Coronary Artery Bypass Grafting in Patients with Severe Coronary Artery Lesions Caused by Kawasaki Disease

Abstract:Objective: To investigate the result of coronary artery bypass grafting (CABG) in patients with severe coronary artery lesions caused by Kawasaki disease. Methods: We retrospectively analyzed 15 CABGs performed in 6 patients (5 male and 1 female) by angiography. Lesions involved the left anterior descending coronary artery (LAD) in 6patients, the right coronary artery (RCA) in 5, the left circumflex artery (LCX) in 3 and the left main coronary artery (LMT) in 2. Three patients had history of previous myocardial infarction. Left internal thoracic artery (LITA) grafts to the LAD were performed in six patients, right internal thoracic artery (RITA) grafts to the LAD in 1 and to the LCX in 1. Right gastroepiploic artery (RGEA) grafts to the RCA were performed in two patients. Saphenous vein grafts (SVGs) to the LAD were performed in three patients and to LCX in 2. Results: One month after the operation, RGEA graft to the RCA was stenotic in one patient, LITA graft to the LAD was occluded in one patient, and the remaining grafts were patent. During the follow-up period of 1 to 4 years, a SVG to the LCX and a LITA graft to the LAD were severely stenotic in one patient who received four CABGs. The left ventricular contraction was improved and no myocardial infarction was noted in any patients after bypass surgery. Conclusion: CABG could improve the left ventricular contraction and decrease the occurrence of myocardial infarction after surgery. However, occlusion and stenosis rate of grafts is comparatively higher.
Clinical Outcomes and Risk Factors of Coronary Artery Aneurysms after Successful Percutaneous Coronary Intervention and Drug-Eluting Stent Implantation for Chronic Total Occlusions

Abstract:ObjectiveThe study aimed to analyze the risk factors and long-term outcomes associated with coronary artery aneurysms (CAAs) after successful percutaneous coronary intervention (PCI) and drug-eluting stent (DES) implantation in patients with CTOs.BackgroundThere are sporadic data available on post-procedure CAAs after transcatheter revascularization for CTOs.Methods and resultsA total of 141 patients with 149 CTOs who underwent successful CTO-PCI and DES implantation with angiographic follow-up from 2004 to 2010 were included. Patients were divided into CAA group and non-CAA group according to the presence of CAAs in the follow-up angiography. The independent predictors and major adverse cardiac events (MACEs) including cardiac death, myocardial infarction (MI) and target-vessel revascularization (TVR) were compared between two groups. The incidence of CAAs was 11.4% (17/149) after index procedure. Multivariate analysis showed that age (OR: 0.925, CI 0.873–0.980, P = 0.008), ostial occlusion (OR: 6.715, CI 1.473–30.610, P = 0.014), the parallel wire technique (OR: 6.167, CI 1.709–22.259, P = 0.005) and DES length (OR: 1.030, CI 1.002–1.058, P = 0.036) were the independent predictors of CAAs after successful CTO-PCI and DES implantation. MACEs were similar between two groups (adjusted hazard ratio 0.670; 95% CI 0.160–2.808; P = 0.584) during the 5-year follow-up.ConclusionsThe independent predictors of CAAs after successful CTO-PCI and DES implantation are age, ostial occlusion, the parallel wire technique and DES length. CAAs after index procedure are not frequently associated with adverse clinical events under dual antiplatelet therapy. Further large clinical studies are warranted to explore the clinical implications of patients with this distinct new entity.
Heart Team Intervention for Calcified Left Main Coronary Disease and Jeopardized Left Internal Mammary Artery Graft

Abstract:It is sometimes difficult to identify the culprit lesion and treatment strategy in patients with acute coronary syndrome who have complex coronary lesions and jeopardized left internal mammary artery graft. This report describes a heart team approach for a non-ST-segment elevation myocardial infarction case with complex coronary vasculature. A 73-year-old man presented to the emergency department with crescendo angina. He had a history of total aortic arch replacement with concomitant coronary artery bypass graft using left internal mammary artery. Emergent coronary angiography demonstrated severe stenosis at left main trunk bifurcation caused by calcified nodule. While the bypass graft to left anterior descending coronary artery was patent, the proximal segment of left subclavian artery was occluded. Following the prompt discussion with our heart team, we performed percutaneous coronary intervention in the first step for treating the left main stenosis using rotational atherectomy into the unprotected left circumflex artery. After clinical recovery, stress myocardial scintigraphy identified the presence of anteroseptal ischemia, which indicated coronary subclavian steal syndrome due to left subclavian artery occlusion. Contrast-enhanced CT visualized that the occlusion originated from the anastomosis, suggesting the potential procedural risk of endovascular treatment by dilatation. Our heart team discussed again and decided to undergo axillo-axillary artery bypass surgery. He was discharged 8 days after the surgery without any sequelae. This is the rare case report of non-ST-segment elevation myocardial infarction who had similar condition to coronary subclavian steal syndrome after total aortic arch replacement. This case highlights the importance of a collaborative approach of the heart team to identify the best therapeutic strategy in a patient with complex coronary vasculature.

Abstract:RATIONALE:With the development and standardization of modern chronic total occlusions (CTOs) recanalization technique, percutaneous coronary intervention has become a promising treatment alternative to surgery after bypass graft failure. Treatment of a native coronary CTO lesion is preferable to treatment of a saphenous vein graft (SVG) CTO supplying the same territory; however, technical expertise is required.PATIENT CONCERNS:This is a 69-year-old male with prior history of coronary artery bypass grafting presented with severe dyspnea at mild exertion (NYHA III) of 2 months duration.DIAGNOSIS:The patient was diagnosed as heart failure caused by ischemia after SVG failure (SVG to right coronary artery) according to electrocardiogram, plasma N-terminal pro-B-type natriuretic peptide levels, and coronary angiogram.INTERVENTIONS:We recanalized native right coronary artery CTO by retrograde approach using septal collaterals by surfing technique after recanalization of totally occluded left coronary artery.OUTCOMES:Dyspnea was relieved at discharge. At 6-month follow-up, the patient had no recurrence of dyspnea.LESSONS:In case of SVG failure, percutaneous coronary intervention of native vessel should be considered as a treatment option. Retrograde approach through native vessel is safe but has requirements for operators' volume, skill, and experience.
[Coronary Intervention in 1148 Patients with Chronic Total Coronary Occlusion].

Abstract:OBJECTIVE:To assess the technique and outcomes of coronary intervention treatment for patients with chronic total occlusion (CTO) lesions.METHODS:Retrospectively analyzed the data from 1148 patients with CTO who were treated by percutaneous coronary intervention (PCI) between June 1993 and December 2004. There were 1494 CTO lesions in 1458 vessels with a mean occlusion time of (49.1 +/- 31.6) months. A total of 1499 stents were implanted to those target lesions.RESULTS:Total success rates of patients and lesions were 90.2% (1036/1148) and 88.2% (1317/1494), respectively. The success rate of PCI was declined in CTO lesions with long occlusion time, stump missing, bridging collaterals and >or= 15 mm in length (P < 0.05). Operative failures were occurred in 112 patients. Guide wire and balloon failed to pass through the occlusion accounted for 82.1% and 17.9% of all failures respectively. No death was occurred during operation, but 2 patients suffered from acute stent thrombosis and other 9 patients suffered from acute or later pericardium perforation. After procedure, there were 3 patients died and 3 patients suffered from subacute stent thrombosis during in-hospital period. The total in-hospital major adverse cardiac events rate was 0.6% (7/1148). The angina-free survival rate was 87.1% at discharge.CONCLUSION:In an experienced heart center, it is possible to obtain a relatively high success rate of PCI and ideal clinical outcomes in patients with CTO coronary lesions.

Abstract:Objective To analyze the surgical characteristics and clinical outcomes of 11 cases with total occlu- sion of left main(LM)coronary artery.Methods 11 patients with total occlusion of LM underwent coronary artery bypass grafting(CABG),accounted for 0.7% of total CABGs in the same period of time.The mean age was 60.0 years old.There were 4 cases of acute occlusion of LM with cardiogenic shock,including 2 cases of cardiopulmonary resuscitation.The oth- er 7 cases were chronic occlusion of LM with mean left ventricular ejection fraction of 43.3%.Eight cases were concur- rently with severe lesion in right coronary artery and 6 cases without right-left collateral circulation.In all 11 cases,4 cas- es were rescue procedures,3 were emergency and 14 were urgent.Results Eight were off-pump and 3 were on-pump CABGs.The mean number of distal anastamosis was 3.1±0.7.There were 6 patients with intra-aortic balloon pump,1 case with left ventricular assist device and 1 with extracorporeal membrane oxygen(ECMO).Five cases died,3 of which were rescue surgery with acute occlusion of LM.Another 2 deaths related to ventricular fibrillation postoperatively in patients with low LVEF.The 6 survivals were followed up from 1 to 54 months without late deaths.Conclusions Total occlusion of LM accounts for high hospital mortality and high risks for surgical treatment.There will still be a challenge to surgeons in acute occlusion of LM with ongoing deterious hemodynamics.

Abstract:Coronary chronic total occlusion (CTO), which occurs in 18. 4–52% of all patients referred for coronary angiography, represents one of the last barriers in coronary intervention. Approximately half of all patients with prior coronary artery bypass graft (CABG), who undergo coronary angiography, are diagnosed with coronary CTO. In fact, these patients often develop recurrent symptoms and events, necessitating revascularization. Currently, there is neither a consensus nor developed guidelines for the treatment of CTO patients with prior CABG, and the prognosis of these patients remains unknown. In this review, we discuss current evidence and future perspectives on CTO revascularization in patients with prior CABG, with special emphasis on clinical and lesion characteristics, procedural success rates, periprocedural complications, and long-term outcomes.

Percutaneous Coronary Intervention for Aorto-Ostial Chronic Total Occlusion: Evaluating Lesion Complexity and Procedural Outcomes

Abstract:Background: PCI for aorto-ostial CTO remains challenging. The techniques for guidewire in aorto-ostial CTO may differ from those used in non-aorto-ostial CTOs, influenced by clinical and angiographic characteristics. Objectives: This study aimed to assess the technical aspects and outcomes of percutaneous coronary intervention (PCI) in patients with aorto-ostial chronic total occlusion (CTO). Methods: This analysis included 420 patients with ostial CTO from the Japanese CTO-PCI Expert Registry, spanning January 2014 to December 2022. It examined the strategies and procedural outcomes of CTO PCI. Results: Ostial CTO represented 420 of 10,814 (3.9%) of all CTO PCI cases. Within this subset, aorto-ostial CTO accounted for 218 of 420 (52%) cases. The technical success rate for aorto-ostial CTO was 88% (191/218). Aorto-ostial CTOs exhibited longer lesion lengths and were more likely to present with challenges such as distal target lumen ambiguity, calcification, and tortuosity compared with non-aorto-ostial CTOs. The retrograde approach was more commonly used in aorto-ostial CTO, with retrograde direct crossing being the most successful technique, especially in cases of flush CTO. A multivariate logistic analysis identified several factors significantly associated with unsuccessful aorto-ostial CTO PCI, including difficulties in engaging a guiding catheter, estimating the collateral channel (as assessed by the J-Channel score), and tortuosity. Conclusions: Aorto-ostial CTOs are more complex and frequently require a retrograde approach. The availability of suitable interventional collateral channels is crucial for the success of these procedures.
Impact of coronary total occlusion on graft failure and outcomes of coronary artery bypass grafting

Abstract:ObjectiveThe study objective was to assess the impact of chronic total occlusion on long-term graft failure and outcomes in patients who underwent coronary artery bypass grafting.MethodsWe conducted an observational study involving a single-center subgroup of the CORONARY trial. At 6 to 9 years after coronary artery bypass grafting, all alive patients were invited for coronary computed tomography angiography and clinical follow-up. We assessed the association between chronic total occlusion graft and failing graft showing Fitzgibbon type B or O. Risk factors associated with chronic total occlusion graft failure were assessed. The impact of chronic total occlusion on clinical outcomes was analyzed, including death, myocardial infarction, and repeated revascularization.ResultsA total of 349 patients undergoing coronary artery bypass grafting were enrolled between May 2007 and October 2011. Of 301 alive patients at follow-up time (median, 6.8 years; interquartile range, 6.0-8.0 years), repeat coronary computed tomography angiography was performed in 206 patients (68.4%) with 723 grafts (154 chronic total occlusion grafts and 569 nonchronic total occlusion grafts). Chronic total occlusion graft was significantly associated with an increased risk of long-term graft failure after adjustment for patient- and graft-level characteristics (adjusted odds ratio, 2.27; 95% confidence interval, 1.42-3.62; P < .001). Arterial graft, side-to-side anastomosis, higher graft flow, and antiplatelet therapy at discharge were associated with chronic total occlusion graft patency. The presence of 1 or more chronic total occlusions was not significantly associated with long-term composite of death, myocardial infarction, or repeat revascularization (adjusted hazard ratio, 0.91; 95% confidence interval, 0.54-1.51; P = .707).ConclusionsChronic total occlusion graft was associated with an increased risk of graft failure. Surgical technique and guideline-directed medical therapy should be noted to improve chronic total occlusion graft patency.

Potential risk of provisional stenting in left main coronary artery bifurcation in multivessel-related acute coronary syndrome

Abstract:We present a case of an elderly man suffering from an acute coronary syndrome (ACS) with preshock vital signs and remarkable ST-T wave depression in leads V4-V6, and ST elevation in lead aVR. Coronary angiography showed total occlusion of the right coronary artery (RCA) and impending occlusion in the distal left main coronary artery (LMCA) with a tandem lesion in the proximal left anterior descending artery (LAD). After insertion of an intra-aortic balloon pump both the LAD and left circumflex artery (LCX) were dilated alternatively; and cross-over stenting in the LMCA bifurcation was subsequently performed. However, total occlusion of the LCX occurred and it caused acute hemodynamic collapse and ventricular fibrillation storm. Immediate installation of percutaneous cardio-pulmonary support system allowed stent deployment to be performed in the RCA and subsequent reopening of the LCX that led to a return to sinus rhythm. The patient recovered almost normal left ventricular wall motion and previous activity without any neurological deficit within 2 weeks. Provisional stenting in ACS in the LMCA bifurcation with multivessel disease has a potential risk of acute hemodynamic collapse; a planned two-stent deployment strategy may assure a higher rate of safety in such cases.

Abstract:Abstract Background and aim of the study : There is still controversy with regard to adequate therapy for symptomatic patients who are refractory to medical management with myocardial bridges of the left anterior descending artery. This study sought to evaluate the treatment of symptomatic coronary myocardial bridges with coronary artery bypass graft surgery. Methods: Thirteen patients who underwent surgery between October 2005 and May 2008 were included in this study. The mean patient age was 51.3 ± 10.2 years (range, 39–75 years). There were ten men and three women. All 13 patients had angina pectoris preoperatively, and they had myocardial bridges only. Coronary angiography prior to surgery demonstrated myocardial bridging of the left anterior descending artery with systolic compression ≥75% in all patients. They underwent off‐pump coronary artery bypass grafting using the internal mammary artery. Results: The acute clinical success rate was 100% with respect to the absence of myocardial infarction, death, or other major in‐hospital complications. Postoperative coronary CT angiography studies in seven patients after one year demonstrated no graft stenosis. During follow‐up, no patient had symptoms of angina. Conclusions: Coronary artery bypass graft surgery using a LIMA graft is a safe and effective treatment for symptomatic coronary myocardial bridges. (J Card Surg 2012;27:405‐407)
[Off-pump Coronary Artery Bypass Grafting for Anomalous Origin of the Left Coronary Artery: Report of a Case]

Abstract:Anomalous origin of the left coronary artery from the right sinus of Valsalva is a rare anomaly known to cause sudden death in young competitive athletes. We report a case of 58-year-old male who was admitted to our hospital because of acute coronary syndrome. Emergency coronary angiography documented a critical stenosis in the right coronary artery. A cardiologist implanted a stent with good angiographic result. It was not possible to place a catheter in left coronary artery ostium at normal position. The patient was diagnosed to have an anomalous origin of the left coronary artery arising from the right sinus of Valsalva, passing between the aorta and the main pulmonary artery by coronary 3 dimensional computed tomography angiography (3D-CTA). After a stent insertion, he still had angina. Further examination of the left coronary artery including intravascular ultrasound revealed a hard plaque in the left main trunk as the culprit lesion. Because previous percutaneous coronary intervention was ineffective, off-pump coronary artery bypass grafting was performed using bilateral internal thoracic arterial grafts to the left anterior descending artery and the obtuse marginal branch. Postoperative 3D-CTA demonstrated patent grafts. Two years after the operation, he is free from chest discomfort.

Abstract:A 75-year-old man who presented with chest pain at rest was admitted to our hospital. He had a history of percutaneous coronary intervention (PCI) on the obtuse marginal branch of the left circumflex artery (LCX) with the implantation of a sirolimus-eluting stent 15 years prior. Coronary angiography revealed an in-stent occlusion of LCX and an occlusion of the proximal right coronary artery (RCA). Urgent PCI was performed on LCX as it was considered the culprit lesion. Even after revascularization, a contralateral injection showed inadequate collateral flow to RCA, and the distal end of the chronic total occlusion (CTO) remained unclear. To assess the extracardiac collateral circulation, we conducted computed tomography angiography (CTA). Computed tomography angiography revealed a relatively short CTO with a collateral channel originating from an anomalous bronchial artery exhibiting dilation and tortuosity ( Panel A ). Selective bronchial arteriography allowed visualization of the distal end of CTO of RCA ( Panel B and Supplementary material online , Video ), and PCI was effectively performed with an antegrade approach.

[Hybrid Coronary Revascularization of Minimally Invasive Coronary Artery Bypass Grafting and Percutaneous Coronary Intervention in a Patient with Left Main Coronary Stenosis]

Abstract:We report a successful case of hybrid coronary revascularization of minimally invasive coronary artery bypass grafting( MICS-CABG) and percutaneous coronary intervention(PCI). The patient was a 78-year-old man with angina pectoris due to left main trunk (LMT) lesion, and had a history of repeated PCI to the left anterior descending artery (LAD) and the left circumflex artery (LCX) for angina pectoris. He presented with a chest pain on effort in June, 2015. A coronary angiogram showed a severe stenosis in the LMT extending to LAD and LCX. We performed hybrid therapy of CABG to LAD, and PCI to LMT and the proximal portion of LCX because the lesion was technically and suitable for PCI. CABG to LAD was performed via left mini thoracotomy using the left inter mammary artery (LIMA). LIMA was harvested under 3-dimentional endoscope. On the 5th post-operative day, PCI was performed to LMT and LCX. The postoperative course was uneventful and he was discharged on the 11th post-operative day. This case suggests that hybrid coronary revascularization is less invasive and feasible for selected patients with multi-vessel disease.
Impact of Coronary Chronic Total Occlusion on Long-term Clinical Outcome in Patients with Unprotected Left Main Disease Undergoing Percutaneous Coronary Intervention

Abstract:Objectives: Reported data regarding the prevalence, prognostic impact, and safety and efficacy of revascularization of coronary chronic total occlusion (CTO) in patients with left main coronary artery (LMCA) disease who undergo percutaneous coronary intervention (PCI) are scarce. The aim of the present study was to compare clinical outcomes among patients with LMCA disease undergoing PCI. Outcomes were compared between those with and without coronary CTO and between those with CTO who had successful and unsuccessful CTO recanalization procedures. Methods: All consecutive patients with significant LMCA disease (>50% stenosis at coronary angiography) who underwent PCI between July 2014 and December 2018 were retrospectively included in our study. The primary endpoint of the study was long-term mortality. Secondary endpoints included the incidence of myocardial infarction, repeat percutaneous or surgical revascularization, stroke, and stent thrombosis. Results: Between July 2014 and December 2018, 578 patients underwent PCI for LMCA disease at Pederzoli Hospital and University of Turin were enrolled. They were divided into 3 groups: group A: 374 (65%) patients without CTO, group B: 108 (19%) patients with untreated or unsuccessfully treated CTO, and group C: 96 (17%) patients with successfully treated CTO. At a median follow-up of (1090 ± 279) days, there were no statistically significant differences between the groups in terms of the primary and secondary endpoints. However, there was a trend towards higher mortality in patients with untreated or unsuccessfully treated CTO (13% vs. 19% vs. 14% in groups A, B, and C, respectively; P = 0.12). The primary and secondary endpoints were further analyzed based on the presence or absence of myocardial viability: subgroup C1: 54 (56%) patients with successful percutaneous transluminal coronary angioplasty (PTCA) having viability, and subgroup C2: 42 (44%) patients with successful PTCA not having viability. There was a trend toward a statistically significant higher rate of death among patients in group B, who underwent unsuccessful recanalization with viable myocardium (19% vs. 9% vs. 19% in groups B, C1, and C2, respectively, P = 0.05). On multivariable analysis, the propensity for successful revascularization of CTO was associated with a reduced risk of death (P = 0.01; odds ratio, 0.75; 95% confidence interval: 0.62–0.87). Conclusions: Among patients with LMCA disease undergoing PCI, CTO represents a common finding associated with worse prognosis. Successful revascularization of CTO in patients with viable myocardium appears to significantly improve prognosis.

Abstract:Perfusion, Ahead of Print. Aneurysms of the left main coronary artery are rare clinical entities with an incidence of 0.1%. We herein report a case of left main trifurcation aneurysm in a 57-year-old woman whose coronary angiography demonstrated a giant saccular aneurysm of trifurcation of the left main coronary artery, measuring 32 × 21 mm with tight post-aneurysmal stenosis in the intermediate artery. The patient underwent coronary artery bypass grafting following the resection of CAA and reconstruction with saphenous vein roof-plasty, which ensured the preservation of the native coronary vasculature. At the 6-month follow-up, she remained asymptomatic and preserved preoperative left ventricular function.

Abstract:Objective To analyze the clinical, imaging and interventional data of patients with chronic total occlusion (CTO) lesions without myocardial infarction (MI) and to summarize the clinical and imaging characteristics of these patients. Methods The data of 2651 patients with CTO verified by coronary angiography between January 1995 and December 2014 were analyzed retrospectively. Results There were 1466 CTO patients (55. 3%) without MI (the control group) and 1185 CTO patients (44. 7%) with MI ( the MI group). The age, percentage of female patients, unstable angina, hypertention, mean triglyceride levels, left ventricular ejection fraction ( LVEF) were lower in the MI group than in the control group ( all P﹤0. 05). The rates of heart failure and serum creatinine levels were higher in the MI group than the control group (both P﹤0. 05). The rate of multi-vessel disease was higher in the control group than in the MI group (81. 4% vs. 76. 5%, P﹤0. 05). According to the target CTO vessel location, patients in the control group had lower rates of CTO in LAD (36. 2% vs. 40. 7%, P=0. 007) and higher rates of CTO in LCX (17. 0%vs. 12. 7%, P﹤0. 001). Patients in the control group without MI had better collateral circulation than that in the control group (32. 7% vs. 27. 0%, P﹤0. 001). There were no differences in success rate of PCI and complete revascularization between the two groups. Conclusions The present study showed that the CTO patients without MI were associated with better collateral development compared with the CTO patients with MI. Age, gender, unstable angina encouraging ischemic preconditioning and hypertension may be beneficial by facilitating collateral development through endogenous cardioprotective mechanisms.
