Abstract:OBJECTIVE:To evaluate the long-term effects of retrograde liberated highly selective vagotomy (RLHSV) in the treatment of duodenal ulcer.METHODS:Seventy patients with duodenal ulcers complicated by stenosis, bleeding, or perforation were operated on by retrograde liberated highly selective vagotomy. Among these patients, 61 had perforated duodenal ulcers, 6 bleeding and 3 stenosis.RESULTS:Followed up for 30 to 120 months in 65 patients showed a recurrence rate 7.69% and no hemorrhage occurs. According to the modified Visick grading system, 56 patients (86.2%) were of Visick I, 4 (6.1%) of Visick II, 2 (3.0%) Visick III, 3 (4.6%) Visick IV, and (92.3%) Visick I or II.CONCLUSIONS:This modified procedure is rapid, easy and radical operation with excellent long-term results. It can be considered an effective alternative for the treatment of duodenal ulcer with complication.
Surgical Management and Outcomes of Duodenal Gastrointestinal Stromal Tumors.

Abstract:BACKGROUND AND STUDY AIMS:This retrospective study purports to examine these characteristics and compare the surgical procedures available and appropriate for the treatment of patients affected by duodenal GISTs.PATIENTS AND METHODS:A retrospective examination of reports and studies carried out between May 2012 and March 2017, and covering patients with primary GISTs of the duodenum was performed using modules from the SPSS package. Comparisons of treatment effects resulting from the administration of two differential methods of surgical treatment namely pancreaticoduodenectomy (PD), and limited resection (LR), were effected on the reports of the GIST patients thus selected.RESULTS:Out of these 62 patients who had undergone resection of duodenal GISTs, 47 (76%) had limited resection (LR) and 15 (24%) underwent pancreaticoduodenectomy (PD). In Multivariate analyses, tumor size was an independent predictive factor for recurrence (p=0.008). ASA, tumor size, and PD were independent and significant prognostic factors on OS (p=0.021, p=0.024, and p=0.030, respectively). In the very low and low risk group, and high-risk group, there were no significant differences in the RFS (recurrence-free survival) and OS (overall survival) between the LR and PD groups.CONCLUSIONS:When technically feasible, LR should be given due consideration as a reliable and curative option for duodenal GISTs achieving satisfactory RFS and OS.

Abstract:Background Uncontrolled ulcer bleeding of duodenal ulcer (DU) after endoscopic therapy often needs surgery. At present, cutting the bottom of the ulcer with ligation and performing its excision-lesion are the common ways to achieve immediate efficacy in stopping bleeding. For the problem of its great trauma, we seek an easy and useful technical method to reach the same therapeutic effect to stop acute bleeding. Methods We determined the distribution of the lesion and its innervated blood vessels under the guidance of the endoscopy and then performed suture and hemostasis on the external surface of the stomach and duodenum. Results An immediate efficacy in stopping bleeding was shown and the hemoglobin (Hb) level returned to normal after operation with no recurrence of bleeding. Conclusion We created a successful and novel strategy for laparotomy-endoscopic assisted suture for DU emergency hemostasis without duodenectomy.

Abstract:To summarize clinical application and experience of abdominal retrograde gastric and jejunal feeding tube and study its clinical value.Methods We viewed 45 adult patients with esophageal carcinoma treated with two-incision esophagectomy from January 1st 2014 to December 31th 2016 in Department of Thoracic Surgery the First Affiliated Hospital of Zhejiang University.During the surgeries 31 patients were treated with nasogastric tube and jejunostomy tube control group and 14 patients were treated with retrograde gastric tube and jejunostomy tube abdominal retrograde gastric and jejunal feeding tube experimental group .All of them were diagnosed with esophageal squamous cancer according to the postoperative pathological results.Results Differences of general information between two groups including gender age body mass index smoking history preoperative albumin preoperative alanine aminotransferase preoperative aspartate aminotransferase preoperative hemoglobin location and pathological TNM stage were not statistically significant P＞0.05 .Operative time operative blood loss postoperative albumin postoperative hemoglobin duration of using gastric tube length of postoperative stay and complication rate between the two groups were not statistically different P＞0.05 . The experimental group had less postoperative drainage volume of gastric tube in 24h P<0.05 .Conclusion For resectable esophageal carcinoma abdominal retrograde gastric and jejunal feeding tube can provide effective gastrointestinal decompression and enteral nutrition while avoiding nasal irritation and inconvenience.So it deserves to be further studied and widely applied.
A Novel Self-Assembled Gel Protects Against Endoscopic Resection- Induced Colonic Ulcers: a Randomized Clinical Trial A Novel Gel That Could Promote Ulcer Healing

Abstract:Abstract Background: Although associated with high en bloc and complete resection rates, endoscopic resection, which is widely performed for gastrointestinal mucosal and submucosal lesions, may cause bleeding and perforation. However, novel agents are being introduced to minimize these complications. Methods: In this multicenter, randomized, single blind, controlled trial, participants who underwent colonic endoscopic resection across four hospitals in China were randomly assigned to the control or experimental group(1:1 ratio); the gel was used after colonic endoscopic resection in the latter. Participants were instructed to report adverse events on the first day, 14th day, and one-month post-intervention. Repeat endoscopy was performed at two-week follow-up to confirm resection site healing. Results: Of the 92 screened participants, 73 completed the study. Each ulcer bed was successfully coated with the gel over an 87.4±13.4% area within approximately three minutes in the experimental group. The calculated healing rates were significantly higher in the experimental than in the control group (84.7±10.8% vs. 72.6±16.0%, P=0.0004). No severe relative adverse event occurred during follow-up. Conclusions: This novel gel is a safe, effective, and convenient agent that promotes wound healing after colonic endoscopic resection. And we recommend application of this gel to protect the ulcer bed in daily clinical practice. Trial registration This trial has been registered in Chinese Clinical Trial Registry (ChiCTR2100053692).

Abstract:Objective:To investigate the effect of selective esophagogastric devascularization and splenectomy plus transection and reanastomosis of distal esophagus with a circular stapler on esophageal variceal bleeding due to portal hypertension.Methods:A retrospective analysis was made on the results of 18 patients with liver cirrhosis,portal hypertension,esophageal and gastric variceal bleeding who were treated with selective esophagogastric devascularization plus transection and reanastomosis of the distal esophagus with esophagus stapler in our department.Results:All the patients were followed up from 3 to 42 months.There was no operative mortality,no esophageal anastomotic leakage and no encephalopathy after operation.The rebleeding rate was 0%.Anastomotic stenosis took place in one case during the early postoperative period(5.6%)and meliorated after 3 months of operation.The varicose veins in 11 patients(61.1%)were eliminated after operation.Seven cases had slight varicose veins.The portal hypertensive gastropathy in 12 cases was improved significantly after operation.Conclusion:In treatment of esophageal and gastric variceal bleeding due to portal hypertension,the effect of selective esophagogastric devascularization plus transection and reanastomosis of distal esophagus with esophagus stapler is a satisfactory method with a low rebleeding and complication rate.

Abstract:To study the clinical effect of non-ampullary duodenal mucosal lesions treated by endoscopic resection. A retrospective analysis was performed on the data of 58 cases of duodenal non-ampullary mucosal lesions treated by endoscopic resection from January 2016 to June 2019 from 3 hospitals. Among 58 cases, 27 lesions (46.6%) were located in the duodenal bulb and 31 (53.4%) in the duodenal descending part (including the ball-drop boundary). Forty-six patients (79.3%) received endoscopic mucosal resection, 7 (12.1%) received endoscopic submucosal dissection and 5 (8.6%) received pre-cut endoscopic mucosal resection. Few postoperative complications were found except for 1 case of intraoperative bleeding, 1 case of delayed bleeding 2 days after surgery, 1 case of lesion residual and 2 cases of postoperative abdominal pain. No perforation occurred. Endoscopic treatment of non-ampullary duodenal mucosal lesions is safe and effective.
Prevention and treatment of duodenal stump leakage after radical gastrectomy for gastric cancer

Abstract:The occurrence of duodenal stump leakage after gastric cancer surgery is mainly related to its anatomical features, tumor invasion and metastasis. The duodenum wall is thin and fixed. It has poor blood supply,and massive corrosive fluid in it. Also,the duodenum is connected to a variety of important structures of the digestive system,and only the duodenal bulb is mobile,limiting the maneuverability of suture devices for duodenal stump closure. The duodenal bulb is the most common site of metastasis of lower part gastric cancer. When cancer spreads to the duodenal bulb,it will cause difficulty in the dissection and closure of the duodenal stump,leading to a higher risk of duodenal stump leakage. The clinical symptoms are related to the time of leakage occurring, the amount of leakage contents,the establishment of effective drainage,etc. For treatment,the key to the control of infection is effective drainage,and reasonable nutrition support is an essential mean to promote healing. The purpose of a salvage surgery is to place an exact drainage and establish a long-term nutritional supportive route. If the purpose can be achieved by non-operative procedure,the salvage surgery is not necessary. Preoperative chemotherapy inducing tumor shrinkage,would improve the chance to safely suture the duodenal stump,which is of great significance to prevent the duodenal stump leakage. Preoperative chemotherapy via intra-arterial and intravenous administration could higher the response rate to around 75%.
Endoscopic variceal ligation and endoscopic variceal obturation for the management of gastric varices:a prospective com parative study

Abstract:Objective To analyze and evaluate the efficacy of gastric variceal ligation ( GVL) and gastric variceal obturation ( GVO) for patients with gastric varices , accordingly providing some reference for the clinical treatment of gastric varices .Methods Ninty-three patients with gastric varices were included at the Second Affiliated Hospital of Xi′an Jiaotong University from January 2010 to December 2013 .Among them,48 patients received GVL and 45 received GVO.Recording clinical and endscopic change after treatment, we compared elimination coefficient , recurrence rate and rebleeding rate , mortality and complications between two groups ( GVL and GVO) .By comparing the above items ,we finally evaluated the efficacy of GVL and GVO for gastric varices .Results In patients who received GVL , 64.6% of them achieved obliteration of varicose veins , while 62.2% with GVO.There was no significant difference between two groups (P=0.813).GVL and GVO were both effective in controlling acute gastric varices bleeding,and the difference between two methods had no statistical significance (GVL 91.7%,GVO 100%, P>0.05).Rebleeding occurred in 31.3%patients in the GVL group and 26.7%in the GVO group (P=0.672).The rebleeding rate was still higher in IGV1 petients compared with GOV1 and GOV2 ( P =0.037).Complications and mortality rates showed no significant difference between the groups ( P =0.608),as well as cumulative survival rates (P=0.350).The efficacy of GVL to obliterate gastric varices is similar to GVO.The ability of GVL to control active gastric variceal bleeding showed no difference to GVO, as well as complications and survival .Conclusions GVO is a safe and effective method for gastric varices, while it still needs to be aware of the occurrence of embolic complications especially systematic embolic complications .GVL is a feasible strategy to handle gastric varices , particularly for GOV1 and GOV2.
Application of Retrocolic Approach with Uncinate Process Priority in Laparoscopic Pancreaticoduodenectomy

Abstract:Background: Pancreaticoduodenectomy (PD) is a complex operative procedure, which remains the primary curative treatment for pancreatic, distal bile duct, and periampullary cancers. In recent years, with the continuous development of laparoscopic technology and equipment, laparoscopic pancreaticoduodenectomy (LPD) has been performed gradually in many high-volume surgical centers. However, it is still challenging even for experienced pancreatic surgeons to perform LPD, at the same time, with the accumulation of surgical experience, different surgical approaches are also constantly discussed.Methods: We retrospectively analyzed the clinical data of 323 patients who received LPD at a single institution. Among them, 200 patients received operations with retrocolic approach, 123 patients were treated with traditional approach. In this study, we analyzed perioperative data and compared survival time for patients with pancreatic cancers in two groups.Result: Compared with traditional approach, retrocolic approach with uncinate process priority has a shorter operative time (94.25 +/- 6.46 minutes versus 116.43 +/- .4.78 minutes, P = .009) and less intraoperative blood loss (80 mL versus 150 mL, P = .562). However, there is no statistical significance in the incidence of postoperative complications (>= Clavien-Dindo [CD] III) (65 [32.5%] versus 45 [36.58%], P = .871), R0 resection rates (41 versus 38, P = .826), and the number of lymph nodes harvested (16.64 +/- 5.93 versus 15.37 +/- 4.65, P = .785) between two groups. Meanwhile, the median survival time of patients with pancreatic cancers in posterior approach group was longer than those in traditional approach group (30.34 months versus 28.54 months, P > .05); however, there was no statistical significance between them.Conclusion: Retrocolic approach with uncinate process priority is a feasible method for pancreatic cancer, which could reduce operating time and intraoperative bleeding, meanwhile, not increase the incidence of postoperative complications. Retrocolic approach with uncinate process priority can be generalized to larger group sizes.

Abstract:BackgroundPractitioners of traditional Chinese medicine know that auricular point acupressure (APP) using vaccaria seeds on the large intestine point (CO7) has a significant effect on postoperative gastrointestinal dysfunction. A standardized, clinical, research design will transform this clinical experience into scientific evidence, thus providing a basis to promote the wider use of this therapy. We aim to carry out a double-blind, randomized, controlled trial (RCT) to evaluate the efficacy and safety of APP treatment for gastrointestinal dysfunction after laparoscopic cholecystectomy.Methods/designThis study is a randomized, double-blind, controlled, single-center, clinical, pilot trial. It has been designed according to the Consolidated Standards of Reporting Trials (CONSORT 2010) guidelines as well as the Standards for Reporting Interventions in Controlled Trials of Acupuncture (STRICTA). Study subjects are being selected from among hospitalized patients who have undergone laparoscopic cholecystectomy at the Department of Minimally Invasive Surgery of Tianjin Nankai Hospital. Qualified subjects will be assigned randomly either to the APP group or to the APP sham stimulation group on the basis of random numbers generated using SPSS 19.0. A specifically appointed investigator will be responsible for the randomization. The APP therapy (or sham stimulation) will be performed 6 h after surgery and every 12 h subsequently; six sessions will be conducted, each lasting 3 min. The first evaluation will be performed immediately before the first treatment (6 h after surgery) and, then, every 12 h for seven evaluations. The primary outcome is the time to first passage of flatus after surgery; the secondary outcome measures are abdominal distension, nausea, vomiting, time to first defecation, psychological status, and quality of life.DiscussionThis pilot trial is a standardized, scientific, clinical trial designed to evaluate the efficacy and safety of APP treatment—using vaccaria seeds on CO7—for gastrointestinal dysfunction after laparoscopic cholecystectomy. We aim to provide objective evidence to promote this therapy in clinical practice.Trial registrationChinese Clinical Trial Registry, ChiCTR-IPR-15007643. Registered on 14 December 2015.

Abstract:Intra-abdominal bleeding resulting from inadequate drainage of duodenal leakage (DL) is typically caused by the corrosiveness of duodenal fluid. Open abdomen (OA) treatment addresses both the drainage and bleeding simultaneously. However, a sequential treatment (ST) approach involving hemostasis through transcatheter arterial embolization (TAE) followed by percutaneous drainage of source control has emerged as an alternative method. This study aimed to evaluate the prognosis of ST in cases of DL-induced intra-abdominal bleeding. Abstract:BACKGROUND: How to precisely protect and preserve anterior and posterior vagal trunks and all their branches during the procedure of splenectomy and azygoportal disconnection is studied rarely. We firstly developed a vagus nerve-guided robotic-assisted laparoscopic splenectomy and azygoportal disconnection (VGRSD). The aim of this study was to evaluate whether VGRSD is feasible and safe and to determine whether VGRSD can effectively eliminate postoperative digestive system complications by protecting vagal nerve precisely.METHOD: In this prospective clinical study, 10 cirrhotic patients with oesophagogastric variceal bleeding and hypersplenism who underwent VGRSD between January 2022 and March 2022 were gathered, and compared with a retrospective cohort who received a part of the vagus nerve-preserving robotic-assisted laparoscopic splenectomy and azygoportal disconnection (VPRSD). They were all followed up for 6 months.RESULTS: In VGRSD group, the operation time was 173.5 ± 16.2 min, blood loss was 68.0 ± 39.1 ml, VAS pain score on the first day was 1.9 ± 0.7, and the postoperative hospital stay was 7.7 ± 0.7 days. There was no incisional complications, pneumonia, gastric fistula, pancreatic fistula, and abdominal infection. No patients suffered from diarrhoea, delayed gastric emptying, and epigastric fullness. Compared with VPRSD, operation time was significantly longer for VGRSD (p < 0.05). However, VGRSD was significantly associated with less diarrhoea and shorter postoperative hospital stay (all p < 0.05).CONCLUSION: VGRSD procedure is not only technically feasible and safe, it also effectively eliminate postoperative digestive system complications.TRIAL REGISTRATION: We registered our research at https://www.CLINICALTRIALS: gov/. Abstract:Objective:To evaluate the integrated emergency medical treatment of acute non-variceal upper gastrointestinal bleeding(acute non-variceal upper gastrointestinal bleeding,ANVUGIB) the clinical effect.Methods:Selected the 189 patients in shenzheng hospital from the February 2006 to 2009,all the patients had a comprehensive emergency medical ANVUGIB diagnosis and treatment.Analyzed statistical lesion detection rate,the rate of disease detection in the different time periods,treatment effect,observation of patients with an average length of hospital stay.Results:The sooner the emergency endoscopy,the higher detection rate of lesions,especially for acute gastricmucosal lesions,such as the detection rate of superficial lesions.Conclusion:The emergency medical treatment can be integrated to ensure that emergency endoscopy safe and effective and to reduce the average number of hospitalized days and observation times of patients.
