COMPARATIVE OBSERVATION OF BILATERAL TRIGEMINAL NEURALGIA AFTER RADIOFREQUENCY THERMOCOAGULATION AT DIFFERENT TEMPERATURES

Abstract:Objective:To comparatively observe the outcomes of RFT at varying temperatures in patients with bilateral trigeminal neuralgia (TN), including pain relief, complications, and patient satisfaction.Methods:From Jan 2013 to Dec 2015, 34 patients with bilateral TN in Shengjing hospital of China Medical University were recruited into the study. These patients underwent bilateral RFT at 70℃ or 75℃ respectively. After RFT, a 3-year follow-up was completed for evaluating pain relief, complications and patient satisfaction. Results:All patients were satisfied with the pain relief after RFT. The proportion of excellent pain relief at both sides was 97.4% (70℃) and 98.2 (75℃) at discharge, 93.7% and 96.7% at 1 year, 84.0% and 92.6%at 2 years, and 84.0% and 84.9% at 3 years, respectively. There were 4 and 3 cases of recurrence at different temperature sides, respectively. The incidence rate and severity of complications including facial numbness, masticatory atonia and corneal hypoesthesia were more serious at 75℃ RFT side than that at 70℃ side (P< 0.05), and patient satisfaction was lower at 75℃ RFT side. Conclusion: Our results show that there is consistent pain relief after both 70℃ and 75℃ RFT. However, the 70℃ RFT is superior to the 75℃ RFT in the incidence rate, severity of complications and patient satisfaction. Therefore, the clinicians are suggested to select 70℃ temperature for V2 and V3 TN RFT, so as to reduce complications.
