Abstract:Public health events, as the common concern faced by the international community, call for the joint response from all mankind. The outbreak of the COVID-19 has highlighted the problems confronting the global governance of international public health, such as limited functions of international organizations and difficulties in achieving objectives, poor collaboration between governance subjects and their limited performance, overlapping legal basis of governance and blurred core function, and lack of solutions to special problems. The corresponding approaches can be taken to improve the efficiency of the governance of global public health, including supporting the role of international organizations to achieve the objectives, enhancing coordination among international governance subjects to form synergy, promoting the compliance with IHR2005 to avoid conflict of law application and upholding the vision of a community with a shared future for mankind to jointly respond to the special problems.

Abstract:Abstract The global response to the COVID-19 pandemic has laid bare weaknesses and major challenges in the international approach to managing public health emergencies. Populist sentiment is spreading globally as democratic nations are increasing their support for or electing governments that are perceived to represent “traditional” native interests. Measures need to be taken to proactively address populist sentiment when reviewing the IHR (2005) effectiveness in the COVID-19 pandemic. We discuss how populism can impact the IHR (2005) and conversely how the IHR (2005) may be able to address populist concerns if the global community commits to helping states address public health threats that emerge within their borders.

Abstract: Cultural taboos inhibit communication about sex, deny and stigmatize deviant sexual practices, and are incorporated into sexual cultures characterized by silence, shame and secrecy. In contemporary Indonesian societies, sex is often infused with negative connotations and referred to as taboo. Considering the threat that an AIDS pandemic poses, HIV/AIDS prevention and treatment represent a critical development issue that requires ongoing commitment from the Indonesian Government and the international donors that provide critical financial support in the health sector. These taboos that inhibit communication about sexuality clearly constrain attempts to challenge negative understandings of sexuality as inherently shameful. However, promotion of reproductive/sexual health and HIV/AIDS prevention is highly contingent upon expanded dialogue about sexuality in both private and public domains. Hence, the author emphasized the need to acknowledge the complex ways in which sexual communication is circumscribed by enduring cultural taboos, and also how sexual dialogue operates to subvert and erode those taboos.

Abstract:The outburst and rapid worldwide spread of the COVID-19 pandemic have become the single greatest challenges to humankind today. The adverse impact of the pandemic on the world economy and social order raises the question of whether the existing international public health order is capable of meeting the needs of humankind facing serious infectious diseases like the COVID-19 pandemic. The WHO-led international public health order was formulated at the end of the Second World War and reflected the culture, tradition, and perception of security of Western countries. Stimulated by the process of globalization, national states’ perception of sovereignty, human rights, and interdependence has undergone profound changes. By the very nature of public health, it is closely related to trade and intellectual property, which encouraged and enabled the World Health Organization, the World Trade Organization, and the World Intellectual Property Organization to cooperate closely. Hand in hand with the ever deepening of globalization, public health has become a genuine and important international issue drawing collaboration of many international organizations and ad hoc bodies such as the G20. “There are sure to be bad years following the movement of great armies.” The current international public health order has revealed certain weaknesses and shortcomings that require actions. The system is bound to be reviewed and reformed, which must take into account the interests, cultures, and traditions of the developing countries in the context of global collaboration.

Abstract:The coronavirus disease 2019 (COVID-19) pandemic continues to claim lives around the world and, to some extent, reflects the failure of international cooperation. Global health diplomacy (GHD)can be a bridge for international cooperation for tackling public health crises, strengthening health systems through emphasizing universal health coverage for sustainable and equitable development, and rebuilding multilateral organizations. It can be a catalyst for future global health initiatives. Health should not be used as a political tool at the cost of people’s lives, nor should it become a proxy for geopolitics but can be used to diffuse tensions and create a positive environment for political dialogue. Health diplomacy’s focus should be to mitigate inequality by making available diagnostics, therapeutics, and vaccines as a global public good. The implications for the lack of international cooperation will lead to increased global disparities and inequities as the countries that cannot procure vaccines will find their population more vulnerable to the pandemic’s repercussion. Though the international cooperation on trade has suffered the impact of geopolitical shifts and competition, through engaging in GHD, the governments can align the trade and health policies. Amid this global health crisis, the World Health Organization (WHO) has faced an increase in International Health Regulations violations, limiting its influence and response during this COVID-19 pandemic. Nations need to develop a sense of cooperation that serves as the basis for a mutual strategic trust for international development. The priorities of all the countries should be to find the areas of common interest, common operational overlap on development issues, and resource allocation for this global fight against COVID-19.

Abstract:The COVID-19 pandemic has exposed a global health and political crisis like no other in recent history. As ground zero of the virus outbreak, significant criticism and blame have been directed at China for covering up the outbreak. Yet a systematic assessment of China's responses to international opprobrium of its pandemic measures has been largely lacking in the literature. Drawing on the concept of "blame" from public administration, this article seeks to fill this gap by investigating China's COVID-19 crisis and blame (mis)management practices. We make two key contributions in this article. First, we highlight how Beijing engaged in the politics of blame and outline three modes (defensive, aggressive and proactive benevolence) of its blame management practices. Second, we suggest that China sought to articulate and refine its identity during the COVID-19 pandemic, providing insights into a China that is increasingly assertive yet vulnerable to reputational damage. We contend that China's efforts to counter international opprobrium and shift strategic narratives speak directly to issues of autocratic legitimation and its conceived "responsible great power" identity, with greater success among domestic rather than global audiences.

Abstract:The COVID-19 pandemic exposed gaps in global health governance, catalyzing proposals for a new WHO pandemic treaty. This paper investigates China's stance on the treaty, recognizing it as reflective of many developing countries' concerns, through a qualitative analysis of its interventions during the treaty's drafting and negotiations and an examination of historical and geopolitical factors. Findings reveal China's emphasis on respecting state sovereignty, differentiated obligations for developing nations, preventing stigma, and concrete capacity building—concerns shared across the Global South. Its posture balances pragmatism and principle, reflecting differentiated responsibilities as a major power and developing country along with philosophical divergences from Western legal thinking. While endorsing global cooperation, China insists on voluntary terms without impinging on policy space. Implications suggest that accommodating China's concerns about invasive compliance mechanisms and inequitable burdens through flexible provisions can shape the treaty's acceptance and architecture. Creative solutions reconciling sovereignty and collective action combined with concrete equity measures and depoliticized cooperation will determine the treaty's success. China's major role indicates its endorsement, representative of the Global South's voice, is essential for an impactful pandemic treaty and reformed global health governance.

Abstract:The COVID‐19 epidemic has sent economic and social shockwaves reverberating across the globe, and has shaken governance institutions at every level and across a range of issues, from trade to health and climate change. An important question is whether COVID will accelerate, slow, or perhaps reverse some key trends in global governance. Most prominently, the global and multilateral governance regimes that developed post‐World War II were already weakening and fragmenting before COVID (Acharya, 2016). In a closely related phenomenon, cosmopolitan liberalism, as a set of institutions and ideologies committed to a democratic open society, human rights, and multiculturalism, has also retreated in the face of rising populism and authoritarianism in recent years (Norris and Inglehart, 2019). COVID could also affect a third important trend, the decline of state regulation and the rise of private, voluntary, and disclosure‐based governance (Levy et al., 2010). The impact of COVID on these trends suggests some fruitful new directions for scholars of global governance and organizations.

Abstract:The call to strengthen global health governance against future outbreaks through a binding treaty on pandemics has attracted global attention and opinion. Yet, few of these perspectives have reflected the voices from early career global health professionals in Africa. We share our perspectives on the Pandemic Treaty, and specifically our scepticism on the limitations of the current top-down approach of the treaty, and the need for the treaty to centre equity, transparency and fairness to ensure equitable and effective cooperation in response to global health emergencies. We also highlight the challenges intergovernmental organisations for health faced in coordinating nation states during the COVID-19 crisis and how a Pandemic Treaty would address these challenges. We argue that lessons from the COVID-19 pandemic provide a critical opportunity to strengthen regional institutions in Africa-particularly in a multipolar world with huge disparities in power and resources. However, addressing these challenges and achieving this transformation may not be easy. Fiscal space in many countries remains constrained now more than ever. New tools such as the Pandemic Fund should be designed in ways that consider the specific needs and capacities of countries. Therefore, strengthening countries' capacities overall requires an increase in domestic investment. This paper calls for wider structural reforms such as debt restructuring among other tools to strengthen countries' capacities.

Abstract:The COVID-19 pandemic has revealed the contested politics of global health governance, though we still don't know enough about the dynamics of domestic pandemic responses, or about the relationship between the politics of those responses and the politics of global health governance, both of which have changed significantly in recent decades. Focusing on three cases (HIV/AIDS, SARS, and COVID-19) of cross-border infectious diseases, this article explores the trajectory of China's pandemic responses in the context of globalization. Attending to changing politics at domestic, international, and global levels, I argue that those responses have been a complex combination of China's domestic politics (e.g., priorities, institutions, leadership, and timing), its international relations (especially with the US), and its engagements with global health governance. It is concluded that the increasing divergence of pandemic responses in a time of ubiquitous global health crisis demands urgent attention to the connections (including contestations) between domestic pandemic responses and the evolvement of global health governance from a broader perspective that considers changes in geopolitics.

Abstract:Global Health Promotion, Ahead of Print. Global health governance is a strategic priority for the World Health Organization (WHO), and the public health surveillance system (PHSS) is a fundamental element of the global health governance structure to timely identify emerging diseases and guide global public health decisions and actions. This analysis explores the overall landscape of global health governance, with a specific focus on the PHSS to understand whether the existing governance landscape facilitates or undermines the WHO's ability to formulate and implement global health policies and initiatives. To achieve this, the existing evidence was reviewed, and synthesized with the experts' perspectives. It is reported that fragmentation is the main drawback of the global health governance landscape, necessitating reorganization and restructuring. The disintegration of PHSS at the global, regional and local levels is associated with a lack of leadership, misalignment with global health priorities, imbalance in coverage of surveillance systems, inadequate innovative technology and digitalization, and fragmented data and information systems. The fragmentation and disintegration of global health governance undermine the effectiveness of the WHO's global health strategic directions and programmes and hinder its ability to govern and guide the global, regional and national public health emergency response. Strategic rethinking of the WHO's governance is essential because strong governance and leadership lead to a robust, aligned and effective PHSS.

Abstract:The paper summarises the extent and incidence of the Covid-19 pandemic and evaluates government policies in different countries. The consequences of lock-down policy are discussed. The most negatively affected groups of the population are the low-skilled workers and self-employed. Psychological welfare has seriously declined and provision of health services have deteriorated. Population density, age structure and state management strongly influence the spread and containment of the disease. State coordinated economies and social-democratic countries have managed the effects of the disease better than market coordinated economies, though there are important exceptions. A failure of some governments to provide sufficient reserve capacity of healthcare facilities indicates government policy failure. The paper suggests that a more focused policy of social isolation of people susceptible to coronavirus with less stringent lockdown measures would have significantly reduced social costs and increased public health benefits.

Abstract:Over the past two decades, global health diplomacy, foreign policy for health, and global health policy have changed substantially. Diplomacy is a constitutive part of the system of global health governance. COVID-19 hit the world when multilateral cooperation was subject to major challenges, and global health has since become integral to geopolitics. The importance of global health diplomacy, especially at WHO, in keeping countries jointly committed to improving health for everyone, has once again been shown. Through a systematic review, this Series paper explores how international relations concepts and theories have been applied to better understand the role of power in shaping positions, negotiations, and outcomes in global health diplomacy. We apply an international relations perspective to reflect on the effect that those concepts and theories have had on global health diplomacy over the past two decades. This Series paper argues that a more central role of international relations concepts and theories in analysing global health diplomacy would help develop a more nuanced understanding of global health policy making. However, the world has changed to an extent that was not envisioned in academic discourse. This shift calls for new international relations concepts and theories to inform global health diplomacy.

Abstract:During the Covid-19 pandemic, the World Health Organization (WHO) was faced with the task of regular public updating—about both the pandemic itself, and hundreds or potentially thousands of other health emergencies. Here, we examined the 242 reports published in the WHO Disease Outbreak News (DON) during the first four years of the Covid-19 pandemic (2020 to 2023), and document the diseases and regions that were reported. We find that multinational epidemics of diseases like Ebola virus and MERS-CoV continue to dominate the DON. However, recent years have also seen more reports of climate-sensitive infectious diseases, as well as a state shift in influenza outbreak reporting in both China and the rest of the world. Surprisingly, the DON was only minimally used to document the Covid-19 pandemic and the global mpox epidemic, almost exclusively before the declaration of a public health emergency of international concern. Notably, inconsistent reporting related to Covid-19 variants of concern speaks to the ongoing evolution of the DON as a resource, and potentially, to its complicated relationship with international travel and trade restrictions. We suggest that researchers should continue to exercise caution when treating the DON as a global record of outbreak history, but that the DON is a compelling record of the WHO itself, including the process it uses to assess outbreak risk.

Abstract:Abstract The COVID-19 pandemic affects all countries, but how governments respond is dictated by politics. Amid this, the World Health Organization (WHO) has tried to coordinate advice to states and offer ongoing management of the outbreak. Given the political drivers of COVID-19, we argue this is an important moment to advance International Relations knowledge as a necessary and distinctive method for inclusion in the WHO repertoire of knowledge inputs for epidemic control. Historical efforts to assert technical expertise over politics is redundant and outdated: the WHO has always been politicized by member states. We suggest WHO needs to embrace the politics and engage foreign policy and diplomatic expertise. We suggest practical examples of the entry points where International Relations methods can inform public health decision-making and technical policy coordination. We write this as a primer for those working in response to COVID-19 in WHO, multilateral organizations, donor financing departments, governments and international non-governmental organizations, to embrace political analysis rather than shy away from it. Coordinated political cooperation is vital to overcome COVID-19.

Abstract:Is the contestation of international institutions always a one-sided process that originates from nation-states? In research to date, there has been little discussion of the extent to which international institutions endure, or even form counter-reactions to national contestation strategies. This study examines the reasons for which WHO engages in counter-contestation vis-à-vis its member states. The paper analyzes the evolution of global health governance by relating a principal-agent approach and contestation considerations. The WHO (agent) wants to reshape the principal-agent relationship with the member states (principals) in order to maximize its autonomy and eventually ensure stronger independence. The WHO pursues its efforts to become more independent on the basis of its own logic of action: To achieve this autonomy from member states, WHO on the one hand uses a strategy we call counter-contestation. On the other hand the member states want their interests to be represented by the WHO and ensure this goal through the logic of action known as contestation. Four international health crises are used to show how different the logics of action are and what effects they have. This study explores how and to what extent WHO actively engages in the political exchange of diplomatic moves and manoeuvres, creating contestation as a mutual game between states and International Organizations.

Abstract:Assessment of the impact of COVID-19 on international institutions and international relations is essential for shaping global governance for the post COVID crisis world. The authors review the actions of the key international institutions in response to the pandemic undertaken in January-March 2020 reflecting on three questions. First, were the actions undertaken by the international institutions adequate, coordinated and timely? Second, could the outbreak have been contained if the global governance system was not in a state of severe strain, with many of the gaps exposed and reforms promised in the wake of the 2009 financial and economic crisis unfulfilled, its key causes unaddressed and unilateralism rising among its key members? In addition, was the COVID-19 crisis exacerbated by the crisis of multilateralism? Third, and most difficult, what is the future of global governance after the COVID-19 crisis ends? The analysis of international institutions performance three months into the crisis leads to authors to conclude that there have been inadequate actions to produce a timely, coordinated international response from all the major multilateral organizations and from the newer plurilateral summit institutions of the BRICS, G7 and G20. The failure of these global governance institutions was due not only to the severe strains from leading members’ unilateralism and competition, but from the very architecture designed in 1945 that now poorly matches intensely globalized world. Global governance in the post COVID world should not descend into the old war-prone balance of power, nor flow from a new Bretton Woods-San Francisco as in 1944–1945 but from an intensification and expansion of G20 governance that will generate and coordinate more comprehensive, stronger multilateral organizations for the benefit of all.

Abstract:Global health law is essential in responding to the infectious disease threats of a globalizing world, where no single country, or border, can wall off disease. Yet, the Coronavirus Disease (COVID-19) pandemic has tested the essential legal foundations of the global health system. Within weeks, the SARS-CoV-2 coronavirus has circumnavigated the globe, bringing the world to a halt and exposing the fragility of the international legal order. Reflecting on how global health law will emerge in the aftermath of the COVID-19 pandemic, it will be crucial to examine the lessons learned in the COVID-19 response and the reforms required to rebuild global health institutions while maintaining core values of human rights, rule of law, and global solidarity in the face of unprecedented threats.
