outpatients and inpatients in 10 third class hospitals and 13 second class hospitals were analyzed statistically in respect of consumption sum, order of varieties. RESULTS: The proportion of antibiotic use in Shanghai second and third class hospitals showed a different degree of decrease. However, in terms of the number of prescriptions and consumption sum, the proportions of antibiotics used in outpatient department were significantly higher in second class hospitals than in third class hospitals. The proportion of antibiotic use in inpatients (both non-surgery and surgery ones) were still on the high side, most had a postoperative antibiotic using coure of 3 to 7 days. CONCLUSION: The proportion and duration of antibiotic use should be strictly under control to ensure safe, effective and economical use of antibiotics.

Abstract:OBJECTIVE: To investigate the utilization status of antibiotics in rural area of Ningbo and to put forward improvement measures.METHODS: The data regarding the utilization of antibiotics were obtained by questionnaire survey and the on-the-spot investigation among the randomly selected 52 township hospitals,451 village clinics,and 117 drugstores in Ningbo.RESULTS: The irrational drug utilization phenomena were widespread in township hospitals,village clinics,and drugstores in Ningbo,which involved technical problems as well as administrative loophole,and manifested as insufficient etiology evidence,nonstandard dosage regimen,unclear indications for preventive medication or combined uses of antibiotics,and lack of effective drug control measures etc.CONCLUSION: The irrational drug utilization phenomena are widespread in rural area of Ningbo,which should be given proper intervention.
Trend of Antibiotics Usage among Outpatients in Primary Public Medical Institutions in a County of Zhejiang Province， from 2015 to 2020

Abstract:ObjectiveTo understand the usage of antibiotics in outpatients in a county of Zhejiang Province from 2015 to 2020， so as to provide a basis for further standardizing the clinical application of antibiotics in this region.MethodsOutpatient records and prescription records of 146 public medical institutions from 2015 to 2020 were extracted from the electronic medical record data sharing platform system of medical institutions in a county of Zhejiang Province. The utilization rates and the number of types of antibiotics used in outpatient patients were described. According to the drug anatomy， therapeutic and chemical classification system （ATC）， clinical application classification and dosage form， the use status of different types of antibiotics was observed. The annual percentage change （APC） was used to analyze the time trend of antibiotic use. Among the antibiotic prescriptions in each year， the proportions of prescriptions aimed for different use reasons and patients were compared to analyze the changes in the structures of antibiotic prescriptions.ResultsDuring 2015‒2020， a total of 2 861 438 prescriptions were issued in 146 primary public medical institutions in this county， including 314 642 prescriptions for antibiotics， and the overall utilization rate of antibiotics was 11%. The utilization rate of antibiotics showed a decreased trend across the years （from 14.23% in 2015 to 7.59% in 2020， APC=-11.51%， 95%CI=-0.01%‒-21.7%， t=-12.05， P<0.001） and an obvious seasonal variation character （higher in winter and spring， lower in summer and autumn）. Cefoxitin sodium for injection was the most used antibiotic in 2015， and cefuroxime was the most used antibiotic from 2015 to 2020， respectively. The utilization rates of cephalosporins （APC=-11.06%， 95%CI=0‒-20.89%， t=-22.233， P<0.001）， quinolones （APC=-9.74%， 95%CI=0‒-18.53%， t=-7.652， P=0.002）， macrolides （APC=-17.52%， 95%CI=0‒-17.52%， t=-3.626， P=0.022） and lincoamides （APC=-49.01%， 95%CI=0‒-74%， t=-3.35， P=0.029） showed a decreasing trend across the years. There were differences in the distribution of drug use reasons （χ2=9 458.427， P<0.001）， as well as the age （χ2=5 421.709， P<0.001） and sex （χ2=245.322， P<0.001） of patients. During the observation period， 50.69% of antibiotic prescriptions were used to treat respiratory diseases， with the highest proportion of 57.67% in 2015 and the lowest proportion of 34.93% in 2020. 58.37% of patients were aged 50‒70 years， with a decreasing proportion of patients under 50 years （17.83% in 2015， 6.64% in 2020） and an increasing proportion of patients over 70 years （24.1% in 2015， 38.41% in 2020）.ConclusionThe utilization rate of antibiotics used in outpatients from primary public medical institutions in a county of Zhejiang Province shows a decreasing trend across the years， but higher than that of secondary and above hospitals in China， and also higher than that of primary medical institutions in economically-developed areas. The management and monitoring of the clinical use of antibiotics in the county should be actively strengthened.

Abstract:IntroductionThe irrational use of antibiotics is a major driver of antimicrobial resistance. This study aimed to explore how antibiotics were used in secondary and tertiary hospitals in Shaanxi Province, western China from 2013 to 2015.MethodA retrospective study was conducted with a sample of 16 hospitals in Shaanxi Province (2 tertiary and 14 secondary hospitals; 8 public and 8 private hospitals) using a stratified random sampling method. All of the macro data from these hospitals from 2013 to 2015 were analyzed. All collected data were double-entered and analyzed using Excel 2007.ResultsThe percentage of injectable antibiotic prescriptions was 26.6% of all of the antibiotic prescriptions in the secondary hospitals and 14.2% in the tertiary hospitals. Injectable antibiotic prescriptions in private tertiary hospitals (enter %) were more than two times that of public tertiary hospitals (enter %). In both tertiary and secondary hospitals, the percentage of antibiotic prescriptions for outpatients, emergency patients and inpatients were within the scope of the national standards, while the intensity of antibiotic use was higher than the national standard of 40 DDD/100 bed-days. The prophylactic antibiotic use rate in clean surgery was 40.4% in tertiary hospitals and 60.7% in secondary hospitals, which were both higher than the national standard of 30%. The preventive use rate of antibiotics in private tertiary hospitals (55.00%) was more than two times that of public tertiary hospitals (25.90%), and the rate was also higher in private secondary hospitals (61.50%) than in public secondary hospitals (59.70%).ConclusionsSubstantial antibiotic abuse occurred in the sample Chinese hospitals, especially in secondary hospitals. The government should continue to strengthen the administration of antimicrobial use in hospitals. At the same time, medical professional training and interventions for physicians should be conducted to fundamentally reduce the irrational use of antibiotics.

Abstract:OBJECTIVE:To investigate the utilization of antibiotics in our hospital and analyze the irrational use of antibiotics so as to provide reliable basis for rational drug use in the clinic as well as scientific management on the use of antibacterials. METHODS:A total of 1 200 outpatient prescriptions (100 monthly from January to December in 2008) randomly sampled in our hospital were subjected to evaluation in respect of the rational use of antibiotics. Another 11 103 outpatient prescriptions randomly sampled monthly from January to December in 2008 were analyzed in respect of prescription error and the irrational use of antibiotics. The consumption quantity of antibiotics and the order of DDDs of antibacterials (injections and oral preparations) within the 12 months were investigated. RESULTS:Six kinds of β-lactams,two kinds of quinolones and one kind of macrolides ranked among the first 10 places on the DDDs list of the antibacterial drugs used in the outpatients. The outpatient utilization of antibacterials in our hospital was basically rational,but there were some problems such as high starting point and irrational administration and dosage which remain to be tackled urgently. CONCLUSION:The outpatient utilization of antibacterials in our hospital was rational on the whole,but the management on the use of antibiotics in the hospital should further strengthened by taking intervention measures such as prescription evaluation etc so as to promote rational drug use and ensure medication safety.

Abstract:Objective The aim of this study is to explore factors that affected antibiotic prescribing in rural areas of w estern China.Methods This study was conducted in 40 counties of Western China.We used a 3-stage sampling method to select the data collection sites.We randomly selected 5 tow nships from each county and then randomly selected 4 villages from each tow nship.The last 30 prescriptions for outpatients up to May 31st 2005 at each selected village health clinic w ere collected.Village doctors w ere interview ed using a structured questionnaire to determine the information about village doctors and clinics.To take into account the possible correlation among patients in the same villages,w e used the multilevel logistic regression models to identify the independent predictors of antibiotic prescribing.Results The main predictors of antibiotic prescribing in village health clinics were number of drug items and number of antibiotic items in village clinics,subsidy for doctors from government,village doctor receiving professional training and assessment,number of drug items prescribed,injection prescribing,patient's age and gender.Conclusion The situation of antibiotic prescribing of village doctors was not optimistic.Village doctors receiving subsidy from government and receiving professional training and assessment were positive significance for improving the status of rural area.

Abstract:Objective:To investigate outpatient clinic prescriptions in a hospital and analysis the use of antibiotic agents and standard of prescription writing.Methods:A cross section investigation was used to survey the clinical application of antibiotic agents and standard of prescription writing within a day.Results:The usage rate of antibiotic agents was 23.59%. The proportion of combination use of antibiotic agents was 18.48%.It was involved in 10 categories of antibiotic agents, the third generation carbostyril(31.58%)was used at most,next were the first generation cephalosporin(23.16%)and nitromidazole(15.09%).Nonstandard prescription writing contained the substandard usage and dosage of drugs(67%)and incorrect writing names of drugs(55%).Conclusion:It is totally reasonable for the use of antibiotic agents in outpatient clinic prescriptions,but it is recommended to emphasize on germiculture and antibiotic sensitive tests and enhance the monitoring on antibiotic rational use of special crowds.The writing of prescription should be standard and avoid medical dispute from incorrect writing names of drugs.

Abstract:Objective:To study the use of antibiotics in out-patient of our hospital to promote the rational use of antibiotics.Method: The data of antibiotics used for outpatients in our hospital from 2005 to 2007 were collected and analyzed.Result:Antibiotics accounted for 16.1%among medicine incomes.Most antibiotics were oral drug preparation.The first three which rank in sum of money were cephalosporins,antifungal agents and penicillins.Antibiotics were used in accordance to the therapeutic principle of stomatic infection. Conclusion:The use of antibiotics for outpatients was rational on the whole,while the rational use of drugs should be further improved.

Abstract:Objectives: To explore the trends of antibiotic consumption in public healthcare institutions through analysing the provincial centralized bidding procurement (CBP) data in Shandong, China.Methods: The Shandong CBP system has been established since 2011, covering public healthcare institutions of 500 secondary and tertiary hospitals, 600 urban primary healthcare centres (PHCs) and 1600 rural PHCs. We collected all the antibiotic procurement records from the CBP system between 2012 and 2016. Antibiotic consumption data were calculated using Anatomical Therapeutic Chemical (ATC)/DDD methodology and normalized using DDD per 1000 inhabitants per day (DID).Results: Overall antibiotic consumption increased from 12.859 DID in 2012 to 15.802 DID in 2014, then decreased to 13.802 DID in 2016. The top three antibiotics consumed in 2016 were penicillins (4.251 DID), quinolones (2.957 DID) and macrolides (2.057 DID). PHCs consumed 80% of the total antibiotics, of which rural PHCs accounted for 88%. Antibiotic consumption peaked in 2014 for rural PHCs and in 2015 for hospitals, and declined thereafter. In urban PHCs, the consumption steadily increased from 2012 to 2016.Conclusions: Zero mark-up drug policies and national policies to improve rational use of antibiotic were associated with the reduction of antibiotic consumption in public healthcare institutions in Shandong Province. Regulations for antibiotic use in PHCs should be strengthened.

Abstract:Objective To investigate the use of cephalosporins use in out-patient department.Methods Randomly 2400 out-patient or emergency prescriptions during the year 2012 were sampled and analyzed to summarize the prescriptions according to Management Standard of Hospital Prescription Comments,Instructions of the Clinical Application of Antibiotics or Pharmacopoeia of People 's Republic of China(2010 edition).Results 235 irrational prescriptions were found(accounting for 9.79%),including improper use of antimicrobial drugs,inappropriate dosage,improper combination therapy,unreasonable solvent use,etc.Conclusion Although cephalosporins prescriptions of out-patientl are reasonable on the whole,the quality of the prescription writing should be further improved for rational drug use.

Abstract:Objective: To evaluate the usage of antibiotics and to enhance the rationality of antibiotics usage in department of Obstetrics and Gynecology.Methods: cluster sampling was introduced and the usage of antibiotics was investigated retrospectively from Jan 2004 to Dec 2006.Results: 5817 medical records were surveyed,the percentage of antibiotics usage is 44.15% and preventive use took up 93.73%.Conclusion: We should pay more attention to the management of antibiotics usage and elevate the reasonable application of antibiotics in clinic.

Abstract:Objective: To investigate antibiotics uses in Shanghai during the first half of 2005. Method: Antibiotics management guidelines, patient records, out - patient prescriptions and uses of 20 Shanghai second- and third - class hospitals were collected. Result: The percentage of antibiotics income out of the total medication income was 21.57-25.01%. The use rate of antibiotics was as high as more than 70% , in which cephalosporin nearly acounts for 50 %. The rates of bacterial detections and drug sensitivity tests were as low as 15% and 9% , respectively. The average prescription fee in the third - class hospitals was 18.5% higher than that in the second-class hospitals. The levofloxacin of defined daily doses (DDDs) of antibiotics uses in hospitalized patients was on the top list, and was followed by several cephalosporins. Conclusion: It is still very common that irrational uses of antibiotics results in the waste of medical sources. More strict measures are needed in hospital antibiotics controls.

Abstract:Objective To explore the status of antibacterial usage in the outpatient department in order to manage and use the antibacterial more scientifically.Methods A total of 36378 prescriptions from January 2012 to December 2012 were collected,of which the prescriptions using antibacterial were analyzed.Results Of the 36378 prescriptions,19883 were antibacterial prescriptions,accounting for 54.66%;18684 had antibacterial indications,accounting for 93.97%;11862 were single-use prescriptions,accounting for 59.66%,7902 were double combination prescriptions,accounting for 39.74%,and 119 were triple prescriptions,accounting for 0.60%;14866 were injection prescriptions,accounting for 40.87% and 18656 were oral use prescriptions,accounting for 51.28%;The use of azithromycin was common,which was found in 6358 prescriptions,accounting for 17.48%.Conclusion The use of antibacterial is basically rational and standard in the ourpatient department,but further standarization and management is needed in order to enhance rational and safe medication and reduce drug-induced diseases.

Abstract:OBJECTIVE: To investigate the necessity of Special Rectification Activities of Clinical Application of Antibacterials in China. METHODS: The original electronic prescriptions including antibiotics were collected from 9 comprehensive Grade Three hospitals in Shanghai between 2008 and 2010. Excel software was used to summarize all the prescription data, calculating DDDs,compound annual growth rate(CAGR) and antibiotics use density (AUD), etc. RESULTS: The number of antimicrobial agents’ prescriptions and the amount of prescriptions showed a rising trend year by year in the recent 3 years. However, its proportion in the total number and total amount of prescriptions remained stable. Among treatment categories of top 10 drugs in the list of DDDs, the second generation cephalosporins ranked first place and the rising CAGR of DDDs in the 3 years was also the highest. Among the top 10 drugs in the list of DDDs, cephalosporin had four. Cefuroxime (tablets) rose fastest with CAGR of 17.3%. Drugs of restricted use took up the highest proportion of antimicrobials, which was more than 50%; followed by drugs of non-restricted use with a declining trend year by year. Although the drugs of special use accounted for the smallest, they showed a very obvious increasing trend year by year in the three years. The utilization ratio of antimicrobials in hospitalized patients in hospital A among sample hospitals had been above 65% in three years. The AUD of antimicrobials in five key departments were 40.37～198.43 DDD/100 persons/ day. There was a large gap with the standard in this special campaigns activity. CONCLUSION: Although the clinical use of antimicrobials has been limited in recent years, antimicrobials abuse remains serious. It needs further efforts of health administrative departments and all levels of medical institutions.
Antibiotic Prescribing Patterns in Village Health Clinics Across 10 Provinces of Western China

Abstract:Objectives: To explore antibiotic prescribing patterns in rural areas across 10 provinces of Western China and to compare the patterns among these provinces.Methods: About 20 125 prescriptions were collected from 680 primary health clinics in villages from 40 counties in 10 provinces of Western China. Percentage of prescriptions with antibiotics and number of antibiotics per 100 prescriptions were used as measurements of antibiotic utilization.Results: The percentage of prescriptions with antibiotics was 48.43 (range: 41.12-57.47) in the study areas. There were 49 kinds of antibiotics prescribed in total, and 17 of them accounted for 90% of all usage. The number of antibiotics per 100 prescriptions was 54.62 (range: 43.78-69.56).Conclusions: The frequency and proportion of prescribed antibiotics in the rural areas of Western China are higher compared with the developed countries, and the patterns of antibiotic prescription differ greatly among provinces. The findings have important policy implications for recommendations on the utilization of antibiotics in China.

Abstract:Objectives To analyze the medical knowledge,drug purchases and using behavior of rural residents and estimate the problems and influencing factors of drug use by residents.Methods Stratified sampling method was employed to choose samples and SPSS,was used forstatistical analysis.Results Residents in investigated rural areas were lack of drug knowledge.Of the self-medicators,more than 40% used antibiotics,while 38%(Shandong) and 47.8%(Ningxia) used Chinese medicines.Of the patients who used drugs in the four weeks preceding the survey,5.1% and 7.8% always forgot their medication,25.3% and 36.1% occasionally forgot their medication in Shandong and Ningxia respectively.Conclusions The survey suggested the existence of irrational drug use in rural area.Measure should be made to improve the medical knowledge and drug use behavior of rural residents.

Abstract:Objective This study was performed to investigate the status and exiting problem of antibiotic use in surgical patients in Beijing Friendship Hospital and provide reference for safe,rational and effective in antibiotic use clinic.Methods The data of surgical patients(n=178) in 2000 and surgical patients(n=172) in 2004 were collected randomly and analyzed retrospec-tively.Results Of the selected patients in 2000 and 2004,100% were receiving antibiotics.The rate of rational antibiotic use was 65.7% and 46.5% respectively.The purpose of antibiotic use was for simple prophylaxis in 43.8% and 43.6%,for both treatment and prophylaxis in 56.2% and 56.4% respectively.The empiric use of antinations composed 100% and 98.3% respectively.In 56.5% and 44.4%,36.6% and 50.0%,6.9% and 4.4%,0 and 1.1% of the users,a single,two,three and four or more agents were used in combinations,respectively.Conclusion There is unrational antibiotic usage in clinical practice in our hospital,and the clinicians should pay attention to it.

Abstract:OBJECTIVE:To investigate the utilization of antibacterials in the emergency and out-patient department in our hospital so as to find out the problem and improve the rational level of drug use.METHODS:12 550 prescriptions of western medicines were randomly sampled from the emergency and out-patient department in our hospital in 2008 for a statistical analysis of antibacterials regarding the indexes such as the proportion of patients using antibacterials,rate of drug combination,DDDs etc and the rationality of using antibacterials.RESULTS:The rate of using antibacterials was 41.92% in the outpatients.The rate of using only one kind of antibacterials was 76.64% versus 23.36% of using two or more than two kinds of antibacterials.Prescriptions of antibiotic injections represented 41.46% in the prescriptions of antibacterials.The top 3 oral antibacterials were Cefaclor Sustained-release Tablets,Azithromycin Dispersible Tablets and Clarithromycin Capsules.Cefmetazole took the lead among all the antibiotic injections.Quinolones took the lead among all the antibacteirals for external use.CONCLUSION:The utilization of antibacterials in the emergency and out-patient department in our hospital was rational on the whole,but there were irrational prescriptions.The management on the rational use of antibacterials should be further strengthened in hospital and the intervention measure such as evaluation on prescriptions should be taken by concerned department to and increase the consciousness on rational use of drugs and ensure safe and rational medication.

Abstract:OBJECTIVE To estimate the status quo and existing problem of antibiotic use in elderly in patients of four hospitals in Beijing and provide reference for safe,rational and effective clinic antibiotic use. METHODS The data of elderly patients in 2000(n=254) and in 2004(n=274) were collected randomly and analyzed retrospectively. RESULTS Of the selected patients in 2000 vs 2004,73.2% vs 74.1% were receiving antibiotics.The rate of rational antibiotic use was 78.0% vs 68.0%.The purpose of antibiotic use was for simple prophylaxis in 26.3% vs 35.5%,for simple therapy in 43.5% vs 36.9%,for both therapy and prophylaxis was 29.0% vs 27.1%,for ambiguous aim was 1.1% vs 0.5%.The purpose of antibiotic use was as an empiric decision in 95.2% vs 92.6%,respectively. CONCLUSIONS There is unreasonable antibiotic usage in elderly patients of the four hospitals,and clinician should pay attention to it.
Effects Evaluation of Antibacterial Drug Prescriptions Comment in Outpatient and Emergency Depart-ment of a Hospital

Abstract:OBJECTIVE:To provide reference for rational use of antibacterial drugs in the clinic. METHODS:In retrospective study,outpatient and emergency antibacterial drugs prescriptions were randomly selected from a hospital during 2013-2014,and then analyzed statistically. DDDs of antibacterial drugs were analyzed by using DDD method. RESULTS:From 2013-2014,the ap-plication rate of antibacterial drugs in the hospital declined substantially from 13.8% to 11.9%,and the rate of irrational use dropped from 2.7% to 1.2%. The application rate of antibacterial drugs was higher in pediatric department,emergency department and urology department;drug cost per capita was higher in infectious department,nephrology department and emergency depart-ment. The application rate of antibacterial drugs in pediatric department decreased greatly in 2014,compared with 2013,maintain-ing about 40%. The application rate of antibacterial drugs in emergency department during Jan.-Sept. in 2014 was lower than corre-sponding period in 2013,and that of the hospital during Jan.-Sept. in 2014 was lower than corresponding period in 2013,but dur-ing Oct.-Dec. in 2014 was slightly higher than corresponding period in 2013. Single type of antibacterial drugs were mainly used in outpatient department,accounting for 91.50% and 90.77% of antibacterial drug prescriptions in 2013 and 2014;two-drug account-ed for 8.44% and 9.11%. The top 3 antibacterial drugs in the list of DDDs during 2013-2014 were roxithromycin,cefuroxime and cefminox. CONCLUSIONS:The use of antibacterial drugs is up to the requirements of Nation Special Rectification Actirity for Clin-ical Application of Antibacterial Drugs in 2013. The prescription comment improve the quality of antibacterial drug prescriptions and rational use index greatly in outpatient department,but the application rate of antibacterial drugs is too high in some depart-ment. There still is irrational use of antibacterial drugs.
