Abstract:This study aims to evaluate the radiation dosimetry of a new technetium-99m‒labelled small-molecule inhibitor of prostate-specific membrane antigen (HYNIC-Glu-Urea-A, 99mTc-HYNIC-PSMA) and its feasibility as a tumor-imaging agent in prostate cancer (PCa) patients. A total of 15 PCa patients were enrolled in this study. For the dosimetry study, 5 PCa patients received whole-body planar scans at 0.5 h, 1 h, 2 h, 4 h and 8 h after 99mTc-HYNIC-PSMA injection. The Dosimetry Toolkit (GE, Milwaukee) was used to process the data and segment the organs in the SPECT/CT images, which were then projected onto planar images. The organ-specific absorbed doses, total-body absorbed doses and 99mTc-HYNIC-PSMA effective doses of patients were calculated using OLINDA/EXM 1.1 software. Whole-body SPECT/CT images were also acquired from additional 10 prostate patients to investigate the feasibility of 99mTc-HYNIC-PSMA for imaging tumors by calculating the ratio of tumor-to-background tracer uptake at 2 h after 740 MBq administration. The total-body absorbed dose was 1.54E-03 ± 2.43E-04 mGy/MBq, and the effective dose was 3.72E-03 ± 4.5E-04 mSv/MBq. Compared to published studies of other similar PSMA tracers and 99mTc-targeted conventional tracers, the absorbed doses of 99mTc-HYNIC-PSMA in all organs showed that it could be used safely in the human body. In addition, 99mTc-HYNIC-PSMA showed high tracer uptake (with a tumor-to-background ratio of 9.42 ± 2.62) in the malignant lesions of PCa patients, making it a promising radiopharmaceutical imaging method for site-specific management of PCa.
Dosimetry estimation and preliminary clinical application of [99mTc]Tc-HYNIC-PSMA-XL-2 in prostate cancer

Abstract:Objective: Molecular imaging of prostate-specific membrane antigen (PSMA) inhibitors has become a favorite for prostate cancer (PCa). This study aimed to estimate the dosimetry and the preliminary clinical application of the [99mTc]Tc-HYNIC-PSMA-XL-2, which is a novel imaging tracer invented by our team that can specifically targets PSMA for PCa and its metastases. Methods: The single-photon emission computed tomography (SPECT) whole-body (WB) planar images were collected on 6 patients at 0.5, 1.0, 2.0, 4.0 and 8.0 h after 99mTc-PSMA-XL-2 injection, respectively. The SPECT/computed tomography (CT) scan was carried out immediately following the WB planar image scan performed after 2.0 h. The volumes of interest (VOIs) of the bladder, heart wall, intestines, kidneys, liver, lungs, and spleen were segmented in the SPECT/CT images. VOIs of the salivary glands and the whole body were drawn in SPECT planar images. The dosimetry toolkit was used to process the data and project the SPECT/CT images onto planar images. The dosimetry analysis was performed using the IDAC-Dose dosimetry software. Furthermore, other PCa patients were enrolled to study the preliminary clinical application of [99mTc]Tc-HYNIC-PSMA-XL-2. Results: The clearance of [99mTc]Tc-HYNIC-PSMA-XL-2 is primarily by the hepatobiliary and intestinal system, due to its lipophilic characteristic. The effective half-life of [99mTc]Tc-HYNIC-PSMA-XL-2 is about 3.90 h. High absorbed doses were observed in the salivary glands (1.93E-02 ± 3.88E-03 mSv/MBq), kidneys (1.63E-02 ± 7.32E-03 mSv/MBq) and spleen (1.21E-02 ± 2.64E-03 mSv/MBq). The total body effective dose was 4.84E-03 ± 9.30E-05 mSv/MBq. The preliminary clinical case indicated that [99mTc]Tc-HYNIC-PSMA-XL-2 SPECT/CT could detect the primary prostate lesion, lymph node and bone metastases comprehensively. Conclusion: [99mTc]Tc-HYNIC-PSMA-XL-2 is a safe SPECT/CT tracer, which can detect prostate malignant lesions without interference from the bladder. In addition, the malignant lesions of the lymph node and bone of PCa patients also can be detected efficiently.

Abstract:Purpose: To quantify interfractional anatomical variations and their dosimetric impact during the course of fractionated proton therapy (PT) of prostate cancer and to assess the robustness of the current treatment planning techniques.Methods: Simulation and daily in-room CT scans from ten prostate carcinoma patients were analyzed. PT treatment plans (78 Gy in 39 fractions of 2 Gy) were created on the simulation CT, delivering 25 fractions to PTV1 (expanded from prostate and seminal vesicles), followed by 14 boost fractions to PTV2 (expanded from prostate). Plans were subsequently applied to daily CT, with beams aligned to the prostate center in the sagittal plane. For five patients having a sufficiently large daily imaging volume, structure contours were manually drawn, and plans were evaluated for all CT sets. For the other five patients, the plans were evaluated for six selected fractions. The daily CT was matched to the simulation CT through deformable registration. The registration accuracy was validated for each fraction, and the three patients with a large number of accurately registered fractions were used for dose accumulation.Results: In individual fractions, the coverage of the prostate, seminal vesicles, and PTV1 was generally maintained at the corresponding prescription dose. For PTV2, the volume covered by the fractional prescription dose of 2 Gy (i.e., V2) was, on average, reduced by less than 3% compared to the simulation plan. Among the 225 (39 x 5 + 6 x 5) fractions examined, 15 showed a V2 reduction larger than 5%, of which ten were caused by a large variation in rectal gas, and five were due to a prostate shift in the craniocaudal direction. The fractional dose to the anterior rectal wall was found to increase for one patient who had large rectal gas volume in 25 of the 39 fractions, and another who experienced significant prostate volume reduction during the treatment. The fractional bladder dose generally increased with decreasing fullness. In the total accumulated dose for the three patients after excluding a few fractions with inaccurate registration due to a large amount of rectal gas (a condition inconsistent with RTOG protocol), 98.5%, 96.6%, and 98.2% of the PTV2 received the prescription dose of 78 Gy. The V75 and V70 of the anterior rectal wall and bladder both remained within tolerance.Conclusions: The results confirm that the PT planning techniques and dose constraints used at our institution ensure that target coverage to the prescription dose is maintained in the presence of interfractional anatomical variations. Dose coverage in individual fractions can be compromised, and normal tissue dose increased, due to deviations in the bladder and rectal volume compared to the simulation plans or progressive changes in the prostate volume during the treatment. 99Mtc-Labeling and Evaluation of a HYNIC Modified Small-Molecular Inhibitor of Prostate-Specific Membrane Antigen.

Abstract:INTRODUCTION:Prostate-specific membrane antigen (PSMA) is a well-established target in the development of radiopharmaceuticals for the diagnosis and therapy of prostate cancer (PCa). In this study, we evaluated a novel 99mTc-labeled small molecular inhibitor of PSMA.METHODS:This new small-molecular inhibitor of PSMA, 6-hydrazinonicotinate-Aminocaproic acid-Lysine-Urea-Glutamate (HYNIC-ALUG) was radiolabeled by 99mTc and was evaluated both in vitro and in vivo using PCa models (PC-3 and LNCaP). Radiation dosimetry was assessed in mice.RESULTS:99mTc-HYNIC-ALUG showed excellent stability in different media. A cell assay preliminarily displayed its specificity for PSMA. The inhibitor showed good pharmacokinetics making it suitable for in vivo imaging. PC-3-derived tumors showed no obvious radioactive uptake; however, the LNCaP-derived tumors showed very high radioactive uptake which was significantly decreased by the selective PSMA inhibitor 2-PMPA. Biodistribution in LNCaP xenografts showed an optimum tumor-to-blood ratio of 24.23±3.54 at 2h. Tumor uptake was also decreased in the inhibition experiment with 2-PMPA (19.45±2.14%ID/g versus 1.42±0.15%ID/g at 2h). The effective dose of the 99mTc-HYNIC-ALUG was 8.4E-04mSv/MBq.CONCLUSIONS:A new 99mTc-labeled PSMA inhibitor with specific accumulation in PSMA-positive tumors and low background in other organs was synthesized. The radiopharmaceutical also showed very low radiation dosimetry. This agent may significantly improve the diagnosis, staging, and subsequent monitoring of therapeutic effects in PCa patients.
Preliminary Clinical Study of (99M)tc-Labelled Small Molecules Against PSMA for Prostate Cancer Imaging

Abstract:Background and purpose:Prostate-speciifc membrane antigen (PSMA), a cell surface protein with high expression in prostate carcinoma (PC) cells, is an attractive target for PC imaging and therapy. Small-molecule radiopharmaceuticals targeting PSMA can detect the location and extent of disease with high sensitivity and speciifcity. The aim of this study was to evaluate the value of technetium-99m-labelled small molecule against PSMA (HYNIC-Glu-Urea-A,99mTc-PSMA) for the detection of primary and metastatic prostate cancers.Methods:Twenty-four prostate cancer patients and 1 patient with benign prostate hyperplasia received whole-body scan followed by abdominopelvic SPECT/CT 2 h after intravenous injection of99mTc-PSMA. Tumor to muscle uptake ratio of99mTc-PSMA was calcu-lated using region of interest (ROI) technology. The sensitivity and specificity of99mTc-PSMA were evaluated. The relationships between positive99mTc-PSMA and prostate speciifc antigen (PSA) level and Gleason Score were analyzed. Results:Based on per patient, the sensitivity and speciifcity of99mTc-PSMA were 72.7% (16/22) and 100% (3/3), re-spectively. The level of PSA in patients with positive99mTc-PSMA imaging was signiifcantly higher than that in patients with negative99mTc-PSMA imaging [(PSA median 17.31 ng/mL, range: 2.26-3 239.0 ng/mL)vs(PSA median 0.49 ng/mL, range: 0.07-9.28 ng/mL)] (Z=-3.51,P<0.001). Among newly diagnosed patients and recurrent patients with PSA more than 2.0 nm/mL, it was apparent that99mTc-PSMA imaging was able to detect lesions with improved sensitivity of 94.1% (16/17). Gleason Scores between positive99mTc-PSMA patients and negative99mTc-PSMA patients were not significantly different (Z=-0.69,P=0.52).Conclusion:With the combination of whole-body scan and tomography, 99mTc-PSMA SPECT/CT can be an excellent and speciifc molecular imaging strategy to detect prostate cancer and its metastases.
[99mTc]Tc-HYNIC-ALUG SPECT/CT in the initial staging of 227 consecutive patients with newly diagnosed prostate cancer: a 5-year monocentric retrospective study

Abstract:Purpose The purpose of this study was to assess the effectiveness of [ 99m Tc]Tc-HYNIC-ALUG SPECT/CT in the initial staging of patients with newly diagnosed PCa. Methods A retrospective analysis was conducted on 227 consecutive patients who underwent [ 99m Tc]Tc-HYNIC-ALUG SPECT/CT imaging for the primary staging of newly diagnosed PCa. The presence and location of PSMA-positive lesions were determined, and the maximum standardized uptake values (SUVmax) of the primary prostate tumor were also measured. The metastatic findings and SUVmax were stratified according to International Society of Urological Pathology (ISUP) grade, prostate-specific antigen (PSA) levels, and D’Amico classification. Furthermore, the [ 99m Tc]Tc-HYNIC-ALUG SPECT/CT findings were compared to the histopathological findings in patients who had undergone radical prostatectomy with pelvic lymph node dissection (PLND). Results Of the 227 patients, 92.1% (209/227) had positive [ 99m Tc]Tc-HYNIC-ALUG SPECT/CT findings. Advanced disease was detected in 38.8% (88/227) of the patients and was positively correlated with increasing ISUP grade and PSA levels. Lymph node metastases (both pelvic and extrapelvic), bone metastases, and visceral metastases were detected in 30.0% (68/227), 25.6% (58/227), and 3.1% (7/227) of the patients, respectively. For the 129 patients who underwent radical prostatectomy with PLND, the sensitivity of [ 99m Tc]Tc-HYNIC-ALUG SPECT/CT in the evaluation of PCa was 90.7% (117/129). The sensitivity, specificity, accuracy, and positive and negative predictive values for detecting pelvic lymph node metastases on [ 99m Tc]Tc-HYNIC-ALUG SPECT/CT were 23.5% (12/51), 93.6% (73/78), 65.9% (85/129), 70.6% (12/17), and 65.2% (73/112), respectively. Among the 209 patients with PSMA-avid primary prostate disease, the SUVmax of the primary prostate tumor was significantly associated with ISUP grade (p<0.0001), PSA levels (p<0.0001), D’Amico classification (p<0.0001), and advanced disease (p<0.0001). Receiver operating characteristic (ROC) analysis revealed that a PSA level >19.8 ng/ml and SUVmax of the primary prostate tumor >7.4 had a sensitivity of 71.6% and 71.6% and specificity of 76.9% and 82.6%, respectively, for detecting metastatic disease. Conclusions [ 99m Tc]Tc-HYNIC-ALUG SPECT/CT emerges as a valuable imaging tool for the initial staging of newly diagnosed PCa. The presence of advanced disease and the SUVmax of the primary prostate tumor were positively correlated with ISUP grade and PSA levels.

Abstract:Background: Prompt and accurate diagnosis of prostate cancer (PCa) is of paramount importance for effective treatment planning. While Gallium-68 labeled prostate-specific membrane antigen (PSMA) positron emission tomography (PET)/computed tomography (CT) has proven efficacy in detecting PCa, limited availability poses challenges. As a potential alternative, [99mTc]Tc-PSMA single photon emission computed tomography (SPECT)/computed tomography (CT) holds promise. This systematic review and meta-analysis aimed to evaluate the diagnostic value of [99mTc]Tc-PSMA SPECT/CT for prostate cancer. Methods: A comprehensive search of PubMed, Cochrane, EMBASE, Scopus, Ovid, and Web of Science databases was conducted until July 2024. Sensitivity and specificity data were extracted to assess the diagnostic accuracy of [99mTc]Tc-PSMA SPECT/CT, while the Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) tool was used to evaluate study quality. Statistical analyses were performed using STATA 18, with MetaDisc 1.4 employed to detect threshold effects. Diagnostic accuracy indicators, including sensitivity, specificity, diagnostic odds ratio (DOR), negative likelihood ratio (LR-), and positive likelihood ratio (LR+), were pooled. The area under the curve (AUC) of the combined model was calculated using summary receiver-operating characteristic (SROC) curves. Results: Seven studies meeting the inclusion criteria were identified from an initial pool of 1467 articles, with no publication bias observed. The pooled sensitivity, specificity, and AUC of [99mTc]Tc-PSMA SPECT/CT were found to be 0.89 (95% CI, 0.84-0.93), 0.92 (95% CI, 0.67-0.99), and 0.93 (95% CI, 0.90-0.95), respectively. Additionally, the comprehensive diagnostic odds ratio, diagnostic score, positive likelihood ratio, and negative likelihood ratio were calculated as 95.24 (95% CI, 17.30-524.41), 4.56 (95% CI, 2.85-6.26), 11.35 (95% CI, 2.31-55.71), and 0.12 (95% CI, 0.08-0.18), respectively. Conclusions: In conclusion, our findings demonstrate that [99mTc]Tc-PSMA SPECT/CT exhibits favorable diagnostic performance for prostate cancer and can provide valuable supplementary information, particularly in regions and settings where [68Ga]Ga-PSMA PET/CT availability is limited, such as remote areas. These results highlight the potential of [99mTc]Tc-PSMA SPECT/CT as a valuable tool in the diagnosis and management of prostate cancer, warranting further investigation and validation in larger patient cohorts.

Abstract:Recently, prostate-specific membrane antigen (PSMA) has gained momentum in tumor nuclear molecular imaging as an excellent target for both the diagnosis and therapy of prostate cancer. Since 2008, after years of preclinical research efforts, a plentitude of radiolabeled compounds mainly based on low molecular weight PSMA inhibitors (PSMA-i) have been described for imaging and theranostic applications, and some of them have been transferred to the clinic. Most of these compounds include radiometals (e.g., 68Ga, 64Cu, 177Lu) for positron emission tomography (PET) imaging or endoradiotherapy. Nowadays, although the development of new PET tracers has caused a significant drop in single-photon emission tomography (SPECT) research programs and the development of new technetium-99m (99mTc) tracers is rare, this radionuclide remains the best atom for SPECT imaging owing to its ideal physical decay properties, convenient availability, and rich and versatile coordination chemistry. Indeed, 99mTc still plays a relevant role in diagnostic nuclear medicine, as the number of clinical examinations based on 99mTc outscores that of PET agents and 99mTc-PSMA SPECT/CT may be a cost-effective alternative for 68Ga-PSMA PET/CT. This review aims to give an overview of the specific features of the developed [99mTc]Tc-tagged PSMA agents with particular attention to [99mTc]Tc-PSMA-i. The chemical and pharmacological properties of the latter will be compared and discussed, highlighting the pros and cons with respect to [68Ga]Ga-PSMA11.

Abstract:AIMS: Proton beams deposit energy along their paths and stop abruptly without penetrating the opposite side, making it difficult to detect their actual paths. However, confirming the path may lead to evaluating the actual doses to organs at risk in proton therapy for prostate cancer. As proton beams produce positron emitters through nuclear fragmentation reactions, theoretically, proton beam paths can be measured by positron emission tomography/computed tomography (PET/CT). Therefore, this study investigated whether conducting PET/CT examinations immediately after proton beam therapy helps to assess the doses delivered to the rectal and urinary bladder walls, which are the major sites of radiation-related toxicity.MATERIALS AND METHODS: Between June 2022 and June 2023, 51 consecutive patients with prostate cancer who underwent proton beam therapy were enrolled and imaged with PET/CT to measure these radioactive particles and validate the actual dose delivered to the rectal and urinary bladder walls.RESULTS: The delivered doses assessed using PET/CT after proton beam therapy strongly correlated with the planned volume for proton beam treatment.CONCLUSIONS: PET/CT exhibited potential as a valuable tool for validating the irradiated dose to organs at risk.

Clinical value of 99mTc-PSMA SPECT/CT in detection of prostate cancer

Abstract:Objective:To evaluate the clinical value of technetium-99m labelled small molecule against prostate speciifc membrane antigen (PSMA) (HYNIC-Glu-Urea-A, 99mTc-PSMA) in the detection of primary and metastatic prostate cancers. Methods:A total of 69 consecutive prostate cancer patients who received 99mTc-PSMA SPECT/CT were retrospectively analyzed. The sensitivity of 99mTc-PSMA for the detection of the primary and metastatic lesions was evaluated. The relationships between positive 99mTc-PSMA and prostate speciifc antigen (PSA) level and Gleason score were analyzed. Results:Of all patients, 6 cases with PSA<0.2 ng/mL after radical prostatectomy remained biochemically disease-free. Based on per patient, in 77.8%(49/63) of the patients at least one lesion indicative of prostate cancer was detected by 99mTc-PSMA SPECT/CT. The level of PSA in the patients with positive 99mTc-PSMA imaging was signiifcantly higher than that in the patients with negative 99mTc-PSMA imaging (PSA median 25.52 ng/mL, range:0.85-3 239 ng/mL) vs. (PSA median 0.35 ng/mL, range:0.003-9.28 ng/mL)(P<0.001). Among newly diagnosed patients and recurrent patients with PSA>1.0 ng/mL, 99mTc-PSMA imaging was able to detect lesions with improved sensitivity of 97.4%(37/38);furthermore, among recurrent patients with PSA>10.0 ng/mL, the number and extent of lesions detected by 99mTc-PSMA SPECT/CT increased signiifcantly. Gleason score in the patients with positive 99mTc-PSMA imaging was signiifcantly higher than that in the patients with negative 99mTc-PSMA imaging (P<0.001). Conclusion:Among newly diagnosed prostate cancer patients and recurrent patients with PSA>1.0 ng/mL, 99mTc-PSMA SPECT/CT can detect prostate cancer and metastases in a high number of patients and can provide important information for clinical treatment decision.

Materials and methods Fifty-six men with suspected PCa between October 2019 and November 2022 were prospectively enrolled in this study. The median age of the patients was 70 years (range, 29-87 years). Patients were divided into high-(Gleason score>7, n=31), medium- (Gleason score=7, n=6) and low-risk groups (Gleason score < 7, n=6). All patients underwent 99m Tc-PSMA SPECT/CT and mpMRI at an average interval of 3 days (range, 1-7 days). The maximum standardized uptake value (SUV max ), the minimum apparent diffusion coefficient (ADC min ), and their ratio (SUV max /ADC min ) were used as imaging parameters to distinguish benign from malignant prostatic lesions. Results Of the 56 patients, 12 were pathologically diagnosed with a benign disease, and 44 were diagnosed with PCa. 99m Tc-PSMA SPECT/CT and mpMRI showed no significant difference in the detection of primary PCa (kappa =0.401, P =0.002), with sensitivities of 97.7% (43/44) and 90.9% (40/44), specificities of 75.0% (9/12) and 75.0% (9/12), and AUC of 97.4% and 95.1%, respectively. The AUC of SUV max /ADC min was better than those of SUV max or ADC min alone. When SUV max /ADC min in the prostatic lesion was >7.0×10 3 , the lesion was more likely to be malignant. When SUV max /ADC min in the prostatic lesion is >27.0×10 3 , the PCa patient may have lymph node and bone metastases. SUV max was positively correlated with the Gleason score ( r =0.61, P=0.008), whereas ADC min was negatively correlated with the Gleason score ( r =-0.35, P =0.023). SUV max /ADC min was positively correlated with the Gleason score ( r =0.59, P =0.023). SUV max /ADC min was the main predictor of the high-risk group, with an optimal cut-off value of 15.0×10 3 . Conclusions The combination of 99m Tc-PSMA SPECT/CT and mpMRI can improve the diagnostic efficacy for PCa compared with either modality alone; SUV max /ADC min is a valuable differential diagnostic imaging parameter.

Abstract:Objective:Prostate speciifc membrane antigen (PSMA) is a well-established target for diagnosis and therapy of prostate cancer (PCa). We have synthesized an amino-functionalized PSMA inhibitor (Glu-Urea-A) and labeled technetium-99m via HYNIC. Thein vivo target capacity of the new probe was evaluated in animal model and compared with18F-lfuorodeoxyglucose (18F-FDG) and11C-choline which were mostly used in clinic.Methods:The new probe was studied in severe combined immunodeifciency (SCID) mice bearing PC-3 and LNCaP tumor xenografts on small animal SPECT/CT. The comparative analysis of18F-FDG and11C-choline imagings was done in LNCaP tumor xenografts on small animal PET/CT. The tumor to muscle ratio wasobtained by the analysis software accompanying with small animal SPECT/CT and PET/CT respectively.Results:The imaging results showed that the metabolism of99mTc-HYNIC-Glu-Urea-A was very fast. Kidney was the primary excretory organ and little radioactive uptake was found in intestines. There was no signiifcant uptake in other organs. PC-3 tumor did not have any radioactive uptake. LNCaP tumor had very high radioactive uptake, and the tumor to muscle ratio was 20.4. The blocking experiment showed that the tumor uptake reduced signiifcantly. The tumor to muscle ratio obtained from PET/CT imaging were only 2.1 and 2.6 for 18F-FDG and11C-choline respectively, which were much lower than99mTc-HYNIC-Glu-Urea-A.Conclusion:The probe99mTc- HYNIC-Glu-Urea-A with fast metabolism and high tumor uptake was a promising molecular imaging probe targeting PSMA. It is superior to18F-FDG and11C-choline in prostate cancer imaging.

Abstract:The focus of this paper in on quality control and decay property analysis of 68Ga-DKFZ-PSMA-617 for positron emission tomography (PET) imaging of prostate cancer. 68Ga-DKFZ-PSMA-617 was synthesized in high radiochemical yield and specific activity (77 GBq/µmol) under optimized conditions. The decayed tracer was characterized by MADLI-TOF to support its decay and coordination characterization. Contrast PET imaging studies were performed using normal nude mice after intravenous injection of 18.5 MBq of 68Ga-DKFZ-PSMA-617 and 18F-FDG respectively. These results warrant the further clinical application of 68Ga-DKFZ-PSMA-617 in prostate cancer PET imaging.
Evaluation of PSMA-Targeted Theranostic Gold Nanoparticles for Prostate Cancer Imaging and Radiosensitization

Abstract:Radiation therapy is a commonly applied initial definitive treatment for prostate cancer. Prostate Specific Membrane Antigen (PSMA) is overexpressed in prostate cancer cells and is used as an effective diagnostic and therapeutic target. Targeting prostate cancer with anti-PSMA nanoparticles is a promising methodology, with the potential to minimize the side effects and toxicity of anticancer drugs. The objective of this study is to evaluate the radiobiological effects of PSMA-targeted gold nanoparticles (GNPs) as a theranostic tool for prostate cancer imaging and radiosensitization. The preparation of PSMA-targeted GNPs (PSMA-GNPs) was performed by covalently conjugating anti-PSMA monoclonal antibodies to 15 nm diameter GNPs. The number of GNPs that bind to PSMA positive prostate cancer cells were imaged and quantified by dark-field microscopy. The number of radiation-induced damage foci per cell were quantified by immunofluorescent staining for γ-H2AX. The ability of prostate cancer cells to grow into colonies was measured by colony formation assay. Prostate tumor xenograft models were propagated by the injection of prostate cancer cell lines orthotopically into immunodeficient mice. Mouse tumor xenografts were monitored by multimodality tumor imaging, including X-ray fluorescence (XRF), cone beam computed tomography (CBCT), and bioluminescence tomography. By dark-field microscopy, we found that PSMA-GNPs preferentially bind to PSMA positive LNCaP cells over PSMA negative PC3 cells. In PSMA positive LNCaP cells, we observed a 30% increase in DNA double strand breaks, as quantified by γ-H2AX foci number when PSMA-GNPs were added to radiotherapy (RT, single fraction 5 Gy), compared to RT plus control IgG nanoparticles. However, we did not observe that PSMA-GNPs enhanced DNA damage in PSMA negative PC3 cells. We also found a reduction in LNCaP colony formation by when PSMA-GNPs were added to RT. However, we did not observe radiosensitization in PSMA negative PC3 cells. In the tumor xenograft models, we found that PSMA-GNPs preferentially accumulate in PSMA positive LNCaP tumor xenografts by X-ray fluorescence. PSMA targeted GNPs have a highly specific binding efficiency to PSMA positive prostate cancer cells, which through X-ray fluorescence tomography may be used for the identification of tumor spatial distribution and quantification of nanoparticle binding. The addition of PSMA-GNPs to RT resulted in increased double strand breaks, cell death and inhibition of tumor growth. PSMA-GNPs are novel theranostic tools for tumor imaging and radiosensitization.

Abstract:Abstract Radiation therapy is moving from CT based to MRI guided planning, particularly for soft tissue anatomy. An important requirement of this new workflow is the generation of synthetic-CT (sCT) from MRI to enable treatment dose calculations. Automatic methods to determine the acceptable range of CT Hounsfield Unit (HU) uncertainties to avoid dose distribution errors is thus a key step toward safe MRI-only radiotherapy. This work has analysed the effects of controlled errors introduced in CT scans on the delivered radiation dose for prostate cancer patients. Spearman correlation coefficient has been computed, and a global sensitivity analysis performed following the Morris screening method. This allows the classification of different error factors according to their impact on the dose at the isocentre. sCT HU estimation errors in the bladder appeared to be the least influential factor, and sCT quality assessment should not only focus on organs surrounding the radiation target, as errors in other soft tissue may significantly impact the dose in the target volume. This methodology links dose and intensity-based metrics, and is the first step to define a threshold of acceptability of HU uncertainties for accurate dose planning.

Abstract:Objectives 99mTc-HYNIC-PSMA is a novel technetium-99m-labeled small-molecule inhibitor of prostate-specific membrane antigen (PSMA) for detection of prostate cancer. The present study investigated the diagnostic yield of 99mTc-HYNIC-PSMA Single photon emission computed tomography (SPECT)/CT in 147 patients with biochemically recurrent prostate cancer after radical prostatectomy. Methods 147 patients with biochemical relapse after radical prostatectomy were finally eligible for this retrospective analysis. The median prostate-specific antigen (PSA) level was 8.26 ng/mL (range, 0.22-187.40 ng/mL). Of the 147 patients, 72 patients received androgen deprivation therapy (ADT) at least 6 months before the 99mTc-HYNIC-PSMA SPECT/CT. All patients underwent planar whole-body scans and subsequent SPECT/CT of the thoracic and abdominal regions after intravenous injection of 705 ± 70 MBq of 99mTc-HYNIC-PSMA. Images were evaluated for the presence and location of PSMA-positive lesions, in which SUVmax were also measured. Detection rates were stratified according to PSA levels, ADT and Gleason scores. The relationships between SUVmax and clinical characteristics were analyzed using univariate and multivariable linear regression models for patients with positive findings. Results Of the 147 patients, 99mTc-HYNIC-PSMA SPECT/CT revealed at least one positive lesion in 118 patients with a high detection rate (80.3%). The detection rates were 48.6% (17/35), 85.1% (40/47), 92.1% (35/38), and 96.3% (26/27) at PSA levels of greater than 0.2 to 2, greater than 2 to 5, greater than 5 to 10, and greater than 10 ng/mL, respectively. PSMA SPECT/CT indicated local recurrence, lymph node metastases, bone metastases, and visceral metastases in 14 (9.5%), 73 (49.7%), 48 (32.7%) and 3 (2.0%) patients. The detection rates of local recurrence and metastasis increased with increasing PSA levels. The detection rate was higher in patients treated with ADT than those without (90.3% vs. 70.7%; P =0.0029). In patients with Gleason scores ≥8, detection rate was slightly higher than those with ≤7 (81.7% vs. 78.5%), but not statistically significant (P = 0.6265). Multivariable linear regression analysis showed a significant correlation of PSA levels and ADT with SUVmax (P=0.0005 and P=0.0397). Conclusions 99mTc-HYNIC-PSMA SPECT/CT offers high detection rates for biochemically recurrent prostate cancer after radical prostatectomy. The detection rate and SUVmax were positively correlated with PSA levels and ADT.

Abstract:Abstract Background Clinical management decisions on prostate cancer (PCa) are often based on a determination of risk. 68 Ga-prostate-specific membrane antigen (PSMA)-11-positron emission tomography (PET)/computer tomography (CT) is an attractive modality to assess biochemical recurrence of PCa, detect metastatic disease and stage of primary PCa, making it a promising strategy for risk stratification. However, due to some limitation of 68 Ga-PSMA-11 the development of alternative tracers is of high interest. In this study, we aimed to investigate the value of 18 F-PSMA-1007 in identifying non-metastatic high-risk PCa. Methods A total of 101 patients with primary non-metastatic PCa who underwent 18 F-PSMA-1007 PET/CT were retrospectively analyzed. According to the European Association of Urology guidelines on PCa, patients were classified into intermediate-risk (IR) group or high-risk (HR) group. The maximum standardized uptake values (SUVmax) of the primary prostate tumor were measured on PET/CT images. The diagnostic performance of PET/CT for IR and HR PCa was calculated, and the relationship between the SUVmax of primary prostate tumor, prostate-specific antigen (PSA) level and Gleason score (GS) was analyzed. Results Of all 101 patients, 49 patients were classified into IR group and 52 patients were classified into HR group. There was a significant positive correlation between PSA level/GS and SUVmax ( r = 0.561, r = 0.496, P < 0.001, respectively). Tumors with GS 6 and 7a showed significantly lower 18 F-PSMA-1007 uptake compared to patients with GS 8 and 9 ( P < 0.01). SUVmax in patients of HR was significantly higher than those of IR (median SUVmax: 16.85 vs 7.80; P < 0.001). In receiver operating characteristic curve analysis, the optimal cutoff value of the SUVmax for identifying high-risk PCa was set as 9.05 (area under the curve: 0.829; sensitivity: 90.4%; specificity: 65.3%). Conclusion 18 F-PSMA-1007 PET/CT showed the powerful diagnosis efficacy for high-risk PCa, which can be used as an objective imaging reference index for clinical reference.

Abstract:43 Background: Prostate cancer (PC) is a leading cause of cancer death in men worldwide. Imaging plays a key role in disease detection and initial staging. Emerging data has shown the superiority of 68 Gallium and 18 Fluorine labeled-PSMA imaging with PET/CT over conventional imaging for primary diagnosis. Despite the success, there are challenges in meeting the high demand and expensive costs especially in low-income countries. These challenges stimulated interest in 99m Technetium-labeled-PSMA agents. Single photon emission computer tomography (SPECT/CT) is more worldwide available, and the imaging agent is low cost, which is more promising. The aim of this study is to compare the diagnostic accuracy of 99m Tc‐PSMA-SPECT/CT to 18 F-PSMA-PET/CT in the primary diagnosis of PC and impact on clinical staging. Methods: Prospective controlled study, total of 16 patients with histologically confirmed PC (intermediate unfavorable and high risk) were recruited to undergo 18 F-PSMA-PET/CT and 99m Tc-HYNIC-PSMA-SPECT/CT. Mean age of patients was 71 (54-75) years and the median PSA level was 31.5 (4.3-920) ng/ml. Lesions were divided into prostate, seminal vesicles, lymph nodes (LN) (locoregional [LR] and non-locoregional [NLR]), bone, and visceral metastases. The size of LN with PSMA avidity on 99m Tc‐PSMA-SPECT/CT were determined. Lesions detected were read independently of PET/CT findings with two experienced nuclear medicine physicians. Volumetric analysis was also performed between the two imaging modalities (Tumoral Total Volume [TTV]) and correlated with PSA levels. Results: A total of 233 lesions were detected on 18 F-PSMA-PET/CT; prostate (n=16), seminal vesicles (n=11), LN LR (n=54), NLR (n=66), bone (n=82) and visceral (n=4); of these 99m Tc‐PSMA-SPECT/CT both reviewers detected 100% of the lesions in prostate (16/16), seminal vesicles (11/11) and visceral (4/4); LN LR (51/54; 94%), NLR (51/66; 77%) and bone (70/82; 85%). Between reviewers there was discordance in just 3 LN (<4 mm) and 1 bone metastases. The lowest diameter of positive LN detected by SPECT/CT was 4 mm (range 4-18). Pearson-correlation coefficient between PSA and TTV SPECT was r=0.68 (p=0.003) and for TTV PET r=0.71 (p=0.002). There were not statistically significant differences between TTV SPECT and TTV PET (T-student p=0.48). Five cases with high volume disease were detected by both methods. All patients were classified according to miTNM showing no difference. Conclusions: 99m Tc‐PSMA-SPECT/CT may be a useful in primary diagnosis of PC. Despite it showed a slightly lower lesion detection rate compared to 18 F-PSMA-PET/CT, it exhibited no impact on clinical staging and consequently the initial treatment intension. Our results show that 99m Tc‐PSMA-SPECT/CT could be used as a cost-efficient alternative with wider availability and no significant difference.

Abstract:Purpose To evaluate the diagnostic performance of PSMA PET/CT, including [ 68 Ga]Ga-PSMA-11 and [ 18 F]DCFPyL, in comparison with the [ 99m Tc]Tc-MDP bone scan (BS) in identifying bone metastases among prostate cancer patients. Methods A search was performed in the PubMed and Embase databases to locate pertinent publications from inception to February 12, 2024. The studies included were those that examined the diagnostic effectiveness of PSMA PET/CT (covering [ 68 Ga]Ga-PSMA-11 and [ 18 F]DCFPyL) compared to [ 99m Tc]Tc-MDP BS in identifying bone metastases among prostate cancer patients. The quality of the selected studies was evaluated using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) checklist. Results The meta-analysis included nine articles involving 702 patients. The sensitivity of PSMA PET/CT was higher compared to [ 99m Tc]Tc-MDP BS (0.98 vs. 0.85, P < 0.01), while the specificity of PSMA PET/CT was also higher than [ 99m Tc]Tc-MDP BS (0.97 vs. 0.70, P < 0.01). In subgroup analysis, the sensitivity of [ 68 Ga]Ga-PSMA-11 PET/CT was higher compared to [ 99m Tc]Tc-MDP BS (0.98 vs. 0.86), while the specificity of [ 68 Ga]Ga-PSMA-11 PET/CT was also higher than [ 99m Tc]Tc-MDP BS (0.98 vs. 0.65). Conclusion Our meta-analysis demonstrates that PSMA PET/CT exhibits superior sensitivity and specificity in comparison with [ 99m Tc]Tc-MDP BS for identifying bone metastases in prostate cancer patients. Further research with head-to-head design is necessary to validate these results and evaluate the clinical effectiveness of these imaging methods. Abstract:Objective To investigate the distribution of 99mTc-HYNIC-TOC in normal mice and its imaging features in the somatostatin receptor(SSTR)-positive nude mice bearing pancreatic tumor(AR42J) as well as toxicity, radiation dose and radiation assessment. Methods Mice were injected with 99mTc-HYNIC-TOC through tail vein and killed respectively at 0.5, 1, 2, 4, 6 h after injection. The radioactive counting of different organs were determined and converted to standard distribution in humans, and then the radiation and absorbed doses of 99mTc-HYNIC-TOC in human organs and whole body were calculated with OLINDA/EXM software. Serial animal SPECT/CT scans were acquired 2 and 4 h after injection of 99mTc-HYNIC-TOC(0.2 mL/1 mCi) in nude mice bearing AR42J tumor. Results The blood radioactivity decreased rapidly and the tracer excreted mostly via the kidneys and urinary system in normal mice. SSTR-positive nude mice bearing AR42J tumor was imaged clearly 2 h after injection when the ratio of tumor to non-tumor was up to 7.1. The study of radiation dose demonstrated that the intestine absorbed the most body radiation which was 6.40E-03μGy/MBq, and the effective dose was 4.59μSv/MBq. Conclusion 99mTc-HYNIC-TOC is a promising and safe tracer for SSTR-positive tumor imaging.
