The Role of Internet Hospitals during the COVID-19 Era in China：a retrospective, cross-sectional study (Preprint)

Abstract:BACKGROUND The rapid development of internet hospitals in China has successfully contributed to reduce potential disease spread and avoid overloading of healthcare system during the COVID-19 era. The epidemic also triggered us to develop a unique research on human online medical behaviors under strict quarantine policy. Long-term obveration was followed to clarify target hospital’s online health service capacity, and capture user demand fluctuations and trends. OBJECTIVE To provide implications for the construction of Internet hospitals in the COVID-19 era. METHODS We collected data from The Second Affiliated Hospital of Zhejiang University School of Medicine (SAHZU) internet hospital from November 1st, 2019, to September 16th, 2021. Over 900,000 users were analyzed with respect to the demographic characteristics, distributions of departments and user selections, vibration of registrations and consultations, and consulting behaviors. Interrupted time series (ITS) analysis was adapted to evaluate the impact of this momentous emergency event and its long-term trends. Three investigations were conducted synchronously to determine users’ authentic demands and medical appeals on online hospitals. RESULTS 1. The general portrait of internet hospital users is young or middle-aged women, located in Zhejiang and surrounding provinces. 2. The ITS model indicated that after the intervention (the social distancing policy) was implemented in outbreak period, the number of users significantly increased (β_2=105.78, P<0.001). And long-term waves of COVID-19 lead to the increasing number of online users post-pandemic period (β_3=0.167, P<0.001). 3. We summarized eight major demands at internet hospital during the national shutdown period and afterwards. Online consultation and information service persisted to be the universal demands. Users’ medical behavior patterns changed from onsite to online as improved demand. Concerns about medical safety and quality, with timing and cost comes next. CONCLUSIONS The epidemic has spawned the explosive growth of tele-health and this trend is irreversible. As we shared the practical examples of one public Internet hospitals in China, we put forward suggestions on the future direction of tele-health. Vital experience in the construction of Internet hospital were provided in the normalization of COVID-19 prevention and control, which can be demonstrated as a model of Internet hospital management practice for different medical institutions. Abstract:To the Editor:Since December 2019, the outbreak of the novel coronavirus SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) that originated in Wuhan, Hubei province has spread rapidly across China. Many hospitals in Wuhan are the epicenter of the outbreak. The first 14 staff members who were infected with SARS-Cov-2 were from our department. All of them recovered. In such an urgent and unexpected situation, our department began immediate and effective prevention and control strategies to stop the spread of the epidemic in the department. We believe that clinical departments, especially those related to non-infectious diseases in geographic areas of high risk for virus transmission, should take appropriate management and control measures to improve safety during this epidemic. For this purpose, we summarize and share our experiences which should help medical staff prepare in advance for a similar situation. These include the characteristics of SARS-Cov-2 infection, principles of prevention and control of infection, management of infected patients, and epidemic prevention in the outpatient department, ward, operating room, and medical staff.

Abstract:Abstract Objective: The novel coronavirus pneumonia (COVID-19) has spread rapidly across the globe with the movement of people. How to diagnose COVID-19 quickly and accurately is a concern for all. We retrospectively assessed the clinical characteristics of patients with COVID-19 detected by outpatient screening in areas outside Wuhan, China, to guide early screening outside the epidemic area, to isolate and treat COVID-19-positive patients, and to control the spread of this virus in the region.Results: Among the 213 patients treated in the fever clinic of our hospital, 41 tested positive for novel coronavirus (2019-nCoV) and 172 were negative. Among the positive patients, 13 (31.7%) of the patients had been to Wuhan, while 28 (68.3%) had not been to Wuhan. There were 4 cases of clustering occurrence. The main symptoms exhibited by COVID-19-positive patients were fever (87.8%), cough (68.3%), and expectoration (34.1%). The C-reactive protein (CRP) levels were increased in 35 (85.3%) positive patients; the hydroxybutyrate dehydrogenase in the myocardial zymogram was increased in 22 positive patients (53.6%) and 38 negative patients (22.1%); computed tomography (CT) findings revealed lung lesions in all 41 positive patients (100%).Conclusion: We classified the patient population and analyzed the data to understand the early clinical performance of COVID-19. Our research illustrate that screening for COVID-19 outside Wuhan should focus on early symptoms such as fever and cough, in combination with lung CT findings, epidemiological history, and sputum pathogen detection to determine whether patients need further isolation.

Abstract:Coronavirus disease 2019 (COVID-19) has had a tremendous impact in China and abroad since its onset in December 2019 and poses a major threat to human health. Healthcare workers (HCWs) are at the forefront of the response to outbreaks. This study reviewed literature data and found that HCWs were at high risk of infection during the COVID-19 pandemic, especially at the early stage of the epidemic, and many factors greatly affected their occupational safety. Although SARS-CoV-2 transmission was controlled in China, the Chinese experience can help protect HCWs from COVID-19 and other respiratory diseases.

Abstract:Abstract Background As COVID-19 has become a global pandemic, early prevention and control of the epidemic is extremely important. Telemedicine, which includes medical advice given over telephone, Internet, mobile phone applications or other similar ways, may be an efficient way to reduce transmission and pressure on medical institutions. Methods We searched MEDLINE, Web of science, Embase, Cochrane, CBM, CNKI and Wanfang databases for literature on the use of telemedicine for COVID-19, SARS and MERS. from their inception to March 31st, 2020. We included studies about the content of the consultation (such as symptoms, therapy and prevention, policy, public service), screening of suspected cases, the provision of advice given to those people who may have symptoms or contact history. We conducted meta-analyses on the main outcomes of the studies. Results A total of 2041 articles were identified after removing duplicates. After reading the full texts, we finally included nine studies. People were most concerned about symptoms (64.2%), epidemic situation and public problems (14.5%), and psychological problems (10.3%) during COVID-19 epidemic. During the SARS epidemic, the proportions of people asking for consultation for symptoms, prevention and therapy, and psychological problems were 35.0%, 22.0%, and 23.0%, respectively. Two studies demonstrated that telemedicine can be used to screen the suspected patients and give advice. One study emphasized the limited possibilities to follow up people calling hotlines and difficulties in identifying all suspect cases. Conclusions Telemedicine services should focus on the issues that the public is most concerned about, such as then symptoms, prevention and treatment of the disease, and provide reasonable advice to patients with symptoms or people with epidemic history.

Abstract:Hospitals are suffering from a critical challenge induced by the rapid spread of Coronavirus disease 2019 (COVID-19). Not only have patients been marginalized, but many clinicians working in the region-al hospitals have limited access to the specialist consultations and treatment guidelines they need from provincial-level hospitals to manage pneumonia cases caused by COVID-19. Telemedicine has been acknowledged as a breakthrough technology in combating epidemics. This study aims to demonstrate the implementation of Emergency Telemedicine Consultation System (ETCS) since COVID-2019 first emerged in Henan Province, beginning in late December 2019. This system was developed for coronavirus care across 126 connected hospitals, serving as the overarching authoritative source for diagnostic decision making and knowledge sharing for treatment. The information shared could rapidly expand to enable open collaborations with key stakeholders such as government authorities, research institutions and laboratories. The experience from building this system during this crisis can provide insights to guide public health institutions as they implement telemedicine to increase resilience to future epidemic outbreaks.

Abstract:Coronavirus disease (COVID-19) is caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that is rapidly spreading across the globe. The clinical spectrum of SARS-CoV-2 pneumonia ranges from mild to critically ill cases and requires early detection and monitoring, within a clinical environment for critical cases and remotely for mild cases. The fear of contamination in clinical environments has led to a dramatic reduction in on-site referrals for routine care. There has also been a perceived need to continuously monitor non-severe COVID- 19 patients, either from their quarantine site at home, or dedicated quarantine locations (e.g., hotels). Thus, the pandemic has driven incentives to innovate and enhance or create new routes for providing healthcare services at distance. In particular, this has created a dramatic impetus to find innovative ways to remotely and effectively monitor patient health status. In this paper we present a short review of remote health monitoring initiatives taken in 19 states during the time of the pandemic. We emphasize in the discussion particular aspects that are common ground for the reviewed states, in particular the future impact of the pandemic on remote health monitoring and consideration on data privacy.

Abstract:Coronavirus disease 2019 (COVID-19) has emerged as a pandemic with serious clinical manifestations including death. A pandemic at the large-scale like COVID-19 places extraordinary demands on the world's health systems, dramatically devastates vulnerable populations, and critically threatens the global communities in an unprecedented way. While tremendous efforts at the frontline are placed on detecting the virus, providing treatments and developing vaccines, it is also critically important to examine the technologies and systems for tackling disease emergence, arresting its spread and especially the strategy for diseases prevention. The objective of this article is to review enabling technologies and systems with various application scenarios for handling the COVID-19 crisis. The article will focus specifically on 1) wearable devices suitable for monitoring the populations at risk and those in quarantine, both for evaluating the health status of caregivers and management personnel, and for facilitating triage processes for admission to hospitals; 2) unobtrusive sensing systems for detecting the disease and for monitoring patients with relatively mild symptoms whose clinical situation could suddenly worsen in improvised hospitals; and 3) telehealth technologies for the remote monitoring and diagnosis of COVID-19 and related diseases. Finally, further challenges and opportunities for future directions of development are highlighted.

Abstract:Since the first outbreak of the coronavirus disease (COVID-19) in the winter of 2019, this pandemic has resulted in significant global social and economic disruption, as well as global public health and medical crises[1]. Climate, weather conditions, and even latitude have now been acknowledged to be directly associated with COVID-19 outbreaks[2-4]. It was reported that 60％ of the confirmed cases of COVID-19 occurred in areas where the air temperature ranged from 5℃ to 15℃[4]. These earlier research results strongly implied that the COVID-19 pandemic might spread cyclically and outbreaks might recur in large cities in the mid-latitudes (for example, the northern part of China) in autumn 2020[4], which has now been confirmed by the third wave of the pandemic. With quarantines and other restrictive legislations and control measures being put in action to contain the spread of the disease[5], the pandemic has also manifested additional obstacles in the treatment of other diseases. For example, a median dose delay of 14 (6-20) days was found in patients who received one cycle of gastric chemotherapy in our department from the first outbreak till the spring festival of 2020 (n = 79).

Abstract:The outbreak of coronavirus disease 2019 (COVID-19) has spread rapidly around the world. As of May 30, 2020, a total of 84 568 confirmed COVID-19 cases have been recorded in China, with a mortality rate of approximately 5.5%. Taizhou is a prefecture-level city in Zhejiang Province. A total of 146 cases were diagnosed in this epidemic, with a fatality rate of 0%. This condition is due to the establishment of an "Internet +" diagnosis and treatment model based on online medical application (APP), telemedicine, WeChat service, and consultation hotline in Taizhou. Taizhou led in opening the "COVID-19 Prevention and Treatment Special Line" in China, which is conducive to pre-hospital screening, suppressing social panic, and clinical support. Hospitals also carried out related online lectures and popularization of science. We summarize Taizhou's COVID-19 prevention and control experience with telemedicine features, with a view to providing reference for the control of the epidemic at home and abroad.

Abstract:In the year 2020, the coronavirus disease 2019 (COVID-19) crisis intersected with the development and maturation of several digital technologies including the internet of things (IoT) with next-generation 5G networks, artificial intelligence (AI) that uses deep learning, big data analytics, and blockchain and robotic technology, which has resulted in an unprecedented opportunity for the progress of telemedicine. Digital technology-based telemedicine platform has currently been established in many countries, incorporated into clinical workflow with four modes, including “many to one” mode, “one to many” mode, “consultation” mode, and “practical operation” mode, and has shown to be feasible, effective, and efficient in sharing epidemiological data, enabling direct interactions among healthcare providers or patients across distance, minimizing the risk of disease infection, improving the quality of patient care, and preserving healthcare resources. In this state-of-the-art review, we gain insight into the potential benefits of demonstrating telemedicine in the context of a huge health crisis by summarizing the literature related to the use of digital technologies in telemedicine applications. We also outline several new strategies for supporting the use of telemedicine at scale.

Abstract:COVID-19 is an emerging infectious disease that is extremely contagious and can cause serious consequences and even death. Convalescent plasma, an unregistered therapy, from which the antibodies might suppress virus, has been proven effective in the treatment of SARS, Ebola and H1N1, without severe adverse events(Chen et al. 2020). It is unclear whether it can play a role or not in the emerging infectious disease COVID-19, but it may be a potential therapy (Chen et al. 2020). According to the guidelines for the diagnosis and treatment of novel coronavirus infection, the use of convalescent plasma is recommended in critically ill patients. Social media is a dynamic and interactive computermediated communication tool that has high penetration rates in the general population in both high-income and middle-income countries (Grajales et al. 2014). In health care, social media use allows health information to be shared easily and globally(Katz 2014). To some extent, social media has become an important tool in medicine, and the possible ethical dilemmas in its application have often been discussed (Chretien and Kind 2013; Terrasse et al. 2019; Wardrope and Reuber 2019), but very few cases have shown that it can be used to solve the ethical dilemmas of medicine. In this paper, we share our experience in solving medical ethical dilemmas and legal risks of severe COVID-19. Our experience shows that it is easy to encounter medical ethical dilemmas and legal risks when applying unregistered treatments to critically ill patients. In the context of treating severe COVID-19 with convalescent plasma, we propose a social media-based strategy to address this problem.

Abstract:Coronavirus becomes officially a global pandemic due to the speed spreading off in various countries. An increasing number of infected with this disease causes the Inability problem to fully care in hospitals and afflict many doctors and nurses inside the hospitals. This paper proposes a smart health system that monitors the patients holding the Coronavirus remotely. Due to protect the lives of the health services members (like physicians and nurses) from infection. This smart system observes the people with this disease based on putting many sensors to record many features of their patients in every second. These parameters include measuring the patient's temperature, respiratory rate, pulse rate, blood pressure, and time. The proposed system saves lives and improves making decisions in dangerous cases. It proposes using artificial intelligence and Internet-of-things to make remotely quarantine and develop decisions in various situations. It provides monitoring patients remotely and guarantees giving patients medicines and getting complete health care without anyone getting sick with this disease. It targets two people's slides the most serious medical conditions and infection and the lowest serious medical conditions in their houses. Observing in hospitals for the most serious medical cases that cause infection in thousands of healthcare members so there is a big need to uses it. Other less serious patients slide, this system enables physicians to monitor patients and get the healthcare from patient's houses to save places for the critical cases in hospitals.
