Abstract:Objective To evaluate the characteristics of atrial function and synchronism in patients with preexcitation syndrome by two dimensional speckle tracking imaging.Methods The conventional echocardiographic examination and two dimensional speckle tracking imaging were performed in 26 patients with preexcitation syndrome and 38 controls.Firstly,the conventional echocardiographic parameters including IVST,LVPWT,LVIDd,LVIDs,LAD,LVEF,E and A were measured.Then,a dynamic echocardiographic image of the apical four chamber view was obtained and the speckle tracking imaging of the right atrium free wall,atrial septum and left atrium free wall was analysed.The peak systolic strain rates(S wave)and peak systolic strain rate in atrial systole(A wave)were measured and the time from the onset time of P wave to the onset time of A wave of speckle tracking image was measured,that is to say,the time to peak of electrical mechanical motion of the right atrium free wall(P-RA),atrial septum(P-IAS)and left atrium free wall(P-LA),respectively.The synchrony of intra-atrial and inter-atrial electro-mechanical motion based on the formula was calculated.The differences of P-RA,P-IAS,P-LA and atrial synchrony parameters between different groups were compared and the relationship between P-RA,P-IAS,P-LA,atrial synchrony parameters and preexcitation syndrome was analysed.Results There was no significant difference of the clinical parameters and the conventional echocardiographic parameters between different groups.There was no significant difference of the S wave and the A wave between different groups.The parameters of synchronism of the intra-right atrium,the intra-left atrium and the inter-left and right atrium were significantly longer in the patients with preexcitation syndrome than in the control group,and the difference was statistically significant(all P<0.05).The correlation analysis showed that the synchronism of the intra-right atrium,the intra-left atrium and the inter-left and right atrium was correlated with preexcitation syndrome and that the correlation between the inter-left and right atrium synchronism and the preexcitation syndrome is the best(r=0.578).Conclusions The decrease of atrial synchrony in patients with preexcitation syndrome is closely related to preexcitation syndrome.
