Abstract:【Objectives】 To investigate the characteristics and time trends of clinical data of patients with gastric carcinoma in our hospital over the past ten years.To compare the demographic,clinical and pathologic features of young patients with older patients.To compare the demographic,clinical and pathologic features of cardia carcinoma patients with those of non-cardia carcinoma.【Methods】 656 patients with gastric carcinoma in our hospital from 1999.1～2009.1 had been included: ①The common information,pathologic type,location and TNM stage were investigated;②The gender,pathologic type,location,TNM stage and family history of young patients were compared with those of old patients;③The mean age,gender,pathologic type,location,TNM stage and family history of cardia carcinoma were compared with those of non-cardia carcinoma.【Results】 Of the 656 patients,464(70.7%) were male and 192(29.3%) were female,with a mean age of 62.1±12.9 years old(median age: 64.5 years old).Gastric carcinoma more often arised in gastric antrum,acounting for 46.3%(304 cases);and the incidence of gastric cardia carcinoma did not increased.Patients of the early stage accounted for 12.3%(81 cases) and the advanced stage account for 87.7%(575 cases).Regarding TNM stage,stage III(33.0%) and IV(34.8%) were in the majority.From 34 years old,the percentage of patients increased gradually and the peak age was 64～74 years old.The mean age of patients diagnosed from 1999 to 2003 was(61.1±13.7) years old and that of patients diagnosed from 2004 to 2008 was(63.1±11.9) years old.There was a statistic difference.②The male-to-female ratio of young patients(≤40 years old) was 1∶1 and that of old patients(≥60 years old) was 3∶1,the difference was significant.The high malignant group(low-differentiation adenocarcinoma and signet-ring cell carcinoma) accounted for 97.7% in young patients and 71.6% in old patients,the difference was significant.There were no significant differences between young and old patients in location,TNM stage or family history.③There was a significant difference between cardia carcinoma and non-cardia carcinoma in mean age(65.6±11.1 vs 61.8±12.8 years old).Gastric cardia carcinoma had a higher male-to-female ratio than non-cardia carcinoma and the difference was significant(5.4 vs 2.3).Regarding TNM stage,there was a significant difference between the two groups in stage IV(52.6% in cardia carcinoma vs 29.2% in non-cardia carcinoma).There were no significant differences between the two groups in pathologic type or family history.【Conclusions】 ①Gastric carcinoma has a higher incidence in old male.Stage III,IV,low-differentiation carcinoma has higher probability as well as carcinoma in gastric antrum.The percentage of gastric cardia carcinoma did not increased in the past ten years.The mean age of patients diagnosed from 2004 to 2008 is older than that of patients diagnosed from 1999 to 2003.②Compared with old patients,there are more female and higher malignant carcinoma in young patients with gastric carcinoma.There are no significant differences between young and old patients in location,TNM stage or family history.③Compared with non-cardia carcinoma,cardia carcinoma has an older mean age and a higher male-to-female ratio.There are more stage IV patients in cardia carcinoma.There are no significant differences between the two groups in pathologic type or family history.
