Abstract:This report presents a case of nasopharyngeal diffuse large B-cell lymphoma and a literature review concerning the use of [F-18]fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT). A 37-year old man was admitted to hospital complaining of nasal secretions with minor epistaxis and a 20-year history of snoring. Nasal endoscopy found diffuse swelling in the nasopharynx and a biopsy was performed. Prior to chemotherapy, FDG-PET/CT showed soft-tissue diffuse thickening and FDG accumulation in the nasopharynx and bilateral cervical lymph nodes; FDG did not accumulate elsewhere. After four cycles of chemotherapy (rituximab, cyclophosphamide, doxorubicin, vincristine) and prednisone treatment, FDG-PET/CT showed that FDG still accumulated in the nasopharynx and bilateral cervical lymph nodes, therefore radiotherapy was initiated. At 1 year, FDG-PET/CT showed no FDG accumulation. Immunohistochemical analysis revealed that the tumour was positive for phosphorylated protein kinase B (Akt), suggesting that FDG uptake may be associated with factors activated by the phosphatidylinositol 3-kinase/Akt signalling pathway.

Abstract:BACKGROUND:We investigated the relationships between glucose transporter protein-1 (GLUT-1) and hypoxia inducible factor-1α (HIF-1α) expression, 2-deoxy-2-[18F]-fluoro-D-glucose (18F-FDG) uptake and prognosis in laryngeal and hypopharyngeal carcinomas patients.METHODS:Levels of GLUT-1 and HIF-1α proteins were examined by immunohistochemistry in 55 patients with laryngeal and hypopharyngeal carcinomas. All patients underwent 18F-FDG positron emission tomography-computed tomography (PET/CT) before surgery.RESULTS:Levels of GLUT-1 and HIF-1α in laryngeal and hypopharyngeal carcinomas were significantly higher than in vocal cord polyps. GLUT-1 expression was correlated with HIF-1α expression (r=0.571, P<0.001) and SUVmax (r=0.495, P<0.001). HIF-1α expression was not correlated with SUVmax (P=0.199). HIF-1α expression was correlated with tumor size (r=0.351, P=0.009). Multivariate analysis demonstrated that treatment method (P=0.025), tumor size (P=0.008), GLUT-1 expression (P=0.032), and HIF-1α expression (P=0.032) were correlated with overall survival (OS).CONCLUSIONS:Treatment method, tumor size, GLUT-1 and HIF-1α levels were correlated with prognosis in laryngeal and hypopharyngeal carcinomas patients.

Abstract:Granulomatous slack skin (GSS), a subtype of mycosis fungoides, is an extremely rare disease in cutaneous T-cell lymphoma. Only I reported case was documented in the nuclear medicine literature demonstrating this entity with gallium-67. A 16-year-old boy with GSS exhibited moderate F-18 fluorodeoxyglucose uptake in corresponding areas of cutaneous disease on positron emission tomography/computed tomography scan. F-18 fluorodeoxyglucose positron emission tomography/computed tomography may be a valuable tool for staging and evaluating the therapeutic response in a patient with GSS.
Gorham-Stout Syndrome in Mainland China: a Case Series of 67 Patients and Review of the Literature.

Abstract:Gorham-Stout syndrome (GSS) is a rare disorder of uncertain etiology and unpredictable prognosis. This study aims to present a comprehensive understanding of this rare entity.A literature search in PubMed and three Chinese databases was performed to screen histologically proven GSS cases among Chinese residents in the mainland. We analyzed the patients' clinical characteristics, the value of different treatment modalities and their influence on the clinical outcome.Sixty-seven cases were finally enrolled. There were 43 men (64.2%) and 24 women (35.8%). The mean age at diagnosis was 28 years (1.5-71 years). The most common clinical symptoms included pain (n=40, 59.7%), functional impairment (n=13, 19.4%), and swelling (n=12, 17.9%). The radiographic presentation of 37 cases (55.2%) was disappearance of a portion of the bone. The others presented as radiolucent foci in the intramedullary or subcortical regions. A total of 42 cases provided data on therapy, these included surgery (n=27, 40.3%), radiation therapy (n=6, 9.0%), surgery combined with radiation therapy (n=2, 3.0%), and medicine therapy (n=7, 10.4%). For 30 of these 42 cases, follow-up data were available: 21 cases had the disorder locally controlled and 9 had a symptom progression. Fortunately, the disease is not fatal in the majority of cases.GSS has no specific symptoms and it should be taken into consideration when an unclear massive osteolysis occurs. The efficacies of different treatment modalities are still unpredictable and further research is required to assess the values of different treatments.

Abstract:Introduction The Gorham-Stout Syndrome (Gorham's massive osteolysis) is a rare condition in which spontaneous, progressive resorption of bone occurs. The etiology is poorly understood. Case presentation We reported here a patient who had begun insidiously and was characterized by extensive osteolysis in the left upper limb, with progressive carpal bone resorption. The characteristic radiological and CT findings were presented and the clinical presentation, diagnosis and surgical treatment were discussed in the case. Conclusion Gorham's disease is rarely involved in the whole upper limb. Histopathological evidences suggest that extensive osteolysis is caused by hemangiomatosis. The characteristic imaging findings may help us to differentiate from malignant neoplasm with progressive bony destruction.

Abstract:Gorlin-Goltz syndrome is an autosomal dominant inherited disease characterized by cutaneous basal cell carcinomas, odontogenic keratocysts, intracranial calcifications, and rib abnormalities, among other manifestations. A 13-year-old male patient visited a Bucomaxilofacial Surgery Service with his father, presenting a complaint of swelling on the right hemiface. The parent reported a diagnosis of Gorlin-Goltz syndrome for his son, with a medical history of basal cell carcinoma, bifid ribs, and intracranial calcification.

Abstract:A 72-year-old man with diffuse large B-cell lymphoma underwent fluorine-18 fluorodeoxyglucose (FDG) PET/CT, revealing lymphoma lesions and no evidence of aortitis. The patient received chemotherapy and was treated with granulocyte colony-stimulating factor (G-CSF) for neutropenia. During chemotherapy, the patient underwent PET/CT again, revealing FDG accumulation and wall thickening at the aortic arch, which suggested aortitis. The patient was only experiencing fatigue. G-CSF-associated aortitis was suspected, and the original G-CSF was switched to another G-CSF while continuing chemotherapy. Three months later, the third round of PET/CT showed that FDG accumulation and wall thickening of the aortic arch vanished. PET/CT may be useful for not only the diagnosis but follow-up of G-CSF-associated aortitis. Radiologists should recognize incidental aortitis on PET/CT in patients receiving G-CSF administration.

Abstract:Gorlin syndrome can be caused by pathogenic/likely pathogenic (P/LP) variants in the tumor suppressor gene PTCH1 (9q22.1‐q31), which encodes the receptor for the sonic hedgehog (SHH) ligand. We present a 12‐month‐old boy clinically diagnosed with Gorlin syndrome who was found to have significantly delayed development, palmar pitting, palmar and plantar keratosis, short hands, frontal bossing, coarse face, hypertelorism, a bifid rib, misaligned and missing teeth, and SHH‐activated medulloblastoma. Genetic testing, including a pediatric cancer panel and genome sequencing with peripheral blood, failed to identify any P/LP variants in PTCH1. Paired tumor/normal exome sequencing was performed, which identified a germline NM_000264.5 (PTCH1): c.361_362ins? alteration through manual review of sequencing reads. Clinical RNA sequencing further demonstrated an Alu insertion at this region (PTCH1: c.361_362insAlu), providing molecular confirmation of Gorlin syndrome. This finding exemplifies a unique mechanism for PTCH1 disruption in the germline and highlights the importance of comprehensive analysis, including manual review of DNA sequencing reads and the utility of RNA analysis to detect variant types which may not be identified by routine genetic screening techniques.

Evaluation of lung lesions using FDG γ-camera PET equipped with a one-inch crystal

Abstract:The aim of this study was to evaluate the clinical value of 18-fluorodeoxyglucose (FDG) imaging with gamma-camera positron emission tomography (GCPET) equipped with a one-inch crystal for diagnosing lung lesions and determining the stage of non-small cell lung cancer (NSCLC) in regions with a high prevalence of inflammatory disease and tuberculosis. FDG-GCPET was used to examine 103 patients with suspected malignant lesions in the lung. The results of FDG-GCPET and conventional workup (CWU) including computed tomography (CT), ultrasonography and radionuclide bone scintigraphy were compared. The final diagnosis was based on the results of a histological analysis or follow-up of at least six months. The results showed 82 patients with malignant and 21 patients with benign lesions. If a lesion to background ratio > or = 2.0 was used as the threshold, then the diagnostic sensitivity, specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) of FDG-GCPET for NSCLC were 93.9, 57.1, 86.4, 89.5 and 70.6%, respectively. In 36 patients who underwent open-chest surgery, the diagnostic positive values of FDG-GCPET and CT for lymph-node involvement were 85% (17/20) and 65% (13/20), respectively. The diagnostic sensitivity, specificity, accuracy, PPV and NPV of FDG imaging were 85, 81.3, 83.3, 85 and 81.3%, respectively compared to the CT values of 65, 75, 69.4, 76.5 and 63.2%, respectively (NS). For the evaluation of distant metastases, 31 true-positive patients were identified during the follow-up. FDG imaging correctly identified 28 patients compared to 25 by CWU. In conclusion, FDG imaging with GCPET equipped with a one-inch crystal revealed a high lesion detection capability but a low level of clinical effectiveness for differentiating between malignant and benign lesions in the lung in regions with a high prevalence of inflammatory disease and tuberculosis. For N and M staging of NSCLC, this method may provide additional data that are not available from the CWU.
Case Report: Papillary thyroid carcinoma in Goltz-Gorlin syndrome

Abstract:Goltz-Gorlin syndrome (GGS), also known as focal dermal hypoplasia, is a rare X-linked disorder caused by pathogenic variants in the PORCN gene and characterized by several abnormalities, including skin and limb defects, papillomas in multiple organs, ocular malformations, and mild facial dysmorphism. To date, only approximately 300 cases have been described in the literature. A 16-year-old female patient, born with multiple congenital dysmorphisms consistent with GGS and confirmed by genetic exam, was referred to our outpatient clinic for the workup of a thyroid nodule. A thyroid ultrasound showed a bilateral nodular disease with a 17-mm large hypoechoic nodule in the right lobe. Cytological exam of fine needle aspiration biopsy was suspicious for malignancy. Thus, she underwent total thyroidectomy plus lymphadenectomy of the right central compartment. A histological exam disclosed a papillary thyroid carcinoma (PTC) with lymph node micrometastases. Radioiodine (131-Iodine) therapy was performed. At 3- and 6-month follow-up, the patient did not present either ultrasound or laboratory PTC recurrence. To our knowledge, we report the first case of PTC in a patient with GGS. Since thyroid cancer is rare among children and adolescents, we hypothesize that the PORCN pathogenic variant could be responsible for tumor susceptibility. We also provide an overview of the clinical findings on GGS patients already reported and discuss the possible pathogenetic mechanism that may underlie this rare condition, including the role of PORCN in tumor susceptibility.
Features of IgG4-related Lung Disease on 18F-FDG PET/computed Tomography Imaging

Abstract:Objectives The aim of the study was to summarize the features of immunoglobulin G4-related lung disease (IgG4-RLD) on fluorine 18-fluorodeoxyglucose (18F-FDG) PET/computed tomography (CT). Methods In this retrospective case series, 12 consecutive patients (9 men and 3 women, mean age 55.4 ± 13.7 years) with IgG4-RLD were included. The clinicopathological information and features of 18F-FDG PET/CT imaging were analyzed. Results Six (50%) patients had pulmonary involvement alone and six (50%) patients had extrapulmonary involvement with intense 18F-FDG uptake. Pulmonary manifestations included mass (25%, 3/12), solid nodule (solitary 25%, 3/12; multiple 50%, 6/12), multiple ground-glass opacities (GGOs) (50%, 6/12), thickening of alveolar interstitium (50%, 6/12), and thickening of bronchovascular bundle (33.3%, 4/12). The maximum standardized uptake value (SUVmax) of the solid nodules and masses, multiple GGOs, bronchovascular bundle and the thickening of septa was 4.0 ± 2.5, 2.3 ± 1.8, 1.4 ± 0.6, and 0.9 ± 0.5, respectively. The SUVmax statistically significant linear association with the diameter of masses or solid nodules (P value = 0.03), but no significant inverse linear association (P value = 0.06) with the concentration of serum IgG4 concentration. Conclusions The image patterns of IgG4-RLD on 18F-FDG PET/CT are varying. Multiple pulmonary manifestations or multiple organ involvement, especially in combination with elevated levels of serum IgG and IgG4, may help to make the diagnosis. A potential major application of PET-CT would be evaluation of response to treatment, and the impact of PET/CT on IgG4-RLD management is worth investigating further in the future.

Abstract:Purpose The purpose of this study was to compare dynamic 18 F-FGln PET/CT images of healthy subjects and cancer patients and explore the best imaging phase for different cancers. Methods Thirteen healthy volunteers and 31 cancer patients separately underwent 18 F-FGln and 18 F-FDG PET/CT scans within 1 week. The distributions of 18 F-FGln and 18 F-FDG in the whole body and the tumor avidity were analyzed and compared. The tumor maximum standardized uptake values (SUVmax) and tumor-to-nontumor SUV ratio (SUR) of 18 F-FGln/PET at different scan phases were compared. Results Compared to the healthy subjects, the cancer patients had lower 18 F-FGln activity (SUVmean) in most normal organs, especially in the lung, muscle, spleen, and heart ( p < 0.05). Additionally, the FGln-avid tumors did not necessarily manifest as FDG-avid and vice versa. Overall, among the 31 primary malignant lesions confirmed by biopsy or postoperative pathological analysis, 29 showed increased radioactive uptake on all 18 F-FGln PET/CT imaging phases. The peak of SUVmax in breast and thyroid cancers was within 10 min, while in lung cancers, the plateau of SUVmax was within 30 min to 60 min. The SURs of lung cancer ( p = 0.046) and thyroid cancer ( p = 0.794) increased from the early-phase to the late-phase acquisition; however, a significant decrease was observed in the breast lesions ( p = 0.022). Conclusions 18 F-FGln images may further supplement the diagnostic ability of 18 F-FDG in cancer patients and detect metabolic changes in different tumors. Furthermore, the imaging time for 18 F-FGln PET/CT needs to be optimized for different cancer types to improve the contrast resolution of tumors.

Abstract:Introduction: Gorlin syndrome is a rare, autosomal dominant multi-systemic disorder with a predisposition to the development of cancers such as medulloblastoma and nevoid basal cell carcinoma. Heterozygous pathogenic variants in PTCH1 are responsible for 90% of Gorlin syndrome cases. Pathogenic variants in PTCH1 cause overstimulation of the sonic hedgehog signaling pathway, which plays a role in the development of embryonic structures and tumorigenesis. Clinical major and minor diagnostic criteria for Gorlin syndrome have been determined. Odontogenic keratocyst (OKC) is the most common reason for medical admission in Gorlin syndrome. In this article, it is aimed to draw attention to the fact that patients with Gorlin syndrome are not very rare in our country and the variability in phenotypic and dysmorphic findings may be a clue for the diagnosis. Methods: Exome sequencing was performed on the Illumina NextSeq550 System platform by using the Ion Ampliseq exome RDY kit for Illumina. Sanger sequencing was performed accordingly for the other affected individuals in both families. Results: In this study, the clinical and molecular findings of 9 Gorlin syndrome patients from three unrelated families are presented. Macrocephaly, calcification of falx cerebri, palmar-plantar pits, rib anomalies, and OKC were detected in decreasing order in more than half of the patients. A novel heterozygous frameshift PTCH1 variant in family 1, a nonsense previously reported PTCH1 variant in family 2, and a novel heterozygous splice-site PTCH1 variant in family 3 were detected. Conclusion: Gorlin syndrome should be kept in mind in patients presenting with macrocephaly, palmoplantar pits, and OKC history. Careful examination of all family members is essential in the timely diagnosis of other affected individuals with minor phenotypic findings.

Abstract:Objective: Adult liver Langerhans cell histiocytosis (LCH) is an extremely rare disease. This paper reports a 40 years old male patient who was diagnosed as liver LCH though ultrasound-guided liver biopsy. The initial Fluorrine-18-fluorodeoxyglucose positron emission tomography/computed tomography (F-18-FDG PET/CT) showed multiple nodular low-density lesions in liver without obvious elevated F-18-FDG uptake. Four years later, the follow-up F-18-FDG PET/CT showed the liver multiple lesions with slightly elevated F-18-FDG uptake. Conclusion: We describe this case, to highlight the importance of F-18-FDG PET/CT in differential diagnosis for the primary disease and the multiple liver nodules.

Dynamic PET/CT imaging of 18F-(2S, 4R)4-fluoroglutamine in healthy volunteers and oncological patients

Abstract:Purpose The purpose of this study was to compare dynamic F-18-FGln PET/CT images of healthy subjects and cancer patients and explore the best imaging phase for different cancers. Methods Thirteen healthy volunteers and 31 cancer patients separately underwent F-18-FGln and F-18-FDG PET/CT scans within 1 week. The distributions of F-18-FGln and F-18-FDG in the whole body and the tumor avidity were analyzed and compared. The tumor maximum standardized uptake values (SUVmax) and tumor-to-nontumor SUV ratio (SUR) of F-18-FGln/PET at different scan phases were compared. Results Compared to the healthy subjects, the cancer patients had lower F-18-FGln activity (SUVmean) in most normal organs, especially in the lung, muscle, spleen, and heart (p < 0.05). Additionally, the FGln-avid tumors did not necessarily manifest as FDG-avid and vice versa. Overall, among the 31 primary malignant lesions confirmed by biopsy or postoperative pathological analysis, 29 showed increased radioactive uptake on all F-18-FGln PET/CT imaging phases. The peak of SUVmax in breast and thyroid cancers was within 10 min, while in lung cancers, the plateau of SUVmax was within 30 min to 60 min. The SURs of lung cancer (p = 0.046) and thyroid cancer (p = 0.794) increased from the early-phase to the late-phase acquisition; however, a significant decrease was observed in the breast lesions (p = 0.022). Conclusions F-18-FGln images may further supplement the diagnostic ability of F-18-FDG in cancer patients and detect metabolic changes in different tumors. Furthermore, the imaging time for F-18-FGln PET/CT needs to be optimized for different cancer types to improve the contrast resolution of tumors.

Abstract:Objective: To explore the imaging manifestation and clinical characteristics of primary salivary gland-type lung cancer using (18)F-fluorodeoxy glucose ((18)F-FDG) positron emission tomography/computed tomography (PET-CT). Methods: From March 2009 to January 2017, 12 patients with pathologically confirmed primary salivary gland-type lung cancer were enrolled in First Affiliated Hospital of Nanjing Medical University. Their images and clinicopathological data were retrospectively analyzed. Results: Six out of 12 patients had mucoepidermoid carcinoma (MEC), and the other six patients had adenoid cystic carcinoma (ACC). Five MEC were located in the main bronchus, and the other one was in segmental bronchus. Intrabronchial nodule or mass with smooth or lobulated margin and calcification(n=3) was the main (18)F-FDG PET-CT features of MEC. Two ACC involved trachea, two involved the main bronchi, and the other two involved lobular bronchi. The main (18)F-FDG PET-CT features of ACC were diffuse or circumferential irregular thickness of the bronchial wall, distorted lumen, and the longitudinal extent of the tumor was greater than its transverse axis. The (18)F-FDG uptake of all lesions was increased in varying degree. The median (25th percentile, 75th percentile) value of maximum standardized uptake value (SUVmax), metabolic tumor volume (MTV) and total lesion glycolysis (TLG) were 5.1(3.1, 8.1), 5.7(1.2, 21.4)cm(3) and 18.6(0.6, 93.7), respectively. All of them were related to pathological grading and nodal tumor involvement( all P<0.05), but not associated with tumor location or pathological type( all P>0.05). MTV and TLG were also related to clinical stage( all P<0.05). Tumor size was correlated with MTV, TLG of primary lesions(r=0.607, P=0.036; r=0.579, P=0.049), but not with SUVmax(r=0.568, P=0.054). Conclusions: Primary salivary gland-type lung cancer mainly occurs in segmental bronchus. The MTV and TLG of the tumor calculated by (18)F-FDG PET-CT are correlated with clinicopathological characteristics, and are helpful for clinical diagnosis and treatment.

Abstract:Objectives 18 F-fluorodeoxyglucose positron emission tomography-computed tomography ( 18 F-FDG-PET/CT) is a diagnostic imaging method that is based on the Warburg effect, which is the increased uptake of glucose through aerobic glycolysis in cancer cells. The diagnostic value of 18 F-FDG-PET/CT for thyroid cancer is controversial. However, uptake of 18 F-FDG and the corresponding maximum standardized uptake value (SUVmax) is expected to reflect the metabolic status of cancer cells. In the present study, we sought to determine the relationship between 18 F-FDG uptake and tumor metabolism- associated factors. Methods This was a single-center retrospective study. In the present study, SUVmax was compared with the expression of hexokinase 2 (HK2), glucose transporter 1 (GLUT1), vascular endothelial growth factor (VEGF), and glutaminase 1 (GLS1) in 41 patients with thyroid cancer. Results GLS1 expression was found to be moderately correlated with SUVmax ( p < 0.001, r = 0.51), whereas HK2 and VEGF expression were weakly correlated ( p = 0.011, r = 0.28, p = 0.008, r = 0.29, respectively) and GLUT1 did not correlate with SUVmax ( p = 0.62, r = 0.06). Conclusion Our findings suggest 18 F-FDG PET/CT reflects GLS1 expression in thyroid cancer and could be used to select suitable candidates for GLS1 inhibitor treatment.

Abstract:Altered biodistribution of [F-18]2-fluoro-2-deoxyglucose (FDG) is sometimes encountered in pediatric patients undergoing chemotherapy for lymphoma on post-induction positron emission tomography (PET) imaging. A characteristic pattern of increased FDG uptake in white adipose tissue can be seen, particularly in the buccal regions, body wall and gluteal regions, with a shift of radiotracer away from the blood pool and liver. This altered biodistribution has been attributed to effects of corticosteroids in pediatric and adult patients and is important to recognize because of its potential for limiting the diagnostic quality of the PET scan and interfering with therapeutic response assessment.
