Abstract:The world is still in heightened awareness of the potential threat of another influenza pandemic, although it has been 5 years since the 2009 influenza A H1N1 pandemic. Evaluation of the adopted intervention strategies for handling the 2009 H1N1 pandemic is helpful for dealing with future outbreaks. In this paper we developed a hybrid model combining meta-population and agent-based models to evaluate the containment and mitigation strategies e.g., contact tracing and quarantine of contacts at assembly sites in the early phase, more medical institutions to detect cases and treat serious patients in the developing phase, and rapid vaccines delivered to students first for an H1N1 pandemic that were adopted in China. We find that the presented model can retrospectively fit relatively well to the spreading progress through comparison of the simulation results with actual infections data, and it can be used for practical application in evaluating the containment and mitigation strategies for an influenza pandemic if model validation and parameter estimation can be conducted by using actual data. The results will contribute to understanding the spread of viruses and the control of infectious diseases, and to helping government officials create policies on handling an influenza pandemic, especially beneficial to a large and diverse country such as the People's Republic of China.

Abstract:Background: Since the novel coronavirus disease 2019 (COVID-19) emerged in Wuhan city and then rapidly spread throughout china, the practices of the government's emergency response and management of medical system to prevent the COVID-19 was vitally crucial. However, the studies about China's emergency management of COVID-19 prevention and control were scarce. We aimed to explore the government's emergency response practices to COVID-19 prevention by comparing with H7N9 avian influenza (H7N9) in China.Methods: A qualitative comparative study was conducted in the study, by using a set of 6 key time nodes selected from international literature to form a frame of reference for the comparative analysis of the emergency response to H7N9 (2013) in Shanghai and COVID-19 (Wuhan city) prevention and control in China.Findings: 1) The hospital reporting speed--the first case report to the local CDC, for H7N9(2013) in Shanghai and COVID-19 in Wuhan city was 6 and 19 days, respectively. 2). The pathogen inspection speed-- the period from case reporting to technically confirming and re-checking pathogens, for COVID-19 was much quicker than that in H7N9(2013) in Shanghai (12days vs. 31days). 3). The government responding speed --from re-checking pathogens to the local government's emergency response, for H7N9 (2013) in Shanghai and COVID-19 in Wuhan was 4 and 15 days, respectively. 4) The total emergency disposal time of local government was 5 days longer in COVID-19 in Wuhan city than H7N9 avian influenza (2013) in Shanghai (46 days vs. 41 days).InterpretationFrom H7N9 to COVID-19, the speed of detecting unknown pathogens greatly improved in China, whereas the hospital reporting of epidemics and decision-making by government of Hubei province was significantly slow, which might be an important influential factor of the outbreak of COVID-19. Abstract:BACKGROUND: The focus of the study is to assess the advantages and shortcomings of China's public health system in the process of the COVID-19 prevention and to discuss the future reform of China's public health system.METHODS: By searching literature and reports related to the COVID-19 prevention of China, we compared the prevention effectiveness with the prevention policies in the process of the COVID-19 prevention.RESULTS: China's public health system can effectively combine national power to maximize the effectiveness of pandemic prevention. It improved the pandemic prevention ability of communities continuously and promoted the fairness of prevention. Traditional Chinese Medicine has also been used in pandemic prevention, which reduces the drug resistance of the virus. At the same time, the combination of the disease diagnosis and the Internet has reduced the spread speed of the pandemic. China's public health system also has some problems in response to the COVID-19, such as the shortage of medical resources, insufficient alerts, the low efficiency of reporting to superior government and the shortage of reward and punishment system for pandemic prevention.CONCLUSIONS: China's practice and efforts of the COVID-19 prevention can provide experience for other countries to improve their public health systems and accelerate the end of the COVID-19 pandemic.

Abstract:Two U.S. specialists (on the governance and foreign policy aspects of China's public health issues as well as its human and medical geography) examine how two different sets of policies implemented by the government of China have affected both the geography and political ecology of pandemic disease outbreaks (HIV/AIDS, SARS, and H1N1) over the past two decades. More specifically, they argue that: (1) broad development and reform policies largely responsible for China's rapid modernization/urbanization and increasingly successful perfomance in the global economic arena have generated unexpected side-effects in terms of the location, incidence, and spread of pandemics as well as the state's capacity to mount an adequate health care response; and (2) politically motivated public health policies implemented in response to the spread of specific pandemics in China have had unanticipated impacts on the progression of disease outbreaks and their outcomes. Abstract:Background: Recently, the novel coronavirus disease (COVID-19) has already spread rapidly as a global pandemic, just like the H1N1 swine influenza in 2009. Evidences have indicated that the efficiency of emergency response was considered crucial to curb the spread of the emerging infectious disease. However, studies of COVID-19 on this topic are relatively few. Methods: A qualitative comparative study was conducted to compare the timeline of emergency responses to H1N1 (2009) and COVID-19, by using a set of six key time nodes selected from international literature. Besides, we also explored the spread speed and peak time of COVID-19 and H1N1 swine influenza by comparing the confirmed cases in the same time interval. Results: The government’s entire emergency responses to the epidemic, H1N1 swine influenza (2009) completed in 28 days, and COVID-19 (2019) completed in 46 days. Emergency responses speed for H1N1 was 18 days faster. As for the epidemic spread speed, the peak time of H1N1 came about 4 weeks later than that of COVID-19, and the H1N1 curve in America was flatter than COVID-19 in China within the first four months after the disease emerged. Conclusions: The speed of the emergency responses to H1N1 was faster than COVID-19, which might be an important influential factor for slowing down the arrival of the peak time at the beginning of the epidemic. Although COVID-19 in China is coming to an end, the government should improve the public health emergency system, in order to control the spread of the epidemic and lessen the adverse social effects in possible future outbreaks.

Abstract:Public health emergencies severely affect people’s lives and health, national security, social stability, and economic development. Against the background of coronavirus disease 2019 pandemic, this study analyzes the current status of China’s public health emergency prevention and control system from the aspects of its six major systems: monitoring, early warning, and evaluation; diagnosis and treatment; basic scientific research; personnel training; material, equipment, and industry support; and social mobilization and coordinated action. We conclude that strengthening system construction is a realistic demand of China in the new development stage. China’s public health system still faces challenges regarding institutional positioning, capacity building, hardware investment, and management system. Therefore, we propose that China should establish a national overall coordination and emergency command mechanism, strengthen clinical, science, education, and industrial support capacity, and further promote international cooperation.

Abstract:Based on the systematic evaluation of the capacity of the new infectious diseases control and prevention of the 32 provincial capital cities and municipalities with independent planning, the advantages and shortcomings of Wuhan could be described as follows: (1) The legal framework for epidemic prevention has been established, but the effective actions have not been carried. (2) With an organizational basis for responding to the epidemic, a coordination mechanism has not been effectively established. (3) The management mechanism has been improved, but responsibilities among different departments were not clear enough. (4) The disease reporting system has been running well, the capacity of the incidence trend forecasting and early warning was still needed to be enhanced. (5) The insufficiency of public health service delivery was observed owing to a lack of financial investment. Abstract:To summarize the present status of health emergency management assessment in China, a comprehensive search of Chinese databases for research that explicitly mention health emergency assessment indicators and indicator systems was performed. Studies were evaluated using the Ekman quality assessment tool, and data were extracted with an original extraction form. Sixty-one studies were included. There are many types and methods of health emergency management assessment in China, and the dimensions and the indicators involved are complex. Legal, regulatory, and policy bases for such assessment need to be further strengthened. The relevance of the entire assessment process and its practical application should be enhanced. In the occupational practice, appropriate evaluation methods should be selected according to respective evaluation purposes, evaluation objects, and contents. Laws, regulations, and policies in the evaluation of health emergency management should be improved. Finally, further correlational research on health emergency management evaluation system processes should be explored and improved.

Abstract:Chinese government had adopted strict measurements in the earlier prevalence of novel H1N1 influenza virus. As with the coming of the pandemic, the control strategy had been modified. Many controversies had been aroused among the professionals, the officials and the publics. As influenza was mainly transmitted through the respiratory tract, it was very difficult to cut off the transmitting pathway. In addition, the transmissibility of the novel H1N1 virus was almost the same as that of seasonal influenza. The whole population was more susceptible to it and the complete control of the infective source was impossible. The aim of the present strategy was to inhibit the peak of the pandemic and retard the spread speed of the virus. The main task of medical service was to reduce the mortality rate of the pandemic so as to do the best to decrease the negative influence on the whole society.

Abstract:As public health emergencies become increasingly complex and frequent worldwide, modernization of the public health emergency system is urgently required for improving the overall security level of a country; it is also crucial for the modernization of the national governance system. In this study, we summarize China’s response to public health emergencies from three aspects: epidemic surveillance and reporting system, sentinel surveillance and multipoint trigger mechanism, and mobile terminal application for individuals. Moreover, we explore the development paths for precise control and integrated management of public health emergencies and propose corresponding suggestions. Specifically, precision control can be realized by combining the following aspects: temporal and spatial modeling and calculation for the epidemic, epidemic data collection and information statistics, grassroots community prevention and control, and emergency resource supply. Integrated management should focus on: collection and perception of social governance information, data analysis and calculation platforms, rapid response and command at the grassroots level, epidemic monitoring/early warning/prediction, and continuous risk assessment. Furthermore, we suggest that China should strengthen information technology to enable epidemic prevention and control, improve its epidemic monitoring and reporting system, and build an integrated prevention and control system for public health governance.

Abstract:Since the spread of the novel coronavirus (SARS-CoV-2), China has launched an unprecedented fight against the epidemic. At the beginning of the outbreak, China responded quickly by informing the World Health Organization (WHO) and sharing sequence information internationally soon after the draft genome of the virus was revealed by Chinese researchers. China’s effort in transparency, data sharing and quick response for controlling the outbreak was praised by the international community. Although the outbreak is not over yet, the important consideration for this and future outbreaks has aroused wide public discussion. This article is aimed to identify the lessons we could learn from the outbreak and propose suggestions for future improvement from aspects including improving the public health and disease control and prevention system, improving the emergency response mechanism, promoting technological innovation, and increasing medical supplies and reserves. An enhanced holistic system is in urgent need for disease control and prevention through strengthening the public health and disease control and prevention system, improving the emergency response system, building relevant legal system, and establishing a national biosafety system. Future outbreaks might be inevitable, but we can save more lives and avoid much financial loss by effective countermeasures when public health emergency happens.

Abstract:Since the emergence of influenza A/H1N1 pandemic virus in March-April 2009, very stringent interventions including Fengxiao were implemented to prevent importation of infected cases and decelerate the disease spread in mainland China. The extent to which these measures have been effective remains elusive. We sought to investigate the effectiveness of Fengxiao that may inform policy decisions on improving community-based interventions for management of on-going outbreaks in China, in particular during the Spring Festival in mid-February 2010 when nationwide traveling will be substantially increased. We obtained data on initial laboratory-confirmed cases of H1N1 in the province of Shaanxi and used Markov-chain Monte-Carlo (MCMC) simulations to estimate the reproduction number. Given the estimates for the exposed and infectious periods of the novel H1N1 virus, we estimated a mean reproduction number of 1.68 (95% CI 1.45-1.92) and other A/H1N1 epidemiological parameters. Our results based on a spatially stratified population dynamical model show that the early implementation of Fengxiao can delay the epidemic peak significantly and prevent the disease spread to the general population but may also, if not implemented appropriately, cause more severe outbreak within universities/colleges, while late implementation of Fengxiao can achieve nothing more than no implementation. Strengthening local control strategies (quarantine and hygiene precaution) is much more effective in mitigating outbreaks and inhibiting the successive waves than implementing Fengxiao. Either strong mobility or high transport-related transmission rate during the Spring Festival holiday will not reverse the ongoing outbreak, but both will result in a large new wave. The findings suggest that Fengxiao and travel precautions should not be relaxed unless strict measures of quarantine, isolation, and hygiene precaution practices are put in place. Integration and prompt implementation of these interventions can significantly reduce the overall attack rate of pandemic outbreaks.

Abstract:There were 27 novel coronavirus pneumonia cases found in Wuhan, China in December 2019, named as 2019-nCoV temporarily and COVID-19 formally by WHO on 11 February, 2020. In December 2019 and January 2020, COVID-19 has spread in large scale among the population, which brought terrible disaster to the life and property of the Chinese people. In this paper, we will first analyze the feature and pattern of the virus transmission, and discuss the key impact factors and uncontrollable factors of epidemic transmission based on public data. Then the virus transmission can be modelled and used for the inflexion and extinction period of epidemic development so as to provide theoretical support for the Chinese government in the decision-making of epidemic prevention and recovery of economic production. Further, this paper demonstrates the effectiveness of the prevention methods taken by the Chinese government such as multi-level administrative region isolation. It is of great importance and practical significance for the world to deal with public health emergencies.

Abstract:Abstract For China, the coronavirus disease 2019 (COVID-19) outbreak is a major public health emergency with the fastest spread, the most extensive infection, and the hardest to contain over the past 70 years. The different organizations and institutions in China have taken unprecedented public health responses to interrupt the virus transmission in the past several months. The outbreak in China was under control, but the number of confirmed cases abroad is still rising. Coronavirus disease 2019 has presented a global pandemic. We summarized the response measures adopted by different organizations at different levels (country, province, and hospital) in China, such as setting up an effective integrated system for disease prevention and control, effective deployment of medical staff, adjusting measures according to local conditions, establishing Fangcang hospitals, strengthening scientific research on COVID-19, epidemic prevention knowledge education, mass rapid testing for severe acute respiratory syndrome coronavirus 2, and correct personal protection including high compliance of wearing masks, hoping to provide some help for disease control in some regions.

Abstract:Objective: This study aimed at exploring the current development status and problems of health emergency management in China and provides a reference for improving, constructing, and implementing a public health emergency management system.Methods: Cases of major and severe public health emergencies in China were analyzed along with the relevant health emergency management literature from the last decade.Results: China's health emergency system gradually improved during the study period. Monitoring and early warning systems were significantly strengthened. Material reserves and transfer management systems were constantly improved. However, the operational efficiency of command and decision systems was low, versatile talent accounted for a relatively small proportion, and emergency fund investment was insufficient.Conclusion: Constructing a sound and scientific emergency management mechanism is a lengthy and challenging process. To establish an emergency management mode for public health emergencies that is appropriate for China, it is necessary to solve existing problems and learn from the models and experiences of developed foreign countries.

Abstract:Objective To study current situation and existed problems in the public health emergency alert and response system so as to formulate more advices to consummate it and help to establish a more reasonable system adapting to our country. Methods A qualitative study was carried out among CDC working staff including two focus group discussions(FGD). Results Although government had put more emphasis on public health,increasing its devotion and strengthening the alert and response capability after SARS outbreak, some problems still existed, such as ① absence of correlative laws and rules; ② lack of government devotion; ③ insufficient utilization of surveillance information; ④ lack of the canonical response guidance. Conclusions With regard to the building up of the public health emergency alert and response system, the following proposals are put forward: increasing payout on public health; consummating the construction of disease surveillance network; affording more education and training to correlative staff engaged in the disease control to improve their abilities.
