Abstract:Chronic total occlusion (CTO) coronary artery disease is defined as the occurrence of complete occlusion of 1 or more coronary arteries, resulting in the interruption of forward blood flow, and the duration of occlusion is >3 months. [ 1 ] Coronary CTO lesions is a serious stage of coronary artery disease, and the clinical manifestations are dominated by chest tightness, chest pain, and other manifestations of coronary artery disease. CTO lesions can lead to angina pectoris, myocardial infarction, heart failure, and even sudden death. Epidemiologic investigation shows that the incidence of coronary CTO lesions in patients with conventional angiography is about 20%, [ 2 ] accounting for >30% of coronary heart disease, but <10% of patients receive surgical treatment. [ 3 ] Currently, there are 3 main treatment modalities for CTO lesions: drug therapy, coronary artery bypass grafting therapy, and percutaneous coronary intervention (PCI). [ 4 , 5 ] Interventional therapy has the advantages of less trauma and faster postoperative recovery, which is the key development direction of current surgical treatment and the most valuable treatment modality for research. [ 6–8 ] Studies have shown that recanalization of the coronary artery at occlusion can significantly reduce the incidence of adverse cardiovascular events. [ 9 , 10 ] However, it has been suggested that after the establishment of collateral circulation in the occluded coronary artery, the opening of the occluded coronary artery does not have much effect on the improvement of adverse outcomes in patients. [ 11 ] There are still many research gaps regarding the pathogenesis, natural history, and long-term prognosis of CTO lesions after recanalization. As well as the low recanalization rate, high restenosis rate, unclear long-term prognosis, and high risk of intraoperative complications, the treatment difficulties of CTO lesions have hindered the research process of this disease.

Clinical Outcomes and Risk Factors of Coronary Artery Aneurysms after Successful Percutaneous Coronary Intervention and Drug-Eluting Stent Implantation for Chronic Total Occlusions

Abstract:ObjectiveThe study aimed to analyze the risk factors and long-term outcomes associated with coronary artery aneurysms (CAAs) after successful percutaneous coronary intervention (PCI) and drug-eluting stent (DES) implantation in patients with CTOs.BackgroundThere are sporadic data available on post-procedure CAAs after transcatheter revascularization for CTOs.Methods and resultsA total of 141 patients with 149 CTOs who underwent successful CTO-PCI and DES implantation with angiographic follow-up from 2004 to 2010 were included. Patients were divided into CAA group and non-CAA group according to the presence of CAAs in the follow-up angiography. The independent predictors and major adverse cardiac events (MACEs) including cardiac death, myocardial infarction (MI) and target-vessel revascularization (TVR) were compared between two groups. The incidence of CAAs was 11.4% (17/149) after index procedure. Multivariate analysis showed that age (OR: 0.925, CI 0.873–0.980, P = 0.008), ostial occlusion (OR: 6.715, CI 1.473–30.610, P = 0.014), the parallel wire technique (OR: 6.167, CI 1.709–22.259, P = 0.005) and DES length (OR: 1.030, CI 1.002–1.058, P = 0.036) were the independent predictors of CAAs after successful CTO-PCI and DES implantation. MACEs were similar between two groups (adjusted hazard ratio 0.670; 95% CI 0.160–2.808; P = 0.584) during the 5-year follow-up.ConclusionsThe independent predictors of CAAs after successful CTO-PCI and DES implantation are age, ostial occlusion, the parallel wire technique and DES length. CAAs after index procedure are not frequently associated with adverse clinical events under dual antiplatelet therapy. Further large clinical studies are warranted to explore the clinical implications of patients with this distinct new entity.

Abstract:Coronary chronic total occlusions (CTO) are present in up to one-third of patients with coronary artery disease (CAD). It is thus essential for all clinical cardiologists to possess a basic awareness and understanding of CTOs, including optimal evaluation and management. While percutaneous coronary intervention (PCI) for CTO lesions has many similarities to non-CTO PCI, there are important considerations pertaining to pre-procedural evaluation, interventional techniques, procedural complications, and post-procedure management and follow-up unique to patients undergoing this highly specialized intervention. Distinct from other existing topical reviews, the current manuscript focuses on key knowledge relevant to non-interventional cardiologists.

[Coronary Intervention in 1148 Patients with Chronic Total Coronary Occlusion].

Abstract:OBJECTIVE:To assess the technique and outcomes of coronary intervention treatment for patients with chronic total occlusion (CTO) lesions.METHODS:Retrospectively analyzed the data from 1148 patients with CTO who were treated by percutaneous coronary intervention (PCI) between June 1993 and December 2004. There were 1494 CTO lesions in 1458 vessels with a mean occlusion time of (49.1 +/- 31.6) months. A total of 1499 stents were implanted to those target lesions.RESULTS:Total success rates of patients and lesions were 90.2% (1036/1148) and 88.2% (1317/1494), respectively. The success rate of PCI was declined in CTO lesions with long occlusion time, stump missing, bridging collaterals and >or= 15 mm in length (P < 0.05). Operative failures were occurred in 112 patients. Guide wire and balloon failed to pass through the occlusion accounted for 82.1% and 17.9% of all failures respectively. No death was occurred during operation, but 2 patients suffered from acute stent thrombosis and other 9 patients suffered from acute or later pericardium perforation. After procedure, there were 3 patients died and 3 patients suffered from subacute stent thrombosis during in-hospital period. The total in-hospital major adverse cardiac events rate was 0.6% (7/1148). The angina-free survival rate was 87.1% at discharge.CONCLUSION:In an experienced heart center, it is possible to obtain a relatively high success rate of PCI and ideal clinical outcomes in patients with CTO coronary lesions.

Abstract:Coronary chronic total occlusion (CTO), which occurs in 18. 4–52% of all patients referred for coronary angiography, represents one of the last barriers in coronary intervention. Approximately half of all patients with prior coronary artery bypass graft (CABG), who undergo coronary angiography, are diagnosed with coronary CTO. In fact, these patients often develop recurrent symptoms and events, necessitating revascularization. Currently, there is neither a consensus nor developed guidelines for the treatment of CTO patients with prior CABG, and the prognosis of these patients remains unknown. In this review, we discuss current evidence and future perspectives on CTO revascularization in patients with prior CABG, with special emphasis on clinical and lesion characteristics, procedural success rates, periprocedural complications, and long-term outcomes.

Abstract:Coronary chronic total occlusions (CTO) are an increasingly frequent entity in clinical practice and represent a challenging percutaneous coronary intervention (PCI) scenario. Despite data from randomized trials that have not yet demonstrated a clear benefit of CTO recanalization, the widespread of CTO-PCI has substantially increased. The improvement in operators' techniques, equipment, and training programs has led to an improvement in the success rate and safety of these procedures, which will represent an important field of future development of PCI. The present review will summarize clinical outcomes and technical and safety issues of CTO revascularization with the aim to guide clinical daily cath-lab practice.

Abstract:Coronary chronic total occlusion (CTO) often occurs in patients with obstructive coronary artery disease, which remains one of the greatest challenges for interventional cardiologists. Coronary computed tomography angiography (CCTA) with its emerging post-processing techniques can provide a detailed assessment of CTO lesions before percutaneous coronary intervention (PCI), playing an important role in the clinical management of CTO PCI, from early diagnosis, pre-procedural outcome prediction, the crossing algorithm planning, intraprocedural guidance, and finally post-procedural assessment and follow-up. In addition, the feasibility of CT perfusion (CTP) in patients with CTO has been validated. Combined CCTA and CTP have the great potential to be the one-stop-shop imaging modality for assessing both anatomy and function of CTO lesions. This review aims to make radiologists understand the role of CCTA in the diagnosis and assessment of CTO lesions, thus assisting interventionalists in optimizing CTO PCI crossing strategies with the expertise of radiologists. Critical relevance statement The anatomical features of CTO on CCTA can reveal the complexity of CTO lesions and are associated with CTO PCI outcome, thus helping interventionalists optimize CTO PCI crossing strategies. Key points • CTO is the common lesion in invasive coronary angiography, and CTO PCI is technically difficult and its success rate is relatively low. • Length, collaterals, and attenuation-related signs can help distinguish CTO from subtotal occlusion. • The anatomical features of CTO lesions can help grade the difficulty of CTO PCI and predict procedural outcomes and long-term outcomes of CTO PCI. • The real-time fusion of CCTA with fluoroscopic angiography can be applied in highly complicated CTO lesions. • After CTO PCI, CCTA can help guide a second CTO PCI re-entry or follow up stent patency.

Abstract:Chronic total occlusion(CTO)is the difficult point for percutaneous coronary intervention(PCI).Because of its special pathological basis,the patient's requirements are very high,the operation success rate is low,and the complication rate is high.Perioperative myocardial injury(PMI)is a complication of CTO-PCI.Compared with non-occlusive lesions,CTO-PCI can lead to PMI when it involves larger branch vessels and collateral vessels.This article reviews the causes,prevention and treatment progress of PMI associated with CTO-PCI.

Abstract:Chronic total occlusion (CTO) is considered as the most challenging frontier in interventional cardiology and the last one to be conquered. With availability of state of the art hardware, wires and catheters in particular and increased skills of the operators, the success rate for recanalization of CTO by percutaneous catheter intervention (PCI) has improved. Yet the complications rate and longterm adverse events are high, mostly due to failure in tracking or navigation of hardware through the occluded CTO segment, prolonged exposure to radiation and high doses of contrast used. Therefore, proper selection of patient is of utmost importance. One of the major challenges for successful CTO recanalization is satisfactory visualization of the occluded CTO segment. Conventional invasive catheterization fails to fill the gap and the shortcomings and handicaps of such invasive imaging can be resolved with the use of non-invasive CT coronary angiography (CTCA). CTCA helps to better define the morphological features of the occluded CTO segment, which are established predictors of success, like the actual length of the occluded segment and any calcification or tortuosity in its course. Integration of reconstructed three-dimensional CT coronary images with twodimensional fluoroscopic images, offers directional guide to select the best angiographic plane for visualization of angiographically “missing segment”. With advances in CT technology, CTCA has now become an established technology for pre-procedure evaluation of CTO segment, thereby help in planning and execution of successful PCI.

Abstract:Chronic total occlusions (CTOs) represent a challenging scenario in coronary artery disease (CAD). The prevalence of CTOS in patients undergoing coronary angiography underscores the need for effective diagnostic and therapeutic strategies. Coronary angiography, while essential, offers limited insights into lesion morphology, vessel course, and myocardial viability. In contrast, coronary imaging techniques-including optical coherence tomography (OCT), intravascular ultrasound (IVUS), and coronary computed tomography angiography (CCTA)-provide comprehensive insights for each stage of CTO percutaneous coronary intervention (PCI). OCT facilitates the assessment of plaque morphology and stent optimization, despite low evidence and several limitations in CTO-PCI. IVUS offers deeper penetration, allowing managing proximal cap scenarios and guiding subintimal navigation. CCTA provides a non-invasive, three-dimensional view of coronary anatomy, enabling the precise evaluation of myocardial mass at risk and detailed procedural planning. Despite their individual limitations, these imaging modalities have enhanced the success rates of CTO-PCI, thus reducing procedural and long-term complications and improving patient outcomes. The future of CTO management lies in further technological advancements, including hybrid imaging, artificial intelligence (AI) integration, and improved fusion imaging. These innovations promise to refine procedural precision and personalize interventions, ultimately improving the care of patients with complex coronary artery disease.
Percutaneous treatment of coronary chronic total occlusions in China:the current status

Abstract:Objective To preliminarily evaluate the current status of percutaneous treatment of coronary chronic total occlusions(CTO) in China.Methods Forty-five cardiac interventionalists were surveyed by questionnaire when they were attending the activity organized by the Chronic Total Occlusions Club,China (CTOCC).The questionnaire consisted of 10 questions with multiple choice.Results Forty-three(95.6%) valid questionnaires were collected.Stiff wires(48.8%) and polymer-coated hydrophilic wires(44.2%) were commonly used during CTO,and the soft wires were used as the first attempt wire only in 3.6%of patients. Miracle(34.5%) and Pilot(33.3%) series of guide wires were the most frequently used ones.Parallel wire technique was mainly used when the first guide wire failed to cross the lesions,and under such condition Miracle (46%) and Conquest(28.6%) series of guide wires were usually chosen.The intravascular ultrasound(IVUS)-guided wire was seldom chosen.The application rate of IVUS was less than 5%in 93.1%of the operators.Multi slice computed tomography prior to operation was infrequently performed.The application rate of CT was less than 5%in 67.4%of the operators.The number of patients who received retrograde guide wire technique surpassed 20 only in one(2.2%) interventional center.The success rate was less than 80%in 83.7%of the operators. Conclusion Compared with their overseas counterparts,interventionalists in China need to further improve their technique.To fully understand the situation of CTO in China,large scale retrospective studies are warranted.
Clinical and imaging analysis of chronic total occlusion in 2051 patients

Abstract:Objective To analyse the clinical and imaging characteristics of coronary heart disease patients with chronic total occlusion (CTO) lesions shown by coronary angiography. Methods The data from 2?051 patients admitted between June 1989 and May 2005 were undergone retrospective analysis. Results There were 2?848 CTO vessels in 2?051 patiens aged 25-86 (60.5±10.5) years old. Among them, 80.5% of the patients were male. There were 50.1% of the patients with UAP, 42.8% with OMI and 43.9% of them smoked. In terms of co-morbidity, 55.7% of the patients with hypertension. 22.0% with diabetes and 19.2% with hyperlipkdemia. Analysis of the coronary angiography showed that among all the CTO leisons, 34.6% with bridge collateral perfusion, 31.9% of them were abrupt occlusion and 57.3% with occlusion length ≥15 mm. There were 39.5% of the leisons showed moderate proximal segemnt tortuosity and 17.6% showed moderater to heavy calcification. In terms of the leisons location, 29.3% was bifureation, 47.1% was ostial and 24.1% was distal to the vessels. Conclusion Most of the patients enrolled in the study presented with the known risk factors of coronary heart disease including smoking, hypertension, diabetes and hyperlipidemia. A great proportion of CTO patients had the clinical menifestation of UAP, OMI, heart failure, arrhythmia and stroke. Coronary angiography showed a higher ratio of complicated lesions such as collateral bridge formation, severe calcification and tortuosity and long leisons etc.. These clinical and angiogrophic characteristics may make PCI of CTO leisons difficult and may influence its the success rate.

Abstract:Background: PCI for aorto-ostial CTO remains challenging. The techniques for guidewire in aorto-ostial CTO may differ from those used in non-aorto-ostial CTOs, influenced by clinical and angiographic characteristics. Objectives: This study aimed to assess the technical aspects and outcomes of percutaneous coronary intervention (PCI) in patients with aorto-ostial chronic total occlusion (CTO). Methods: This analysis included 420 patients with ostial CTO from the Japanese CTO-PCI Expert Registry, spanning January 2014 to December 2022. It examined the strategies and procedural outcomes of CTO PCI. Results: Ostial CTO represented 420 of 10,814 (3.9%) of all CTO PCI cases. Within this subset, aorto-ostial CTO accounted for 218 of 420 (52%) cases. The technical success rate for aorto-ostial CTO was 88% (191/218). Aorto-ostial CTOs exhibited longer lesion lengths and were more likely to present with challenges such as distal target lumen ambiguity, calcification, and tortuosity compared with non-aorto-ostial CTOs. The retrograde approach was more commonly used in aorto-ostial CTO, with retrograde direct crossing being the most successful technique, especially in cases of flush CTO. A multivariate logistic analysis identified several factors significantly associated with unsuccessful aorto-ostial CTO PCI, including difficulties in engaging a guiding catheter, estimating the collateral channel (as assessed by the J-Channel score), and tortuosity. Conclusions: Aorto-ostial CTOs are more complex and frequently require a retrograde approach. The availability of suitable interventional collateral channels is crucial for the success of these procedures.

Abstract:Background:Treatment of chronic total occlusions (CTOs) is referred to as the last frontier of percutaneous coronary interventions and is currently performed in 10% to 20% of procedures. Improved outcomes with newer generation drug-eluting stents require further research. Methods:The TARGET CTO trial (NCT03040934) is a prospective, multicenter, randomized, noninferiority trial that plans to randomize 196 subjects (1:1) to either a newer-generation sirolimus target-eluting stent or an everolimus-eluting stent. Patients are candidates if they present with at least 1 CTO lesion in a native coronary artery with a diameter of ≥2.50 mm to ≤4.00 mm and a length of <100 mm. In addition, 44 subjects will participate in an optical coherence tomography (OCT) substudy. Clinical follow-up is planned up to 5 years after stent implantation. Angiographic follow-up is planned at 12 months, whereas OCT will be obtained after the procedure, at 3 and 12 months. The primary end point is in-stent late lumen loss by quantitative coronary angiography at 12 months. The key secondary end point is neointimal thickness by OCT at 3 months. Imaging end points are assessed by an independent core lab. Clinical end points are adjudicated by an independent clinical events committee. Conclusion:The TARGET CTO trial compares a sirolimus target-eluting stent with an everolimus-eluting stent for management of CTOs according to contemporary interventional practices. The primary angiographic end points will be reported at 12 months and clinical follow-up will continue for up to 5 years.
The Retrograde Recanalization of Coronary Chronic Total Occlusion: Approaching Through the Backdoor

Abstract:Coronary chronic total occlusion (CTO), defined as occlusion in the coronary arteries with TIMI 0 flow of at least 3 months duration, is observed in 15-30% of patients undergoing diagnostic coronary angiography [1]. Percutaneous coronary intervention (PCI) of CTOs provide symptomatic relief, improves regional and global left ventricular function, quality of life and long-term survival compared to failed recanalization [2,3]. CTOs remain challenging lesions for interventional cardiologists, and have been called the 'last frontier' due to technical complexity, poor success rate, and the greater experience needed to effectively and safely perform the procedure. Despite meaningful benefit associated CTO PCI, several misconceptions prevailed: a chronically occluded artery is clinically benign; Collateral channels (CCs) are sufficient for angina relief; the area subtended by the CTO is non-viable, CTO PCI is associated with high complication rate; a lack of strong randomized data demonstrating clinical benefit. However, the relative reluctance to attempt CTO PCI is waning with improvement in the technique and technology leading to further procedural success and better outcomes. The recognition that traditional guidewire techniques were not adequate to achieve higher procedural success rates led to development of dedicated guidewires, micro catheters, innovative methods like parallel wire technique, anchor wire, anchor balloon technique, intravascular ultrasound (IVUS) guidance, side branch techniques and dissection and re-entry methods played significant roles in improving the success rate via antegrade approach [4]. Even then, the technical success rate for CTO via the antegrade approach remained suboptimal, in the range of 65%-70% worldwide. The limitations of the conventional antegrade approach are overcome by the retrograde approach that involves targeted CC crossing, retrograde lesion crossing and management of subintimal space with use of balloon dilatation for connecting antegrade and retrograde channels [5]. Adoption of this technique has potentially improved the success rate of complex CTO to the extent of 90-95%. Angiographic features of blunt occlusion with a large side branch, bridging collaterals, calcification, and long CTO are no longer considered negative predictors of success while using a retrograde approach. This approach can be used after antegrade crossing failure or as an initial situation like ostial occlusions, long occlusions, heavy calcification, occlusions with ambiguous proximal cap, and occlusions with a diffusely diseased distal vessel, occlusion involving a distal major bifurcation and CTO vessels that are difficult to engage such as anomalous coronary arteries [4].
