Abstract:Objective: To investigate the effect of axillary lymph node status in evaluating the prognosis of patients with local recurrence after breast-conserving surgery. Methods: The clinical data of 72 patients with local-regional recurrence who had undergone breast-conserving therapy in Tianjin Cancer Hospital from February 2001 to December 2009 were collected and retrospectively analyzed. These patients were divided into axillary lymph node positive group (23 cases) and negative group (49 cases), according to their axillary lymph node status. Results: Among 72 patients, 21 cases were local recurrence, 35 cases were regional recurrence, and 16 cases were local-regional recurrence. In the axillary lymph node positive group, 7 cases were local recurrence, 10 cases were regional recurrence, 6 cases were local-regional recurrence. In the axillary lymph node negative group, 14 cases were local recurrence, 25 cases were regional recurrence, 10 cases were local-regional recurrence. There was no significant difference between these two groups (P=0.807). Moreover, no significant differences of the age, recurrent site, estrogen receptor (ER) and/or progesterone receptor (PR) status, human epidermal growth factor receptor 2 (HER-2) status, disease-free period, local and systemic therapeutic effect, non-distant metastasis survival between these two groups were observed (P>0.05). However, the overall survival of axillary lymph node positive group after recurrence was significantly lower than that of negative group (P=0.014). Cox multivariate analysis showed that recurrent site is an independent prognostic factor of disease-free survival of patients with regional recurrence after breast-conserving surgery (OR=2.050, P=0.002). The axillary lymph node status and recurrent site were independent prognostic factors of overall survival of these patients (OR=3.469, P=0.003; OR=3.676, P<0.001). Conclusions: Axillary lymph node status is an independent factor of overall survival of patients with regional recurrence after breast-conserving surgery, but it is marginally related with their non-distant metastasis survival.

Abstract:The combination impact of axillary lymph node status and biological subtypes on prognosis of locally-invasive breast cancer and feasibility levels of breast conserving surgery have not been thoroughly examined. The current study collected 3,302 patients with histologically-confirmed primary invasive breast cancer undergoing curative surgery. Expression statuses of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor type 2 (HER2), along with axillary lymph nodes status and follow-up information, were collected for all patients. Patients were divided into six subgroups, accordingto immunohistochemical results and axillary lymph nodes status, including G1-G6: G1 (LN+/Iuminal-like), G2 (LN+/basal-like), G3 (LN+/HER2+HR-), G4 (LN-/Iuminal-like), G5 (LN+/basal-like), and G6 (LN+/HER2+HR-) groups. Prognostic effects of the G subgroup were detected and its association with the feasibility of breast conserving surgery was explored. A total of 2,031 patients underwent mastectomy procedures (61.5%), while 1,271 (38.5%) underwent breast conserving surgery. There were 1,016 (30.8%), 223 (6.8%), 457 (13.8%), 1,002 (30.3%), 257 (7.8%), and 457 (10.5%) patients in G1 to G6 subgroups, respectively. The median follow-up time was 92 months (ranging from 3 to 187), while disease recurred in 387 patients (11.7%). The G subgroup was an independent factor for DDFS and DFS (both P<0.001) and the only independent factor for LRFS (P<0.001). The G subgroup was also significantly correlated with surgery type (P<0.001). Combination of axillary lymph node status and biological subtype was associated with multiple prognosis of primary breast cancer. Thus, it is a potential indicator for feasibility of breast conserving surgery.
Impact of examined lymph node count on prognosis in patients with lymph node-negative pancreatic body/tail ductal adenocarcinoma.

Abstract:Background: Because the overall prognosis remains dismal for patients with resected pancreatic cancer (PC), we aimed to explore the prognostic impact of examined lymph node (ELN) count on lymph node (LN)-negative pancreatic body/tail ductal adenocarcinoma. Methods: Patients' data were extracted from the Surveillance, Epidemiology, and End Results (SEER) database (National Cancer Institute, USA) to investigate the relationship between ELN count and survival outcomes of LN-negative pancreatic body/tail ductal adenocarcinoma. Results: A total of 700 patients were included, and the median number of ELNs was 11. The respective 1-, 3-, 5-year overall survival (OS) rates were 75.3%, 37.7%, 30.3%, and the 1-, 3-, 5-year cancer-specific survival (CSS) were 78.3%, 41.7%, 34.5%. The X-tile analysis showed that <= 14 was the most optimal cutoff for both OS and CSS. Kaplan-Meier survival analysis indicated that patients with ELNs <= 14 had better OS and CSS than ELNs Multivariate Cox analysis showed ELNs was an independent risk factor for both OS [hazard ratio (FIR), 1.357; 95% confidence interval (CI), 1.080-1.704; P=1).(X)9] and CSS (FIR, 1.394; 95% CI, 1.092-1.778; P=0.008). Conclusions: ELN count is associated with the survival rate in patients with LN-negative pancreatic body/tail ductal adenocarcinoma. Accurate nodal staging for these patients requires more than 14 ELNs.
Comparison of three lymph node staging methods for predicting outcome in breast cancer patients with mastectomy

Abstract:BACKGROUND: Axillary lymph node (ALN) staging is essential in predicting the clinical outcome of breast cancer (BC) patients. Traditionally, it follows the tumor-node-metastasis (TNM) staging, but its accuracy needs further improvement.METHODS: A total of 9,616 BC patients from the Surveillance, Epidemiology, and End Results (SEER) database and 675 patients from the First Affiliated Hospital of China Medical University underwent mastectomy together with ALN dissection were reviewed. Univariate and multivariate logistic analyses were conducted to find the most meaningful factors relevant to prognosis.RESULTS: After univariate and multivariate analyses, age, race, primary site, radiation, chemotherapy, grade, T-stage, estrogen receptor (ER), progesterone receptor (PR), total number of positive lymph nodes (pN), positive lymph node ratio (LNR) and log odds of positive LNs (LODDS) were found to be significantly associated with overall survival (OS). Using these non-LN risk factors, we further compared the efficacy of three different ALN staging methods in prognosis via nomograms. Harrell's concordance index (C-index) and Akaike Information Criterion (AIC) were used to measure nomogram performance of the ALN staging methods: pN: C-index=0.687 (95% CI: 0.678-0.696), AIC =61,398.24; LNR: C-index =0.691 (95% CI: 0.683-0.701), AIC =61,313.56; and LODDS: C-index =0.691 (95% CI: 0.682-0.700), AIC =61,315.60. We found that the nomogram incorporating LODDS had better predictive ability compared with other two methods. Furthermore, an external validation revealed a C-index of 0.753 (95% CI: 0.690-0.816) for the Asian population, which indicates the nomogram based on LODDS may have universality for both Western and Asian populations.CONCLUSIONS: Compared with pN and LNR, LODDS showed higher homeostasis in LN evaluation, and showed marked efficacy in evaluating survival differences among patients with negative LN staging. We constructed a BC prognosis model by incorporating highly relevant clinical pathological factors and a new method of LN staging, which may greatly aid in guiding postoperative treatment.

Effect of Nodal Status on Clinical Outcomes of Triple-Negative Breast Cancer: A Population-Based Study Using the Seer 18 Database

Abstract:Triple-negative breast cancer (TNBC) is an aggressive malignancy with a poor prognosis. Data from the Surveillance, Epidemiology and End Results database (2010-2012) were used to identify 10,771 patients with TNBC, and we assessed the effects of lymph node (LN) status on breast cancer-specific survival (BCSS) and overall survival (OS). In our study, a Kaplan-Meier plot showed that LN-negative patients (N0) had better survival outcomes than LN-positive patients and that patients with ≥10 positive LNs (N3) exhibited the worst survival outcomes regardless of tumor size. A pairwise comparison showed no difference in survival outcomes among each group stratified by tumor size. Further, for LN-positive patients with a tumor size ≤2 cm (T1) or >5 cm (T3), there were similar outcomes between patients with one to three LNs (N1) and those with four to nine LNs (N2), whereas N1 patients experienced significantly better survival outcomes than N3 patients (P<0.001). Therefore, ten metastatic lymph nodes was the cut-off value for poor prognosis. Nevertheless, for patients with a tumor size of 2-5 cm (T2), the extent of LN involvement contributed prognostic value to OS but not BCSS. In summary, we found that nodal status and tumor size exhibited distinct interaction patterns for predicting the outcomes of TNBC. These results provide deeper insight into the prognostic value of nodal status in TNBC.

Abstract:To identify which patients with axillary node-negative breast cancer would be most likely to benefit from adjuvant therapy, we analysed the records of 1,484 consecutive patients with this condition who underwent surgery between 1956 and 1993. Twenty-two individual variables were evaluated statistically by cumulative survival rate using Cox multivariate analysis. Tumour size, intramammary lymph node status, pregnancy-association, number of pregnancies, preoperative history, menopausal status, postoperative radiotherapy, and the use of tamoxifen for postoperative adjuvant therapy were verified to be independent predictors of prognosis. Although certain clinical and biological parameters were shown to benefit from adjuvant therapy, the selection of high-risk patients with axillary node-negative breast cancer who should receive adjuvant chemotherapy remains controversial.

Abstract:Objective To explore the influence of adjuvant chemotherapy on the prognosis of hormone receptor negative breast cancer with residual lymph node disease(RLND)after neoadjuvant chemotherapy.Methods A total of 110 hormone receptor negative breast cancer patients treated with 4-8 cycles of neoadjuvant chemotherapy were respectively analysed between 2002 and 2012.Residual lymph node disease was comfirmed by subsequent radical mastectomy.Then all these patients were classified into two groups: patients treated with adjuvant chemotherapy(group A) and patients untreated with adjuvant chemotherapy(group B).Results All patients were female,the median age was 54.5 years old(IQR:47-59 years).The median follow-up time was 61 months(IQR:51-88 months).There were 82 patients (74.5%) in group A,and 28 patients (25.5%) in group B.The five-year disease-free survival (DFS) rate was 76.2% in group A and 57.6% in group B.The distant disease-free survival (DDFS) rate was 78.9% in group A and 60.4% in group B.Overall survival (OS) rate was 81.0% in group A and 60.0% in group B.Multivariate analysis showed that there were significant differences for DDFS rate (group A vs group B,P=0.033;hazard ratio [HR],5.256;95% confidence interval [95%CI],1.14-24.17) and OS rates (group A vs group B,P=0.011;HR,7.478;95%CI,1.58-35.30) between two groups.Conclusion The patients who have hormone receptor negative breast cancer with RLND after neoadjuvant chemotherapy,may benefit from postoperative adjuvant chemotherapy.

Abstract:Objective To evaluate the effect of non-sentinel lymph node status on prognosis of Luminal B (HER2 negative) early breast cancer.Methods Luminal B (HER2 negative) early breast cancer with positive sentinel lymph node treated from January 2008 to December 2014 in Peking University First Hospital entered the retrospective study.The clinicopathological features and their relationship with non-sentinel lymph nodes status were analyzed statistically.The effect of non-sentinel lymph nodes status on prognosis of Luminal B (HER2 negative) early breast cancer with positive sentinel lymph nodes were also analyzed.Results In the group of 142 patients with positive sentinel lymph nodes (SLN),the average number of SLNs detected in each patient were 2.06± 1.26.The number of positive SLNs was associated with the status of non-sentinel lymph nodes,but not the age,BMI,menopause,T stage,histological grade,lympho-vascular invasion.Non-SLN axillary lymph node non-metastasis occurred in 78 patients (54.9％),DFS 93％,OS 93.1％;non-SLN axillary lymph node metastasisoccurred in 64 patients (45.1％),DFS 95.2％,OS 91.9％.Based on Log-rank (Mantel-Cox) test,There was no significant difference between two groups in DFS (x 2=0.011,P=0.918) and OS (x 2=0.348,P=0.555).Conclusion The number of positive sentinel lymph nodes in Luminal B (HER2 negative) early breast cancer patients with sentinel lymph nodes metastasis were associated with the status of non-sentinel lymph nodes.There were no statistical differences between non-sentinel lymph nodes metastasis group and non-metastasis group in disease free survival and over-roll survival.

Abstract:Abstract Background Occult metastases in axillary lymph nodes have been reported to be associated with poor prognosis in patients with breast cancer. However, studies on the prognostic value of occult metastases have shown controversial results. This meta-analysis aimed to evaluate the prognostic significance of occult lymph node metastases in breast cancer. Methods Studies published until May, 2020, which retrospectively examined negative lymph nodes by stepsectioning and/or immunohistochemistry, were retrieved from MEDLINE, EMBASE, CNKI, and Cochrane Library databases. The pooled Relative Risk (RR) with 95% confidence interval (95% CI) for overall survival (OS) and disease-free survival (DFS) were calculated to examine the associations between occult metastases and prognosis. Results Patients with occult metastases in axillary lymph nodes had poorer five-year DFS (RR = 0.930; 95% CI = 0.907–0.954) and OS (RR = 0.972; 95% CI = 0.954–0.990). Furthermore, the DFS (RR = 0.887; 95% CI = 0.810–0.972) and OS (RR = 0.896; 95% CI = 0.856–0.939) of patients with occult metastases were significantly lower after a ten-year follow-up. Conclusions Occult metastases in the axillary lymph nodes are associated with poorer DFS andOS of patients with breast cancer. Occult metastases might serve as a predictive factor of survival outcomes in patients with breast cancer.

Effect of the Number of Positive Lymph Nodes and Lymph Node Ratio on Prognosis of Patients after Resection of Pancreatic Adenocarcinoma

Abstract:BACKGROUND: The prognostic factors related to lymph node involvement [lymph node status, the number of positive lymph nodes, lymph node ratio (LNR)] and the number of nodes evaluated in patients with pancreatic adenocarcinoma after pancreatectomy are poorly defined.METHODS: A total of 167 patients who had undergone resection of pancreatic adenocarcinoma from February 2010 to August 2011 were included in this study. Histological examination was performed to evaluate the tumor differentiation and lymph node involvement. Univariate and multivariate analyses were made to determine the relationship between the variables related to nodal involvement and the number of nodes and survival.RESULTS: The median number of total nodes examined was 10 (range 0-44) for the entire cohort. The median number of total nodes examined in node-negative (pN0) patients was similar to that in node-positive (pN1) patients. Patients with pN1 diseases had significantly worse survival than those with pN0 ones (P=0.000). Patients with three or more positive nodes had a poorer prognosis compared with those with the negative nodes (P=0.000). The prognosis of the patients with negative nodes was similar to that of those with one to two positive nodes (P=0.114). The median survival of patients with an LNR >= 0.4 was shorter than that of patients with an LNR <0.4 in the pN1 cohort (P=0.014). No significance was found between the number of total nodes examined and the prognosis, regardless of the cutoff of 10 or 12 and in the entire cohort or the pN0 and pN1 groups. Based on the multivariate analysis of the entire cohort and the pN1 group, the nodal status, the number of positive nodes and the LNR were all associated with survival.CONCLUSIONS: In addition to the nodal status, the number of positive nodes and the LNR can serve as comprehensive factors for the evaluation of nodal involvement. This approach may be more effective for predicting the survival of patients with pancreatic adenocarcinoma after pancreatectomy.
Influence of different types of surgical operations on the postoperative recovery status and complications of patients with early-stage breast cancer

Abstract:Objective:To evaluate the influence of different types of surgical operations on the conditions of perio-perative period, postoperative complication and postoperative recovery of patients with early-stage breast cancer. Methods:The 102 cases of patients were retrospectively studied. The 42 patients who accepted sentinel lymph node biopsy were chosen into the study group and the other 60 patients who accepted axillary lymph node dissection were chosen into the control group. The conditions of perioperative period, postoperative complication and postoperative recovery in the two groups were analyzed. Results:In comparisons of the two groups,there were significant differ-ences in the removal of axillary drainage tube time,the total axillary drainage volume and the lymphedema subjec-tive score. However, there were no significantly differences between the two groups in the operation time, the lymphedema subjective measurement,the quality of life and the cosmetic results. In the further analysis, the pa-tients who received breast-conserving surgery had better quality of life and cosmetic result than the patients who re-ceived mastectomy. Conclusion:For patients with early-stage breast cancer,sentinel lymph node biopsy combined with breast-conserving surgery can have better effect.
Effect of Neoadjuvant Chemotherapy in Treatment of Locally Advanced Breast Cancer with Cytologically Proven Axillary Node Metastases

Abstract:OBJECTIVE:To evaluate the effect of neoadjuvant chemotherapy (NC) on locally advanced breast cancer with cytological proven axillary lymph node (ALN) metastases by fine needle biopsy. METHODS:From January 2000 to May 2006,111 patients with cytological proven ALN metastases by fine needle biopsy were identified,and these patients had received at least two cycles of NC in the standard dosage. The relationship between the clinical and pathological responses of ALN with disease-free survival (DFS) and overall survival (OS) was axalyzed. RESULTS:Of 111 patients,67 patients (60.4%) achieved clinical complete response (cCR) in ALN. 29.9% of 67 patients had pathologic complete response (pCR) in ALN. Of 44 patients who had not achieved cCR,36.4% patients had pCR. With a median follow-up of 65 months,compared with the group without ALN pCR,5-year DFS and OS improved in the patients achieving ALN pCR (P=0.036,P=0.038,respectively). Residual primary tumor did not affect the outcome of those with ALN pCR. The clinical response of axillary lymph nodes after the neoadjuvant chemotherapy was not associated with DFS and OS (P=0.182,0.984). CONCLUSIOS:In the patients with ALN positive before NC,pCR is associated with an excellent prognosis,regardless of a residual primary tumor. Only physical examination combined with axillary ultrasound test could not be the predictive and prognostic factors.

Abstract:Aim: Increasing studies have demonstrated lymph node ratio (LNR) to be an accurate prognostic indicator in breast cancer and an alternative to pN staging; however, the AJCC-TNM staging system classified apical or infraclavicular/ipsilateral supraclavicular lymph node-positive (APN(+)) patients with a worse prognosis as the pN3 stage. Until now, different reports on LNR in breast cancer have ignored this possibility. Consequently, it is necessary to discuss the role of APN(+) patients in the LNR system to obtain a precise LNR that predicts the prognosis accurately.Materials and Methods: We collected data on 10,120 breast cancer patients, including 3,936 lymph node-positive patients (3,283 APN(-) and 653 APN(+) patients), who visited our hospital from 2007 to 2012. Then we applied X-tile analysis to calculate cut-off values and conduct survival analysis and multivariate analysis to evaluate patients' prognosis.Results: We confirmed that some APN(+) patients were mis-subgrouped according to previously reported LNR, indicating that APN(+) patients should be excluded in the application of LNR to predict prognosis. Then we applied X-tile analysis to calculate two cut-off values (0.15 and 0.34) for LNR-APN(-) patients and conducted survival analysis and found that LNR-APN(-) staging was superior to pN staging in predicting the prognosis of APN(-) breast cancer patients.Conclusion: From this study, we conclude that excluding APN(+) patients is the most necessary condition for effective implementation of the LNR system. LNR-APN(-) staging could be a more comprehensive approach in predicting prognosis and guiding clinicians to provide accurate and appropriate treatment.
Analysis of the effect and prognosis of endoscopic axillary lymph node dissection for clinical stage Ⅰ and Ⅱ breast cancer patients

Abstract:Objective To study the clinical effect and technical feasibility of endoscopic axillary lymph node dissection for stage Ⅰ and Ⅱ breast cancer patients.Methods From January 2006 to January 2008, 118 patients with stage Ⅰ and Ⅱ breast cancer were treated with endoscopic axillary lymph node dissection(the EALND group, n=55) and conventional axillary lymph node dissection (the CALND group, n=63), respectively. The operation time, bleeding amount,number of lymph nodes dissected, axillary drainage amount and prognosis were compared between the two groups. Student's t test and Chi-square test were used for statistical analysis.Results In the EALND group, the mean operation time was 121.0 minutes, the intraoperative bleeding amount was 51.0 ml, the mean number of lymph nodes dissected was 17.3, the total postoperative axillary drainage was 208.0 ml; the follow-up of 1-3 years showed 2 patients had recurrence and metastasis. In the CALND group, the mean operation time was 70.0 minutes, the intraoperative bleeding amount was 80.0 ml, the mean number of lymph nodes harvested was 19.5, the total postoperative axillary drainage was 350.0 ml; and postoperative recurrence and metastasis were in 2 patients. The operation time was longer in the EALND group than in the CALND group, but the intraoperative bleeding and postoperative drainage of the EALND group were significantly less than those of the CALND group (P<0.05); short term outcome had no significant difference between the two groups(χ2=0.02,P=0.89).Conclusion Endoscopic axillary lymph node dissection can obtain similar therapeutic effect with conventional techniques.
Therapeutic Role of Axillary Lymph Node Dissection in Patients with Stage Iv Breast Cancer: A Population-Based Analysis

Abstract:PURPOSE:To assess the clinical value of axillary lymph node (ALN) dissection in stage IV breast cancer. METHODS:Patients with a diagnosis of stage IV breast cancer from 1990 to 2010 were identified using the Surveillance, Epidemiology, and End Results database. Univariate and multivariate Cox regression analyses were performed to analyze the effect of ALN dissection and ALN status on breast cancer-specific survival (BCSS) and overall survival (OS). RESULTS:A total of 11,645 patients were identified. Of these, 7358 (63.2%) patients underwent ALN dissection, and 6168 (83.8%) patients showed nodal positivity. During this time, the rate of ALN dissection increased. Patients with delayed diagnosis, age <50 years, poorly/undifferentiated disease, larger tumor size (>2 cm), and married women were more likely to undergo ALN dissection. ALN dissection was associated with improved BCSS and OS in multivariate analysis. ALN dissection improved the survival in patients with bone and liver metastasis, and patients with single site of distant metastasis also had survival benefit by ALN dissection. Lymph node staging based on the number of positive lymph nodes was the independent prognostic factor for BCSS and OS in multivariate analysis; patients with advanced lymph node staging had a poor survival. CONCLUSIONS:ALN dissection may improve survival in patients with stage IV breast cancer who received primary tumor surgery, especially in patients with bone, liver, and single site of distant metastasis. The lymph node staging was also the prognostic factor in patients with stage IV breast cancer.

Abstract:Objective To analyze the axillary lymph node metastasis of the different groups of breast cancer patients and to explore the opportunity to minimize the armpit circumscription in early stage patients.Methods 80 breast cancer patients without any neoadjuvant treatment were selected and treated with standard modified radical mastectomy and received total axillary lymph node dissection.The lymph node samples were divided into four groups such as level Ⅰ,level Ⅱ,level Ⅲ and Rotter's lymph node groups,then the pathologic examination was performed.Results There were 55(63.2%) patients with negative nodes and 32(36.8%) patients with positive nodes in total 87 patients.There was one patient with Rotter's lymph node skip metastasis in level Ⅰ negative patients.There were 24 patients with 1-3 positive nodes in level Ⅰ group, and there were 22(91.7%) patients with negative nodes in level Ⅱ group and there was one patient with positive Rotter's lymph node in levels Ⅰ and Ⅱ groups(8.3% together).7 patients had more than 3 positive nodes in level Ⅰ group,there were 2(28.6%) patients with negative nodes in level Ⅱ group,one patient(14.3%) with 1-3 positive nodes,3 patients(42.9%) with 4-9 positive nodes,one patient(14.3%) with more than 9 positive nodes in level Ⅱ group and 4 patients with axillary lymph node metastasis in level Ⅲ group.Conclusion ①It exists the feasibility of giving level Ⅰ dissection to the patients with no clinical evidence of lymph node metastasis and no findings of positive nodes in the procedure of operation.②The patients with 1-3 positive nodes in level Ⅰ group should accept a at least level Ⅱ dissection.③The patients with more than 3 positive nodes should accept level Ⅱ cirumscription if there is no lymph node metastasis and should accept total armpit sirum soription if there is lymph node metastasis.

Abstract:Background: In the era of sentinel node biopsy (SNB), mainly aimed at local control, the axillary dissection (AD) was performed for breast cancer with positive node. It was argued that only level I/II lymph nodes dissection might be inadequate because nodes involvement was found in level III and interpectoral region when axillary node was positive. It is necessary to assess the extent of AD after modern preoperative axillary staging. The aim of this study was to investigate the incidence and associated factors of node involvement in level III/interpectoral region. Methods: A consecutive series of 338 core needle biopsy confirmed T0-2 invasive breast cancer cases were included in this study. Axillary node metastases were proved by ultrasound guided needle biopsy (NB) if ultrasonographic abnormal node was detected prior to SNB or by SNB if no abnormal node was detected. Cases of negative NB but positive SNB with image abnormal node were excluded. Prior to AD included level III and interpectoral lymph nodes, 4 to 8 cycles of neoadjuvant chemotherapy with anthrocyclin and/or taxane regimen were completed for each case. The chi-square test was used to determine the relation between level III/interpectoral lymph nodes metastases and clinicopathological factors. Multivariate logistic regression was analyzed for covariate selection. Results: A median of 19 axillary nodes was harvested per case (range: 5-46, average: 19.2). The pathologic complete response rate of axillary nodes was 35.3% (70/198) in NB positive subgroup. Level III and interpectoral lymph nodes were harvested in 76.9% (260/338) of cases (range: 1-8, average: 1.9, median: 1) and 49.7% (168/338) of cases (range: 1-10, average: 1.6, median: 1), respectively. The incidence of positive level III and interpectoral lymph nodes were 8.9% (30/338) and 8.9% (30/338), respectively. Node involvement of level III/interpectoral region was found in 13.3% (45/338) of these cases. The incidence of node involvement in level III/interpectoral region of NB positive subgroup (14.6%, 29/198) was not significantly higher compared with SNB positive subgroup (11.4%, 16/140, P=0.391). Increasing tumor size was significantly correlated with increasing likelihood of node involvement in level III/interpectoral region (T0-1: 6.3% vs T2: 16.7%, P=0.008). In SNB positive T0-1 subgroup, there was no positive node found in level III/interpectoral region. Multivariate analysis showed that tumor size was the only independent factors predicting node involvement in level III/interpectoral region (OR=3.488, 95%CI:1.427-8.528, P=0.006). Conclusions: The incidence of node involvement in level III/interpectoral region was 13.3% of nodes positive T0-2 breast cancer treated with neoadjuvant chemotherapy. Abstract:Introduction Knowledge of the association between isolated tumor cells (ITCs) in breast cancer patients and the outcome is very limited. We aimed to determine the prognostic value of axillary lymph node ITCs for T1N0M0 female breast cancer (FBC) patients. Methods Data for T1N0M0 FBC patients staged ITCs negative [pN0(i-)] and positive [pN0(i+)] were extracted from the Surveillance, Epidemiology, and End Results database from 2004 to 2015. Prognostic predictors were identified by Kaplan-Meier analysis, competing risk model, and Fine-Gray multivariable regression model. Results A total of 94,599 subjects were included, 88,632 of whom were staged at pN0(i-) and 5,967 were pN0(i+). Patients staged pN0(i+) had worse breast cancer-specific survival (BCSS) [hazard ratio (HR): 1.298, 95% CI = 1.069-1.576,P= 0.003] and higher breast cancer-specific death (BCSD) rate (Gray's test,P= 0.002) than pN0(i-) group. In the Fine-Gray multivariable regression analysis, the pN0(i+) group had higher BCSD rate (HR: 1.321, 95% CI = 1.109-1.575,P= 0.002) than pN0(i-) group. In subgroup analyses, no significant difference in BCSD was shown between the chemotherapy and non-chemotherapy subgroup (Gray's test,P= 0.069) or radiotherapy and non-radiotherapy subgroup (Gray's test,P= 0.096). Conclusion ITC was independently related to the increase of the BCSD rate and could be identified as a reliable survival predictor for T1N0M0 FBC patients.
Indications for Postmastectomy Radiotherapy in Breast Cancer Patients with 1 -3 Positive Axillary Nodes

Abstract:Objective To analyze the prognosis of T1-T2 stage breast cancer with 1 -3 positive axillary nodes after mastectomy, and to explore a subgroup of patients who could benefit from adjuvant radiotherapy. Methods In the retrospective study of 412 eligible patients, survival analysis was performed using the Kaplan-Meier method. Univariate and multivariate analyses were performed using the Log-rank method and Cox regression analysis, respectively. Results The follow-up rate was 98. 7%. 215 and 41patients were followed up for 5 and 10 years,respectively. The 5-and 10-year overall survival (OS) rate was 90. 0% and 81.3%, respectively. The 5-and 10-year locoregional recurrence (LRR) rate was 10. 7% and 18. 6%, respectively. In univariate analysis, T2 statging, more than one positive node, hormone receptornegative ( ER&PR-negative), ratio of positive lymph nodes (LNR) ＞ 25%, Her-2 positive, no hormonal therapy were associated with a significantly higher rate of LRR. T2 staging, more than one positive node,hormone receptor-negative were the risk factors for LRR with statistical significance in the multivariate analysis. Basing on these 3 risk factors, the high-risk group (with 2 -3 factors) had a 10-year LRR rate of 36. 9% compared with 3.9% in the low-risk group ( with 0 - 1 factors;x2 =20. 64,P =0. 000). The 5-year and 10-year distant metastasis (DM) rate was 12.9% and 24. 5%, respectively. LRR, and LNR ＞25%were statistically significant predictors of DM in the multivariate analysis. The 5-year DM rate for patients with LRR was 36. 6% compared with 9. 7% without LRR (x2 = 16. 34,P =0. 000). The 5-year OS rate for patients with LRR was 69. 9% compared with 92. 9% without LRR ( x2 = 20. 79, P = 0. 000). LRR was associated with a higher risk of distant metastasis and worse survival. Conclusions LRR after mastectomy has a significant impact on the outcome of patients with T1 -T2 breast cancer and 1 - 3 positive axillary nodes.Patients who have 2 -3 risk factors might benefit from radiotherapy.
Correlation Analysis of Pathological Features and Axillary Lymph Node Metastasis in Patients with Invasive Breast Cancer

Abstract:Objective. To investigate the risk factors of axillary lymph node metastasis in patients with invasive breast cancer. Methods. This study retrospectively included 122 cases of invasive breast cancer patients admitted to the First Medical Center of PLA General Hospital from January 2019 to September 2020. According to postoperative pathological results, axillary lymph node metastasis was divided into axillary lymph node metastasis (ALNM) group ( n =40) and non-axillary lymph node metastasis (NALNM) group ( n =82). General demographic information was collected and compared between the two groups. Collected pathological results included lymphovascular invasion (LVI) and the expression of estrogen receptor (ER), progestogen receptor (PR), human epidermal growth factor receptor 2 (HER-2), and Ki-67 detected by immunohistochemistry. Imaging parameters of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) including apparent diffusion coefficient (ADC), early enhanced rate, and time-intensity curve (TIC) were also included into univariate analysis. The variables with differences between the two groups were compared by univariate analysis, and the related factors of axillary lymph node metastasis were analyzed by logistic regression model. Results. There was no significant difference in general demographic information between the two groups. No significant differences were found in the positive rates of HER-2, ER, PR, Ki-67, pathological types, and clavicular lymph node metastasis and skin chest wall invasion between the two groups ( P > 0.05 ). The proportion of LVI in ALNM group was significantly higher than that in NALNM group (37.50% vs. 6.10%, P < 0.001 ). The proportion of breast cancer on the left side in the ALNM group was higher than that in the NALNM group, and the difference was statistically significant (70.00% vs. 47.56%, P = 0.019 ). There were no significant differences in the imaging parameters obtained by DCE-MRI between the two groups. Binary logistics regression analysis showed that LVI (OR =12.258, 95% CI =3.681-40.812, P < 0.001 ) and left breast cancer (OR =3.598, 95% CI =1.404-9.219, P = 0.008 ) were risk factors for axillary lymph node metastasis in patients with invasive breast cancer. Conclusion. The formation of vascular tumor thrombi in breast cancer tissue and left breast cancer are risk factors for axillary lymph node metastasis in invasive breast cancer and might be helpful for preoperative detailed assessment of the patient's condition.
