Abstract:Aortic dissection is a life-threatening emergency disease, which needs immediate and effective clinical interventions. In recent years, owing to the widespread use of multidetector computed tomography, the rate of diagnosis of aortic dissection has significantly improved. As a convenient and noninvasive examination, multidetector computed tomography angiography has been used widely in the preoperative diagnosis of aortic dissection and postoperative follow-up. Improving CT scan protocols and powerful post-processing function make the diagnosis of aortic dissection more accurate and provide adequate evidences for clinical treatments. This review summarized the recent application and research progress of multidetector computed tomography angiography in the diagnosis of aortic dissection and its future perspective.

Abstract:Acute aortic dissection (AAD) is a high mortality disease that can lead to acute ischemic strokes (AIS). Some of the patients with AAD combined with AIS initially present with neurological symptoms, which can easily lead to missed or delayed AAD diagnosis. This is attributed to the lack of physician awareness or the urgency of patient thrombolysis. Intravenous administration of thrombolytic therapy (IVT) for AAD is associated with poor prognostic outcomes. We report a patient with AIS combined with AAD who developed a massive cerebral infarction after receiving IVT for a missed AAD diagnosis.A 49-year-old man was admitted to a local hospital with an acute onset of left-sided limb weakness accompanied by slurred speech. The patient had a history of hypertension that was not regularly treated with medication. Physical examination revealed incomplete mixed aphasia and left limb hemiparesis. Cranial computed tomography (CT) scan showed bilateral basal ganglia and lateral ventricular paraventricular infarct lesions. The patient was diagnosed with AIS and was administered with IVT. After IVT, patient's muscle strength and consciousness deteriorated. From the local hospital, he was referred to our hospital for further treatment. Emergency head and neck CT angiography (CTA) scans were performed. Results showed multiple cerebral infarctions, and aortic dissection in the ascending aorta, innominate artery, as well as in the right common carotid artery. Then, the CTA of thoracoabdominal aorta was performed, which revealed a Stanford type A aortic dissection and aortic dissection extending from the aortic root to the left external iliac artery. Laceration was located in the lesser curvature of the aortic arch. AAD complicated with AIS was considered, and the patient was immediately subjected to cardiovascular surgery for treatment. The next day, the patient underwent aortic arch and ascending aortic replacement and aortic valvuloplasty.Clinical manifestations for AAD combined with AIS are diverse. Some patients may not exhibit typical chest or back pains. Therefore, patients should be carefully evaluated to exclude AAD before administering IVT in order to avoid adverse consequences.
Aortic Dissection: an Analysis of 80 Cases

Abstract:Objective To investigate the clinical manifestation,diagnosis and treatment of aortic dissection.Methods According to the clinical data of continuous 80 patients with aortic dissection between August 1999 and April 2006,it analyzed the clinical characteristics,imaging diagnosis,treatment methods and outcomes.Results Twenty-three patients were treated by traditional open operations,of which 3 patients(13.0%)died of postoperative complications.Fifty-two patients were performed endoluminal stent-graft implantation,and one patients(1.9%)died 1 day after the procedure due to false luminal rupture.No death occurred during follow up time.Additionally,3 patients(3.8%)died before operation,and 2 gave up treatment.Conclusion The accurate diagnosis as early as possible and active therapy are the key to improve the prognosis of aortic dissection.
Emergency Treatment of Aortic Dissection and Clinical Pathway Discussion

Abstract:Objective To investigate the clinical features of aortic dissection (AD) and emergency treatments. Methods Data from 784 patients with aortic dissection were collected in the Department of Emergency from January 2000 through December 2009. A retrospective analysis was carried out to determine the survival rate, mortality rate and treatment efficiency. Results Pain was the most common onset symptom (77.7% , 609/784). The majority of patients (86.5%) had essential hypertension (678/784). All the patients with preoperative diagnosis of aortic dissection underwent emergency medical intervention by internists resulting in 81.5% survival rate (639/784) and 18.5% mortality rate (145/784). There were 157 patients without improvement (20.0% ) and the total efficiency rate was (83. 1% ). The efficiency rate of conventional treatment was 76.4% , while the efficiency rate of triple four-procedure treatment was 89. 8% (P<0.05). Of them, 139 patients (17. 7% ) died in the hospital. Among them,. 26 patients died within 24 hours (18.4% ) and 47 cases died within 48 hours (33. 8% ) and 66 patients died within 72 hours (47.2% ). There were 92 patients who refused treatments after diagnosis, and among them, 81 patients died within 72 hours (88.04% ). The difference in mortality rate between two groups was significant (P<0.05). Conclusions The diagnosis of aortic dissection depends on detailed history, physical examination and CT or MRI imaging. Analgesia, sedation and control of blood pressure are essential for emergency treatments. Early diagnosis and effective emergency treatments are the critical strategy for the early surgical intervention and time window for further treatment to improve the survival rate of AD.
Acute Myocardial Ischemia in Patients Receiving Surgery for Acute Type A Aortic Dissection

Abstract:Purpose: Acute type A aortic dissection (ATAAD) is a dissection involving the ascending aorta within 14 days of symptom onset. Acute myocardial ischemia is associated with short-term mortality in patients with ATAAD. However, its relevance to ATAAD’s surgical timing and prognostic implications is unknown. Methods: This retrospective study enrolled 277 patients at our tertiary center between March 1997 and January 2019 and investigated their admission electrocardiograms. Twenty-one patients with left ventricular hypertrophy or left bundle branch block were excluded, and the records of 256 patients eventually underwent data collection, review, and multivariate analysis. Results: In patients with acute myocardial ischemia, the incidence of aortic root involvement, acute coronary involvement, and preoperative stroke was significantly higher. Acute myocardial ischemia was unassociated with 30-day or in-hospital mortality in patients who underwent surgery within 7 h of symptom onset. However, it was independently associated with the 30-day mortality and postoperative stroke when the 7-hour window had elapsed. Conclusions: As a time-dependent surgical prognosticator in ATAAD, acute myocardial ischemia was unassociated with short-term mortality in patients surgically operated within 7 h of symptom onset. Conversely, it was an independent factor for the 30-day mortality and postoperative stroke when the 7-hour window had elapsed.

Abstract:Background: Acute coronary involvement (ACI) due to acute aortic dissection (AAD) type A is potentially fatal. We examined selected patients with AAD type A, which had evolved over 14 years, and acute coronary involvement. The purpose of this study was to determine the characteristics of patients with ACI due to AAD type A.Methods: Between 1997 and 2011, we recruited 20 patients (14.1%) with ACI (14 men, 6 women; mean age: 51.8 +/- 11.8 years; age range: 35-79 years) from 142 patients who had undergone surgical repair of AAD type A.Results: We propose a novel 4-category classification scheme based on the surgical pathological findings. The right coronary artery was involved in 15 patients, and the left was involved in 5 patients. Fourteen patients had preoperative myocardial ischemia. In the other 6 patients, acute coronary involvement was found intra-operatively. Patients with ACI were significantly younger than those without ACI (51.8 +/- 11.8 vs. 61.0 +/- 11.8; p = 0.001), a lower prevalence of intramural hematoma (5.0% vs. 32.8%; p = 0.011), a higher aortic regurgitation rate (95.0% vs. 53.5%; p = 0.001). Patients presenting with ACI had an in-hospital mortality rate of 20.0% (4/ 20), while those without ACI had an in-hospital mortality rate of 19.7% (24/ 122).Conclusions: Acute coronary involvement due to AAD type A is not always associated with coronary malperfusion. Patients with ACI were much younger, had a higher aortic regurgitation rate, and, less commonly, had intramural hematoma. This new classification scheme would make it more convenient for surgeons to decide on treatment options for this special cohort. (C) 2013 Elsevier Ireland Ltd. All rights reserved.

Abstract:Objective To summarize the treatment experience and strategies of patients with acute type A aortic dissection involving coronary arteries in order to improve the efficacy of such patients.Methods Between March 2013 and April 2016,we recruited 37 patients with coronary involvement caused by acute type A aortic dissection,26 men,11 women;mean age (49.7 ± 10.4) years.All procedures were done on an emergency basis within 24 hours after the patient's arrival.Results Acute type A aortic dissection with coronary involvement is a more complex operation associated with high in-hospital mortality(18.9％,7/37) and low short-term survival(64.9％,13/37).There were 9 patients underwent coronary artery bypass graft after completion of the root procedure because of ventricular wall motion abnormality(7 patients) and new ST-segment elevation (2 patients) during weaning from cardiopulmonary bypass.Four of them were survival during follow-up due to the success from rescue coronary artery bypass graft.Conclusion Acute type A aortic dissection with coronary involvement is associated with high in-hospital mortality and low short-term survival.If patients suffered abnormal ventricular wall motion or new ST-segment elevation during weaning from cardiopulmonary bypass,rescue coronary artery bypass graft is essential to salvage these critically ill patients.

Abstract:Objective:To investigate the clinical manifestations,emergency charactoeristics of early diagnosis and treatment and misdiagnosed condition of aortic dissection(AD).Methods:The clinical data in 16 patients with AD in our hospital were summarized retrospectively.Results:The symptoms and physical signs of AD were different,the typical tearing pain was found in 11 cases accounting for 68.8%,while the complications were found as the initial symptom in some patients.The final diagnosis could be confirmed by CT or MRI,and its misdiagnosis rate was 56.2%.Conclusion:The symptoms and physical signs of AD are complex.This disease might be misdiagnosed because of less knowledge about AD in clinical physicians.It is urgent matter to increase the diagnostic consciousness and diagnosis level of the physician of emergency.

Abstract:Objective To investigate how to increase the early diagnosis rate of aortic dissection and decrease its mortality.Methods The pathogenesis and possible precipitating factors,clinical manifestations,accessory examinations,treatments and outcomes of 29 patients with established aortic dissection from January,2000 to december,2004 were analyzed retrospectively.Results In all 29 patients with aortic dissection,8 cases were misdiagnosed at the time of admission,24 cases felt chest discomfort,23 cases were accompanied by hypertension,19 cases received UCG and had typical finding.The sensitivity of MRI and Spiral CT were 100%,10 patients died in hospital.Using Sodium Nitroprusside and Betaloc earlier could stablise and stop aortic dissection separating.The death rate of DeBakeyⅠ and Ⅱ were higher.Conclusion Patient with acute chest pain especially accompanied by diastolic murmur in aortic area should be considered suffering from aortic dissection.MRI,UCG,Spiral CT have great signficance in diagnosing aortic dissection.Synthetical non-invasion imageology examination can take place of the invasional examination.Controlling hypertension and intervention or surgical treatment can reduce its mortality.

Abstract:Aortic dissection is a highly fatal disease with limited predictability requiring emergency response. It remains a challenging clinical problem and has a reported lower 5-year survival rate, especially in acute cases. Studying the epidemiology of aortic dissection can be important for targeting key populations and developing public health policies. Past studies have focused more on the in-hospital and follow-up mortality associated with aortic dissection but the global epidemiology review is still lacking. Incidence data have rarely been generated or provided. We estimated and analyzed the incidence of aortic dissection in all 195 countries and 54 regions worldwide and in the population structures of 15 selected countries. We further reviewed risk factors and baseline characteristics related to aortic dissection. We outlined the topic in terms of the biological, social, environmental, and psychosocial factors. Public health departments should screen target groups and key regions and introduce policies for disease prevention and relieve the high medical burdens.

Abstract:Aortic dissection is a critical condition, which can affect the aorta over its entire length. Characterized by a split between the tunica intima and the tunica media of the aorta, aortic dissection is seen in both sexes, and diagnosis and treatment are decisive for the survival of the patient. Aortic dissection has a wide range of risk factors that contribute to it being one of the most serious of cardiovascular conditions. Diagnosis is based on a thorough knowledge of the condition and its risk factors. This review describes the patient characteristics, drugs, and medical procedures that may contribute to an increased risk of aortic dissection, raising awareness of this life-threatening condition.

Abstract:Aortic dissection is an uncommon but potentially fatal disease with catastrophic complications. A high level of suspicion is required for successful diagnosis as presenting symptoms are so variable that dissection may be overlooked in up to 39% of cases. It most commonly presents in the elderly population with a history of chronic hypertension. Rapid intervention is necessary as delay leads to higher mortality. Despite advances in diagnostic and therapeutic techniques, morbidity and mortality remains high. Advances in diagnostic imaging have raised the awareness of variants of aortic dissection, including intramural hemorrhage and penetrating aortic ulcer. This distinction is important as the clinical course of these variants differs from that of classical aortic dissection, and thus treatment may also differ. Understanding of these variants has also led to the recognition of markers that may help predict progression to classical aortic dissection and thus warrant closer vigilance in selected patient populations. The recognition that rapid diagnosis is required for management of aortic dissection has led to the investigation of serum tests as diagnostic aids. Serum smooth muscle myosin heavy chain, d-dimer, and serum soluble elastin fragments are promising tests that may help raise suspicion for the diagnosis of acute aortic dissection. The high mortality associated with surgical therapy has led to investigation of alternative approaches. Endovascular therapy has emerged as a viable option in patients with type B dissection who are too unstable for surgery. However, long-term follow up is required to validate this procedure.

Abstract:OBJECTIVE:To summarize the diagnostic and therapeutic methods of asymptomatic aortic dissection (AD).METHODS:Ten patients of asymptomatic AD were treated from January 2002 to June 2006. The patients were followed up and their data were reviewed retrospectively.RESULTS:Two patients of type A AD treated by open surgery and the 6 patients of type B AD received endovascular stent-graft repair were successfully treated, and no in-hospital death and severe complication occurred. Furthermore, all of them survived during follow-up. One of the 2 patients of type B AD received medical treatment still survive, but the other suddenly died during follow-up. The most possible cause of his death was fatal aorta rupture.CONCLUSIONS:The diagnosis of asymptomatic AD could be missed or mistaken easily because of its asymptomatic. Keeping a high clinical index of suspicion is crucial in establishing the diagnosis of asymptomatic AD. The asymptomatic AD also has high risk of fatal aorta rupture associated with re-dissection and aneurysmal dilatation. Therefore, type A and B asymptomatic AD should be treated by open surgery or endovascular repair respectively if possible. Endovascular stent-graft repair is a safe and effective method for the treatment of type B asymptomatic AD.

Abstract:Acute aortic syndromes constitute a spectrum of conditions characterized by disruptions in the integrity of the aortic wall that may lead to potentially catastrophic outcomes. They include classic aortic dissection, intramural hematoma, and penetrating aortic ulcer. Although imaging studies are sensitive and specific, timely diagnosis can be delayed because of variability in presenting symptoms and the relatively low frequency with which acute aortic syndromes are seen in the emergency setting. Traditional classification systems, such as the Stanford system, facilitate early treatment decision-making through recognition of the high risk of death and major complications associated with involvement of the ascending aorta (type A). These patients are treated surgically unless intractable and severe co-morbidities are present. Outcomes with dissections that do not involve the ascending aorta (type B) depend on the presence of acute complications (e.g., malperfusion, early aneurysm formation, leakage), the patency and size of the false lumen, and patient co-morbidities. Patients with uncomplicated type B dissections are initially treated medically. Endovascular techniques have emerged as an alternative to surgery for the management of complicated type B dissections when intervention is necessary. Patients with acute aortic syndromes require aggressive medical care, risk stratification for additional complications and targeted genetic assessment as well as careful long-term monitoring to assess for evolving complications. The optimal care of patients with acute aortic syndrome requires the cooperation of members of an experienced multidisciplinary team both in the acute and chronic setting.

Abstract:Thoracic aortic laceration and traumatic aortic dissection are highly lethal injuries due to decelerate trauma after traffic accident or falling, which often associated with other organ injury. Successful management of thoracic aortic laceration and traumatic aortic dissection depends on early diagnosis and appropriate treatment. Management of other injured organs at the same time is mandatory. Endovascular repair has its special advantages in the treatment of aortic laceration and traumatic aortic dissection. The short to middle term follow up results were satisfied. Long term follow up results are needed to assess the overall efficacy of the method.
