Analysis of frequency and predictors of left ventricular reverse remodeling in patients with peripartum cardiomyopathy

Abstract:Objective To investigate the frequency of left ventricular reverse remodeling (LVRR) in a cohort of Chinese patients with peripartum cardiomyopathy (PPCM) from a single center and to identify some baseline predictors. Methods Patients hospitalized in Department of Cardiology and Gynecology in Anzhen Hospital between June 2006 and June 2015 with clinical diagnosis of PPCM and left ventricular ejection fraction (LVEF)<45% and left ventricular end-diastolic diameter (LVEDD)>50 mm detected by echocardiography on admission were enrolled. All patients were followed up at least 6 months with repeat echocardiogram after discharge to determine the frequency of LVRR, defined as an absolute increase in LVEF at least 10%to a final level of≥50%and an absolute decrease in LVEDD at least 5 mm to a final level of≤50 mm. Multivariate logistic regression analysis was used to identify baseline predictors and&nbsp;the prognostic value was evaluated by calculating the area under the curve (AUC) of the receiver operating characteristic curve (ROC). Results A total of 68 patients with PPCM were enrolled in this analysis. After a median follow-up period of 38 months (interquartile range, IQR 17-61 months), 35 patients (51.5%) achieved LVRR, showing a significant increase in LVEF from (34.8 ± 8.2)% at baseline to (58.3 ± 4.9)% at initial recovery accompanied by a significant decrease in LVEDD from (58.4 ± 4.3) mm to (46.8 ± 2.9) mm (both P<0.01). Overall, the median recovery time for the 35 patients with LVRR was 27 (IQR 7-39) months (range 1-59 months), of whom only 12 patients (34.3%) achieved LVRR within 12 months. Multivariate logistic analysis showed that LVEF [odds ratio (OR) 1.098, P<0.01], LVEDD (OR 0.927, P<0.01) on admission and use of angiotensin converter enzyme inhibitor (ACEI) or angiotensin receptor blocker (ARB) (OR 9.924, P<0.01) at discharge were independent predictors of LVRR. ROC analysis showed that the AUC for LVEF and LVEDD on admission to predict LVRR on follow-up was 0.709 (cut-off value of LVEF>35%with sensitivity of 51.4% and specificity of 84.8%) and 0.787 (cut-off value of LVEDD<65 mm with sensitivity of 88.6% and specificity of 60.0%). There was a higher frequency of LVRR in patients with LVEF>35% compared to those with LVEF ≤35% (78.3% vs 37.8%, P<0.01). Similarly, patients with LVEDD<65 mm had a higher frequency of LVRR than those with LVEDD ≥65 mm (70.5% vs 16.7%, P<0.01). Conclusion More than half of the patients hospitalized with PPCM have achieved LVRR after a median follow-up of about 3 years, of whom only one third occurred within 12 months after discharge. A higher LVEF (>35%), a smaller LVEDD (<65 mm) on admission and use of ACEI or ARB at discharge were independent predictors of LVRR.
