Aim of this study is to present a cumulative multi-institutional experience with LESS. METHODS Medical records of patients undergoing LESS procedures at 14 Institutions from US, Europe, Asia and South America between September 2007 and November 2010 were collected and used for this retrospective analysis. RESULTS Overall, 916 patients (mean age 52.1+16.8 years; BMI 25.3+4.4 kg/m2; ASA score 1.7+0/9) underwent LESS urological surgery during the study period. Two hundred twenty three (24.3%) had history of previous abdominal/pelvic surgery. Most (>90%) were upper tract surgery procedures and most were done with a transperitoneal approach (>95%). There was a significant increase in rate number of cases/month during the two years period 2009–10 compared to the 2007–2008 period (30.5 vs 13.3, respectively). One hundred forty (16%) were operated with the use of daVinci robot. Articulating or prebent instrument were used in over 75% of cases. Commercially available single-port device were used in almost 50% of cases, whereas the remaining cases were done by using a home made device (35%) or multiple trocars (single-site approach, 15%). In more than 20% of the cases additional port/instruments were used. Mean operative time was 170+93.5 minutes. Mean estimated blood loss was 154.7+238.6 mL. Mean skin incision length was 3.5+1.3 cm. Intraoperative complications occurred in 31 cases (3.5%). There were 10 conversions to open surgery and 38 to lap for an overall conversion rate of 5.2%. Mean hospital stay was 3.7+2.7 days. Transfusion rate was 1%. Mean pain VAS at discharge was 1.6+1.6. Postoperative adverse events were recorded according to Clavien scoring classification: 21 grade I; 28 grade II; 20 grade III and 5 grade IV. CONCLUSIONS The largest multi-institutional experience of LESS urological surgery is reported herein.
