MR virtual colonography with fat contrast medium enema in detection of colorectal neoplasms

Abstract:Objective To evaluate the potential value of MR virtual colonography (MRVC) with fat contrast medium enema in the detection of colorectal neoplasms. Methods Twenty-six patients underwent MR colonography, after bowel preparation, administration of anisodamine and rectal administration of fat contrast medium. All patients were imaged with T1-weighted three-dimensional fast spoiled gradient-echo with inversion recovery (liver acquisition volume acceleration, LAVA) sequence under fat-saturation in the supine position before and after enhancement. Entire colon was covered in one contiguous coronal breath-hold acquisition lasting for 28 seconds or less. Two radiologists reviewed patient's tolerance, image quality, and sensitivity of colorectal neoplasms. The colorectal neoplasms detected with MRVC were compared with those identified on endoscopic and pathologic results. Results All patients completed the MR examination, including MR virtual endoscopy. Image quality (coronal imaging, multiplanar reformation and MR virtual endoscopy images) was good in all cases. 198/208 (95.2%) colonic segments were well distended in supine position. Mean contrast-to-noise ratio of colorectal masses was 16.2 for the post contrast scans. Sensitivity of lesions larger than 10 mm was 95.8%. Conclusion MRVC with fat contrast medium enema and LAVA sequence is a promising alternative method for detection of colorectal neoplasms. In addition, it allows assessment of extraluminal organs.
