Factors influencing consciousness disorder in acute stroke patients and their outcome

Abstract:Objective To analyze the role of Glasgow coma scale (GCS) in predicting the outcome of acute stroke (AS) patients and the factors influencing consciousness disorder in AS patients and their outcome.Methods The data of 6336 AS patients covered in China QUEST Database were analyzed.The patients were divided into GCS score>8 group (n=5745) and GCS score ≤8 group (n=591).The role of GCS in predicting the outcome of AS patients was analyzed according to the area under the ROC curve.The factors influencing consciousness disorder in AS patients were analyzed by multivariate logistic regression analysis.Results The education level,ambulance delivery rate,incidence of AF and hemorrhagic stroke were significantly higher while the rate of regular alcohol comsumption and the incidence of ischemic stroke were significantly lower in GCS score>8 group than in GCS score ≤8 group (60.9% vs 46.7%,62.1% vs 18.5%,10.0% vs 4.8%,60.9% vs 20.8%,P=0.000;19.3% vs 27.7%,36.9% vs 78.3%,P=0.000).The area under the ROC curve for GCS in predicting the 12-month mortality and inhospital mortality of AS patients was 0.831 and 0.768 respectively (95%CI:0.804-0.858,95%CI:0.748-0.787,P< 0.01).Higher education level,prehospital delay>6 h,regular alcohol comsumption,hemorrhagic stroke,ambulance delivery rate and history of AF were the factors influencing consciousness disorder in GCS score≤8 group.Conclusion GCS can predict the outcome of AS patients.
