Abstract:Non-pharmaceutical midwifery techniques, including perineal warm compresses, to improve maternal outcomes remain controversial. The aims of this study are to assess the effects of perineal warm compresses on reducing perineal trauma and postpartum perineal pain relief. This systematic review included randomized controlled trials (RCTs). We searched seven bibliographic databases, three RCT register websites, and two dissertation databases for publications from inception to 15 March 2023. Chinese and English publications were included. Two independent reviewers conducted the risk of bias assessment, data extraction, and the evaluation of the certainty of the evidence utilizing the Cochrane risk of bias 2.0 assessment criteria, the Review Manager 5.4, and the online GRADEpro tool, respectively. Seven RCTs involving 1362 primiparous women were included. The combined results demonstrated a statistically significant reduction in the second-, third- and/or fourth- degree perineal lacerations, the incidence of episiotomy, and the relief of the short-term perineal pain postpartum (within two days). There was a potential favorable effect on improving the integrity of the perineum. However, the results did not show a statistically significant supportive effect on reducing first-degree perineal lacerations and the rate of perineal lacerations requiring sutures. In summary, perineal warm compresses effectively reduced the second-, third-/or fourth-degree perineal trauma and decreased the short-term perineal pain after birth.

Abstract:Background: Perineal trauma during childbirth and postpartum perineal pain significantly affected the new mother’s physical, psychological, and social experiences toward spontaneous vaginal birth. The effects of perineal warm compresses remain inconclusive. Objectives: To assess the effects of perineal warm compresses during the second stage of labor on reducing perineal trauma and postpartum perineal pain relief. Search Strategy: We searched seven bibliographic databases, three register websites of randomized controlled trials (RCTs), and two dissertation databases for publications from inception to March 15, 2023. Selection Criteria: RCTs published in Chinese and English. Data Collection and Analyses: Two independent reviewers conducted the risk of bias assessing, data extraction, and the certainty of the evidence evaluation utilizing the Cochrane risk of bias 2.0 assessment criteria, the Review Manager 5.4, and the online GRADEpro tool respectively. The combined results demonstrated a statistical reduction on the second-(RR=0.40, 95 %CI: [0.27,0.59]), third- and/or fourth- degree perineal lacerations(RR=0.34, 95 %CI: [0.20,0.57]), the incidence of episiotomy (RR=0.69, 95 %CI: [0.58,0.83]) and relief on the short-term perineal pain postpartum (within two days) (MD=-0.94, 95 %CI: [-1.10,-0.77]); a potential favorable effect on improving the integrity of perineum (RR=3.36, 95 %CI: [1.22,9.27]); did not show a statistically significant supportive effect on reducing the first-degree perineal lacerations ((RR=1.43, 95 %CI: [1.05,1.95])) and the rate of perineal lacerations requiring suture (RR=0.68, 95 %CI: [0.45,1.02]). Conclusions: Perineal warm compresses effectively reduced the second-, third-/or fourth-degree perineal trauma and decreased the short-term perineal pain after birth.

Abstract:Perineal massage, as a preventive intervention, has been shown to reduce the risk of perineal injuries and may have a positive impact on pelvic floor function in the early postpartum period. However, there is still debate concerning the best period to apply perineal massage, which is either antenatal or in the second stage of labor, as well as its safety and effectiveness. Meta-analysis was used to evaluate the effect of implementing perineal massage in antenatal versus the second stage of labor on the prevention of perineal injuries during labor and early postpartum pelvic floor function in primiparous women.

Abstract:Negative maternal outcomes such as perineal trauma and related perineal pain may cause both long-and short- term morbidities. Hands on and hands off/poised technique are two kinds of techniques to protect perineum during the second-stage of labor. Hands on technique has been the routine midwifery practice for a long time; nevertheless, the effects of hands on technique on protecting perineum has been doubted. Hands off/poised as a promising technique seems prevalent in clinical practice. However, there is no consensus on use of hands off/poised technique and hands on technique. The effects of these techniques on maternal outcomes have not been fully investigated. To evaluate the effects of hands on hands off/poised technique on maternal outcomes during the second-stage of labor. Systematic review and meta-analysis of relevant quantitative studies. Seven databases: PubMed, EMBASE, The Cochrane Library, CINAHL, WanFang Data, China National Knowledge Infrastructure (CKNI), SinoMed were systematically searched from inception to July 23,2018 for relevant quantitative studies comparing the effects of hands on and hands off/poised technique on maternal outcomes. Quantitative studies were retrieved for relevant studies. Two reviewers independently screened the studies, evaluated the methodological quality using JBI appraisal checklist tools and extracted the data. The included studies were divided into two groups for analysis according to study types. Nine RCTs with a total of 7112 participants and eight non-RCTs with 37,786 participants were included for meta-analysis. Based on the results from RCTs, this study did not find difference between hands on and hands off/poised technique regarding the risk of 2nd perineal tears, 3rd /4th degree perineal tears, duration of second-stage labor and incidence of postpartum hemorrhage (PPH>500 ml). The results from 9 non-RCTs were similar with that of RCTs, except for showing less 2nd degree perineal tears in hands off/poised technique than in hands on technique. Evidence in the present study indicated that hands off/poised technique may be a promising delivery technique to maintain intact perineum, and reduce perineal pain and episiotomy use among women with low-risk pregnancy undergoing vaginal delivery. In addition, hands off/poised technique might be safe to use as it did not increase the risk of severe perineal trauma, postpartum hemorrhage, and longer duration of second-stage labor when compared with hands on technique. More studies with stringent study design, especially large randomized controlled trial, should be conducted before strong recommendation of the hands off/poised technique.

Abstract:Perineal massage during childbirth has been recommended as an effective measure to prevent perineal injury. However, the overall effects of perineal massage during childbirth on maternal and neonatal outcomes in primiparous women remain inconclusive. Particularly, the effects of perineal massage begun during different stages of labor need to be further investigated.To comprehensively review the effects of perineal massage during childbirth on primiparous health outcomes, including perineal-related outcomes, duration of labor, hemorrhage and postpartum perineal pain, and neonatal outcomes, including Apgar scores and neonatal complications, and to further explore the effects of perineal massage begun during different stages of labor.A systematic review and meta-analysis following the Cochrane Handbook guidelines and PRISMA2020.A systematic search strategy was developed following the three-phase search approach, and the literature search was conducted in electronic databases and clinical trial registers from inception to 7th January 2022. Study selection and data extraction were completed independently by two researchers. The updated Cochrane risk of bias 2.0 tool for randomized trials was chosen to evaluate the quality of included studies. Data analyses were conducted using the Revman5.4 software, and subgroup analyses were performed based on the different start times of perineal massage. Furthermore, the certainty of body of evidence for each outcome was assessed utilizing the GRADEpro online tool.Seventeen randomized controlled trials involving 3248 primiparous women were included in the review. The pooled results of meta-analyses indicated that perineal massage begun during the second stage of labor significantly increased the occurrence of intact perineum (RR = 2.78, 95 % CI: [1.52, 5.05], P < 0.001), reduced the rate of second- and third-degree perineal lacerations (P < 0.05), and decreased the incidence of episiotomy (RR = 0.63, 95 % CI: [0.50, 0.79], P < 0.001), while perineal massage during the first stage of labor effectively shortened the duration of the first and second stages of labor (P < 0.05). The available evidence also suggests the potential role of perineal massage on hemorrhage and long-term postpartum perineal pain (P < 0.05). However, the aggregated results failed to demonstrate the beneficial effects of perineal massage on neonatal outcomes (P > 0.05).Perineal massage begun during the second stage of labor effectively improves the perineal-related outcomes in primiparous women, while perineal massage during the first stage of labor significantly shortens the duration of labor. High-quality studies exploring the standardized procedure for perineal massage and the short- and long-term effects of perineal massage are warranted.CRD42022302336 (PROSPERO).

Abstract:Postpartum haemorrhage (PPH) is responsible for around 27% of global maternal deaths. Perineal tears are common in vaginal births and a significant contributor to excessive blood loss. A diversity of perineal techniques are utilised to prevent perineal trauma and reduce the incidence of PPH; however, they lack evidence‐based comparisons to understand their effects. To assess the effect of perineal techniques during the second stage of labour on the incidence of and morbidity associated with perineal trauma to prevent postpartum complications. We searched four databases and two trial registers up to 16 April 2024. We checked references, searched citations and contacted study authors to identify additional studies. We included randomised controlled trials (RCTs) of women in the second stage of labour who intended to give birth vaginally, comparing any perineal techniques with control or another perineal technique. We excluded studies that performed perineal techniques outside the second stage of labour. Our critical outcomes were second‐, third‐ and fourth‐degree tears measured immediately after birth, and PPH ≥ 500 mL measured within 24 hours after birth. We used the Cochrane risk of bias 2 tool to assess bias in the included RCTs. We synthesised results for each outcome within each comparison using meta‐analysis where possible. Where this was not possible due to the nature of the data, we synthesised results narratively. We used GRADE to assess the certainty of evidence for each outcome. We included a total of 17 studies with 13,695 participants. Hands off (or poised) versus hands on Hands off (poised) may result in little to no difference in second‐degree tears (risk ratio (RR) 0.73, 95% confidence interval (CI) 0.32 to 1.64; 2 studies; low‐certainty evidence) and third‐ or fourth‐degree tears when data are combined (RR 1.27, 95% CI 0.81 to 1.99; 2 studies; low‐certainty evidence). The evidence is very uncertain about the effect of hands off (poised) on third‐degree tears and fourth‐degree tears when reported separately (RR 0.50, 95% CI 0.05 to 5.27; 1 study; very low‐certainty evidence and RR 3.00, 95% CI 0.13 to 71.22; 1 study; very low‐certainty evidence). Hands off (poised) may result in little to no difference in PPH ≥ 500 mL (RR 1.16, 95% CI 0.92 to 1.47; 1 study; low‐certainty evidence). Hands off (poised) probably results in little to no difference in breastfeeding two days after birth (RR 1.02, 95% CI 0.99 to 1.06; 1 study; moderate‐certainty evidence) and perineal pain (RR 0.98, 95% CI 0.94 to 1.01; 1 study; moderate‐certainty evidence). Vocalisation versus control Vocalisation may result in a reduction in second‐degree tears (RR 0.56, 95% CI 0.23 to 1.38; 1 study; low‐certainty evidence) and third‐degree tears (RR 0.13, 95% CI 0.01 to 2.32; 1 study; low‐certainty evidence), but the CIs are wide and include the possibility of no effect. No events were reported for fourth‐degree tears (low‐certainty evidence). Vocalisation may increase maternal satisfaction (RR 1.19, 95% CI 0.93 to 1.51; 1 study; low‐certainty evidence). The evidence is very uncertain about the effect of vocalisation on perineal pain (RR 1.44, 95% CI 0.81 to 2.58; 1 study; very low‐certainty evidence). Warm compress on the perineum versus control (hands off or no warm compress) Warm compress on the perineum may result in little to no difference in second‐degree tears (RR 0.94, 95% CI 0.72 to 1.21; 2 studies; low‐certainty evidence), but likely results in a reduction in third‐ or fourth‐degree tears (RR 0.46, 95% CI 0.27 to 0.79; 3 studies; moderate‐certainty evidence). Evidence from two smaller studies is very uncertain about the effect of warm compress on the perineum on third‐degree tears (RR 0.51, 95% CI 0.04 to 7.05; 2 studies; very low‐certainty evidence) or fourth‐degree tears (RR 0.11, 95% CI 0.01 to 2.06; 2 studies; very low‐certainty evidence) when reported separately. Warm compress likely results in a large reduction in perineal pain (mean difference (MD) ‐0.81, 95% CI ‐1.18 to ‐0.44; 1 study; moderate‐certainty evidence). The evidence is very uncertain about the effect of warm compress on the perineum on maternal satisfaction and PPH ≥ 500 mL. Massage of the perineum versus control (hands off or no usual care) Massage of the perineum may have little to no effect on second‐degree tears (RR 1.04, 95% CI 0.89 to 1.21; 4 studies; low‐certainty evidence). The evidence is very uncertain about the effect of massage of the perineum on third‐degree tears (RR 0.57, 95% CI 0.16 to 2.02; 4 studies; very low‐certainty evidence). Massage of the perineum may reduce fourth‐degree tears but the CIs are wide and include the possibility of no effect (RR 0.26, 95% CI 0.04 to 1.61; 4 studies; low‐certainty evidence). Abstract:Background Numerous interventions to reduce perineal trauma during childbirth have been studied in recent years, including perineal massage. Objective To determine the efficacy of perineal massage during the second stage of labor to prevent perineal damage. Search strategy Systematic search in PubMed, Pedro, Scopus, Web of Science, ScienceDirect, BioMed, SpringerLink, EBSCOhost, CINAHL and MEDLINE with the terms Massage, Second labor stage, Obstetric delivery and Parturition. Selection criteria The articles must have been published in the last ten years; the perineal massage was administered to the study sample; and the experimental design consisted of randomized controlled trial. Data collection and analysis Tables were used to describe both the studies' characteristics and the extracted data. The PEDro and Jadad scales were used to assess the quality of studies. Main results Of the 1172 total results identified nine were selected. Seven studies were included in the meta‐analysis and indicated a statistically significant decreased number of episiotomies in perineal massage. Conclusions Massage during the second stage of labor appears to be effective in preventing episiotomies and reducing the duration of the second stage of labor. However, it does not appear to be effective in reducing the incidence and severity of perineal tears.

Risk Factors for Severe Perineal Lacerations During Childbirth: A Systematic Review and Meta‐analysis of Cohort Studies

Abstract:Aims and objectives To evaluate and quantify the best available evidence regarding risk factors for severe perineal lacerations. Background Many studies have evaluated the risk factors for severe perineal lacerations. However, the results of those studies are inconsistent, and meta‐analysis which thoroughly evaluates the risk factors for severe perineal lacerations is still lacking. Design Systematic review and meta‐analysis of cohort studies based on the PRISMA guideline. Methods PubMed, Embase, the Cochrane Library, CINAHL, ClinicalTrials.gov , CNKI, Wanfang Data, VIP and SinoMed were systematically searched for cohort studies reporting at least one risk factor for severe perineal lacerations from 1 January 2000 to 2 June 2021. Two reviewers independently conducted quality appraisal by NOS scale and extracted data. Data synthesis was conducted via RevMan 5.3 using a random‐effects or fixed‐effects model. Results A total of 47 studies with 7,043,218 women were included. The results showed that prior caesarean delivery (OR: 1.46, 95% CI 1.12–1.92) and pre‐pregnant underweight (OR: 1.31, 95% CI 1.22–1.41) significantly increased the risk of severe perineal lacerations. The results also demonstrated that episiotomy was protective against severe perineal lacerations in forceps delivery (OR: 0.56, 95% CI 0.42–0.74), but not spontaneous vaginal delivery (OR: 1.30, 95% CI 0.81–2.07) or vacuum delivery (OR: 0.76, 95% CI 0.45–1.28). Nulliparity, foetus in occipitoposterior or occipitotransverse position, and midline episiotomy were also independent risk factors for severe perineal lacerations. Conclusions Severe perineal lacerations are associated with many factors, and evidence‐based risk assessment tools are needed to guide the midwives and obstetricians to estimate women's risk of severe perineal lacerations. Relevance to clinical practice This systematic review and meta‐analysis identified some important risk factors for severe perineal lacerations, which provides comprehensive insights to guide the midwives to assess women's risk for severe perineal lacerations and take appropriate preventive measures to decrease the risk.
Effect of Perineal Massage on the Rate of Episiotomy

Abstract:Objective: Women frequently experience perineal damage after a vaginal delivery. This study aimed to investigate the effect of perineal massage (PM) during labor on the need for episiotomies. Materials and methods: The study is a double-blind randomized clinical trial conducted with 99 patients (n=49 controls; n=50 cases). Participants comprised of nulliparous pregnant women aged from 18 to 35 years in the 37th-42nd week of gestation, who referred to the Um-al-Banin Hospital of Mashhad from July to October 2018, for vaginal delivery and were in the active stage of labor. Allocation to study groups was based on a random allocation list generated by a software application. PM was performed for the cases in the active stage four times, each lasting for two minutes at intervals of half an hour. The massage was continued at the beginning of the second stage of labor for ten minutes. Control women received routine care. The delivery was practiced by a midwife who was blinded to the study groups and the performance or non-performance of massage. Data were analyzed in SPSS software version 16. Results: The need for episiotomy was significantly lower in the PM group than in the control group (p = 0.05). Spontaneous perineal tears were significantly higher in mothers of the PM group (p = 0.05. The spontaneous tear degree in the 20 mothers who did not require episiotomy (p = 0.5) and the degree of perineal tear in mothers who needed an episiotomy (n = 79; p = 0.1) were not significantly different in the two groups. In the PM group members who did not require episiotomy (n = 14) and the mother underwent a spontaneous tear, first-degree tears were more frequent than second-degree ones. The median duration of the active stage of labor until the stage completion was lower in the PM group than in the control group, although the difference did not reach statistical significance (p = 0.3). The median of the second stage duration in the control and intervention groups were 55 and 45 minutes, respectively, where the difference was significant (p = 0.002), and the median time of completion of the active stage until delivery in the PM group had reduced. Conclusion: PM had a significant impact on the reduction of the need for episiotomies and the duration of the second stage of labor. Thus, it can be suggested as a safe, simple, low-cost, and effective technique to reduce the perineal damage during delivery.
Effectiveness of small-angle episiotomy on incisional laceration rate, suturing time, and incisional bleeding in primigravida: A meta-analysis

Abstract:Objective To investigate the effect of small-angle lateral perineal incision on postoperative perineal rehabilitation in primiparous women. Method The Cochrane Library, PubMed, Embase, CINAHL, CNKI, WanFang, VIP, and the Chinese Biomedical Literature Database were searched for randomized controlled trials (RCTs) on the effect of small-angle episiotomy on postoperative maternal perineal wound rehabilitation in puerpera until April 3, 2022. Two researchers independently performed literature screening, data extraction and evaluation of risk of bias in the included literature, and statistical analysis of the data was performed using RevMan 5.4 and Stata 12.0 software. Result A total of 25 RCTs were included, with a total sample of 6,366 cases. Meta-analysis results showed that the use of small-angle episiotomy reduced incisional tearing [ OR = 0.32, 95% CI (0.26, 0.39)], shortened incisional suture time [ MD = −4.58 min, 95% CI (−6.02, −3.14)] and reduced incisional bleeding [ MD = −19.08 mL, 95% CI (−19.53, −18.63)], with statistically significant differences (all p < 0.05). There was no significant difference in the rate of severe laceration between the two groups [ OR = 2.32, 95% CI (0.70, 7.70), p > 0.05]. Conclusion The use of a small-angle episiotomy during vaginal delivery can reduce the incision tear rate without increasing the incidence of severe perineal laceration, while shortening the incisional suturing time and reducing incisional bleeding. It can be used clinically according to birth canal conditions of the maternal, the intrauterine condition of the fetus and maternal needs. The STROBE guidelines guided the reporting of this study.RESULTS:Among primiparous women, when compared with those who adopted lateral positions during the passive phase of the second stage of labour and changed to lithotomy positions during the active phase, women who assumed lateral positions during both the passive and active phases had a higher rate of intact perineum/first-degree perineal tears and a reduced risk of episiotomy, and episiotomy/second-degree perineal tears. No significant difference was found in perineal outcomes between those who assumed lithotomy positions during the passive and active phases and those who adopted lateral positions during the passive phase and changed to lithotomy positions during the active phase. No significant difference was found in perineal outcomes among multiparous women, regardless of different positions adopted at the passive and/or the active phase.CONCLUSION:Lateral positions during the active phase may have protective effects on the perineum in primiparous women. However, adopting lateral positions only during the passive phase did not promote better perineal outcomes, regardless of parity.RELEVANCE TO CLINICAL PRACTICE:This study will provide a further reference for the use of lateral positions during different phases of the second stage of labour among primiparous and multiparous women in clinical practice, contributing to the perineal protection during childbirth.
Effects of Warm Compresses on the Perineal Area During the Active Phase of Labor: A Quasi-Experimental Study

Abstract:Background A perineal tear refers to any injury to the female genitalia during labor, which can occur spontaneously or with the use of instruments. An episiotomy, a surgical procedure, is sometimes performed to enlarge the vaginal orifice to facilitate delivery. Ineffective management of labor pain can result in negative physiological and psychosocial outcomes. Objective This study aims to investigate the effect of warm compresses on the perineal area during active labor. Methods This quasi-experimental study utilized a convenience sample, dividing 80 mothers into intervention and control groups. Participants were randomly assigned to groups after meeting the inclusion criteria and signing consent forms. The inclusion criteria encompassed pregnant women aged 18 years or older with a healthy singleton pregnancy, a cephalic presentation, and a gestational age of 37 weeks or more. The women were in the active phase of the first stage of labor (6-7 cm cervical dilation). In Part I, a data collection tool was developed to gather sociodemographic data, obstetrical history, and initial assessment information. Part II assessed perineal pain using the Visual Analogue Scale. Part III collected data on the perineal status post-delivery, and Part IV gathered information about the newborns. The research was conducted in the labor unit at East Jeddah Hospital, with data collection occurring from November 2022 to February 2023. Results The results of this study revealed that the mean second-stage labor pain score was 9.73 for the control group and 8.68 for the intervention group. In the third stage of labor, the mean pain score was 4.52 for the control group and 2.75 for the intervention group. During the fourth stage of labor, the mean pain score for the control group was 2.90, compared to 1.50 for the intervention group. In terms of perineal outcomes, 17 participants (42.5%) in the intervention group had an intact perineum, compared to 11 participants (27.5%) in the control group. However, there was no significant difference in the episiotomy rate between the two groups. Conclusions The findings of this study indicate that applying a warm compress to the perineal area during active labor can enhance perineal intactness and significantly reduce perineal pain. Future research could explore the challenges nurses encounter when applying warm compresses to the perineum during labor.
Impact evaluation of comprehensive assessment system application during childbirth on the outcomes of perineum protection

Abstract:Objective To evaluate the impact of the comprehensive assessment system application on decision of perineum protection methods during childbirth and the outcomes of perineum.Methods A mixed method of qualitative and quantitative research was uti lized,and 223 single birth primiparas with normal fetal position and underwent vaginal delivery were selected as study participants,and 18 midwives who grasped different perineum protection methods assessed the puerperas using a comprehensive assessment system.Then the perineum protection methods,rate of perineal laceration and episiotomy among puerperas delivered by midwives with different seniority were compared,and 10 midwives were interviewed deeply to understand strength and weakness of the com prehensive assessment system.Results There was significant difference in episiotomy rate among puerperas delivered by midwives with different seniority (P＜0.05),while there were no significant differences in rate of perineal laceration and episiotomy among puerperas (P＞0.05 for both),and there was no significant difference between the real perineum protection methods and recommended method (P＞0.05).And two themes were extracted based on interview data:the comprehensive assessment system during childbirth could guide clinical practice to some extent,while some limitations existed.Conclusion The comprehensive evaluation system during childbirth has some clinical guiding significance for choosing suitable perineum protection methods,but it has some shortcomings in item setting,which result in limitations in clinic application.It is necessary to modify the system according to existing research evidence and clinical practice to meet the needs of midwives better.

Abstract:Background: Early puerperal rehabilitation can interfere with a woman's ability to care for herself and her infant. Acupoint hot compress, with a combination of acupoints and natural physical agent heat, has significant potential to alleviate symptoms experienced during early puerperium. Current evidence regarding the effects of acupoint hot compress therapy on early puerperal rehabilitation is insufficient. The aim of this study is to address this with a multicenter design and large sample size. Methods: This is a prospective, multi-center, and randomized controlled clinical trial. A total of 1400 nulliparous women with a singleton pregnancy experiencing natural childbirth from 14 hospitals will be enrolled and randomly allocated to either an intervention group or a control group in a 1:1 ratio. Subjects in the control group will only receive routine postpartum care. In addition to routine postpartum care, the subjects in the intervention group will be administered a 4-hour acupoint hot compress with a constant temperature of 45 +/- 2 degrees C respectively within 30 minutes after delivery, 24 hours and 48 hours after delivery. The primary outcome will be the time elapsed from delivery to the first urination. The secondary outcomes will be postpartum uterine contraction pain intensity, the Edinburgh Postnatal Depression Scale for screening postpartum depression and the assessment of lactation including recording the lactation initiation time, postpartum diet, appetite, weight, neonatal weight. Discussion: These results will provide evidence for obstetricians and parturients on considering nonpharmacologic and noninvasive intervention in early puerperal rehabilitation.
Effect of Prenatal Perineal Massage on Postpartum Perineal Injury and Postpartum Complications: A Meta-Analysis

Abstract:Background. The efficacy of perineal massage is controversial. The study was aimed at comparing the effects of perineal massage on perineal injury and complications. Methods. PubMed, Embase, the Cochrane Library, and ISI Web of Science were searched for literature on the relationship between prenatal perineal massage and postpartum perineal injury and complications until April 2022. Indicators included postpartum perineal tears, perineotomy, postpartum perineal pain, natural labour, and postpartum incontinence. Finally, RevMan5.4 software was used to analyze the extracted data. Results. A total of 6487 subjects in 16 studies were included, with 3211 who received perineal massage and 3276 did not. There was no significant difference in 1-2 degree perineal tearing between the intervention group and the control group ( RR = 0.96 , 95% CI [0.90, 1.03], P = 0.30 ), and there was no heterogeneity between studies ( P = 0.62 , I 2 = 0 % ), indicating publication bias. Compared with the control group, prenatal perineal massage significantly reduced the incidence of 3-4 degree perineal tears ( RR = 0.56 , 95% CI [0.47, 0.67], P < 0.00001 ), and there was no heterogeneity between studies ( P = 0.16 , I 2 = 30 % ), indicating publication bias. Compared with the control group, prenatal perineal massage reduced the risk of lateral perineal resection ( RR = 0.87 , 95% CI [0.80, 0.95], P = 0.001 ), and there was no heterogeneity between studies ( P = 0.14 , I 2 = 31 % ), and there was no publication bias. Compared with the control group, prenatal perineal massage reduced the risk of postpartum pain at 3 months ( RR = 0.64 , 95% CI [0.51, 0.81], P = 0.0002 ). Application Effects of “Hands-Off” Delivery Method in Puerpera with Vaginal Delivery

Abstract:Objective To explore clinical application effects of hands-off method among puerperas delivered through vagina. Methods To utilize retrospective study design, a total of 1 040 puerperas, who could not received natural labour from July to November 2013, were selected as observation group by convenient sampling method and underwent hands-off method to deliver children, while 1 005 prerperas were taken deliver technical protection of perineum from July to November 2010 as control group. We compared puerperas′delivery conditions and injury of perineum, and observed and analyzed the influencing factors of puerpera′s episiotomy and perineal laceration. Results No incidences of Ⅲ and Ⅳ degree perineal laceration and obstetric anal sphincter injury happened. The rate of puerpera′s episiotomy in the observation group was 25. 38% compared with 13.93% in the control group (P <0.01). The Ⅱ degree of perineal laceration was 48. 85% in the observation group lower than 82. 09% in the control group (P<0. 01). Comparing injury of perineum between two groups, the medium injury of observation group (Ⅱdegree of perineal laceration and episiotomy, 74. 23%) was lower than 96. 02% puerperas of control group (P <0. 01). The rate of vaginal wall hematoma in the observation group was 0. 77% lower than 4. 68% in the control group (P<0. 01). The length of second stage of labor and length of perineum wound suture were shorter than those of the control group (P<0. 05);the amount of bleeding 2 h after labor in two groups had no statistical significance (P>0. 05). Regression analysis showed that, maternal age, analgesia, newborn weight were the influence factors of lateral episiotomy (P <0. 05);maternal age, weeks of gestation, analgesia, newborn weight were the influence factors of perineal laceration (P<0. 05). Conclusions The hands-off method to deliver child can elevate the perineal laceration degree of perineum, reduce the length of the second stage labor, and shorten the length of perineal wound suture.
Influences of different midwifery techniques on pelvic floor function of puerperas

Abstract:Objective To explore the influences of different midwifery techniques on pelvic floor function of puerperas,and to identify one of the midwifery technique causing least trauma to pelvic floor function.Methods Totally 242 puerperas by vaginal delivery were randomized into an episiotomy group (underwent episiotomy,and protected perineum by flattening muscle of thenar with palm merging against perineal body) with 95 cases,a traditional midwifery group (without episiotomy,protected perineum by flattening muscle of thenar with palm merging against perineal body) with 86 cases and a noninvasive midwifery group (controlled fetal head delivery speed with one hand,without supporting perineal body) with 61 cases.Results The incidence of postpartum stress urinary incontinence and sexual dysfunction of the noninvasive midwifery group were significantly lower than those of the episiotomy group and traditional midwifery group (P＜0.0125 for all),and the postpartum 2 h blood loss volume was significantly less than that of the episiotomy group (P＜0.05).Conclusion The noninvasive midwifery technique is conducive to alleviate trauma to pelvic floor function of puerperas,and improve their quality of life.
Effects of Upright Positions During the Second Stage of Labour for Women Without Epidural Analgesia: A Meta‐analysis

Abstract:AbstractAimsTo assess the effects of upright positions on maternal outcomes for women without epidural analgesia in comparison with recumbent positions during the second stage of labour.BackgroundUpright positions have many physiological advantages. The underlying benefits and risks of upright positions during the second stage of labour have been reported in many studies but the results are divergent.DesignA meta‐analysis of randomized controlled trials.Data sourcesThe Cochrane Library, PubMed, Embase, CINAHL and ProQuest databases were systematically searched from inception to 17 June 2019.Review methodsWe conducted the quality appraisal using the Cochrane Collaboration's tool and performed meta‐analyses using the Review Manager 5.3 software. The primary outcomes were instrumental vaginal delivery and the duration of the second stage of labour.ResultsOverall, 12 studies including 4,314 women were included. Upright positions significantly decreased the rate of instrumental vaginal delivery (risk ratio [RR] = 0.74, 95% CI 0.59–0.93), shortened the active pushing phase (mean difference [MD] = −8.16 min, 95% CI −16.29 to −0.02), decreased the rate of severe perineal trauma (RR = 0.35, 95% CI 0.14–0.87) and episiotomy (RR = 0.52, 95% CI 0.29–0.92), but significantly increased the rate of second‐degree perineal trauma (RR = 1.45, 95% CI 1.10–1.90). However, there was no significant difference in the duration of the second stage of labour or postpartum haemorrhage.ConclusionsUpright positions are beneficial for improving maternal outcomes. Several results should be considered with caution. Researchers need to clarify the definition of upright positions and conduct large, robust studies in the future to provide stronger evidence.ImpactThis meta‐analysis explores a crucial issue in intrapartum care and clarifies the benefits and possible risks of upright positions in the second stage of labour. Midwives and obstetricians are encouraged to apply upright positions depending on women's preferences and labour progress but should take measures to prevent perineal trauma.

Abstract:Background: Warm compress is believed to reduce perineal tear occurrence and decrease pain intensity during the second stage of childbirth.Objective: This study aimed to determine the effect of warm compresses on perineal tear and pain intensity during the second stage of labor. Methods: A randomized controlled trial was conducted between 28 September 2018 to 30 October 2018 in King Khaled Hospital (KKH), affiliated with the Ministry of National Guard Hospital Affairs, King Abdul-Aziz Medical City Western Region, Saudi Arabia. According to eligibility criteria, the sample randomly recruited through lottery included 100 primigravida pregnant women, with 50 in each intervention and control group. Data were presented in the form of frequency and percentages, standard deviation, and mean. Chi-square test was used for data analysis, with p-value significance at ˂ 0.05.Results: Sociodemographic data revealed no statistically significant difference in the age, education, occupation, and residence of both the control and experimental group. The perineum area’s pain intensity after birth showed a statistically significant difference between the intervention and the control groups (p = 0.001). The perineal tear was also significantly different between both groups (p = 0.001). Conclusion: The applied warm compresses on the perineum area positively reduce second and third-degree perineal tear and pain intensity during the second stage of labor and after childbirth. Therefore, midwives and nurses are expected to actively provide effective planned in-service training programs regarding warm compresses' advantages in the second stage of labor and apply this procedure actively.
