Air Medical Evacuation of Nepalese Citizen During Epidemic of COVID-19 from Wuhan to Nepal

Abstract:In December 2019, the world was disrupted by the news of a new strain of virus known as Novel Corona virus, taking lives of many in China. Wuhan, the capital of Central China's Hubei province is said to be the place where the outbreak started. The city went on a lockdown as the disease spread rapidly. After the lockdown, most countries like India and Bangladesh airlifted their citizens who were studying in Wuhan. Similarly, Nepal also has many youth studying medicine in Wuhan. Pleas for help from the students reached the government. This was a first encounter of such experience for Nepal government. With the help of Health Emergency Organizing committee, Epidemiology and Disease Control Division, Nepal Army Hospital, Nepal Police Hospital, Waste Management team, Nepal Ambulance service, Tribhuwan Airport and Royal Airlines the government of Nepal planned, organized and successfully brought back all the 175 students on 15 the February, 2019 from Wuhan, China. The aim of the present article is to share the experience, the challenges faced and recommendations for future similar cases. Keywords: evacuation; Nepal; Novel Corona virus; Wuhan.

Abstract:The emerging outbreak of the 2019 novel coronavirus (2019-nCoV) originated from Wuhan poses a great challenge to healthcare system in China.[1][1] Primary care practitioners (PCPs) have an important role in district communicable disease control.[2][2] However, because primary health-care system in China still needs to be substantially strengthened,[3][3],[4][4] whether PCPs are proactive and capable in responding to the outbreak remains unclear. Abstract:Protecting the citizens who are living abroad has been a challenge for many countries during the current COVID-19 pandemic. In Bhutan, since the start of the pandemic, it has repatriated its citizens living and working abroad and putting them into a 21-day state-sponsored quarantine. They were also tested for COVID-19 on reverse transcription polymerase chain reaction (RT-PCR) that was funded by the government. While this measure is a blessing for its people, for the government it is a huge economic challenge.

Abstract:The COVID-19 pandemic is continuing to ravage countries across the world. It has displaced families and impacted economies around the world. Our fight against the pandemic is never ending, but COVID-19 vaccines offer unique promises and opportunities to win this war. As more and more vaccines are approved for use, many countries including Bhutan have started vaccinating their population against the SARS-CoV2 disease. Bhutan has fully vaccinated 61.5% of its people and 11.2% partially including children aged between 12 and 17 years. The country used many strategies to increase vaccine accessibility and availability such as government's exhaustive efforts to secure enough vaccine doses, careful preparations, use of international partners, and integration of its tradition and culture for wider vaccine acceptance. The protection from vaccines coupled with other public health measures has helped keep the number of COVID-19 cases to a manageable number without straining its health system too much. The success story is a "beacon of hope for the region" as the world looks to rebuild itself from this long exhausting fight against the COVID-19 pandemic.

Abstract:In March 2020, the country saw its first case when a foreign tourist tested positive for COVID-19 at Thimphu. There was immediate activation of national preparedness and response plan including the requirements for surge in human resource capacity to handle the novel infection. One key emphasis of the national response was to support the existing 3500 health care workforce by partnering with community in sharing pandemic-related activities. One such initiative was training a pool of people to shoulder basic responsibilities at quarantine and non-hospital isolation facilities. Therefore, Khesar Gyalpo University of medical Sciences of Bhutan was tasked to develop a training program and thus the De-suup plus program was designed with focus on people from non-medical background. This article attempts to describe the development and implementation of the training. The training constituted basic nursing competencies, personal protective equipment and essential public health preventive measures with the purpose of serving at quarantine and non-hospital isolation facilities in the Country.

Employing medical anthropology approach as an additional public health strategy in promoting COVID-19 vaccine acceptance in Bhutan

Abstract:Bhutan has been commended for their COVID-19 vaccination campaign, in which the country became the fastest in the world to inoculate nearly all of its adult population in just a few weeks. As a profoundly religious Buddhist kingdom in South Asia, the country successfully utilized a very unique strategy in promoting COVID-19 vaccine acceptance nationwide. Bhutan incorporated religion, culture, and tradition in their campaign wherein the royal government collaborated with the Zhung Dratsang, the country's central monastic body, in deciding the most auspicious day for the launch of their vaccination rollout by avoiding the one-month long Dana, selecting the first person to be vaccinated in accordance to the Buddhist astrology, and leading the country in chanting the Sangay Menlha, a mantra believed by many as a powerful prayer in warding off diseases. The launch turned out as a feast for all Bhutanese since it seemingly marked the beginning of the last phase of their COVID-19 struggle. Incorporating medical anthropology in public health approaches can greatly influence the public, especially when the people are deeply rooted in folk beliefs. This unique strategy can be emulated by many localities with strong influence of cultural traditions and folk beliefs.

Abstract:Background Coronavirus Disease’s (COVID-19) rampant rise has caused numerous challenges to the already strained health infrastructure of Nepal. The combination of underlying geographic and socioeconomic risk factors, and Nepal’s unique challenges with its rural majority population, the influx of migrant workers returning during the pandemic and catastrophic monsoon flooding has potentiated disease spread. This review describes the country’s health sector response, key strategies, challenges and strengths, and provides lessons learned for future response. Methods Open access information from the Nepali government was used to describe the COVID-19 response plan and implementation. The World Health Organization’s weekly situation reports, United Nations documents, and local news articles were also reviewed to find key evolution of COVID-19 and challenges faced in response. Findings Nepal’s COVID-19 response strategy was detailed in the government’s Health Sector Emergency Response Plan (HSER) in May 2020, and the strategy can be divided into four main areas: public health and social measures, hospital-based intervention, management and oversight, and budget allocation. Nepal has had COVID-19 cases in all 7 provinces and 77 districts, and due to a large rural population, the sudden influx of more than 1.3 million people from reverse migration and massive monsoon floods contributed to the rise in community spread of the virus. The second wave (Delta wave) found Nepal with the worst viral replication in the world and collapsed the health infrastructure. Conclusion Nepal’s COVID-19 response was strained by its unique challenges and key lessons were identified from this review that will be needed for future pandemic and disaster response. These include early planning, data centralization, transparent leadership, clear communication and coordination between three tiers of government, private-public partnerships, and utilization of its working population.

Abstract:Abstract Noble CORONA Virus (COVID-19) is an infectious disease similar form of pneumonia/ SARS-CoV-2- impacting globally. The fear of coronavirus looks pandemic, but its severity is uncertain. Nepal was one of the first nine countries outside of China to report a COVID-19 case. Also, its unpredictability of mode or range of surface, the lifespan of the virus, objects of transmission (a distance of air/air currents, living duration in air, humidity, duration on objects, surface). The first case was found in Wuhan in December 2019 in China. The purpose is to summarize the current information about COVID-19 and to explore in terms of why Nepal is not hitting severely, while other countries are on death toll? We summarized the published articles form the web sources and news, Academic Journals, Ministry of health and population Nepal, WHO/CDC update reports/guidelines, Google search engine. Thematic analysis is made to explore the situation. Although, Nepal has a lack of health services, testing kits, advance lab and protecting equipment (PPE), why COVID-19 does not hit Nepal than China, Europe and North America, it still tremendous uncertainty. Is lockdown, isolation, social distance and quarantine the best ways of prevention? The hypothesis is floating globally – do BCG vaccinated countries are safer than non-user OR due to not having enough kits to screen populations at risk for the virus – while lack of testing a big cause for missing case OR Nepalese have better immune systems? It has attracted global attention. We believe that the COVID-19 is still evolving and it is too early to predict of an outbreak in Nepal. The government needs to increase funding for local health departments, begin planning for future epidemics and be prepared to bolster the economy by supporting consumer spending the midst of a serious outbreak. COVID-19 is a serious health challenge for Nepal, but so far the number of death has been lower than was foretell. It is, therefore essential to carry out more scientific evidence to explore results. Nepalese health services need to maintain up than today and follow lockdown, isolation, social distance and an advance screening test kit through the country.
Retrospective analysis of the clinical features of outpatients with novel coronavirus in areas outside Wuhan, China

Abstract:Abstract Objective: The novel coronavirus pneumonia (COVID-19) has spread rapidly across the globe with the movement of people. How to diagnose COVID-19 quickly and accurately is a concern for all. We retrospectively assessed the clinical characteristics of patients with COVID-19 detected by outpatient screening in areas outside Wuhan, China, to guide early screening outside the epidemic area, to isolate and treat COVID-19-positive patients, and to control the spread of this virus in the region.Results: Among the 213 patients treated in the fever clinic of our hospital, 41 tested positive for novel coronavirus (2019-nCoV) and 172 were negative. Among the positive patients, 13 (31.7%) of the patients had been to Wuhan, while 28 (68.3%) had not been to Wuhan. There were 4 cases of clustering occurrence. The main symptoms exhibited by COVID-19-positive patients were fever (87.8%), cough (68.3%), and expectoration (34.1%). The C-reactive protein (CRP) levels were increased in 35 (85.3%) positive patients; the hydroxybutyrate dehydrogenase in the myocardial zymogram was increased in 22 positive patients (53.6%) and 38 negative patients (22.1%); computed tomography (CT) findings revealed lung lesions in all 41 positive patients (100%).Conclusion: We classified the patient population and analyzed the data to understand the early clinical performance of COVID-19. Our research illustrate that screening for COVID-19 outside Wuhan should focus on early symptoms such as fever and cough, in combination with lung CT findings, epidemiological history, and sputum pathogen detection to determine whether patients need further isolation.

Abstract:Background: COVID-19, like every other pandemic, has imposed an unprecedented threat to doctors' physical and mental health. Literature in this area is sparse. The present study has been done to explore the knowledge, attitude, and behavior of doctors regarding this pandemic and how it influences their depression, anxiety, and stress level. Materials and Methods: This online survey has been done for 10 days. Data were collected on background characteristics, knowledge, attitude, and behavior of the respondents in a semi-structured pro forma, and psychiatric morbidity was measured by the Depression, Anxiety, and Stress Scale-21. A total of 152 complete responses have been received. The data were assessed using SPSS software. Results: Out of 152 study participants, 34.9% were depressed and 39.5% and 32.9% were having anxiety and stress, respectively. Significant predictors for psychiatric morbidities were experience in health sector, duty hours, use of protective measures, and altruistic coping. Multivariable logistic regression showed most of the factors to be significantly associated with depression, anxiety, and stress level. Discussion: Doctors who were working during COVID pandemic have a high prevalence of psychiatric morbidity. Age and having multiple comorbidities are significant predictive factors. Adequate protective measures should be warranted. Altruistic coping and a sense of greater goal are significant among the doctor community, in this pressing time. The doctors are pushing themselves to the best of their capacity and also protecting their patients' best interest. A large-scale, multicentric study will probably give a larger picture and will guide us for better service planning and delivery.

Abstract:Background: Coronavirus disease which has become a pandemic has also impacted cancer service to a small country like Nepal which has very low resources. Aims and Objectives: Covid infection itself and the lockdown implemented by the government resulted negative impact in our cancer management. Hence the aim of the study was to delineate the impact of Covid in cancer management. Materials and Methods: We did a prospective descriptive study from 1st March 2020 till 1st August 2020 in our medical oncology department at Birat Medical College and Teaching Hospital (BMCTH). Results: We had 60% reduction in the newly diagnosed cases, 40% decrement in the chemotherapy admissions and total fourteen mortalities. PCR test was advised for all the COVID suspects with features of fever and separate observation ward was started for such patients. Delayed in the cancer diagnosis and treatment could result in increase in the advanced cancer burden which can overwhelm the health service in Nepal. Conclusion: COVID 19 had negative impact on cancer treatment and management at our institution.

Abstract:<p>The Coronavirus-19 (COVID-19) hasn't seen the dawn since its emergence, however waxing and waning has resulted in the emergence of deadly variants. The effects of pandemic have not been limited to its virulence, but have rather conferred multiple collateral effects, especially in developing countries; thereby, designating it as a SYNDEMIC. The same culminated in neglect of non-COVID-19 conditions like tuberculosis (TB) and human immunodeficiency virus-acquired immunodeficiency syndrome (HIV/AIDS). Besides being the prognostic factor for severe COVID-19, these infections in hidden pockets served as reservoir for emergence of the deadly Omicron. Another significant impact of this juxtaposition was on the delivery of healthcare services for TB and HIV. <p> The unanticipated COVID-19 pandemic turned the path of ongoing progress of elimination programs. Direct consequences of the COVID-19 pandemic were pronounced on diagnosis, treatment, and services for patients with TB and HIV. Essential TB services were reallocated to the COVID-19 rapid response task force. However, despite escalating the tribulations, this triple burden has simultaneously taught lessons to escalate the progress of halted programs. The pandemic has catalyzed an unusual level of collaboration among scientists, which can be exploited for TB and HIV. Fast-track diagnostics, digitalization, contact tracing, and vaccine development have enabled world to envision the same for TB/HIV.</p>.

Abstract:Background and aim: The coronavirus disease 2019 pandemic has brought deteriorating physical and mental burdens to health care workers (HCWs) in Indonesia, mainly attributed to the lack of protection and screening among HCWs, patients' concealment of their travel and medical history, and perceived social stigma and discrimination. Hence, we deliver our perspectives and recommendations based on the current situation in Indonesia to enforce their safeties. We encourage stakeholders to implement a systematic approach by employing stringent prevention strategies, ensuring adequate personal protective equipment (PPE) provision and equitable PPE distribution, and routine HCWs screening to prevent nosocomial clusters, in addition to the provision of psychosocial support to HCWs by offering social aids and psychological sessions. Furthermore, social stigma and discrimination toward HCWs and patients should also be addressed and mitigated, thus preventing concealments of patients' history and alleviating emotional burdens. We believe that providing continuous support to HCWs would lead to key benefits in ensuring a winning battle against the COVID-19 pandemic. Relevance for patients: HCWs are pivotal players in winning the battle against the COVID- 19 pandemic. Ensuring their safety and well-being will enable them to deliver better healthcare services, thus resulting in mutual benefit for themselves, the patients, and the nation's recovery.

Abstract:Many popular tourist attractions and trekking routes in Bhutan and Nepal are situated between 3000 and 6000 m in elevation. High-altitude emergencies are becoming more common and medical providers must be aware of the practical and medical issues in managing these disorders. We reflect on the challenges in providing high-altitude emergency medical services in Bhutan and Nepal.

Abstract:COVID-19 has resulted in large number of mortalities across the globe. However, Cambodia has recorded low number of COVID-19 cases with no death. A number of factors buttress the accuracy of this phenomenon such as significant support from international health partners, culture of wearing a face mask when sick, timely response of Cambodia's neighbouring countries, and the compliance of the general public to the restrictions. Cambodia started to take stringent measures and augmented efforts to initiate policies and plans to curb the spread of the virus, including but not limited to: closure of inbound and outbound borders, shutting down of schools, and banning religious activities, gatherings and meetings, with more than 50 people. Another source of success of Cambodia is extensive mass testing, complemented with contact tracing. A strategy called "box in" the virus was introduced. Healthcare workers were trained to help in contact tracing and detection at the community level. Measures enacted so far has helped Cambodia control the pandemic. Other countries could adopt and adapt to the policies and best practices of Cambodia. However, possibilities of new waves of the pandemic may affect the country, thus, the Cambodian government needs to be cautious when lifting restrictions to avoid explosion of new cases.
