Abstract:Objective:To evaluate the role of 3-dimensional magnetic resonance spectroscopy (3D MRS ) in the diagnosis of prostate lesions.Methods:35 cases had prostate magnetic resonance imaging (MRI) and 3D MRS. Of the 34 cases with prostate lesion,there were 12 patients having prostate carcinoma (PCa),22 cases having benign prostate hyperplasis (BPH).Apart from one healthy volunteer,the pathology diagnosis of 12 cases with PCa were obtained by aspiration biopsy (n=5) and surgery (n=7).All of the pathology diagnosis of 22 cases with BPH were obtained by surgery.The MRS and pathology findings were correlated.The ratio of choline with creatine to citrate (CC/C) in normal and abnormal signal area of central zone and peripheral zone were measured by MRS.The difference between PCa and non-PCa regions were assessed using independent samples T test.Results:The mean CC/C of PCa,nodular hyperplasis of BPH,and peripheral zone with normal signal intensity were 2.57±1.3 (0.9~6.0),0.70±0.20 (0.3±1.60)and 0.56±0.22 (0.17~0.72) respectively.There were significant statistic differences between CC/C of P Ca and normal peripheral zone as well as BPH (t=2.597,P<0.01).No statistic difference was existed between normal peripheral zone and BPH (P>0.05).Conclusion:3D MRS is helpful for the differential diagnosis between benign and malignant prostate lesions.With the combination of MRI,the extent of tumor can be directly and accurately delineated.

Abstract:Prostate cancer (PCa) is the second most common type of cancer among men in the United States. A major limitation in the management of PCa is an inability to distinguish, early on, cancers that will progress and become life threatening. One-dimensional (1D) proton ((1)H) MRS of the prostate provides metabolic information such as levels of choline (Ch), creatine (Cr), citrate (Cit), and spermine (Spm) that can be used to detect and diagnose PCa. Ex vivo high-resolution magic angle spinning (HR-MAS) of PCa specimens has revealed detection of more metabolites such as myo-inositol (mI), glutamate (Glu), and glutamine (Gln). Due to the J-modulation and signal overlap, it is difficult to quantitate Spm and other resonances in the prostate clearly by single- and multivoxel-based 1D MR spectroscopy. This limitation can be minimized by adding at least one more spectral dimension by which resonances can be spread apart, thereby increasing the spectral dispersion. However, recording of multivoxel-based two-dimensional (2D) MRS such as J-resolved spectroscopy (JPRESS) and correlated spectroscopy (L-COSY) combined with 2D or three-dimensional (3D) magnetic resonance spectroscopic imaging (MRSI) using conventional phase-encoding can be prohibitively long to be included in a clinical protocol. To reduce the long acquisition time required for spatial encoding, the echo-planar spectroscopic imaging (EPSI) technique has been combined with correlated spectroscopy to give four-dimensional (4D) echo-planar correlated spectroscopic imaging (EP-COSI) as well as J-resolved spectroscopic imaging (EP-JRESI) and the multi-echo (ME) variants. Further acceleration can be achieved using non-uniform undersampling (NUS) and reconstruction using compressed sensing (CS). Earlier versions of 2D MRS, theory of 2D MRS, spectral apodization filters, newer developments and the potential role of multidimensional MRS in PCa detection and management will be reviewed here.

Abstract:To investigate quantitative 3-D dynamic contrast-enhanced ultrasound (DCE-US) and, in particular 3-1) contrast-ultrasound dispersion imaging (CUDI), for prostate cancer detection and localization, 43 patients referred for 10-12-core systematic biopsy underwent 3-D DCE-US. For each 3-D DCE-US recording, parametric maps of CUDI-based and perfusion-based parameters were computed. The parametric maps were divided in regions, each corresponding to a biopsy core. The obtained parameters were validated per biopsy location and after combining two or more adjacent regions. For CUDI by correlation (r) and for the wash-in time (WIT), a significant difference in parameter values between benign and malignant biopsy cores was found (p < 0.001). In a per-prostate analysis, sensitivity and specificity were 94% and 50% for r, and 53% and 81% for WIT. Based on these results, it can be concluded that quantitative 3-I) DCE-US could aid in localizing prostate cancer. Therefore, we recommend follow-up studies to investigate its value for targeting biopsies. (C) 2018 World Federation for Ultrasound in Medicine & Biology. All rights reserved.

Abstract:Despite being the solid tumor with the highest incidence in western men, prostate cancer (PCa) still lacks reliable imaging solutions that can overcome the need for systematic biopsies. Dynamic contrast-enhanced ultrasound imaging (DCE-US) allows us to quantitatively characterize the vascular bed in the prostate, due to its ability to visualize an intravenously administered bolus of contrast agents. Previous research has demonstrated that DCE-US parameters related to the vascular architecture are useful markers for the localization of PCa lesions. In this paper, we propose a novel method to assess the convective dispersion (D) and velocity (v) of the contrast bolus spreading through the prostate from three-dimensional (3D) DCE-US recordings. By assuming that D and v are locally constant, we solve the convective-dispersion equation by minimizing the corresponding regularized least-squares problem. 3D multiparametric maps of D and v were compared with 3D histopathology retrieved from the radical prostatectomy specimens of six patients. With a pixel-wise area under the receiver operating characteristic curve of 0.72 and 0.80, respectively, the method shows diagnostic value for the localization of PCa.

Abstract:Trans-rectal ultrasound-guided 12-core systematic biopsy (SBx) is the standard diagnostic pathway for prostate cancer (PCa) because of a lack of sufficiently accurate imaging. Quantification of 3-D dynamic contrast-enhanced ultrasound (US) might open the way for a targeted procedure in which biopsies are directed at lesions suspicious on imaging. This work describes the expansion of contrast US dispersion imaging algorithms to 3-D and compares its performance against malignant and benign disease. Furthermore, we examined the feasibility of a multi-parametric approach to predict SBx-core outcomes using machine learning. An area under the receiver operating characteristic (ROC) curve of 0.76 and 0.81 was obtained for all PCa and significant PCa, respectively, an improvement over previous US methods. We found that prostatitis, in particular, was a source of false-positive readings.

Abstract:Objective To address the value of real-time precision targeted prostate biopsy using transrectal ultrasound-magnetic resonance fusion(TRUS/MRI). Methods Twenty four patients with elevated prostate-specific antigen value of 4-20ng/ml,normal or abnormal digital rectum examination,and suspicious mutiparametric magnetic resonance(mpMRI)imaging of T2-weighted or diffusion weighted or dynamic contrast enhanced were enrolled in present study from August 2015 to September 2015.Suspicious areas on mpMRI were delineated and marked by a radiologist.TRUS/MRI fusion biopsy was performed by a urologist and a ultrasonography doctor using Hitachi real-time virtue sonography(RVS)system.At first suspected lesions were biopsied transperineally using RVS system(TB)and then followed by the conventional transperineal systematic biopsy(SB)under general anesthesia.Two cores and 6-12 cores specimens were obtained respectively.The pathological results of TB and SB were compared.The correlation of mpMRI with biopsy results was also evaluated. Results TB revealed prostate cancer(PCa)in 14 of 24men(58.3%),and demonstrated a trend of increasing the cancer detection rate(CDR)compared to SB with a CDR of 37.5%.The positive rate of core in the TB men with PCa was significantly higher than in the SB men(P<0.001).Degree of suspicion on mpMRI had a significant association with CDR(P=0.002). Conclusions TRUS/MRI fusion biopsy provides improved detection of PCa in men with elevated PSA levels and suspicious MR imaging.TB specimens were significant superior to SB.The predictor value of mpMRI has a good concordance with the results of TRUS/MRI fusion biopsy.

Abstract:1H magnetic resonance spectroscopy studies (360 MHz) were performed on specimens of benign (n = 66) and malignant (n = 21) human prostate tissue from 50 patients, and the spectral data were subjected to multivariate analysis, specifically linear-discriminant analysis. On the basis of histopathological assessments, an overall classification accuracy of 96.6% was achieved, with a sensitivity of 100% and a specificity of 95.5% in classifying benign prostatic hyperplasia from prostatic cancer. Resonances due to citrate, glutamate, and taurine were among the six spectral subregions identified by our algorithm as having diagnostic potential. Significantly higher levels of citrate were observed in glandular than in stromal benign prostatic hyperplasia (P < 0.05). This method shows excellent promise for the possibility of in vivo assessment of prostate tissue by magnetic resonance.

Abstract:Objective: To compare the quality of 3D 1H-MRSI of body coil (BODY) and endorectal coil (ERC) in prostate gland examination, and to discuss the clinical feasibility of BODY MRS. Methods: Both BODY and ERC MRS were performed in 40 consecutive patients, and measurable MRS data was obtained in 30 patients. The right and left side of the peripheral and central zone of prostate were divided into bottom, middle and tip regions of interesting (ROI), respectively. Twelve ROIs were obtained in each case. The following parameters relating of MRS quality were compared, i. e. signal-to-noise ratio (SNR), lipid peak and ratio of (Cho+Cre)/Cit. Results: Circled digit oneThe SNR in the peripheral zone of different individuals using ERC was statistically higher than that of using BODY MRS (P<0.05). The SNR in the central gland of bottom using ERC was statistically higher than that of using BODY MRS (P<0.05). Circled digit twoThe Lipid Peak in the peripheral zone of the bottom and the left of tip acquired using ERC was statistically higher than that of using BODY MRS (P<0.05). The Lipid Peak in the central zone of the bottom and the left of middle acquired using ERC was statistically higher than that of using BODY MRS (P<0.05). Circled digit threeThe mean ratios of (Cho+Cre)/Cit in the peripheral zone and central zone of different individuals acquired using both coils were not significantly different (P>0.05). Conclusion: The MRS data can be reliably acquired using BODY MRSI, while the SNR and the Lipid Peak of BODY are inferior to those of ERC MRS.
Improving tissue‐type imaging of prostate cancer by combining magnetic‐resonance spectroscopy and ultrasound spectral parameters

Abstract:This study aims to improve existing prostate tissue-type imaging methods by utilizing independent tissue properties sensed by MR and US. An artificial-neural-network classifier was trained using the blood PSA level and ultrasonic spectral parameters of echo signals derived from biopsied regions of 64 patients. Biopsy-core histology was used as the gold standard. Classifier performance was assessed using ROC analysis. We generated tissue-type images (TTIs) using a look-up table that returned cancer-likelihood scores for spectral parameter and PSA combinations. We assessed the feasibility of integrating MR and US parameters to improve classification. 3-D renderings of the prostate from transverse MR and US scans were generated from data acquired preoperatively. The 3-D rendering obtained from MR data showed that the prostate was considerably distorted (flattened) by the large MR endorectal probe compared to the distortion caused by the smaller US endorectal probe. The MR 3-D rendering was warped successfully to match the US volume. Warping successfully compensated for the deformations introduced during scanning. This suggests that accurate coregistration of 3-D data acquired using MR and US is possible. Such coregistration is essential for developing an improved classifier based on MR and US parameters.

Abstract:Objective To investigate the application of the dynamic contrast enhanced MRI (DCE-MRI) combined with magnetic resonance spectroscopy (MRS) in the diagnosis of prostate cancer.Method A total of 60 patients with prostate cancer and 60 patients with benign prostatic hyperplasia diagnoses in Sichuan Cancer Hospital from January 2011 to January 2014 were included as prostate cancer group and proliferative group respectively.Sixty healthy individuals during the same period were included as the control group.We used Siemens Avanto 1.5 T high field superconducting MRI for DCE-MRI scan and MRS scan.After the MRS scan was finished,we used the workstation spectroscopy tab spectral analysis.Eventually we got the crest lines of prostate metabolites choline (Cho),creatine (Cr) and citrate (Cit).Then we calculated Cho/Cit,(Cho+Cr)/Cit and their average.Results Comparing the signal value in 21 seconds,1 minute,2 minutes of DCE-MRI,the differences among the three groups were statistically significant (P＜0.05).Comparing the results of spectral analysis,the differences among the three groups were statistically significant (P＜0.05).The sensitivity was 89.67％,the specificity was 95.45％ and the accuracy was 94.34％ when using DCE-MRI combined with MRS.Conclusion DCE-MRI combined with MRS greatly improves the sensitivity,specificity and accuracy of the diagnosis of prostate cancer;it has a great application value in the diagnosis of prostate cancer.

Abstract:Background: The most common cancer (non-cutaneous) malignancy among men is prostate cancer. Management of prostate cancer, including staging and treatment, playing an important role in decreasing mortality rates. Among all current diagnostic tools, multiparametric MRI (mp-MRI) has shown high potential in localizing and staging prostate cancer. Quantification of mp-MRI helps to decrease the dependency of diagnosis on readers' opinions.Objective: The aim of this research is to set a method based on quantification of mp-MRI images for discrimination between benign and malignant prostatic lesions with fusion-guided MR imaging/transrectal ultrasonography biopsy as a pathology validation reference.Material and Methods: It is an analytical research that 27 patients underwent the mp-MRI examination, including T1- and T2- weighted and diffusion weighted imaging (DWI). Quantification was done by calculating radiomic features from mp-MRI images. Receiver-operating-characteristic curve was done for each feature to evaluate the discriminatory capacity and linear discriminant analysis (LDA) and leave-one-out cross-validation for feature filtering to estimate the sensitivity, specificity and accuracy of the benign and malignant lesion differentiation process is used.Results: An accuracy, sensitivity and specificity of 92.6%, 95.2% and 83.3%, respectively, were achieved from a subset of radiomics features obtained from T2-weighted images and apparent diffusion coefficient (ADC) maps for distinguishing benign and malignant prostate lesions.Conclusion: Quantification of mp-MRI (T2-weighted images and ADC-maps) based on radiomics feature has potential to distinguish benign with appropriate accuracy from malignant prostate lesions. This technique is helpful in preventing needless biopsies in patients and provides an assisted diagnosis for classifications of prostate lesions.

Abstract:Abstract In this paper, we review the developments of 1 H-MR spectroscopic imaging (MRSI) methods designed to investigate prostate cancer, covering key aspects such as specific hardware, dedicated pulse sequences for data acquisition and data processing and quantification techniques. Emphasis is given to recent advancements in MRSI methodologies, as well as future developments, which can lead to overcome difficulties associated with commonly employed MRSI approaches applied in clinical routine. This includes the replacement of standard PRESS sequences for volume selection, which we identified as inadequate for clinical applications, by sLASER sequences and implementation of 1 H MRSI without water signal suppression. These may enable a new evaluation of the complementary role and significance of MRSI in prostate cancer management.

Abstract:Objective To evaluate three-dimensional reconstruction of MRI images in diagnosis and therapy of prostate cancer. Methods Twenty-eight patients with proven prostate cancers were recruited in this study. Seventeen of them were diagnosed as having prostate cancer according to the ultrasound guided systemic biopsy. Their MR examinations showed fourteen lesions in the peripheral zone and three in the central gland of the prostate. The other eleven patients underwent MR examination after a period of treatment, including endocrinetherapy and brachytherapy. Using endorectal coil, a series of T2-weighted images were acquired on the axial plane. These source images were processed by 3D-Doctor software to reconstruct into three-dimensional images. Results In the fourteen patients with peripheral zone cancer, reconstruction images could display the 3D regions of cancer and the involvement of capsular. The outspread of central gland and the compression of peripheral zone in patients with central gland cancer could be revealed in the same way. The volumetric changes of the lesion and the prostate after endocrinetherapy could also be perceived through these 3D images. Similarly, radioactive seeds were revealed in a spatial manner that could be easily evaluated. Conclusion Three-dimensional reconstruction images were obtained in all patients. They were able to provide stereotyped information about the lesions and their surrounding tissues. MRI three-dimensional reconstruction can be an adjunctive tool in the evaluation of prostate lesions.

Abstract:Purpose: The technical challenges that have prevented routine proton magnetic resonance spectroscopic imaging (1H MRSI) examinations of the breast include insufficient spatial resolution, increased difficulties in shimming compared to the brain, and strong lipid contamination at short echo time (TE) at 1.5 T. The authors investigated the feasibility of high spatial resolution 1H MRSI of human breast cancer in a clinical setting at 3 T.Methods: Ten patient studies (eight cancers and two benign lesions) were performed in a 3 T whole-body clinical imager using a pulse sequence consisting of optional outer volume presaturation, optional CHESS pulse for lipid suppression, CHESS pulse for water suppression, and standard 2D/3D PRESS pulse sequence with an elliptical weighted k-space sampling scheme.Results: All ten studies were technically successful. The spectral quality was acceptable for all cases even the one with a 65 Hz width of water peak at half height. Choline (Cho) signals were clearly visible in malignant lesion areas, while there was no detectable Cho in normal appearing breast or in benign lesions. It was also observed that the distribution of Cho signal can be nonuniform across MRI demonstrated lesions.Conclusions: To the author's knowledge, this is the first 2D/3D MRSI study of human breast cancer with short TE (less than 135 ms) at 3 T and the highest spatial resolution (up to 0.25 cm(3)) to date. In conclusion, the authors have presented a robust technique for high spatial resolution in vivo 1H MRSI of human breast cancer that uses the combined advantages of high field, short TE, multivoxel, and high spatial resolution itself to overcome the major technical challenges and illustrated its potential for routine clinical examination as well as advantages over single-voxel techniques in studying metabolite heterogeneity. Abstract:Objective To study the value of magnetic resonance spectroscopic imaging (MRSD with only surface coils in diagnosing prostate cancer. Methods A total of 48 elderly patients with surgery or biopsy-confirmed benign prostatic hyperplasia or prostate cancer were preoperatively examined by surface coils for three-dimensional simple multi-voxel magnetic resonance spectroscopy scans, and the voxel (choline[Cho] + creatine[Cr])/citrate(Cit) values were calculated. According to the pathological findings, the voxels were divided into prostate cancer and non-cancer. The voxel (Cho + Cr)/Cit values of prostate central zone and prostate peripheral zone were analyzed. The voxel nature of (Cho+Cr)/Cit value was used as the gold standard receiver to plot characteristic curve (ROC curve) and to calculate the area under the curve; the maximal Youden index was used as the standard to obtain the cut-off value and the corresponding sensitivity and specificity. Results The voxel (Cho + Cr)/Cit values of different natures were significantly different (P<0. 001). When the area under the ROC curve of (Cho + Cr)/ Cit value was used for diagnosis of prostate cancer, the central zone was 0. 652, and the peripheral zone was 0. 821. For the cutoff value, the central zone was (Cho + Cr)/Cit = 0. 645, with the corresponding sensitivity and specificity being 0. 512 and 0. 71, respectively; the peripheral zone was (Cho + Cr)/Cit = 0. 815, with the corresponding sensitivity and specificity being 0. 72 and 0. 84, respectively. Conclusion Surface coil alone can successfully complete the three-dimensional proton spectroscopic imaging of prostate, and (Cho + Cr)/Cit = 0. 815 for the cut-off value has a high diagnostic value for peripheral zone prostate cancer.
Three-dimensional Transrectal Ultrasonography of Prostate Cancer

Abstract:Objective:To evaluate the value of three-dimensional and high frequency transrectal ultrasonography(3DUS)in the diagnosis of prostate cancer.Methods：The ultrasonographic images of forty-six patients with prostate cancer which had been obtained by CombisonR 530 three-dimensional imaging system(7.5MHz transrectal transducer)were analysed retrospectively.Results:The ultrasonic signs of prostate cancer could be classified as focal nodular type,nodule-infiltrative type and diffuse infiltrative type.3DUS could provide more informations than those of 2DUS with multi-plane observation and improve the diagnostic accuracy rate(82.6%) of prostate cancer(especially for the last two categories),moreover it could clearly show the spatial relationships of focal lesions,prostatic capsule and nearby structures,and help us to stage tumors.Conclusions:3DUS can provide more informations for the operators,so as to improve the ultrasound diagnostic accuracy rate of prostate cancer obviously.
Application value of multi-parameter magnetic resonance image-transrectal ultrasound cognitive fusion in prostate biopsy

Abstract:To investigate the effect of three-dimensional (3D) reconstruction-assisted cognitive fusion in targeted prostate biopsy. There was no significant difference in the detection rate of prostate cancer (PCa) between targeted biopsy and systematic biopsy, and there was significant difference in the detection rate of clinically significant prostate cancer (csPCa) between targeted biopsy and systematic biopsy. In the low prostate total specific antigen (tPSA) group, there was no statistically significant difference in the detection rate of prostate cancer between the two biopsy modalities. However, compared with systematic puncture, targeted puncture had a higher detection rate for csPCa and a lower detection rate for clinically insignificant prostate cancer (ciPCa), and the difference was statistically significant. In the high tPSA group, there was no significant difference in the detection rate of PCa, csPCa, and ciPCa between the two biopsy types. Single needle positive rate of targeted puncture (29.77%) was significantly higher than that of systematic puncture (10.28%). The detection rate of csPCa in 3D reconstruction-assisted cognitive fusion targeted prostate biopsy is better than that of 12-needle systematic biopsy, which markedly improved the positive rate of prostate biopsy.
