Abstract:Digital literacy is essential for scientific literacy in a digital world. Although the NGSS Practices include many activities that require digital literacy, most studies have examined digital literacy from a generic perspective rather than a curricular context. This study aimed to develop a self-report tool to measure elements of digital literacy among middle and high school students in the context of science practice. Using Messick's validity framework, Rasch analysis was conducted to ensure the tool's validity. Initial items were developed from the NGSS, KSES, and other countries' curricula and related research literature. The final 38 items were expertly reviewed by scientists and applied to 1194 students for statistical analysis. The results indicated that the tool could be divided into five dimensions of digital literacy in the context of science practice: collecting and recording data, analyzing and interpreting (statistics), analyzing and interpreting (tools), generating conclusions, and sharing and presenting. Item fit and reliability were analyzed. The study found that most items did not show significant gender or school level differences, but scores increased with grade level. Boys tended to perform better than girls, and this difference did not change with grade level. Analysis and Interpretation (Tools) showed the largest differences across school levels. The developed measurement tool suggests that digital literacy in the context of science practice is distinct from generic digital literacy, requiring a multi-contextual approach to teaching. Furthermore, the gender gap was evident in all areas and did not decrease with higher school levels, particularly in STEM-related items like math and computational languages, indicating a need for focused education for girls. The tool developed in this study can serve as a baseline for teachers to identify students' levels and for students to set learning goals. It provides information on how digital literacy can be taught within a curricular context.

Abstract:OBJECTIVE:To develop dataset for basic medical information system.METHODS:All items of basic medical information system dataset were drafted by literature review, brain storm and group discussion. A Delphi study was conducted to evaluate and adjust the candidate items; then items were finalized based on comprehensive scores.RESULTS:The final dataset developed through two-round Delphi study contained 20 first-level items, 102 second-level items, 40 third-level items and 88 fourth-level items. The response rates were 71.79% and 92.86% in round 1 and 2, respectively. The authority coefficient was both 0.77 and the coordination coefficient of the specialists' opinion was 0.239 and 0.516, respectively.CONCLUSION:The dataset for basic medical information system determined from Delphi study is credible and can be used for development of related software.

Development and validation of a short form of the medication literacy scale for Chinese College Students

Abstract:Medication literacy is integral to health literacy, pivotal for medication safety and adherence. It denotes an individual's capacity to discern, comprehend, and convey medication-related information. Existing scales, however, are time-consuming and predominantly cater to patients and community dwellers, necessitating a more succinct instrument. This study presents the development of a brief Medication Literacy Scale (MLS-14) utilizing classical test theory (CTT) and item response theory (IRT), targeting a college student demographic. The MLS-14's abbreviated version, a 6-item scale (MLS-SF), was distilled through CTT and IRT methodologies, engaging 2431 Chinese college students to scrutinize its psychometric properties. The MLS-SF demonstrated a Cronbach's {\alpha} of 0.765, with three extracted factors via exploratory factor analysis, accounting for 66% of the cumulative variance. All items exhibited factor loadings above 0.5. The scale's three-factor structure was substantiated through confirmatory factor analysis with satisfactory fit indices (chi2/df=5.11, RMSEA=0.063, GFI=0.990, AGFI=0.966, NFI=0.984, IFI=0.987, CFI=0.987). IRT modeling confirmed reasonable discrimination and location parameters for all items, free of differential item functioning (DIF) by gender. Except for items 4 and 10, the remaining items were informative at medium theta levels, indicating their utility in assessing medication literacy efficiently. The developed 6-item Medication Literacy Short Form (MLS-SF) proves to be a reliable and valid instrument for the expedited evaluation of college students' medication literacy, offering a valuable addition to the arsenal of health literacy assessment tools.

Abstract:Background Information on nutrition literacy of middle schoolers is limited and tools for measuring nutrition literacy of middle schoolers are inadequate. Nutrition literacy has a positive effect on health. Improving children's nutrition literacy can help them to master the necessary nutritional knowledge, develop a healthy lifestyle, and learn to supplement nutrition according to their own needs for healthy growth. Objectives To develop the Chongqing Middle school student Nutrition Literacy Scale (CM-NLS). Methods Three experiments were conducted. A theoretical framework and an initial item pool of CM-NLS were established based on the literature review. And the two-round Delphi method was used to explore the suitable acceptance indicators and items. Item evaluation and reduction were performed using the classical test theory. Then, the items in the final CM-NLS were tested for their validity and reliability amongst 462 middle school students. The construct validity was assessed using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). The internal consistency reliability and split-half reliability were evaluated using Cronbach's alpha coefficients. Results The final CM-NLS consisting of 52 items that were based on three primary items (functional, interactive and critical) and six sub-items (obtain, understand, apply, interact, medial literacy and critical skill) was developed and validated. EFA suggested six factors explaining 69.44% of the total variance (Kaiser–Meyer–Olkin test = 0.916, Bartlett's test χ2 = 5,854.037, P < 0.001). CFA showed that the model fit the data adequately, with χ2/df = 1.911, root mean square error of approximation = 0.063, goodness-of-fit index = 0.822 and adjusted goodness of fit index = 0.790. The total CM-NLS Cronbach's alpha values of internal consistency and split-half reliability were 0.849 and 0.521, respectively, with reasonable reliability. Conclusions CM-NLS is a valid and reliable instrument for assessing nutrition literacy among middle school students in Chongqing. Specifically, it could be used by practitioners for needs assessment before the implementation of a nutrition education program.

Abstract:Objective To study the index system of health literacy among college students, so as to provide the basis for carrying out effective and scientific health education among college students and effectively promoting the health level of college students. Methods This study developed the index system of college students' health literacy through literature analysis and Delphi expert consultation. Results Literature review was conducted for understanding the definitions, connotation, and dimensions of the existing assessment tools on health literacy. Based on governmental documents about the college students health, core issues and key demands of college students' health were defined through experts consulting conference and focus group interview. On the basis, the assessment indicators system of health literacy for college students was developed through Delphi Expert Consultation. Conculsion This research develops a matrix of health literacy assessment system for college students in China, and constructs a four-level health literacy assessment system for college students from the perspective of health-related information processing and two dimensions of health-related field for college students. It covers all aspects of information processing and the level of health literacy of college students in different fields of health content.
Development and Application of Information Literacy Assessment Tool for Primary and Secondary School Teachers

Abstract:The level of teachers' information literacy is not only an important force to promote teacher professional development, but also an important factor to affect the integration and innovation of information and communication technology (ICT) in education. Using scientific and reasonable assessment tools to carry out evaluation is an important part of promoting teachers' information literacy. Therefore, this study used the situational judgment test and the Delphi method to design the initial test paper, and then developed the assessment tool after several rounds of iterative revision, so as to provide support for the information literacy evaluation of primary and secondary school teachers. The assessment tool included five dimensions (i.e. information awareness, information knowledge, information application, information ethics and security, and professional development) with a total of 38 questions. This study introduced the development process, assessment items and scoring rules of the assessment tool, and verified the effectiveness of the tool in terms of difficulty, discrimination, reliability and validity. In addition, the assessment tool was used to evaluate the information literacy of primary and secondary school teachers in X city in China, and the results could clearly reveal the development status quo of teachers' information literacy in this city.

Abstract:Purpose: There are few studies on the intervention of the doctor-patient relationship (DPR) from patients' perspective, because of lacking appropriate measurement instruments for the effect of the patient interveqntion. Understanding the status of patients' literacy (the ability to initiatively obtain, read, and analyze health-related materials, make appropriate decisions, reduce risks of health-related problems) and developing the patients' literacy scale (PLS) align with the interests of patients, doctors, and researchers.Patients and Methods: This study was conducted in two stages: item building and item refinement. A total of 303 subjects were recruited from the outpatient hall in three hospitals with different levels in Hangzhou city, China. Twenty patients and seven experts determined the face and content validity, respectively. The construct validity, convergent, discriminant validity, and known-group validity of the scale were examined by exploratory and confirmatory factor analysis (EFA and CFA). Internal consistency, including Cronbach's alpha, McDonald's omega, split-half reliability, and composite reliability (CR), was also tested.Results: The EFA of PLS showed that Kaiser-Meyer-Olkin (KMO) measure of sampling adequacy was 0.787, and Bartlett's Test of Sphericity showed a significance of p < 0.001. The extracted four-factor (patient activation, knowledge, attitudes, practice) model explained 61.266% of the total variance. For the overall PLS, the Cronbach's alpha, McDonald's omega and split-half reliability coefficient were 0.815, 0.838 and 0.720, respectively. The CFA showed the goodness of fit (RMSEA = 0.065, CMIN/DF = 1.635, GFI = 0.900, CFI = 0.905, TLI = 0.909). The CR of each factor in this scale was 0.740, 0.732, 0.630, and 0.749, respectively.Conclusion: This study showed that the PLS was valid and reliable to measure the patients' literacy (PL). The 15-item PLS can help not only measure PL but also be used as a standard and advocacy target for patients' behavioral model which can further improve the DPR.

Abstract:Objective To develop the ehealth literacy scale for college students,and assess its reliability and validity. Methods The eHealth Literacy Scale( eHEALS) was translated into Chinese version and revised after qualitative interviews and pre-investigation. A total of 1163 college students were investigated by using the scale. Results The eHEALS was included about 3 dimensions and 20 items: the acquiring ability of ehealth,the information evaluation ability of ehealth,the practical ability of ehealth. Cronbach coefficient of the 3 dimensions was between 0. 836- 0. 895,for total items was 0. 915. The factor load coefficients were between 0. 451- 0. 829. The correlation coefficient for the items in each dimension had statically significance( P 0. 05). Average score of the 1163 respondents was( 68. 03 ± 13. 04). Average score of each item was( 3. 40 ± 0. 08). Conclusion The revised ehealth literacy scale for college students( Chinese version) has acceptable constructional validity,content validity and internal consistency,which can be applied assess the ehealth literacy among college students.

Abstract:Objective To establish health literacy assessment framework and tool for primary and secondary school students based on characteristic of school health education in China.Methods Based on literature review,qualitative interviews,Delphi expert consultation and comparisons of national and international school health education guidelines,the health literacy assessment framework was developed.Then the health literacy assessment tool was formed under the assessment framework.Using the health literacy assessment tool,4105 students (1489 high school students,1255 middle school students,1361 primary school students) were recruited to participant field study using stratified random cluster sampling method.The feasibility,reliability and validity of the assessment tool were examined by using SPSS 13.0.Results The framework of health literacy assessment for students consisted of three-level dimensions.The first-level dimensions were comprised of 4 indexes (knowledge,concept,skill and behavior) in each school-age level (high,middle and primary school).The second-level dimensions included 19,19 and 17 indexes and the third-level dimensions were 56,60,49 indexes respectively.The reliability and validity of the health literacy assessment tool for each school-age level were good.Cronbach's α and split-half reliability were higher than 0.7.The correlation coefficient between the assessment tool for high school and questionnaire for Chinese citizen was 0.716,and the coefficient of criterion validity at different grades with NVS,NLAI,eHEALS were statistical significances (P ＜ 0.001).Conclusion The health literacy assessment framework and questionnaire were developed,and its reliability and validity were acceptable.

Abstract:The aim of this research is to develop and validate a scale for the evaluation of medical students’ information literacy self-efficacy beliefs, as this plays a crucial role in the development of lifelong learning objectives. Curriculum developers and medical educators need to have a good understanding of information literacy in order to decide when specific support and training should be integrated in the curricula. The use of a trustworthy, user-friendly tool in a large population able to detect different aspects of students’ information literacy self-efficacy beliefs could help to evaluate an entire curriculum. A 5-factor model was developed and validated within a 6-year medical curriculum (n=1252). Internal consistency of the subscales was high (α: 0.845-0.930). In conclusion, the Information Literacy Self-Efficacy Scale for Medicine (ILSES-M) could be an added value for evaluating medical students’ information literacy self-efficacy beliefs. Furthermore, it could form the basis for curriculum development as well as a guideline for critical curriculum reflection.

Abstract:Introduction: Health information literacy is critical for individuals to obtain, understand, screen, and apply health information. However, there is currently no specific tool available to evaluate all four dimensions of health information literacy in China. Public health emergencies can present an opportunity to evaluate and monitor the health information literacy level of residents. Therefore, this study aimed to develop a questionnaire to evaluate the level of health information literacy and to measure the reliability and validity. Methods: The development process of the questionnaire consisted of the determination of questionnaire items, expert consultation, and validation. Based on the National Residents Health Literacy Monitoring Questionnaire (2020) and the 2019 Informed Health Choices key concepts, the researchers drafted the questionnaire, including all four dimensions of health information literacy. Experts in relevant fields were invited to evaluate the draft questionnaire, and revisions were made accordingly. Finally, the reliability and validity of the finalized version were examined in Gansu Province, China. Results: The research team preliminarily formulated 14 items encompassing the four dimensions of health information literacy. After consulting with 28 experts, modifications were made. A convenience sample of 185 Chinese residents was invited to participate. Cronbach's alpha coefficient was 0.715 and McDonald's omega was 0.739 for internal consistency, and the test-retest intra-class correlation coefficient after 4 weeks was 0.906, indicating that the questionnaire content and measurement structure was relatively stable. Conclusion: This questionnaire is the first evidence-based assessment tool developed for monitoring health information literacy in China, and it has shown good reliability and validity. It can help to monitor the health information literacy levels of Chinese residents, promote evidence-based decision-making, and guide interventions to improve health information literacy.

Abstract:Objective To develop a health literacy scale for middle school students,and examine the reliability and validity of the scale.Methods Based on the definition of health literacy provided by WHO and the content of adolescent health education according to “Outline of health education guidance in primary and secondary school students”and “Health education norms of primary and secondary school students”,the conceptual framework and constructed the items pool of the health literacy scalewas designed.The scale was then administered to secondary school students for pre-investigationin Shang-hai.The validity of the scale was examinedby using factor analysis and structural equation model,and examined the reliabil-ity of the scale and the subscales by measuring their internal consistency.Results 498 middle school students were investi-gated in our survey.And the scalefinally formed and contained 31 items after screened the items by the methods of critical ra-tio,factor loading and Cronbach’s alpha coefficient.Exploratory factor analysis identified five subscales,which together ex-plained 41.3% of the instrument’s variance.Based on confirmatory factor analysis,the items were grouped into five sub-scales representing health concepts,health knowledge,health-related lifestyle and behaviors,health-related cognitive skills and operational skills (χ2 =520.748,P <0.001,CFI =0.920,TLI =0.912,RMSEA =0.022,RMSEA 90% CI:0.014 -0.028,WRMR =1.048).The Cronbach’s alpha coefficient for the health literacy scale was 0.739 and the corre-sponding figures for the subscales ranged from 0.541 to 0.679.Conclusion The health literacy scale for middle school student demonstrated relatively good validity and reliability.

Abstract:Objectives To develop and validate a short-form nutrition literacy (NL) assessment tool for Chinese college students based on a 43-item NL measurement scale. Methods To develop and validate short-form NL scale, 1359 college students were surveyed, the data were analyzed using exploratory factor analysis, linear regression analysis, Item analysis, confirmatory factor analysis, and Pearson correlation. Results The 12-item short-form NL scale (NL-SF12) was developed using factor analysis and regression analysis, which accounted for 96.4% of the variance. The correlation coefficient between the NL-SF12 and NL-43 was 0.969, indicating satisfactory criterion-related validity. The NL-SF12 had a Cronbach's α of 0.890, suggesting strong internal consistency reliability, and content validity index was greater than 0.9, indicating that each domain accurately reflects the connotation of nutrition literacy. The model–data fit and convergent validity of the confirmatory factor analysis results were both good. Conclusion The NL-SF12 is an effective measurement tool with a good reliability and acceptable validity to assess comprehensively NL for college students, and is applicable to quick, widespread use in population study and practice with low respondent burden.

Structural Validation and Measurement Invariance Testing of the Chinese Version of the eHealth Literacy Scale Among Undergraduates: Cross-Sectional Study

Abstract:Background: The eHealth Literacy Scale (eHEALS) was introduced in China in 2013 as one of the most important electronic health literacy measurement instruments. After a decade of development in China, it has received widespread attention, although its theoretical underpinnings have been challenged, thus demanding more robust research evidence of factorial validity and multigroup measurement properties. Objective: This study aimed to evaluate the Chinese version of the eHEALS in terms of its measurement properties. Methods: A cross-sectional survey was conducted in a university setting in China. Item statistics were checked for response distributions and floor and ceiling effects. Internal consistency reliability was confirmed with Cronbach α, split-half reliability, Cronbach α if an item was deleted, and item-total correlation. A total of 5 representative eHEALS factor structures were examined and contrasted using confirmatory factor analysis. The study used the item-level content validity index (I-CVI) and the average of the I-CVI scores of all items on the scale to assess the content validity of the dominance model. Furthermore, the validated dominance model was subsequently used to evaluate the relevance and representation of elements in the instrument and to assess measurement invariance across genders. Results: A total of 972 respondents were identified, with a Cronbach α of .92, split-half reliability of 0.88, and item-total score correlation coefficients ranging from 0.715 to 0.781. Cronbach α if an item was deleted showed that all items should be retained. Acceptable content validity was supported by I-CVIs ≥0.80. The confirmatory factor analysis confirmed that the 3-factor model was acceptable. The measurement model met all relevant fit indices: average variance extracted from 0.663 to 0.680, composite reliability from 0.810 to 0.857, chi-square divided by the df of 4.768, root mean square error of approximation of 0.062, standardized root mean squared residual of 0.020, comparative fit index (CFI) of 0.987, and Tucker-Lewis index of 0.979. In addition, the scale demonstrated error variance invariance (Δnormed fit index=-0.016, Δincremental fit index=-0.012, ΔTucker-Lewis index=0.005, Δcomparative fit index=-0.012, Δrelative fit index=0.005, and Δroot mean square error of approximation=0.005). Conclusions: A 3-factor model of the Chinese version of the eHEALS fits best, and our findings provide evidence for the strict measurement invariance of the instrument regarding gender.
Measuring Health Literacy Regarding Infectious Respiratory Diseases: a New Skills-Based Instrument.

Abstract:BACKGROUND:There is no special instrument to measure skills-based health literacy where it concerns infectious respiratory diseases. This study aimed to explore and evaluate a new skills-based instrument on health literacy regarding respiratory infectious diseases.METHODS:This instrument was designed to measure not only an individual's reading and numeracy ability, but also their oral communication ability and their ability to use the internet to seek information. Sixteen stimuli materials were selected to enable measurement of the skills, which were sourced from the WHO, China CDC, and Chinese Center of Health Education. The information involved the distribution of epidemics, immunization programs, early symptoms, means of disease prevention, individual's preventative behavior, use of medications and thermometers, treatment plans and the location of hospitals. Multi-stage stratified cluster sampling was employed to collect participants. Psychometric properties were used to evaluate the reliability and validity of the instrument.RESULTS:The overall degree of difficulty and discrimination of the instrument were 0.693 and 0.482 respectively. The instrument demonstrated good internal consistency reliability with a Cronbach's alpha of 0.864. As for validity, six factors were extracted from 30 items, which together explained 47.3% of the instrument's variance. And based on confirmatory factor analysis, the items were grouped into five subscales representing prose, document, quantitative, oral and internet based information seeking skills (χ(2) = 9.200, P>0.05, GFI = 0.998, TLI = 0.988, AGFI = 0.992, RMSEA = 0.028).CONCLUSION:The new instrument has good reliability and validity, and it could be used to assess the health literacy regarding respiratory infectious disease status of different groups.

Abstract:Despite the great attention paid to Internet literacy research, little has been done to overcome the problems stemming from the heterogeneity of Internet literacy nomenclature and the use of non-standardized measurement tools, especially for adolescents in developing countries. Considering junior students are the high-risk groups of Internet addiction and have wide access to the Internet, the aim of this study is to develop a new scale to assess Chinese junior students’ Internet literacy (JIL). In the psychometric study (n = 1099 junior students), an 18-item scale was developed using the exploratory and confirmatory factor analyses, which includes five subscales: knowledge and skills for the Internet (KSI), Internet self-management (ISM), awareness and cognition of Internet (ACI), Internet interactions (II), and autonomous learning on the Internet (ALI). Evidence of internal reliability, test-retest reliability, and construct validity provided good psychometric support for the measure. Criterion-related validity of the measures was demonstrated by examining its anticipated theoretical relations to two hypotheses: (1) High JIL level alleviates the adverse effects of an individual’s Internet addiction degree, while pathological use for interacting with others on the Internet exacerbates the adverse effects; (2) an individual’s degree of Internet use self-efficacy is positively associated with JIL level. It is envisaged that the JIL Scale will help facilitate unified research in the field.

Abstract:Objective To develop a health literacy comprehensive index,provide the credible scientific tool for health literacy assessment.Methods The normalization method and entropy value method were used to calculate the subjective weight and objective weight and the index method was used to establish the health literacy comprehensive index model.Results There were 40 items screened which weight were calculated to establish the health literacy comprehensive index including four partial indexes: health knowledge,health behavior,health belief and functional health literacy.Conclusion The comprehensive index model is a credible and scientific tool for health literacy assessment and fills in the gaps about theory and technology in this field.
Construction of a Framework for a Comprehensive Health Literacy Assessment System for Chinese Primary School Students:A Delphi-based Study

Abstract:Objective To develop a framework for a comprehensive health literacy assessment system for primary school students that will provide a basis for establishing measurement tools and promoting healthy student development.Methods An expert survey questionnaire was developed using literature review,Delphi method,expert meeting,and other methods.From December 2021 to February 2022,13 experts in health literacy and related fields were selected for two rounds of Delphi questionnaire surveys.A comprehensive health literacy assessment index system was constructed for primary school students in different grades,including grades 1-2,3-4,and 5-6.The weight coefficient was calculated using the percentage weighting method.Results The constructed comprehensive assessment system includes three grade-specific subsystems.For students in grades 1-2 and 3-4,the subsystem includes six primary indicators(personal hygiene,healthy diet,physical activity,disease prevention and control,safety literacy,and mental health)and fifteen secondary indicators;each of the six primary indicators includes four capability dimensions of health knowledge,motivation,behavior,and skills;the subsystem for students in grades 5-6 has an additional primary indicator of e-health literacy,with seven primary indicators and twenty secondary indicators,along with the same four capability dimensions.The effective response rate to the expert questionnaire was 100％in the two rounds of consultation,indicating high enthusiasm of the experts;the coefficients of authority for the two rounds of consultation were greater than 0.7,confirming the reliability of the results;the Kendall's coefficients of 0.302,0.327,and 0.340 for the primary indicators,secondary indicators,and the four ability dimensions of the primary indicators in the second round of consultation were higher than those in the first round of consultation(P＜0.05 for all).Conclusion The constructed framework of a comprehensive health literacy assessment system for Chinese primary school students can serve as a reference for related research on assessing students'health literacy.
