Questionnaire in patients and doctors with ICD indication

Abstract:Objective Aim to understand the extraordinary low application of implantable cardioverter defibrillator(ICD) in China. Methods patients with ICD indication,their families and the attending doctors in Zhongshan Hospital of Fudan University were investigated by questionnaire from the period of June ,2007 to Janu-ary 2008. In addition, the basic information, examination results and treatment state were collected. Results There were 122 patients enrolled in this study,including 89 patients(73.0%)with the indication of primary pre-vention for SCD,which is 2.7 times compared with the patients of the secondary prevention. Non-ischemic heart diseases are the most common underlying cause in both primary and secondary SCD prevention. 42.4% of sec-ondary prevention patients accepted ICD/CRT-D implantation,while only 2.2% of primary prevention patients re-ceived device-based therapy. Age and education has no effect on the attitude of accepting ICD treatment. In the aspect of the recognition of suffered heart diseases, sudden cardiac death (SCD)and the acceptance of ICD,patients of secondary prevention are much better than the primary prevention ones(P<0.001). When come along with mak-ing the decision of whether accepted ICD implantation, the suggestion of physicians plays the most important role. The state of illness don't need ICD treatment other than economic issue is the main reason of the patients rejecting ICD implantation. The most physicians(93.3%)never introduced ICD to their primary prevention patients due to the same reason,which is much higher(21.1%)than the secondary prevention(P<0.001). Conclusions The clinical applications of ICD are far below its indication range at present,especial in the primary prevention. The lack of un-derstanding SCD and ICD in both physician and patient is the main reason for the low implantation of ICD in China.
