A retrospective study of atrial fibrillation after DDD pacemaker implantation

Abstract:Objective To investigate the clinical predictors of the occurrence of atrial fibrillationafter the implantation of DDD mode pacemaker.Methods One hundred and eighten patients were enrolled(age 69.5 ±13.0 years,59% female) who received DDD pacemakers with features that permitted detectionand storage of information the date, duration, and sequential episodes of AF.Time of the first occurrence ofAF was determined at follow-up visit.Other data such as age, gender, structural heart disease, concomitantnon-cardiac diseases, indication of pacing, the cumulative percentage of atrial pacing (Cum%AP) andventricular pacing(Cum%VP) was collected.Results Atrial fibrillation developed frequently after thedual-chamber pacemaker implantation.48% of the patients developed AF during follow up.A prior historyof AF was an independent predictor of AF occurrence (OR 4.116,95%CI 2.123-7.998,P =0.001).AFoccurred more often in patients with sinus node disease(SND) than those with atrioventricular block(AVB )(OR 2.553,95% CI 1.122-5.814,P =0.045).Patients treated with Calcium channel blocker (CCB) had alower incidence of AF (OR 0.278,95% CI 0.118-0.658,P =0.004).Conclusions AF developedfrequently after dual-chamber pacemaker implantation.Patients who had a prior AF history were more likelyto develop AF after the implantation of dual -chamber pacemaker.Patients with sinus node dysfunction morelikely developed AF compared to the patients with AVB.It seemed that ventricular pacing was not associationwith occurrence of AF.
