Abstract:Purpose: Pediatric prehospital endotracheal intubation (PHETI) is a difficult and rarely performed procedure that remains the gold standard for prehospital airway management when ventilation and/or anesthesia is required, but high complications rates, including malposition continue to concern. We reviewed the experience in our institution of pediatric intubations with particular emphasis on the position of the endotracheal tube (ETT) tip within the trachea and related complications. Method: Intubated pediatric patients presenting directly from the scene to our level 1 trauma center, between 2006 and 2014, were included in our study. Patient records and radiographs were retrospectively reviewed to identify the ETT tip-to-carina distance and possible intubation-related complications. ETT tips identified beyond the carina on radiographs or by clinical diagnosis were defined as misplaced. Because head movement causes a significant ETT movement within the trachea, which is age related, we also defined ETT tip placement (1) less than 2 cm above the carina in children younger than 8 and (2) less than 3 cm above the carina in children 8 years or older as "near miss" intubations. Results: From a total of 34 cases, ETT misplacement was identified in seven cases. Diagnosis was made radiologically in five cases and clinically in two cases. Four of these patients had left lung atelectasis due to tube misplacement. Tube thoracotomy was performed in two of these patients without concurrent evidence of chest injury. "Near miss" intubations accounted for 7/9 and 9/25 in children <8 years and ≥8 years old, respectively, totaling 16/34, with two of these leading to late displacements. Conclusions: Pediatric endotracheal tube intubation carries a high rate of tube malposition and left lung atelectasis in our experience of pediatric trauma patients, with less than a third of ETTs placed in a safe position.

Abstract:Neonatal intubation is one of the most common but also one of the most dangerous and complicated procedures in neonatological practice, which may cause adverse events, including bradycardia and severe oxygen desaturations. Concerning clinical situations, we review the critical points in the procedure of neonatal intubation. The new technique that recommended in neonatal intubation may improve success rates and decrease morbidity.
Point-of-care ultrasound to confirm endotracheal tube cuff position in relationship to the cricoid in the pediatric population

Abstract:Background: Anatomically, the subglottic area and the cricoid ring are the narrowest portions of the larynx. To limit the potential for damage related to mucosal pressure injuries from the presence of an endotracheal tube, the cuff should be placed below the cricoid in children. Previously, no clinical or imaging method has been used in real time to determine the exact location of the endotracheal tube cuff after endotracheal intubation. Point-of-care ultrasound may provide an option as a safe and rapid means of visualizing the endotracheal tube cuff and its relationship to the cricoid ring thereby achieving ideal endotracheal tube cuff positioning-below the cricoid. Methods: In this prospective, nonrandomized trial, point-of-care ultrasound was used following endotracheal intubation in children to evaluate the position of the endotracheal tube cuff in relationship to the cricoid and tracheal rings. After anesthesia was induced and the trachea was intubated, the endotracheal tube cuff and its position in relation to the cricoid and tracheal rings were identified in the longitudinal plane using point-of-care ultrasound. With the patient's neck in a neutral position, the level of the proximal (cephalad) margin of the saline-filled cuff of the endotracheal tube was identified and recorded in relationship to the cricoid and tracheal rings. The ideal position is defined as the cephalad margin of the endotracheal tube cuff below the level of the cricoid. Results: The study cohort included 80 patients, ranging in age from 1 to 78 months. In all patients, the cuff of the ETT, cricoid, and tracheal rings were identified. The cephalad end of the endotracheal tube cuff was found at the level of the cricoid in 16.3% of patients, at the first tracheal ring in 27.5% of patients, at the second tracheal ring in 23.8% of patients, at the third tracheal ring in 17.5% of patients, and at below the fourth tracheal ring in 15% of patients. Initial endotracheal tube cuff position had no significant association with age, height, weight, endotracheal tube size, and endotracheal tube type. Conclusion: Point-of-care ultrasound provides a rapid and effective means of identifying the position of the endotracheal tube cuff in relationship to the cricoid ring. The technique may have applications in the perioperative arena, emergency departments, and intensive care units.
Design and Superior Performance of a New Endotracheal Tube to Avoid the Asphyxiation of Premature Infants

Abstract:Background Neonatal endotracheal intubation attempts often fail, with failures typically attributable to unintended esophageal intubation, with asphyxia, brief or prolonged, as the consequence. Standard-of-care neonatal endotracheal tubes have changed little over recent decades, even as the gestational age of neonates thought eligible for resuscitation and intensive care has decreased. Methods A new neonatal endotracheal tube was patterned after the soft steering mechanism of a two-string fishing line trocar. The new tube remains patent throughout the intubation for air movement and CO<sub>2</sub> detection and allows for a finger on the intubator's hand to stiffen, curve, and elevate the tip of the tube over the epiglottis and into the trachea without occluding the vision through a laryngoscope. This tube's engineering principles were studied prospectively in a controlled open-label pilot study in premature infants. Infants were observed during 12 intubations in a one-to-one comparison with standard practice. Results The new design in comparison to a conventional neonatal endotracheal tube (CNETT) was found to be superior. The average intubation time (mean 36.6 sec, median 30 sec) was shorter (mean 44.6 sec, median 45 sec) in the new design. Intubation attempts were fewer (0 vs. 3), and unintended esophageal intubations were also fewer (0 vs. 4). Conclusion Tracheal intubation of premature infants with the new soft-steering mechanism endotracheal tube was associated with less asphyxia, fewer intubation attempts, and fewer esophageal intubations.
Evaluation of Endotracheal Intubation with a Flexible Fiberoptic Bronchoscope in Lateral Patient Positioning: A Prospective Randomized Controlled Trial

Abstract:Background: There is an unmet need for a reliable method of airway management for patients in the lateral position. This prospective randomized controlled two-center study was designed to evaluate the feasibility of intubation using a flexible fiberoptic bronchoscope in the lateral position during surgery. Methods: Seventy-two patients scheduled for elective nonobstetric surgery in the lateral decubitus position requiring tracheal intubation under general anesthesia at Lishui Central Hospital of Zhejiang Province and Jiaxing First Hospital of Zhejiang Province from April 1, 2015, to September 30, 2015, were enrolled in this study. Patients were randomly assigned to the supine position group (Group S, n=38) and the lateral position group (Group L, n=34). Experienced anesthetists performed tracheal intubation with a fiberoptic bronchoscope after general anesthesia. The time required for intubation, intubation success rates, and hemodynamic changes was recorded. Between-group differences were assessed using the Student's t-test, Mann-Whitney U-test, or Chi-square test. Results: The median total time to tracheal intubation was significantly longer in Group S (140.0 [135.8, 150.0]s) compared to Group L (33.0[24.0, 38.8]s) (P<0.01). The first-attempt intubation success rate was significantly higher in Group L (97%) compared to Group S (16%). Hemodynamic changes immediately after intubation were more exaggerated in Group S compared to Group L (P=0.02). Conclusion: Endotracheal intubation with a flexible fiberoptic bronchoscope may be an effective and timesaving technique for patients in the lateral position.

Abstract:Airway management is an enormous challenge in the critical care of children. Intubation is one way of saving life in the cases of emergency. In children, the airway has significant variations while the oxygen reserve of kids is low. Thus, the appropriate catheter depth is particularly critical. Shallow catheter placement probably leads to vocal cord injury, accidental extubation, hypoxemia and even death. Over-depth placement (such as endobronchial) may result in atelectasis or traumatic lung injury. This review discusses the characteristics of children's airway and the definition of catheter depth. This paper summarize the characteristics of different localization methods of children's tracheal catheter positioning based on universal literature review. And it also demonstrate the new methods of catheter positioning, advantages and disadvantages of these new methods, and future direction for ultrasound guided catheter placement in children.
Diagnostic accuracy of airway ultrasound in confirming the endotracheal tube depth in critically ill children

Abstract:Background: Chest X-ray, the established standard of confirming endotracheal tube (ETT) position, has important drawbacks including radiation exposure. Point-of-care airway ultrasound, which has been insufficiently studied in children, can overcome these problems. Materials and methods: This was a prospective cross-sectional study done on children aged 2 months to 17 years undergoing intubation with cuffed ETT in the PICU. The ETT cuff was filled with saline and three ultrasonographic techniques were used- 1) Suprasternal (SS) method 2) Cricoid (CC) metho and 3) Tracheal ring (TR) method. Position of the ETT as determined by ultrasound and X-ray were compared. The main outcomes were sensitivity, specificity, and area under curve (AUC) for ultrasound-based methods vs. X-ray. For the TR method, concordance between the X-ray and ultrasound categories were taken. Results: Total 62 patients were enrolled. The sensitivity and specificity of SS method were 71% (95% CI: 57-83%) and 100% (40-100%). The CC distance method had an AUC of 0.94 (95% CI: 0.86, 1.0). In the TR method, 98% of correct position on X-ray were correctly classified by USG. The agreement between X-ray and ultrasound categories with the cuff between the first and third tracheal rings, was very good [kappa (95% CI): 0.87 (0.70, 1.00), p ≤0.001)]. Conclusion: Bedside ultrasound is a good method to confirm ETT depth in children. The tracheal ring method had the best diagnostic accuracy and is easy to perform. The new method using cricoid cuff distance needs further validation in different ICU settings.
Application of End-Tidal Carbon Dioxide Monitoring Via Distal Gas Samples in Ventilated Neonates

Abstract:Background: Previous research has suggested correlations between the end-tidal partial pressure of carbon dioxide (PETCO2) and the partial pressure of arterial carbon dioxide (PaCO2) in mechanically ventilated patients, but both the relationship between PETCO2 and PaCO2 and whether PETCO2 accurately reflects PaCO2 in neonates and infants are still controversial. This study evaluated remote sampling of PETCO2 via an epidural catheter within an endotracheal tube to determine the procedure's clinical safety and efficacy in the perioperative management of neonates. Methods: Abdominal surgery was performed under general anesthesia in 86 full-term newborns (age 1-30 days, weight 2.55-4.0 kg, American Society of Anesthesiologists class I or II). The infants were divided into 2 groups (n = 43 each), and carbon dioxide (CO2) gas samples were collected either from the conventional position (the proximal end) or a modified position (the distal end) of the epidural catheter. Results: The PETCO2 measured with the new method was significantly higher than that measured with the traditional method, and the difference between PETCO2 and PaCO2 was also reduced. The accuracy of PErCO2 measured increased from 78.7% to 91.5% when the modified sampling method was used. The moderate correlation between PETCO2 and PaCO2 by traditional measurement was 0.596, which significantly increased to 0.960 in the modified sampling group. Thus, the PETCO2 value was closer to that of PaCO2. Conclusion: PETCO2 detected via modified carbon dioxide monitoring had a better accuracy and correlation with PaCO2 in neonates. Nasotracheal intubation in pediatrics: a narrative review

Abstract:Nasotracheal intubation (NTI) plays an important role in pediatric airway management, offering advantages in specific situations, such as oral and maxillofacial surgery and situations requiring stable tube positioning. However, compared to adults, NTI in children presents unique challenges owing to anatomical differences and limited space. This limited space, in combination with a large tongue and short mandible, along with large tonsils and adenoids, can complicate intubation. Owing to the short tracheal length in pediatric patients, it is crucial to place the tube at the correct depth to prevent it from being displaced due to neck movements, and causing injury to the glottis. The equipment used for NTI includes different tube types, direct laryngoscopy vs. video laryngoscopy, and fiberoptic bronchoscopy. Considering pediatric anatomy, the advantages of video laryngoscopy have been questioned. Studies comparing different techniques have provided insights into their efficacy. Determining the appropriate size and depth of nasotracheal tubes for pediatric patients remains a challenge. Various formulas based on age, weight, and height have been explored, including the recommendation of depth-mark-based NTI. This review provides a comprehensive overview of NTI in pediatric patients, including the relevant anatomy, equipment, clinical judgment, and possible complications.
Effect of Endotracheal Intubation Performed by Trauma Surgeons on Patientswith Severe Traumatic Brain Injury: A Prospective Observational Cohort Study

Abstract:Background: The patients with severe trauma traumatic brain injury (STBI) should be intubated as early as possible, and most of the intubation was performed by anesthesiologists. With the construction of trauma centers in China, more and more trauma surgeons have mastered endotracheal intubation skills after standardized training, more and more trauma patients were intubated by trauma surgeons. We undertook a prospective observational cohort study of endotracheal intubation in emergency resuscitation room to study the effect of endotracheal intubation respectively performed by trauma surgeons and anesthesiologists on patients with STBI. Methods: We collected data of all intubated patients with STBI between January 2018 and March 2020. Patients were divided into trauma surgeons, intubation (TI) group and anesthesiologists, intubation (AI) group according to intubation performer. We recorded age, gender, GCS, ISS, admission RR, admission HR, admission SpO2, admission PO2, admission PCO2, admission SBP, mechanism of injury, type of intracranial lesion, endotracheal intubation time, 1-min success rate of intubation, intubation related complications, the time from arrival at hospital to intubation, incidence of aspiration pneumonia, mechanical ventilation time, 30-day mortality. Comparison was made between the two groups. Results: Among 356 STBI patients, 219 patients met the inclusion criteria (TI group N=142, AI group N=77). There were no significant statistical differences between the groups in age, gender, GCS, ISS, admission RR, admission HR, admission SpO2, admission PO2, admission PCO2, admission SBP, mechanism of injury, type of intracranial lesion. There were also no significant statistical differences between the groups in endotracheal intubation time, 1-min success rate of intubation and intubation related complications. The time from arrival at hospital to intubation in the TI group significantly lower than the AI group (p<0.01). Compared with the AI group, the incidence of aspiration pneumonia and the mechanical ventilation time were decrease (p<0.05). 30-day mortality had no significant statistical difference between the groups. Conclusion: The trauma surgeons can master endotracheal intubation skills through standardized training, and the intubation effect is not significantly different from that of anesthesiologists. Moreover, endotracheal intubation by trauma surgeons allow the patients with STBI to be intubated earlier and reduce the incidence of aspiration pneumonia and the mechanical ventilation time. So early intubation is beneficial for the patients with STBI, and trauma surgeons should be encouraged to master endotracheal intubation skills.
The Sensitivity And Specificity Of Ultrasonic Localization Of Endotracheal Intubation

Abstract:Objective: This study aimed to evaluate the application value of portable ultrasound on clinical judgment of endotracheal tube placement. Methods: This was a blinded prospective study. A total of 94 ASAI similar to III patients in Beijing Chaoyan hospital from September, 2013 to February 2014, were randomly selected. All patients were intended to perform routine orotracheal intubation under general anesthesia. The tracheal intubations with laryngoscopy were performed by residents, 10 mL of saline were injected up to the endotracheal tube cuff (COVIDIENTM. MallinckrodtTM. Taper Guard Evac Oral Tracheal Tube). The endotracheal tube placement (in the trachea or in the esophagus) was confirmed by ultrasound scanning of the neck. End tidal CO2 (ETCO2) wave combined with chest ausculation were used as the gold standard of successful tracheal intubation. Sensitivity, specificity, positive predictive value and negative predictive value of endotracheal tube placement were calculated. Software SPSS 17.0 was used for statistical analysis. Results: Ninety-three patients were enrolled (ASAI similar to III) in this study with 42 males and 51 females. The mean age of the 93 patients was 53.5 +/- 11.8 years old; BMI was 24.91 +/- 3.38. Esophageal intubation was detected in 9 patients. One case of esophageal intubation was misdiagnosed by sonography. The sensitivity and specificity of ultrasonic localization of tracheal intubations was 100% (95% confidence interval, 94.6%-100%) and 88.9% (95% confidence interval, 84%-100%) with the positive and negative predictive value of 98.8% (95% CI 92.7-99.9) and 100.0% (95% CI 59.8-100) respectively. Conclusion: Sonography is portable, non-invasive and repeatable method; it could be used to confirm intracheal intubation with a relatively high sensitivity and specificity.
Fiberoptic Bronchoscopy in Neonatal and Pediatric Intensive Care Units: A 5-Year Experience

Abstract:Objectives: To determine the value and safety of fiberoptic bronchoscopy in neonatal and pediatric intensive care units (NICUs, PICUs). Subject and Methods: A total of 53 fiberoptic bronchoscopy procedures on 47 patients were reviewed. Bronchoalveolar lavage (BAL) was performed in 23 patients. Results: The primary diseases were pneumonia (n = 16), foreign body aspiration (n = 14), congenital airway abnormality (n = 12), trauma and/or following operations (n = 4) and Guillain-Barré syndrome (n = 1). The major bronchoscopic findings included inflammation in 26 patients, foreign body in 14, congenital airway abnormality in 12 and blood clotting in 3. Microbiology on BAL fluid was positive in 19 of 23 patients. In 23 patients with atelectasis, full and partial re-expansion was obtained in 14 and 6 patients, respectively, at 24 h after the procedures. The clinical features of 9 patients with sputum retention or blood clotting improved significantly after BAL. Positive or negative microbiologic BAL fluid results changed treatment in 11 patients, leading to marked clinical improvement in 9 patients. Moreover, 13 patients were extubated within 24 h of bronchoscopy. Conclusion: These data show that fiberoptic bronchoscopy is safe and effective in the diagnosis and therapy of pulmonary disorders in NICUs and PICUs.
Intubated pediatric patients requiring transport: a review of patients, indications, and standards

Abstract:Introduction: We could not find any studies of nontertiary care facilities performing intubation for patients requiring transport to definitive pediatric care. The purpose of our study was to determine the current practices of pediatric airway management in the prehospital and transport environments. Methods: A retrospective analysis of all patients younger than 16 years transported by our flight program during a 2-year period served as the population of interest. The flight records (RN and MD documentation) for intubated patients were analyzed for medications, methods, outcomes, and other descriptive endpoints. As a matter of program policy, all pediatric transports are subjected to peer review in the performance improvement committee. Results: During the review period, 732 patients younger than 16 years (range: 30 days to 15 years) were transported by our flight program. Of the 148 (20%) patients intubated for airway control, 81 were boys (55%), and 67 were girls (45%). Sixteen percent were younger than 1 year, 24% were 1 to 2 years old, 18% were 3 to 5, 20% were 6 to 11, and 22% were 12 to 15. Indicators for intubation included unresponsiveness or arrest, 42 (28%); seizures, 38 (26%); respiratory failure, 28 (19%); decreased level of consciousness (LOC), 14 (9%); airway protection, 13 (9%); combativeness, 11 (7%); and other, 2 (1%). Children were intubated most frequently by the referring physician (92 children, 62% of patients). The flight crew performed 49 (33%) intubations, and EMS staff performed seven (5%). Three children were nasally intubated. Significant variation occurred in medications used, endotracheal tube size and position, and nasogastric decompression. No single group performed better or worse than the others in our review. Conclusion: Variability exists in the application of pediatric airway management techniques, including pharmacologic modes and intubation indications.
Tracheal Intubation Practice and Safety Across International PICUs: A Report From National Emergency Airway Registry for Children*

Abstract:Objectives: It is unknown if variation in tracheal intubation practice and outcomes exist across PICUs in different geographical regions. We hypothesized there would be differences in the process of care and adverse outcomes for tracheal intubation across PICUs in six different geographical regions (New Zealand, Japan, Singapore, Germany, India, and North America). Design: Prospective multicenter pediatric tracheal intubation database (National Emergency Airway Registry for Children) Setting: Six non-North American (International) and 36 North American PICUs. Events: All PICU tracheal intubation encounters from July 2014 to June 2017 Measurements and Main Results: Adverse tracheal intubation–associated events and desaturation occurrence (oxygen saturation < 80%) were evaluated. A total of 1,134 and 9,376 TIs from International and North American PICUs were reported, respectively: primary tracheal intubation 9,060, endotracheal tube change 1,450. The proportion of tracheal intubations for endotracheal tube change was greater in International PICUs (37% vs 11%; p < 0.001). Median age for International tracheal intubations was younger compared with North America (0 yr [interquartile range, 0–2 yr] vs 1 yr (0–7 yr); p < 0.001). Among primary tracheal intubations, the most common indication was respiratory (International 67%, North American 63%; p = 0.049). Direct laryngoscopy was the most common device in both International (86%) and North American (74%) tracheal intubations. Occurrence of any adverse tracheal intubation–associated event was New Zealand 8%, Japan 17%, Singapore 9%, Germany 17%, and India 6%; International 11% versus North American 14%; p value equals to 0.003. Desaturation was reported less commonly in International PICUs: 13% versus North American 17%; p equals to 0.001. International PICUs used cuffed endotracheal tube less often (52% vs 95%; p < 0.001). Proportion of cuffed endotracheal tube use per PICU was inversely correlated with the rate of tube change (r = –0.67; p < 0.001). Conclusions: There were both similarities and differences in tracheal intubation practice and outcomes across international PICUs. Fewer adverse tracheal intubation–associated events were reported from International versus North American PICUs. International PICUs used cuffed endotracheal tube less often and had higher proportion of endotracheal tube change.

Abstract:Objective: The objective of this study was to develop an extubation practice protocol for adult intensive care unit (ICU) patients who underwent endotracheal intubation, providing theoretical guidance for clinical extubation procedures in the ICU. Methods: A research team was established consisting of medical, nursing, anaesthesia, and respiratory therapy professionals; the multidisciplinary team systematically searched domestic and foreign literature, summarised the best evidence, and combined it with clinical practice experience to preliminarily develop an extubation protocol for adult ICU patients who underwent endotracheal intubation. Seventeen experts in critical care medicine, intensive care nursing, clinical anaesthesia, and respiratory therapy were invited to participate in a Delphi expert consultation to screen and modify the draft protocol. Results: The response rates of the two Delphi expert enquiries were 100% and 94.1%, with expert authority coefficients of 0.94 and 0.93, respectively, and Kendall's concordance coefficients were 0.152 and 0.198, respectively, indicating statistically significant differences (p < 0.001). The final protocol included three level I indicators, 14 level II indicators, and 34 level III indicators, covering extubation evaluation, implementation, and postextubation management. Conclusion: The extubation protocol for adult tracheal intubation patients in the ICU constructed in this study is scientific, practical, and reliable. This study can provide theoretical guidance for extubation in ICU patients who have undergone endotracheal intubation.

Abstract:Background:Endotracheal intubation is a common clinical procedure applied for protecting critically ill patient'sairway, and supporting their ventilation needs through the insertion of a large-bore, flexible, plastic tube in thetrachea between the vocal cords called an endotracheal tube. Endotracheal tube care is an essential nursing practicefor maintaining patient safety. The incompetent endotracheal tube care can increase patients’ hospital stay, treatmentcosts, and morbidity and mortality. The literature highlighted that until now nurses’ practices depend greatly onrituals and traditions. Critical care nurses are assigned to deliver safe nursing care directed by the best evidence.Most incompetent nursing care for intubated patients may be due to a lack of nurses’ awareness and knowledge of thecurrent evidence-based practice. To optimize nursing care quality; the nursing practice should be fed by scientificknowledge and standardized evidence-based practice. Hence, this review aims to highlight the recent evidencerelated to endotracheal tube nursing care.

Abstract:Background: Tracheal intubation (TI) practice across pediatric emergency departments (EDs) has not been comprehensively reported. We aim to describe TI practice and outcomes in pediatric EDs in contrast to those in intensive are units (ICUs) and use the data to identify quality improvement targets. Methods: Consecutive TI encounters from pediatric EDs and ICUs in the National Emergency Airway Registry for Children (NEAR4KIDS) database from 2015 to 2018 were analyzed for patient, provider, and practice characteristics and outcomes: adverse TI-associated events (TIAEs), oxygen desaturation (SpO2 < 80%), and procedural success. A multivariable model identified factors associated with TIAEs in the ED. Results: A total of 756 TIs in 13 pediatric EDs and 12,512 TIs in 51 pediatric/cardiac ICUs were reported. Median (interquartile range [IQR]) patient age for ED TIs was higher (32 [7-108] months) than that for ICU TIs (15 [3-91] months; p < 0.001). Proportion of TIs for respiratory decompensation (52% of ED vs. 64% ICU), shock (26% vs. 14%), and neurologic deterioration (30% vs. 11%) also differed by location. Limited neck mobility was reported more often in the ED (16% vs. 6%). TIs in the ED were performed more often via video laryngoscopy (64% vs. 29%). Adverse TIAE rates (15.6% ED, 14% ICU; absolute difference = 1.6%, 95% confidence interval [CI] = -1.1 to 4.2; p = 0.23) and severe TIAE rates (5.4% ED, 5.8% ICU; absolute difference = -0.3%, 95% CI = -2.0 to 1.3; p = 0.68) were not different. Oxygen desaturation was less commonly reported in ED TIs (13.6%) than ICU TIs (17%, absolute difference = -3.4%, 95% CI = -5.9 to -0.8; p = 0.016). Among ED TIs, shock as an indication (adjusted odds ratio [aOR] = 2.15, 95% CI = 1.26 to 3.65) and limited mouth opening (aOR = 1.74, 95% CI = 1.04 to 2.93) were independently associated with TIAEs. Conclusions: While TI characteristics vary between pediatric EDs and ICUs, outcomes are similar. Shock and limited mouth opening were independently associated with adverse TI events in the ED.
Comfort and Coordination among Interprofessional Care Providers Involved in Intubations in the Pediatric Intensive Care Unit

Abstract:Background: Successful execution of invasive procedures in acute care settings, including tracheal intubation, requires careful coordination of an interprofessional team. The stress inherent to the intensive care unit (ICU) environment may threaten the optimal communication and planning necessary for the safe execution of this complex procedure. The objective of this study is to characterize the perceptions of interprofessional team members surrounding tracheal intubations in the pediatric ICU (PICU). Methods: This is a single-center survey-based study of staff involved in the intubation of pediatric patients admitted to a tertiary level academic PICU. Physicians, nurses, and respiratory therapists (RT) involved in tracheal intubations were queried via standardized, discipline-specific electronic surveys regarding their involvement in procedural planning and overall awareness of and comfort with the intubation plan. Qualitative variables were assessed by both Likert scales and free-text comments that were grouped and analyzed thematically. Results: One hundred and eleven intubation encounters were included during the study time period, of which 93 (84%) had survey responses from at least 2 professional teams. Among those included in the analysis, the survey was completed 244 times by members of the PICU teams including 86 responses from physicians, 76 from nurses, and 82 from RTs. Survey response rates were >80% from each provider team. There were significant differences in interprofessional team comfort with nurses feeling less well informed and comfortable with the intubation plan and process compared to physicians and RTs (p < 0.001 for both). Qualitative themes including clear communication, adequate planning and preparation prior to procedure initiation, and clear definition of roles emerged among both affirmative and constructive comments. Conclusions: Exploration of provider perceptions and emergence of constructive themes expose opportunities for teamwork improvement strategies involving intubations in the PICU. The use of a preintubation checklist may improve organization and communication amongst team members, increase provider morale, decrease team stress levels, and, ultimately, may improve patient outcomes during this high stakes, coordinated event.
Endotracheal Tube Obstruction by Unexpected Blood Clot in Anesthetized Children: a Report of Three Cases

Abstract:McGaw, IL, USA) returned a small amount of clear-yellow secretions. ETCO2 and tidal volumes returned to normal. The patient was disconnected from the anesthesia circuit and positioned prone for the procedure. His face was placed on a foam pad (Pediatric Gentletouch 4; Orthopedic Systems, Inc., Union City, CA, USA), and the circuit was reconnected. Shortly after reconnecting the circuit, ETCO2 and tidal volumes decreased to zero, and breath sounds were absent. Positive-pressure ventilation by bag was ineffective. A kink in the tube was suspected, and the patient’s head was repositioned, with no improvement in ventilation. An 8F soft suction catheter was again inserted and upon withdrawal of the catheter, a white foreign body was noted in the endotracheal tube. The foreign body migrated distally with attempts at ventilation, and could not be withdrawn with the 8F catheter. A 10F soft suction catheter was passed, and the foreign body was retrieved. The patient’s oxygen saturation remained above 96% and his heart rate above 100 throughout this time. Tidal volumes and ETCO2 returned to normal, and the rest of the case was uneventful. The foreign body was a 12 · 4 · 5 mm piece of white foam. Upon further inspection, a small gouge in the foam face pad, near the premade passage for the endotracheal tube, was noted and was equal in shape and size to the foreign body. There are numerous case reports of foreign bodies recovered from airways and anesthesia circuits. There are fewer reports of anesthesia equipment obstructing the endotracheal tube. There may be a higher risk with smaller pediatric tubes, as demonstrated by this incident and those cited above. The step of disconnecting and reconnecting the endotracheal tube to the circuit always entails the risk of subsequent complications as the integrity of the circuit is temporarily disrupted. Visual inspection of the circuit should always be undertaken after any disconnection. However, in our situation, only the connector piece of the endotracheal tube was readily visible. The foreign body was initially thought to be a mucus plug, because our patient did have a recent URI and mild ventilatory difficulty at the beginning of the case. Fortunately, positive-pressure ventilation did not push the foam into the bronchial tree, which could have caused more serious complications and required bronchoscopy for retrieval. We are not aware of another instance of this specific complication, despite foam pads being used commonly for surgery in the prone position. Benjamin J . Walker Sally E. Abstract:Background: The lower clinical exposure of Pediatric Emergency Medicine (PEM) fellows to critical procedures may impede skill acquisition. We sought to determine the tracheal intubation learning curve of PEM fellows during training and compared PEM fellow success against standards for tracheal intubation success. Methods: This was a retrospective, video-based study of a cohort of PEM fellows at a single academic pediatric emergency department (PED). All forms of tracheal intubation were included (rapid sequence intubation and crash or no medication). The cohort consisted of 36 PEM fellows from all or part of 5 consecutive fellowship classes. Data were collected by structured review of both existing ceiling-mounted videos and the electronic medical record. The main outcome was PEM fellows' success on the first or second attempt. We used cumulative summation to generate tracheal intubation learning curves. We specifically assessed the proportion of PEM fellows who reached 1 of 4 thresholds for procedural performance: 90% and 80% predicted success on the first and the first or second attempt. Results: From July 2014 to June 2020, there were 610 patient encounters with at least 1 attempt at tracheal intubation. The 36 PEM fellows performed at least 1 attempt at tracheal intubation for 414 ED patient encounters (65%). Median patient age was 2.1 years (interquartile range, 0.4-8.1). The PEM fellows were successful on the first attempt for 276 patients (67%) and on the first or second attempt for 337 (81%). None of the 36 PEM fellows reached the 90% threshold for either first or second attempt success. Four fellows (11%) met the 80% threshold for first attempt success and 11 (31%) met the 80% threshold for first or second attempt success. Conclusions: Despite performing the majority of attempts, PEM fellows often failed to reach the standard thresholds for performance of tracheal intubation. Clinical exposure alone is too low to ensure acquisition of airway skills.
