Abstract:Aortic dissection is a life-threatening emergency disease, which needs immediate and effective clinical interventions. In recent years, owing to the widespread use of multidetector computed tomography, the rate of diagnosis of aortic dissection has significantly improved. As a convenient and noninvasive examination, multidetector computed tomography angiography has been used widely in the preoperative diagnosis of aortic dissection and postoperative follow-up. Improving CT scan protocols and powerful post-processing function make the diagnosis of aortic dissection more accurate and provide adequate evidences for clinical treatments. This review summarized the recent application and research progress of multidetector computed tomography angiography in the diagnosis of aortic dissection and its future perspective.

Abstract:Aortic dissection (AD) is a critical cardiovascular disease due to the separation of media and adventitia caused by the rupture of vascular wall intima. The disease has a high mortality rate of about 1% to 3% for each additional hour, since the adventitia of the aorta can rupture and bleed to death at any time. Although great progress has been made in clinical treatment of aortic dissection, and the mortality rate has been significantly reduced, the pathogenesis is still not very clear. At present, related studies have confirmed that inflammation of aortic wall promotes the occurrence and development of AD. Although the mechanism of aortic dissection is more complicated, some studies have shown that the infiltration of monocytes/macrophages into the aortic wall is the main pathogenic mechanism of the disease. This review introduces the latest research results on the mechanism of macrophage infiltration and plasticity in aortic dissection.

Aortic Dissection: an Analysis of 80 Cases

Abstract:Objective To investigate the clinical manifestation,diagnosis and treatment of aortic dissection.Methods According to the clinical data of continuous 80 patients with aortic dissection between August 1999 and April 2006,it analyzed the clinical characteristics,imaging diagnosis,treatment methods and outcomes.Results Twenty-three patients were treated by traditional open operations,of which 3 patients(13.0%)died of postoperative complications.Fifty-two patients were performed endoluminal stent-graft implantation,and one patients(1.9%)died 1 day after the procedure due to false luminal rupture.No death occurred during follow up time.Additionally,3 patients(3.8%)died before operation,and 2 gave up treatment.Conclusion The accurate diagnosis as early as possible and active therapy are the key to improve the prognosis of aortic dissection.

Abstract:The incidence rate of aortic dissection (AD) is low, but it is highly fatal in the acute phase. Miss diagnosis and misdiagnosis can occur occasionally, resulting the miss of the best intervention time. Research on the epidemiological characteristics and of AD and related risk factors can identify high-risk groups, make screening and diagnosis as soon as possible and effectively control the changeable risk factors to reduce the incidence of AD and improve the outcome of AD cases. This paper summarizes the progress in research of the epidemiological characteristics of AD and related risk factors in order to promote early prevention and diagnosis of AD, improve the AD case management and intervention level.

Abstract:Thoracic aortic dissection was a catastrophic disease with change quickly,and the characteristic of thoracic aortic dissection was acute onset,complex,progress rapidly,difficult to diagnosis,and highly misdiagnosis rate.So the fatality rate of thoracic aortic dissection was high,which easy caused medical disputes.Even so,the cause of the formation of thoracic aortic dissection in detail is not clear,and many risk factors associated with the occurrence of thoracic aortic dissection.These including high blood pressure,gender (male),aortic normal aging,drug using,atherosclerosis,genetic disease and inflammatory diseases,etc.

Abstract:Objective The National Natural Science Foundation of China (NSFC) has made great progress in promoting the development of aortic dissection research in recent years. This study aimed to examine the development and research status of aortic dissection research in China so as to provide references for future research. Methods The NSFC projects data from 2008 to 2019 were collected from the Internet-based Science Information System and other websites utilized as search engines. The publications and citations were retrieved by Google Scholar, and the impact factors were checked by the InCite Journal Citation Reports database. The investigator's degree and department were identified from the institutional faculty profiles. Results A total of 250 grant funds totaling 124.3 million Yuan and resulting in 747 publications were analyzed. The funds in economically developed and densely populated areas were more than those in underdeveloped and sparsely populated areas. There was no significant difference in the amount of funding per grant between different departments' investigators. However, the funding output ratios of the grants for cardiologists were higher than those for basic science investigators. The amount of funding for clinical researchers and basic scientific researchers in aortic dissection was also similar. Clinical researchers were better in terms of the funding output ratio. Conclusion These results suggest that the medical and scientific research level of aortic dissection in China has been greatly improved. However, there are still some problems that urgently need to be solved, such as the unreasonable regional allocation of medical and scientific research resources, and the slow transition from basic science to clinical practice.

Abstract:BACKGROUND: We aimed to comprehensively analyze all the literature related to aortic dissection (AD) in the past decade using Web Scrapping technology from PubMed, revealing the research dynamics in this field.METHODS: Data were retrieved and downloaded from PubMed with search strategy as "(aortic dissection [Title/Abstract]) AND (2010[EDAT]: 2020[EDAT])". Information on the PMID, journal name, title, number of citations, publication year, authors, affiliations, abstract, study type, and keywords of the research was recorded.RESULTS: A total of 7,470 publications were identified. Most of the articles were published in J Thorac Cardiovasc Surg; Japan was the country with the largest publications number; the USA was far ahead of other countries regarding the highly cited studies; Yale University and Baylor College of Medicine took the first place for publishing most of the highly cited articles; the most frequently cited article is the 2014 ESC Guidelines on the diagnosis and treatment of aortic diseases; most of the clinical trials were published on J Vasc Surg; John A. Elefteriades ranked first by cumulative publication numbers; Christoph A. Nienaber took the lead by both cumulative citations and impact factors; Dianna M. Milewicz was the only female researcher on all the three ranking lists; the most common keywords in aortic dissection were Treatment Outcome and Retrospective Studies.CONCLUSIONS: This study provides interesting insights into the AD scientific landscape in recent 10 years and generates some objective evidence for comprehensive understanding and evaluation of this field. This investigation may ultimately inform managers, researchers and policymakers.

Abstract:Aortic aneurysm and dissection (AAD) is a life-threatening medical condition associated with high morbidity and mortality rates. Important mechanisms underlying AAD are the dysregulation of vascular homeostasis and adverse remodeling. Vascular homeostasis maintains normal physiological function. Various physical, chemical, biological, and other internal or external environmental changes dysregulate vascular homeostasis, leading to vascular degeneration and aggravated aortic injury. This process is dependent on the communication between homeostatic mechanisms and the extracellular environment, such as local inflammatory cytokines, vasoactive substances, and hemodynamics. In this article, we summarize recent reports by Chinese researchers who studied the pathogenic mechanisms of AAD mainly from the perspective of communication of the extracellular environment with vascular homeostasis and improving diagnostic methods and therapeutic options for patients with AAD. This review aims to provide a roadmap for AAD that encompasses its pathogenesis and clinical aspects. We hope to facilitate future studies on the development of effective treatments and preventive therapies, and thus improve patient outcomes.

Abstract:Aortic dissection is an age-dependent life-threatening cardiovascular disease with high mortality rates. Recent research has shown that inflammation plays an important role in aortic dissection. Inflammatory cells, such as lymphocytes and macrophages, not only increase the expression of proteases and cell adhesion molecules but also release reactive oxygen species. These cells also contribute to apoptosis of smooth muscle cells in the aortic artery, and finally lead to medial degradation. This process has been considered to be the principal mechanism for aortic dissection. Furthermore, changes in systemic inflammatory biomarkers are associated with acute-phase reactions and complications in aortic dissection. These biomarkers are also used to predict the prognosis of aortic dissection and to distinguish acute aortic dissection from other chest pain diseases. In addition, elevated inflammatory cell activity in aortic wall, identified by positron emission tomography/computed tomography, is associated with serious clinical symptoms and leads to a poor clinical outcome in aortic dissection. Such observations may provide us with new sights into the mechanism of aortic dissection. In this review, we discuss the role of inflammation in the development and progression of aortic dissection. Understanding this inflammatory process may provide us with new strategies for pharmacological treatment of aortic dissection.

Abstract:Aortic dissection is a critical vascular disease that is characterized by a high mortality rate and inflammation significantly influences its onset and progression. Recent studies highlight the integral role of macrophages, key players in the immune system, in the pathological landscape of aortic dissection. These cells are involved in crucial processes, such as the remodeling of the extracellular matrix, immunocyte infiltration, and phenotypic switching of smooth muscle cells, which are essential for the structural integrity and functional dynamics of the aortic wall. Despite these insights, the specific contributions of macrophages to the development and progression of aortic dissection remains unclear. This review explores the pathogenesis of aortic dissection with a focus on macrophages and describes their origins, phenotypic variations, and potential roles based on the most recent research findings. Furthermore, we discuss key molecules related to macrophages during aortic dissection, their interactions with other cellular components within the aorta, and the implications of these interactions for future therapeutic strategies. This comprehensive analysis aimed to improve our understanding of macrophages in aortic dissection and promote the development of targeted interventions.

Abstract:Aortic dissection is a highly fatal disease with limited predictability requiring emergency response. It remains a challenging clinical problem and has a reported lower 5-year survival rate, especially in acute cases. Studying the epidemiology of aortic dissection can be important for targeting key populations and developing public health policies. Past studies have focused more on the in-hospital and follow-up mortality associated with aortic dissection but the global epidemiology review is still lacking. Incidence data have rarely been generated or provided. We estimated and analyzed the incidence of aortic dissection in all 195 countries and 54 regions worldwide and in the population structures of 15 selected countries. We further reviewed risk factors and baseline characteristics related to aortic dissection. We outlined the topic in terms of the biological, social, environmental, and psychosocial factors. Public health departments should screen target groups and key regions and introduce policies for disease prevention and relieve the high medical burdens.

Abstract:Aortic dissection (AD) is a cardiovascular disease that seriously endangers the lives of patients. The mortality rate of this disease is high, and the incidence is increasing annually, but the pathogenesis of AD is complicated. In recent years, an increasing number of studies have shown that immune cell infiltration in the media and adventitia of the aorta is a novel hallmark of AD. These cells contribute to changes in the immune microenvironment, which can affect their own metabolism and that of parenchymal cells in the aortic wall, which are essential factors that induce degeneration and remodeling of the vascular wall and play important roles in the formation and development of AD. Accordingly, this review focuses on the independent and interactive roles of immunity and metabolism in AD to provide further insights into the pathogenesis, novel ideas for diagnosis and new strategies for treatment or early prevention of AD.

Abstract:Thoracic aortic dissection is a dangerous pathological condition where blood intrudes into the vascular wall, creating an artificial channel, the false lumen. The weakened false lumen wall may rupture, resulting in high mortality. Biomechanical factors investigated are the size of the dissecting aneurysm, the blood pressure, the geometry around the distal tear, a configuration of partial thrombosis, the distance between the tears, and the shear stress. The force acting on the walls of the false lumen will generally become larger with increasing size of the dissection or a higher blood pressure. The effects of shear stress, in terms of the ‘average wall shear stress’ and ‘oscillatory shear index’, are elucidated, with low shear stress reversing in direction over time implying damage to the blood vessel. Clinicians can thus assess the danger of imminent rupture quantitatively. This information will help in the decision regarding surgical interventions.

Abstract:Objectives: The acute phase of aortic dissection is very dangerous. The identification of risk factors for disease is beneficial to the early diagnosis and treatment of dissection. The aim of this study was to explore the relationship between the different branches of aortic arch and the occurrence of aortic dissection. Methods: A total of 80 patients with Stanford B aortic dissection treated in our hospital were collected. Computed tomography angiography was performed in all the patients. The patients who were excluded from aortic dissection were selected as the control group. The data of the two groups were treated with three dimensional reconstruction of the post processing workstation. The related data of the branches of the aortic arch were measured and compared. Results: Compared with control group, the diameters of brachiocephalic trunk (BCT), left common carotid artery (LCA) and left subclavian artery (LSA) in aortic dissection patients were 16%, 25% and 20% higher. The straight distances from LCA and LSA to the aortic root were 17% and 21% longer. The angles between BCT, LCA, LSA and the long axis of aortic arch were 6%, 7% and 11% larger. There was no significant difference of adjacent distance branches of the aortic arch. Conclusion: Aortic arch morphology is related to the occurrence of Stanford B aortic dissection.

Abstract:Aortic aneurysm and aortic dissection (AA/AD) are critical aortic diseases with a hidden onset and sudden rupture, usually resulting in an inevitable death. Several pro- and anti-angiogenic factors that induce new capillary formation in the existing blood vessels regulate angiogenesis. In addition, aortic disease mainly manifests as the proliferation and migration of endothelial cells of the adventitia vasa vasorum. An increasing number of studies have shown that angiogenesis is a characteristic change that may promote AA/AD occurrence, progression, and rupture. Furthermore, neocapillaries are leaky and highly susceptible to injury by cytotoxic agents, which promote extracellular matrix remodeling, facilitate inflammatory cell infiltration, and release coagulation factors and proteases within the wall. Mechanistically, inflammation, hypoxia, and angiogenic factor signaling play important roles in angiogenesis in AA/AD under the complex interaction of multiple cell types, such as smooth muscle cells, fibroblasts, macrophages, mast cells, and neutrophils. Therefore, based on current evidence, this review aims to discuss the manifestation, pathological role, and underlying mechanisms of angiogenesis involved in AA/AD, providing insights into the prevention and treatment of AA/AD.

Abstract:Aortic dissection (AD) is a fatal disease that accounts for a large proportion of aortic-related deaths and has an incidence of about 3–4 per 100,000 individuals every year. Recent studies have found that inflammation plays an important role in the development of AD, and that macrophages are the hub of inflammation in the aortic wall. Aortic samples from AD patients reveal a large amount of macrophage infiltration. The sites of macrophage infiltration and activity vary throughout the different stages of AD, with involvement even in the tissue repair phase of AD. Angiotensin II has been shown to be an important factor in the stimulation of macrophage activity. Stimulated macrophages can secrete metalloproteinases, inflammatory factors and other substances to cause matrix destruction, smooth muscle cell apoptosis, neovascularization and more, all of which destroy the aortic wall structure. At the same time, there are a number of factors that regulate macrophages to reduce the formation of AD and induce the repair of torn aortic tissues. The aim of this review is to take a close look at the roles of macrophages throughout the course of AD disease.

Abstract:Aortic dissection, characterized by a high immediate mortality, is primarily caused by excessive bleeding within the walls of the aorta or a severe tear within the intimal layer of the aorta. Inflammation, as well as oxidative stress and the degradation of extracellular matrix (ECM), are significant factors in the development and occurrence of aortic dissection. Matrix metalloproteinases (MMPs) are pivotal enzymes responsible for degrading the ECM. Inflammatory factors and oxidants can interact with MMPs, indicating the potential significance of MMPs in aortic dissection. A substantial body of evidence indicates that numerous MMPs are significantly upregulated in aortic dissection, playing a critical role in ECM degradation and the pathogenesis of aortic dissection. Furthermore, targeting these enzymes has demonstrated potential in facilitating ECM restoration and reducing the incidence of aortic dissection. This review initially provides a brief overview of MMP biology before delving into their expression patterns, regulatory mechanisms, and therapeutic applications in aortic dissection. A profound comprehension of the catabolic pathways associated with aortic dissection is imperative for the future development of potential preventive or therapeutic bio-interventions for aortic dissection.

Abstract:Acute aortic dissection is the most lethal one among the diseases involving the aorta.This article reviewed the operative indications,methods,curative effects and clinical prospect of Endovascular Graft Exclusion (EVGE) for aortic dissection .The indications for EVGE are DeBakeyⅢB type aortic dissections with the intimal tear. EVGE is a minus invasive method with a firmly curative effects, and its clinical prospect will be very bright.

(2) A bibliometric analysis was performed to screen the molecular drivers in aortic dissection. (3) The top five genes related to aortic dissection are transforming growth factor‐β‐related genes, elastin, fibrillin‐1, angiotensinogen and metalloproteinase 9. Background Aortic dissection (AD) is a lethal cardiovascular emergency involving high mortality and disability. However, its specific pathogenesis remains to be elucidated. Methods A bibliometric analysis based on the Web of Science database, VOSviewer software and Citex platforms was conducted to have a knowledge of the development trends, frontiers and hot spots of AD. Subsequently, the top five AD‐related genes from the titles and abstracts of published literature were searched. Lastly, the roles of the top five genes and their encoded proteins in the onset of AD were reviewed. Results The bibliometrics showed that most studies are exploring the molecular drivers related to AD, especially gene mutations. The top five AD‐related genes were transforming growth factor‐β (TGFB)‐related genes, elastin (ELN), fibrillin‐1 (FBN1), angiotensinogen (AGT) and matrix metalloproteinase 9 (MMP9). In particular, regulation of the structure of elastic fiber by TGFB‐related genes, ELN and FBN1, appears to be the principal mechanism contributing to AD onset. Activation of the renin‐angiotensin system is the principal mechanism by which AGT triggers AD. MMP9 promotes the formation and development of AD by degrading extracellular matrix components. Conclusion TGFB, ELN, FBN1, AGT and MMP9 are the five top molecular drivers of AD, providing a comprehensive mechanistic insight into AD.
