Abstract:Objective To explore the clinical features of patients with acute cardiac pulmonary edema(ACPE)with different therapeutic strategies,and to provide the clinical reference for ACPE treatment. Methods The clinical data of 122 patients who got ACPE in the Second Affiliated Hospital of Nanchang University from January 2012 to December 2014 was analyzed retrospectively,cases were divided into conventional therapy group(48 cases),non - invasive mechanical ventilation therapy group(31 cases),and invasive mechanical ventilation therapy group(43 cases). The general data,laboratory test results,department distribution,prognosis and complication of cases were recorded. Results There were no significant difference in gender,age,pH value,PaO2 ,levels of BNP and troponin,and APACHEⅡ score among three groups of patients (P > 0. 05);There was significant difference in PaCO2 among three groups of patients(P < 0. 05);PaCO2 of patients in non -invasive mechanical ventilation therapy group was significantly higher than that of patients in conventional therapy group( P <0. 05). Most ACPE cases(54 cases,44. 3% )were found in department of respiratory medicine,31 ACPE cases(25. 5% ) were found in general ICU,19 ACPE cases(15. 6% )were found in department of cardiology,10 ACPE cases(8. 2% )were found in department of surgery. There were no significant difference in incidences of death after 7 days,cardiogenic shock and cardio - respiratory arrest(P > 0. 05);There were significant differences in incidences of death,death in 7 days,disturbance of consciousness and respiratory failure among 3 groups of patients(P < 0. 05). Conclusion ACPE patients should be treated according to different conditions,cases with hypercapnia can benefit from non - invasive mechanical ventilation therapy. The conventional therapy and invasive mechanical ventilation therapy are the two main treatment methods selected by clinical departments,application of non - invasive mechanical ventilation therapy is limited,cases who receive invasive mechanical ventilation therapy always have refractory hyoxemia,thus a high mortality and high incidence of severe complications can be observed.
