Abstract:PURPOSEThis study aimed to investigate the efficacy and safety of transcatheter arterial chemoembolization (TACE) in combination with three-dimensional conformal radiotherapy (3D-CRT) in the treatment for locally advanced unresectable hepatocellular carcinoma (HCC).METHODSFrom March 2000 to March 2009 a total of 158 patients with unresectable HCC treated and followed at our hospital were divided into TACE group (N=80) and TACE combined with 3D-CRT group (N=78). The TACE group was treated 3-6 times. In the combination group, 2-3 TACE courses were administered and 3D-CRT was performed 2 weeks after the last TACE. Three months after the end of treatment, imaging and serum AFP were carried out to assess the treatment efficacy.RESULTSThe response rates of TACE and the combination groups were 53.7% (43/80) and 71.8% (58/78) (p<0.05). The 1, 2, and 3-year survival rates of patients in the TACE and combination groups were 58.75, 36.25, 16.25%, and 78.48, 55.12 and 25.64% (p<0.05), respectively. Treatment compliance was good, with at least 2 TACE administrations for each case and at least 52 Gy for radiotherapy. In the TACE and the combination group, there were 2 and 3 cases with grade III/IV toxicity, respectively, without treatment-related death.CONCLUSION3D-CRT in combination with TACE significantly improve the therapeutic outcome in patients with locally advanced unresectable HCC, without creating severe toxicity.
Evaluation of the Efficacy for Alternated Treatment on Primary Liver Cancer by Interventional Therapy in Combination with Fractionated Stereotactic Conformal Radiotherapy

Abstract:Objective: To compare effect of the therapeutic hepatic chemoembolization (TACE) alone with that of TACE combined fractionated stereotactic conformal radiation therapy (FSCR) with scheme alternated for primary hepatic cancer were randomly divided into TACE alone group and TACE combined with FSCR group. Acute effects and survival rates were observed. The cox proportional hazards model was used to analyze the prognostic factors. Results: The overall survival rates in group A (91.1%, 62.2% and 24.4% at 1, 3 and 5 years, respectively) were significantly higher than that in group B (73.3%, 37.8% and 8.9%, respectively, P<0.05). Conclusions: Radiotherapy plays an important role for increasing the therapeutic effect by killing off the cancer cells in border area with portacaval perfusion, compared to the treatment with TACE alone. In group A, the number and diameter of the tumor, child-pugh classification and okdua staging are the key factors in clinical prognosis.
Surgical Resection Plus Radiofrequency Ablation for the Treatment of Multifocal Hepatocellular Carcinoma.

Abstract:BackgroundTo compare the efficacy and safety of surgical resection plus radiofrequency ablation (SR-RFA) with transarterial chemoembolization (TACE) in patients with multifocal hepatocellular carcinoma (HCC) beyond the Milan criteria.MethodsFrom 2009 to 2015, a total of 469 consecutive patients with multifocal HCC beyond the Milan criteria (≤5 nodules) treated with SR-RFA (n=59) or TACE (n=410) were analyzed. Their overall survival (OS) rate was compared through propensity score matching (PSM) analysis at a ratio of 1:2, and 141 patients were identified (SR-RFA, n=47; TACE, n=94). Subgroup analysis was conducted according to factors associated with treatment options.ResultsBefore PSM, the 1-, 2- and 3-year OS rates were 81.5%, 68.3% and 64.3% in the SR-RFA group and 58.7%, 35.5% and 24.4% in the TACE group, respectively (all P<0.001). After PSM, the 1-, 2- and 3-year OS rates in the SR-RFA group were 81.8%, 68.7% and 63.4%, whereas those in the TACE group were 59.3%, 36.1% and 19.4%, respectively (all P<0.001). Subgroup analysis also revealed a survival advantage of SR-RFA over TACE in each subgroup. The 30-day mortality rate was 1.22% in the TACE group and nil in the SR-RFA group (P=0.861).ConclusionsSR-RFA provides better a long-term survival rate than TACE for patients with multifocal HCC beyond the Milan criteria. SR-RFA may serve as an alternative treatment for patients with multifocal HCC in a selected patient population.

Abstract:BACKGROUND:This study aimed to compare the safety and efficacy of transradial access (TRA) with transfemoral access (TFA) chemoembolization in treatment of hepatocellular carcinoma (HCC).METHODS:HCC patients who were late for curative treatment on initial diagnosis or HCC patients who had undergone one or several rounds of transarterial chemoembolization (TACE) were enrolled. The clinical and angiographic characteristics, the procedure related details, and the follow-up data from patients who underwent TRA and TFA were analyzed and compared.RESULTS:In total, 112 patients undergoing 160 TRA-TACE and 107 patients undergoing 163 TFA-TACE were included. The technical success rate of TRA was 95.0% and that of TFA was 98.8% (P=0.102). In the TFA-TACE group, 5.5% of cases suffered access site-related complications, including 6 with minor bleeding and 3 with severe bleeding or pseudoaneurysm. In the TRA-TACE group, 1.9% of cases underwent crossover to femoral access for selective cannulation failure. The rate of radial artery occlusion (RAO) was 2.7% (3 of 112 patients), and none of the RAO patients suffered paresthesia, pain at the site of occlusion, hand function loss or distal ischemia. Comparing patients with/without access site-related complications in the TFA-TACE group, there was a statistical difference in patient age and in the percentage of patient with a PT time >15 s (72.6% vs. 57.1%, P<0.001; 44.4% vs. 11.7%, P=0.022).CONCLUSIONS:TRA is a safe and effective method for patients undergoing TACE. Compared with TFA, TRA may reduce the occurrence of access site-related bleeding and vascular complications. TRA-TACE may especially benefit older patients or those with a longer prothrombin time (PT).
Safety and efficacy of transcatheter arterial chemoembolization plus early sequential percutaneous radiofrequency ablation for the treatment of large primary liver cancer with hepatitis B cirrhosis:a single-center experience

Abstract:Objective To assess the safety and efifcacy of transcatheter arterial chemoembolization (TACE) plus early squential percutaneous CT-guided radiofrequency ablation (RFA) in large primary liver cancer (PLC) with hepatitis B cirrhosis. Methods From October 2010 to September 2013, PLC with hepatitis B cirrhosis patients (at least one tumor≥5 cm in diameter) received lipiodol-based TACE on D1. Then CT-guided RFA was done on D5 ± 2. The primary end point was time to progression (TTP). The secondary end points included objective response rate (ORR), overall survival (OS) and safety. Results Total of 45 patients were divided into group A (Child-Pugh A, 25 cases) and group B (Child-Pugh B, 20 cases). The median age of the cohort was 56 years (39-78 years). Median follow-up time was 746 days (90-1410 days). TACE plus RFA treatment-related adverse events were mainly mild to moderate, with 40 cases of ALT elevation (88.9%), 39 cases of fever (86.7%), 38 cases of AST elevation (84.4%). The two groups had similar treatment-emergent safety proifles. The differences in TTP and ORR between two groups were statistically insigniifcant (P>0.05). The median OS time of group A was longer than that of group B, 532 and 302 days, respectively, but no signiifcant difference was found (P=0.211). Multivariate analysis revealed that tumor response was identiifed as an independent favorable prognostic factor for TTP (P = 0.011). However, cases with HBeAg positive had increased risk of disease progression, HR=2.741 (P=0.023). conclusions TACE plus early sequential RFA combined hepatitis B cirrhosis of the liver is safe and effective treatment of liver cancer. No signiifcant differences in liver damage caused by sequential treatment was demonstrated based on different baseline liver function. The short-term curative effect is an important factor to determine the prognostic of patients with large liver cancer.

Abstract:Transarterial chemoembolization (TACE) is the most common modality for treatment of hepatocellular carcinoma (HCC) at the intermediate stage. TACE is typically performed via transfemoral access (TFA). However, transradial access (TRA) is preferred in coronary artery interventions due to decreased complications and mortality. Whether the advantages of TRA can be applied to TACE required investigation. Patients receiving TRA TACE at a single center were retrospectively enrolled for study. Procedural details, technical success, radial artery occlusion (RAO) rate, and access site-related bleeding complications were evaluated. From October 2017 to October 2018, 112 patients underwent 160 TRA TACE procedures. The overall technical success rate was 95.0% (152/160). The rate of crossover from TRA to TFA was 1.9%. No access site-related bleeding complications were found in any cases. Asymptomatic RA occlusion occurred in three patients (2.7%). Compared with TFA, TRA can increase safety and patient satisfaction while decreasing access site-related bleeding complications. Moreover, TRA interventions can benefit patients with advanced age, obesity, or a high risk of bleeding complications.
Efficacy and safety of raltitrexed-based transarterial chemoembolization for colorectal cancer liver metastases.

Abstract:The liver is the most common site of colorectal cancer metastases. The present study aimed to evaluate the efficacy and safety of transarterial chemoembolization (TACE) with raltitrexed and oxaliplatin for colorectal liver metastases in a prospective, multicenter, single-arm trial conducted in 12 hospitals from different areas in China. A total of 90 patients with colorectal liver metastases were enrolled and treated by TACE with raltitrexed 4 mg and oxaliplatin 100 mg, followed by embolotherapy with 50 mg oxaliplatin and 5-20 ml lipiodol, administered every 28 days for four cycles. Patients were followed up every 3 months after the treatment and up to 12 months. The primary endpoint was time to progression. For the full analysis set (FAS), the median time to progression and overall survival were 9.1 and 17.8 months, respectively. The disease control rate in FAS was 71 (78.9%). Grade 3 or 4 adverse events were reported for 24 (26.7%) out of all 90 patients. Grade 3 thrombocytopenia, transglutaminase abnormality, and decreased neutrophil were observed in eight (8.9%), six (6.7%), and five (5.6%) patients, respectively. No unexpected adverse events or toxic deaths were observed. TACE with raltitrexed plus oxaliplatin is feasible, clinically beneficial, and well tolerated with low-grade toxicity for colorectal cancer patients with liver metastases. Abstract:Background: Transcatheter arterial chemoembolization (TACE) combined with radiofrequency ablation (RFA) intervention in prolonging the long-term survival and prognosis of patients with liver cancer are still controversy compared with the traditional interventional therapy of RFA alone. This meta-analysis aimed to compare the efficacy and safety of combination therapy versus RFA alone.Methods: The related articles were searched in PubMed, Embase, MEDLINE, Science Direct, The Cochrane Library, China National Knowledge Infrastructure (CNKI), Wanfang Database, Chinese Science and Technology Journal Database, and China Biomedical Literature Database (CBM). The Chinese and English search keywords included transcatheter arterial chemoembolization, TACE, radiofrequency ablation, RFA, primary liver cancer, and liver tumor. The five evaluation criteria of randomized controlled trials (RCTs) in Cochrane RoB 2.0 repeatedly independently evaluated the bias risks involved in the study and cross-checked the results.Results: A total of 7 articles were included, and the results of bias risk assessment show that 6 articles described the generation of random sequences in detail; There were 3 articles describing allocation concealment in detail; Operator blindness was used in 4 articles; The outcome indicators of 7 documents were complete. The 3-year overall survival rate of the RFA combined with TACE group was significantly better than that of the RFA group [odds ratio (OR) =1.97, 95% confidence interval (CI): 1.42-2.74, Z=4.05, P<0.0001]. The 1-year and 3-year tumor recurrence-free survival rates in the RFA combined with TACE group were significantly better than those in the RFA group (OR =1.88, 95% CI: 1.28-2.76, Z=3.23, P=0.001; OR =2.11, 95% CI: 1.37-3.24, Z=3.38, P=0.0007). There was no significant difference in the complication rate of patients with primary liver cancer between the RFA combined with TACE group and the RFA group (OR =0.79, 95% CI: 0.45-1.39, Z=0.81, P=0.42).Discussion: Meta-analysis results confirmed that TACE combined with RFA was safe and effective in the treatment of primary liver cancer, and can improve the overall survival and recurrence-free survival of patients with primary liver cancer.

Abstract:Objective: To explore the clinical efficacy and safety of transarterial chemoembolization (TACE) combined with targeted therapy for primary hepatocellular carcinoma (PHC). Methods: This was a retrospective study. Retrospective selection of 150 PHC patients admitted to the Renmin Hospital, Hubei University of Medicine January 2019 and June 2021 were included. The patients were divided into the control group and the experimental group according to their treatment regimens. The control group received TACE treatment, while the experimental group received TACE combined with targeted therapy. We analyze the relevant data of two groups of patients and evaluate the clinical efficacy and safety of TACE combined with targeted therapy. Results: The tumor remission rate and control rate in the control group were 41.89% and 75.68%, respectively, while those in the experimental group were 77.63% and 90.79%, with statistically significant differences (p<0.05). The 1-year and 3-year recurrence rates in the control group were 52.71% and 98.65%, respectively, while those in the experimental group were 39.47% and 61.84%, with statistically significant differences (p<0.05). After treatment, the AFP, VEGF, ALT, and AST in the experimental group were significantly reduced compared to the control group (p<0.05). During the treatment period, the incidence and severity of nausea, vomiting, and fever in the experimental group were significantly lower than those in the control group (p<0.05). Conclusion: The clinical efficacy of TACE combined with targeted therapy for PHC is superior to that of TACE alone, with improved disease control rate, improved long-term survival rate, and good safety.
Therapeutic Efficacy of Transcatheter Arterial Chemoembolization for Liver Cancer with High-Risk Factors

Abstract:Objective To explore the therapeutic efficacy of transcatheter arterial chemoembolization(TACE)for liver cancer with high-risk factors.Methods A retrospective analysis of all patients undergoing TACE for liver cancer in our hospital from January 2001 to December 2005 was performed.266 patients with liver cancer underwent 536 TACE procedures,seventy-four patients(27.8%)designated in the high-risk group,as a result of major portal vein thrombosis,and increased serum bilirubin( 2 mg/dl);the others were in the low-risk group.High-risk patients received more superselective embolization with lower dose of embolization agent and fewer numbers of procedure(1.4 vs 2.3,P 0.05).The comparasion was taken between the two groups on the topics of the postoperative complication and survival rates.Results The overall complication rate was 3.5% with no significant statistical difference in complication rates between the two groups(2.5 vs 7.6%,P = 0.07).The 1 and 3 year survival rates were 45% and 16%,in high-risk group and 63% and 18% in low-risk groups respectively.A trend towards increase of survival in the low-risk group,but with no statistical significance between the two groups(P = 0.08).Conclusions High-risk patients undergoing TACE have no significant increase in complication rate.Using rather proper technique,TACE can be performed safely in patients with high-risk factors.

Abstract:The use of percutaneous transcatheter hepatic arterial chemotherapy and embolization in the treatment of primary liver cancer has become increasingly popular in recent years. The authors employed this method, using a combination of cisplatin, mitomycin C, 5-fluorouracil, and ethiodized oil (Lipiodol) or absorbable gelatin sponge in 30 patients with huge liver cancers (diameter range, 5.6-12.0 cm) as a preliminary treatment before liver resection. Significant tumor regression occurred after this treatment, converting these tumors into resectable lesions that were excised successfully later. Before surgery, chemoembolization was done once every 4-6 weeks. The patients underwent 1-5 treatment sessions (mean, 2.9) and then waited 1-4 months (mean, 2.4 months) before undergoing surgery. Alpha-fetoprotein levels decreased to normal in seven patients. The tumor diameters were reduced by 31.6 +/- 15.2% (2.3 +/- 1.2 cm) and the percent tumor necrotic area ranged from 40-100%. Adhesions of the tumor to the diaphragm and thickening of the hepatoduodenal ligament and gallbladder wall were the primary operative findings, but they did not significantly complicate the surgery. There was one postoperative death from acute pulmonary embolism. The 1-year, 2-year, and 3-year survival rates were 88.89%, 77.03%, and 77.03%, respectively. Although these patients still are being followed to assess their long-term survival, this treatment appears promising for patients with advanced huge liver cancers who hitherto have been denied surgery on grounds of unresectability.
Feasibility and Efficacy of Dose-reduced Transcatheter Arterial Chemoembolization (TACE) for Primary Liver Cancer Patients with Portal Vein Thrombosis

Abstract:Objective:To evaluate the efficacy and safety of does-reduced transcatheter arterial chemoembolization (TACE) for primary liver cancer patients with major portal vein thrombosis. Methods: 37 patients with major portal vein and/or its first branch thrombosis underwent does-reduced TACE were retrospectively analyzed. The Child's scale were A for 34 patients and B for 3. Results: 14 patients achieved good precipitation of lipiodol in the portal vein thrombosis, the other 23 patients with poor precipitation of lipiodol in the portal vein thrombosis. The overall median survival time was 11. 67 months. The cumulative survival rates were 75.01 % (6 months), 41.03% (12 months), and 17.61% (18 months). There were14 patients achieved well precipitation of lipiodol in the portal vein thrombosis and the other 23 with poor lipiodol precipitation. For patients with well precipitation of lipiodol, the median survival time was significantly longer than patients with poor lipiodol precipitation (16. 23 VS 6. 80 months, P0. 05). The 6m, 12m and 18m survival rate of patients with well lipiodol precipitation was significantly better than that of with poor lipiodol precipitation (100%, 63. 49% ,31. 75% VS 60. 29% ,13. 03% ,0% ,P0. 05). Cox multi-variable analysis shows that the extent of lipiodol precipitation was the only significant predicting factor for efficacy of therapy (Cox regressive coefficient 1. 67,RR5. 31). All the patients were safely tolerated with dose-reduced chemoembolization except 1 died of liver failure 2 weeks after the procedure, 1 case of upper gastrointestinalt bleeding, the other 2 with increased Child-pugh scale. Conclusion:Dose-reduced chemoembolization for primary liver cancer patients with major portal vein thrombosis is safe, and can partially achieve curative effect especially for patients with good precipitation of lipiodol in the portal vein thrombosis.

Abstract:Objective:This study aimed to assess the treatment response, survival outcomes, and safety of a novel transarterial chemoembolization (TACE) technique with a three-stage mixed chemoembolic regimen (M-TACE) in patients with large unresectable hepatocellular carcinoma (HCC) measuring more than 5 cm in maximum diameter.Methods:Between January 2017 and March 2023, a total of 82 patients were enrolled in this retrospective cohort study. Treatment response was assessed in the first month after M-TACE; progression-free survival and overall survival (OS) were evaluated. The prognostic factors associated with patient survival were statistically analyzed by the Cox regression model. Adverse events were recorded.Results:The maximum diameter of the tumors ranged from 5.3 cm to 20.0 cm (mean 10.71 cm). The objective response (OR) and disease control rates were 74.4 and 92.7%, respectively, at 1-month follow-up. The median survival time was 22 months (95% CI, 13.10-30.90 months). The OS rates were 82.0% at six months, 62.5% at one year, and 43.0% at two years. Targeted therapy and/or immunotherapy (P=0.001) and tumor response at one month (P=0.020) were protective factors for OS. In terms of safety, no major complications occurred and the only observed decrease within the normal range occurred in albumin and platelet levels one month after the embolization procedure. This decrease in levels did not show a significant relationship with the OR rates.Conclusion:M-TACE demonstrated a promising objective tumor response, making it a viable and effective treatment option for patients with large unresectable HCC.
Combination of transcatheter arterial chemoembolization and radiofrequency ablation in hepatocellular carcinoma treatment

Abstract:Objective To investigate the safety, feasibility, and efficacy of transcatheter arterial chemoembolization (TACE) plus radiofrequency ablation (RFA) in hepatocellular carcinoma (HCC) treatment, and to analyze the influencing factors of local efficacy. Methods From August 2011 to June 2013, a series of 10 HCC patients with 12 liver lesions were performed TACE plus RFA. After follow-up of at least 6 months, the efficacy and influencing factors were analyzed. Results A total of 19 TACE and 10 RFA were performed, with technical success rate of 100%. The main complications were bleeding and pain, with rate of 10%and 20%, respectively. The hospital stay were (3.0±1.3) d. After follow-up of (18.0±8.4) months, the local control rate of target lesions was 91.7% (11/12), and the time to local progression was (17.0±9.3) months. Partial of the lesions shrank after TACE. Lesions with good intake of lipiodol, especially in periphery, led to easier identification of the lesion positioning and size, and contributed to the complete ablation followed. After RFA, the lesion size increased at the beginning, then shrank, with significant shrinkage 6-month postablation compared to 1-month postablation. With TACE plus RFA, there was no significant difference in local control between lesions small than 3 cm and 3-5 cm(P>0.05). For lesions smaller than 5 cm, the difference in local control between the perivascular and nonperivascular tumor was not significant(P>0.05). Conclusion The combination of TACE and RFA in hepatocellular carcinoma treatment yields high safety, feasibility and local control, even in the lesions between 3 cm and 5 cm.

Abstract:BACKGROUND:Recent studies suggest that a combination of radiofrequency ablation (RFA) and transarterial chemoembolization (TACE) may have theoretical advantages over TACE alone for treatment of hepatocellular carcinoma (HCC). The purpose of this study was to evaluate the effectiveness and safety of radiofrequency ablation following first-line TACE treatment in the management of HCC beyond the Milan Criteria.METHODS:Forty-five patients who consecutively underwent RFA following first-line TACE treatment for HCC beyond the Milan criteria were enrolled in this study. RFA was performed within 1-2 months after TACE treatment in patients who had incomplete necrotic tumor nodules. Primary effectiveness, complications, survival rates, and prognostic factors were evaluated retrospectively.RESULTS:Complete ablation was achieved in 76.2% of the lesions according to 1-month follow-up computed tomography/magnetic resonance imaging evaluation. The mean follow-up period was 30.9 months (range 3-94 months). There were no major complications after RFA therapy. The median overall survival was 29 months (range 20-38 months), with 1-, 2-, and 3-year survival of 89%, 61%, and 43%, respectively. Multivariate analysis revealed that tumor diameter (P = 0.045, hazard ratio [HR] = 0.228, 95% confidence interval [CI]: 0.054-0.968) and pretreatment serum alpha-fetoprotein level (P = 0.024, HR = 2.239, 95% CI: 1.114-4.500) were independent predictors for long-term survival.CONCLUSIONS:HCC beyond the Milan criteria can be completely and safely ablated by radiofrequency ablation following first-line TACE treatment with a low rate of complications and favorable survival outcome. Further assessment of the survival benefits of combination treatment for HCCs beyond the Milan Criteria is warranted.
Transradial versus transfemoral access for liver cancer patients undergoing hepatic arterial infusion chemotherapy: Patient experience and procedural complications

Abstract:PURPOSE: To determine whether transradial access (TRA) is a more favorable and safe method for hepatic arterial infusion chemotherapy (HAIC) than transfemoral access (TFA).MATERIALS AND METHODS: Retrospective and prospective cohorts of patients with liver cancer were included. Sixty-seven patients in the retrospective cohort were divided into 2 groups: (a) TRA-HAIC (n = 24) and (b) TFA-HAIC (n = 43). Another 33 patients were prospectively enrolled to receive both TRA and TFA for HAIC in a crossover design. Prolonged arterial access was required for up to 48 hours. The primary endpoint was quality of life (QOL) using the visual analog scale. The secondary endpoints mainly included procedural success, adverse events, and operation time.RESULTS: Patient QOL measures revealed significantly lower scores of indices in the TRA-HAIC group than in the TFA-HAIC group in the retrospective cohort (all P < .001). The significant improvement of the QOL indices by TRA-HAIC, such as overall discomfort (P = .019) and pain at the access site (P = .018), was validated in the prospective cohort. The satisfaction scores were significantly higher in the TRA-HAIC group than in the TFA-HAIC group, and patients preferred TRA-HAIC (P < .001). Radial artery occlusion (RAO) as an access-related adverse event occurred more frequently in both the retrospective and prospective cohorts (38% and 33%, P < .001 and P = .001, respectively). Notably, the multivariate analysis of RAO-associated factors showed that enoxaparin use was significantly correlated with a reduced risk of postprocedural RAO (P = .036).CONCLUSIONS: TRA was superior to TFA in patient experience. However, because of the high incidence of access-related adverse events, especially for RAO with a total incidence of 35%, strategies should be optimized for patients to benefit from TRA in future procedures.

Emergency Transarterial Chemoembolization for Spontaneous Rupture and Intraperitoneal Hemorrhage of Primary Liver Cancer

Abstract:Objective:To study the clinical efficacy of emergency transarterial chemoembolization(TACE) in patients with spontaneous rupture and hemorrhage of primary liver cancer.Methods: From March 2001 to March 2008,40 patients diagnosed with primary liver cancer and spontaneous rupture and intraperitoneal hemorrhage underwent emergency TACE in our department.The post-TACE hepatic hemostasis and tumor resectability were analyzed.The prognosis of post-TACE liver resection group and non-resection group was compared.Results: The successful hemostasis was achieved in 82.5%(33/40) cases.The tumor resectability after TACE treatment was 30%(12/40).1-and 3-year survival rate in liver resection group and non-resection group were 75.0%,42.9% and 50.1%,0.0%,respectively(P0.05).Conclusions: Emergency TACE for hepatic hemostasis is effective in patients with rupture and intraperitoneal hemorrhage of primary liver cancer.The long-term prognosis of patients underwent subsequently liver resection is satisfactory.
Clinical Application of Microcatheter in Transcatheter Arterial Chemoembolization Therapy for Primary Liver Cancer

Abstract:Objective To evaluate 3.0F microcatheter in clinical transcatheter arterial chemoembolization therapy (TACE) for primary liver cancer, and to discuss the manipulative technique and indications of using microcatheter. Materials and Methods Using emulsion of Lipiodol with antineoplastic medicine as embolic agents, TACE, using a 3.0F SP microcatheter, was performed in 32 patients of primary liver cancer. The reason to use a microcatheter in this group was as follows. (1) The blood supply of the tumor was from extrahepatic collaterals (n=9). (2) The tumor feeding artery was so small that super selective catheterization was needed (n=16). (3) Anatomic variations of celiac and common hepatic artery (n=6). (4) hepatic artery spasm (n=1).Results The microcatheter was used 42 times in 32 patients. The catheterization was successful in 39 procedures (92.9%) and failure in 3. After therapy, serum AFP reduction over 50% was noted in 27 patients (84.4%), while tumor size reduced over 50% in 8 (25%) and less than 50% in 16 (50%). Successful internal sub segmental hepatectomy was carried out in 2 patients. No serious complication occurred in all patients.Conclusion The manipulation of microcatheter is simple and easy, with a high successful rate. The microcatheter technique has great value in TACE therapy for primary liver cancer, especially for patients with small, severely narrowed, tortuous or easily spastic tumor feeding arteries.

Abstract:Background: Hepatocellular carcinoma (HCC) is one of the most common cancers worldwide. Monotherapy is not very effective for intermediate or advanced stage HCC. Efficacy of combined therapy using transarterial chemoembolization (TACE) with three-dimensional conformal radiotherapy (3-DCRT) for advanced HCC should be evaluated.Methods: HCC patients were selected from our patient database. The sequence of treatments that patients underwent was several courses of TACE followed in 2-4 weeks by 3-DCRT. The median tumor irradiation dose was 44Gy. Toxicity, tumor response, and overall survival rate were analyzed.Results: 140 HCC patients were followed up by the last follow-up time. Among these patients, hepatic toxicities due to treatment were notable in 15 cases. Gastrointestinal bleeding after the overall treatment occurred in 3 cases. Leukopenia of grade III was detected in 1 case. Radiation-induced liver disease (RILD) was observed in 3 patients. Among 140 patients, 27, 97, and 16 cases achieved partial response, stable disease, and progressive disease, respectively. The overall survival rates of 1-year, 3-years, and 5-years were 66%, 29%, and 13%, respectively, with a median survival time of 18 months. Both Child-Pugh grade and radiation dose were determined to be independent predictors for overall survival from multivariate analysis.Conclusion: The combined modality of TACE and 3-DCRT is a promising treatment for unresectable HCC. A large-scale, prospective randomized trial should be performed to confirm the utility of this combined therapy. (C) 2010 Elsevier Ltd. All rights reserved.
