Efficacy of endovascular therapy for traumatic vertebral artery dissecting aneurysms: a preliminary observation

Abstract:Objective:To preliminarily explore the safety and efficacy of endovascular therapy for traumatic vertebral artery dissecting aneurysms (TVADA).Methods:A total of 14 patients with TVADA underwent endovascular therapy at Department of Neurosurgery, The First Affiliated Hospital of Naval Medical University from January 2017 to December 2019 and were consecutively enrolled into this retrospective study. Among them, 11 cases were treated with stent-assisted coiling, 2 cases with stent only, and 1 case with flow diverter device. Surgical complications and outcomes were documented.Results:Successful operations were performed in all 14 cases. Immediate postoperative angiograph revealed partial visualization of the aneurysm in 11 cases and complete occlusion of aneurysm in 3. Post operation, there were 1 patient who died of intracranial hemorrhage and 1 patient developing muscle strength decrease which resolved following treatment. Twelve patients were followed up for an average of 29.5±10.8 months (12-47 months), including 11 patients with Glasgow outcome scale grade Ⅴ and 1 patient with GOS grade Ⅳ. Ten patients underwent imaging follow-up lasting for an average of 24.6±10.4 months (6-40 months), and all reported complete occlusion of dissection aneurysms.Conclusion:Our preliminary observation has suggested that endovascular therapy is safe and effective for TVADA, which could lower the long-term risk of the patient′s re-bleeding.
