Abstract:Big data has been reported widely to facilitate epidemic prevention and control in health care during the coronavirus disease 2019 (COVID-19) pandemic. However, there is still a lack of practical experience in applying it to hospital prevention and control. This study is devoted to the practical experience of design and implementation as well as the preliminary results of an innovative big data-driven COVID-19 risk personnel screening management system in a hospital. Our screening system integrates data sources in four dimensions, which includes Health Quick Response (QR) code, abroad travelling history, transportation close contact personnel and key surveillance personnel. Its screening targets cover all patients, care partner and staff who come to the hospital. As of November 2021, nearly 690 000 people and 5.79 million person-time had used automated COVID-19 risk screening and monitoring. A total of 10 376 person-time (0.18%) with abnormal QR code were identified, 242 person-time with abroad travelling history were identified, 925 person-time were marked based on the data of key surveillance personnel, no transportation history personnel been reported and no COVID-19 nosocomial infection occurred in the hospital. Through the application of this system, the hospital's expenditure on manpower and material resources for epidemic prevention and control has also been significantly reduced. Collectively, this study has proved to be an effective and efficient model for the use of digital health technology in response to the COVID-19 pandemic. Based on the data from multiple sources, this system has an irreplaceable role in identifying close contacts or suspicious person, and can significantly reduce the social burden caused by COVID-19, especially the human resources and economic costs of hospital prevention and control. It may provide guidance for clinical epidemic prevention and control in hospitals, as well as for future public health emergencies.

Abstract:BACKGROUND The rapid development of internet hospitals in China has successfully contributed to reduce potential disease spread and avoid overloading of healthcare system during the COVID-19 era. The epidemic also triggered us to develop a unique research on human online medical behaviors under strict quarantine policy. Long-term obveration was followed to clarify target hospital’s online health service capacity, and capture user demand fluctuations and trends. OBJECTIVE To provide implications for the construction of Internet hospitals in the COVID-19 era. METHODS We collected data from The Second Affiliated Hospital of Zhejiang University School of Medicine (SAHZU) internet hospital from November 1st, 2019, to September 16th, 2021. Over 900,000 users were analyzed with respect to the demographic characteristics, distributions of departments and user selections, vibration of registrations and consultations, and consulting behaviors. Interrupted time series (ITS) analysis was adapted to evaluate the impact of this momentous emergency event and its long-term trends. Three investigations were conducted synchronously to determine users’ authentic demands and medical appeals on online hospitals. RESULTS 1. The general portrait of internet hospital users is young or middle-aged women, located in Zhejiang and surrounding provinces. 2. The ITS model indicated that after the intervention (the social distancing policy) was implemented in outbreak period, the number of users significantly increased (β_2=105.78, P<0.001). And long-term waves of COVID-19 lead to the increasing number of online users post-pandemic period (β_3=0.167, P<0.001). 3. We summarized eight major demands at internet hospital during the national shutdown period and afterwards. Online consultation and information service persisted to be the universal demands. Users’ medical behavior patterns changed from onsite to online as improved demand. Concerns about medical safety and quality, with timing and cost comes next. CONCLUSIONS The epidemic has spawned the explosive growth of tele-health and this trend is irreversible. As we shared the practical examples of one public Internet hospitals in China, we put forward suggestions on the future direction of tele-health. Vital experience in the construction of Internet hospital were provided in the normalization of COVID-19 prevention and control, which can be demonstrated as a model of Internet hospital management practice for different medical institutions. Abstract:Coronavirus disease (COVID-19) is caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that is rapidly spreading across the globe. The clinical spectrum of SARS-CoV-2 pneumonia ranges from mild to critically ill cases and requires early detection and monitoring, within a clinical environment for critical cases and remotely for mild cases. The fear of contamination in clinical environments has led to a dramatic reduction in on-site referrals for routine care. There has also been a perceived need to continuously monitor non-severe COVID- 19 patients, either from their quarantine site at home, or dedicated quarantine locations (e.g., hotels). Thus, the pandemic has driven incentives to innovate and enhance or create new routes for providing healthcare services at distance. In particular, this has created a dramatic impetus to find innovative ways to remotely and effectively monitor patient health status. In this paper we present a short review of remote health monitoring initiatives taken in 19 states during the time of the pandemic. We emphasize in the discussion particular aspects that are common ground for the reviewed states, in particular the future impact of the pandemic on remote health monitoring and consideration on data privacy.

Abstract:Hospitals are suffering from a critical challenge induced by the rapid spread of Coronavirus disease 2019 (COVID-19). Not only have patients been marginalized, but many clinicians working in the region-al hospitals have limited access to the specialist consultations and treatment guidelines they need from provincial-level hospitals to manage pneumonia cases caused by COVID-19. Telemedicine has been acknowledged as a breakthrough technology in combating epidemics. This study aims to demonstrate the implementation of Emergency Telemedicine Consultation System (ETCS) since COVID-2019 first emerged in Henan Province, beginning in late December 2019. This system was developed for coronavirus care across 126 connected hospitals, serving as the overarching authoritative source for diagnostic decision making and knowledge sharing for treatment. The information shared could rapidly expand to enable open collaborations with key stakeholders such as government authorities, research institutions and laboratories. The experience from building this system during this crisis can provide insights to guide public health institutions as they implement telemedicine to increase resilience to future epidemic outbreaks.

Abstract:Currently, many countries are facing the problems of aging population, serious imbalance of medical resources supply and demand, as well as uneven geographical distribution, resulting in a huge demand for remote e-health. Particularly, with invasions of COVID-19, the health of people and even social stability have been challenged unprecedentedly. To contribute to these urgent problems, this article proposes a general architecture of the remote e-health, where the city hospital provides the technical supports and services for remote hospitals. Meanwhile, 5G technologies supported telemedicine is introduced to satisfy the high-speed transmission of massive multimedia medical data, and further realize the sharing of medical resources. Moreover, to turn passivity into initiative to prevent COVID-19, a broad area epidemic prevention and control scheme is also investigated, especially for the remote areas. We discuss their principles and key features, and foresee the challenges, opportunities, and future research trends. Finally, a node value and content popularity based caching strategy is introduced to provide a preliminary solution of the massive data storage and low-latency transmission.

Abstract:In the year 2020, the coronavirus disease 2019 (COVID-19) crisis intersected with the development and maturation of several digital technologies including the internet of things (IoT) with next-generation 5G networks, artificial intelligence (AI) that uses deep learning, big data analytics, and blockchain and robotic technology, which has resulted in an unprecedented opportunity for the progress of telemedicine. Digital technology-based telemedicine platform has currently been established in many countries, incorporated into clinical workflow with four modes, including “many to one” mode, “one to many” mode, “consultation” mode, and “practical operation” mode, and has shown to be feasible, effective, and efficient in sharing epidemiological data, enabling direct interactions among healthcare providers or patients across distance, minimizing the risk of disease infection, improving the quality of patient care, and preserving healthcare resources. In this state-of-the-art review, we gain insight into the potential benefits of demonstrating telemedicine in the context of a huge health crisis by summarizing the literature related to the use of digital technologies in telemedicine applications. We also outline several new strategies for supporting the use of telemedicine at scale.

Abstract:Abstract The current coronavirus disease 2019 (COVID-19) pandemic has caused significant strain on medical centers resources. Thus, concerns about the reducing and management of COVID-19 are on the rise, as there is need to provide diagnosis, treatment, monitoring, and follow-ups during the pandemic. Therefore, the COVID-19 pandemic has radically and quickly altered how medical practitioners provide care to patients. Medical centers are now responding to COVID-19 through rapid adoption of digital tools and technologies such as telemedicine and virtual care which refer to the delivery of healthcare services digital or at a distance using Information and Communications Technology (ICT) for treatment of patients. Telemedicine is expected to deliver timely care while minimizing exposure to protect medical practitioners and patients. Accordingly, a rapid literature review was conducted, and 35 research studies published from 2019 to May 2020 were employed to provide theoretical and practical evidence on the significance of using telemedicine and virtual care for remote treatment of patients during the COVID-19 pandemic. This article provides practical guide based on how to use telemedicine and virtual care during the COVID-19 pandemic. This study provides implication on the potentials of consolidating virtual care solutions in the near future towards contributing to integrate digital technologies into healthcare.

Abstract:Coronavirus disease 2019 (COVID-19) has accelerated the adoption of telemedicine globally. The current consortium critically examines the telemedicine frameworks, identifies gaps in its implementation and investigates the changes in telemedicine framework/s during COVID-19 across the globe. Streamlining of global public health preparedness framework that is interoperable and allow for collaboration and sharing of resources, in which telemedicine is an integral part of the public health response during outbreaks such as COVID-19, should be pursued. With adequate reinforcement, telemedicine has the potential to act as the "safety-net" of our public health response to an outbreak. Our focus on telemedicine must shift to the developing and under-developing nations, which carry a disproportionate burden of vulnerable communities who are at risk due to COVID-19.

Abstract:Abstract Background As COVID-19 has become a global pandemic, early prevention and control of the epidemic is extremely important. Telemedicine, which includes medical advice given over telephone, Internet, mobile phone applications or other similar ways, may be an efficient way to reduce transmission and pressure on medical institutions. Methods We searched MEDLINE, Web of science, Embase, Cochrane, CBM, CNKI and Wanfang databases for literature on the use of telemedicine for COVID-19, SARS and MERS. from their inception to March 31st, 2020. We included studies about the content of the consultation (such as symptoms, therapy and prevention, policy, public service), screening of suspected cases, the provision of advice given to those people who may have symptoms or contact history. We conducted meta-analyses on the main outcomes of the studies. Results A total of 2041 articles were identified after removing duplicates. After reading the full texts, we finally included nine studies. People were most concerned about symptoms (64.2%), epidemic situation and public problems (14.5%), and psychological problems (10.3%) during COVID-19 epidemic. During the SARS epidemic, the proportions of people asking for consultation for symptoms, prevention and therapy, and psychological problems were 35.0%, 22.0%, and 23.0%, respectively. Two studies demonstrated that telemedicine can be used to screen the suspected patients and give advice. One study emphasized the limited possibilities to follow up people calling hotlines and difficulties in identifying all suspect cases. Conclusions Telemedicine services should focus on the issues that the public is most concerned about, such as then symptoms, prevention and treatment of the disease, and provide reasonable advice to patients with symptoms or people with epidemic history.

Abstract:COVID-19 has highlighted the shortcomings of healthcare systems globally as countries struggle to meet the high demand for patient care. The spread of COVID-19 has resulted in unprecedented circumstances that necessitate a shift towards adopting infrastructure to enable care to be provided virtually. This shift is critical to minimize insufficiencies and maximize the quality of care in healthcare systems. While COVID-19 has dramatically accelerated the adoption of technology into care delivery, ongoing work is needed to ensure that our technology infrastructure provides an environment for safe and effective care delivery.Telemedicine usage has substantially increased over the past decade (1), and many hospital systems have robust telemedicine programs. Yet traditional in-person visits remain the cornerstone of clinical care, despite the fact that a significant amount of these visits, including follow-ups, treatment for minor illnesses, and chronic disease management could be substituted by virtual communication. Telemedicine has previously been identified as particularly important during disasters, due to the inaccessibility of traditional care services. This is especially salient for the COVID-19 pandemic where in-person healthcare visits pose a high risk to exposure (2). Additionally, responses to COVID-19, specifically social isolation and the intensified burden on essential workers, are eliciting detrimental psychological effects on large populations, while simultaneously making mental health resources highly inaccessible (3). Overall, with the increased strain and demand on traditional medical resources, telemedicine has emerged as an essential component of clinical care delivery during the COVID-19 pandemic (4) with many healthcare organizations reporting substantial increases in telemedicine use during COVID-19. For example, NYU saw an increase in non-urgent care virtual visits from a pre-COVID-19 average of 95 daily to 4,209 post COVID-19 expansion (4,330% increase) (5).However, as we continue the shift to telemedicine, new issues unravel that need to be addressed, particularly in regard to technology infrastructure. In the US, the Department of Health and Human Services recently lifted many of the restrictions on communication apps, reducing barriers that previously prevented the use of telemedicine services for individuals. That being said, the substantial information security and privacy concerns surrounding telemedicine cannot be overlooked. For example, Zoom, currently one of the most popular video conferencing platforms, has had a 10-fold increase in usage over just a few months including increasing use in healthcare, leading to several important privacy considerations—outsiders joining video conferences, or inadequate encryption of communications (6), leading to the possibility of eavesdropping.State and federal agencies have warned of increased risk of cyberattacks towards healthcare and public health sector and organizations doing research on COVID-19 (7). Ransomware attacks—a type of cybersecurity threat that involves encrypting data and demanding payment in return for unencrypting the data—have continued unabated during the pandemic, with many targeting hospitals specifically. Recent ransomware attacks have included the Illinois Public Health District website and a medical testing facility in the UK (7). Successful cyberattacks negatively impact hospital operations, delay access to clinical services, and lead to significant economic loss (8, 9), all of which would be particularly devastating to organizations already under unprecedented economic and clinical strain during this pandemic.Therefore, while global healthcare systems should allocate significant resources towards improving telemedicine capabilities, improvements must ensure that the technology delivers care that is both safe and effective. Balancing the significant privacy and information security concerns with the enormous potential benefits of virtual care during this pandemic will remain a vital component to our continuously evolving response to COVID-19. Now more than ever, health care workers and organizations need to follow best practices to reduce cyber incidents.

Abstract:COVID-19 is an emerging infectious disease that is extremely contagious and can cause serious consequences and even death. Convalescent plasma, an unregistered therapy, from which the antibodies might suppress virus, has been proven effective in the treatment of SARS, Ebola and H1N1, without severe adverse events(Chen et al. 2020). It is unclear whether it can play a role or not in the emerging infectious disease COVID-19, but it may be a potential therapy (Chen et al. 2020). According to the guidelines for the diagnosis and treatment of novel coronavirus infection, the use of convalescent plasma is recommended in critically ill patients. Social media is a dynamic and interactive computermediated communication tool that has high penetration rates in the general population in both high-income and middle-income countries (Grajales et al. 2014). In health care, social media use allows health information to be shared easily and globally(Katz 2014). To some extent, social media has become an important tool in medicine, and the possible ethical dilemmas in its application have often been discussed (Chretien and Kind 2013; Terrasse et al. 2019; Wardrope and Reuber 2019), but very few cases have shown that it can be used to solve the ethical dilemmas of medicine. In this paper, we share our experience in solving medical ethical dilemmas and legal risks of severe COVID-19. Our experience shows that it is easy to encounter medical ethical dilemmas and legal risks when applying unregistered treatments to critically ill patients. In the context of treating severe COVID-19 with convalescent plasma, we propose a social media-based strategy to address this problem.

Digital health already has a huge impact on the way doctors work, it is very important what tasks it allows them to take on. There are many repetitive and monotonous tasks that most medical professionals do not like to do. Digital health solutions can make them better, faster and cheaper. These tasks usually do not require any creativity or empathy. The process of digital technologies entering healthcare is more complex than simply saying that artificial intelligence (AI) or robotics will take over medical work and activities.We live in unprecedented times. The new strain of coronavirus is spreading at an alarming rate, so much so that the World Health Organization (WHO) declared a pandemic on March 11. However, this strain of COVID-19, which is thought to have originated either by transfer from a non-human host or evolved into a pathogenic strain in the human population, is very real and causes suffering at every level of society. In particular, its virulent nature puts the elderly and the immunocompromised at higher risk.The development of the pandemic contributes to the widespread use of telemedicine. Prior to the pandemic, only 1 in 10 American patients used telemedicine services, according to a study by J.D. Power. But with COVID-19 the use of telehealth applications is increasing to 158% in the United States [3, 5]. The coronavirus pandemic has caused an explosive increase in the need for digital services, including those related to health and medicine. Many medical organizations (public and private) are starting to use telemedicine platforms and have announced the opening of free remote consultations between patients and doctors on COVID-19.

Abstract:Background Most patients with coronavirus disease (COVID-19) who show mild symptoms are sent home by physicians to recover. However, the condition of some of these patients becomes severe or critical as the disease progresses. Objective The aim of this study was to evaluate a telemedicine model that was developed to address the challenges of treating patients with progressive COVID-19 who are home-quarantined and shortages in the medical workforce. Methods A telemedicine system was developed to continuously monitor the progression of home-quarantined patients with COVID-19. The system was built based on a popular social media smartphone app called WeChat; the app was used to establish two-way communication between a multidisciplinary team consisting of 7 medical workers and 188 home-quarantined individuals (including 74 confirmed patients with COVID-19). The system helped patients self-assess their conditions and update the multidisciplinary team through a telemedicine form stored on a cloud service, based on which the multidisciplinary team made treatment decisions. We evaluated this telemedicine system via a single-center retrospective study conducted at Tongji Hospital in Wuhan, China, in January 2020. Results Among 188 individuals using the telemedicine system, 114 (60.6%) were not infected with COVID-19 and were dismissed. Of the 74 confirmed patients with COVID-19, 26 (35%) recovered during the study period and voluntarily stopped using the system. The remaining 48/76 confirmed patients with COVID-19 (63%) used the system until the end of the study, including 6 patients whose conditions progressed to severe or critical. These 6 patients were admitted to hospital and were stabilized (one received extracorporeal membrane oxygenation support for 17 days). All 74 patients with COVID-19 eventually recovered. Through a comparison of the monitored symptoms between hospitalized and nonhospitalized patients, we found prolonged persistence and deterioration of fever, dyspnea, lack of strength, and muscle soreness to be diagnostic of need for hospitalization. Conclusions By continuously monitoring the changes in several key symptoms, the telemedicine system reduces the risks of delayed hospitalization due to disease progression for patients with COVID-19 quarantined at home. The system uses a set of scales for quarantine management assessment that enables patients to self-assess their conditions. The results are useful for medical staff to identify disease progression and, hence, make appropriate and timely treatment decisions. The system requires few staff to manage a large cohort of patients. In addition, the system can solicit help from recovered but self-quarantined medical workers to alleviate shortages in the medical workforce and free healthy medical workers to fight COVID-19 on the front line. Thus, it optimizes the usage of local medical resources and prevents cross-infections among medical workers and patients.

Abstract:Coronavirus becomes officially a global pandemic due to the speed spreading off in various countries. An increasing number of infected with this disease causes the Inability problem to fully care in hospitals and afflict many doctors and nurses inside the hospitals. This paper proposes a smart health system that monitors the patients holding the Coronavirus remotely. Due to protect the lives of the health services members (like physicians and nurses) from infection. This smart system observes the people with this disease based on putting many sensors to record many features of their patients in every second. These parameters include measuring the patient's temperature, respiratory rate, pulse rate, blood pressure, and time. The proposed system saves lives and improves making decisions in dangerous cases. It proposes using artificial intelligence and Internet-of-things to make remotely quarantine and develop decisions in various situations. It provides monitoring patients remotely and guarantees giving patients medicines and getting complete health care without anyone getting sick with this disease. It targets two people's slides the most serious medical conditions and infection and the lowest serious medical conditions in their houses. Observing in hospitals for the most serious medical cases that cause infection in thousands of healthcare members so there is a big need to uses it. Other less serious patients slide, this system enables physicians to monitor patients and get the healthcare from patient's houses to save places for the critical cases in hospitals.
An Internet-Based Multidisciplinary Online Medical Consultation System to Help Cope with Pediatric Medical Needs During the COVID-19 Outbreak: a Cross-Sectional Study.

Abstract:BACKGROUND:Telemedicine is becoming an emerging innovative supplement to the traditional medical system in China. In the present study, we described the rapid implementation of the telemedicine in pediatrics during the coronavirus disease 2019 (COVID-19) outbreak and explored its value in response to the demand for pediatric medical services during the epidemic.METHODS:An Internet-based platform for multidisciplinary online medical consultation was established on February 8, 2020, by a national children's medical center in China. The medical data of the traditional hospital visits and novel online visits from the first two months (February 8 to March 31) after the establishment of the platform were collected and analyzed. The demographic features, changes in the disease spectrum and characteristics of online medical services were described.RESULTS:The total number of visits was significantly lower (by 75.3%) than that of in the same period in the previous year. The disease spectrum was different with a significant decrease of >95% in the number of patients with infectious or contagious diseases. The online visits accounted for 14.7% of all visits. Online patients came from all over the country, covering 91% of the administrative regions of China, and the largest proportion of inquiries were for infants (33.7%) (P<0.01). The response rate of experts to requests for online consultations was 100%. During the study period, 266 experts from 25 pediatric specialties completed 12,318 effective consultations, proving different degrees of online supplementation for various specialties (4.3-168.4%). Online consultations mainly addressed two types of problems: (I) accessibility of the traditional hospital visits and (II) consultations regarding an illness. Data from pediatric nephrology showed that 96% of the patients with stable chronic kidney disease successfully received follow-up medication adjustment guidance through the online platform.CONCLUSIONS:The traditional hospital medical services have been affected by the COVID-19 outbreak. The multidisciplinary online medical consultation system is a strategy to actively respond to the medical needs of children during unique periods.
