Efficacy of arthroscopic treatment for knee osteoarthritis combined with meniscus tear and (or) joint loose bodies

Abstract:Background: Arthroscopy gains its popularity in the treatment of osteoarthritis of the knee. However, in the AAOS Evi-dence-Based Guideline on Treatment of Osteoarthritis of the Knee (2nd Edition), arthroscopy with lavage and (or) debride-ment in patients with a primary diagnosis of symptomatic osteoarthritis of the knee is not recommended. <br> Objective:To observe the clinical effects of arthroscopy in the treatment of knee osteoarthritis (OA). <br> Methods:In this retrospective study, patients with a primary diagnosis of knee OA treated by arthroscopy in our joint center between January 2011 and December 2013 were collected. Different surgical procedures, such as meniscectomy, notchplas-ty, removal of loose bodies, drilling decompression) were recorded by reviewing operation videos. The efficacy of arthrosco-py for knee OA was evaluated by subjective satisfaction and hospital for special surgery knee score (HSS score). The im-provement of clinical symptoms was also assessed. <br> Results:A total of 121 patients were followed up with a mean duration of 24 months (range, 7-44 months). According to Kellgren-Lawrence (K-L) Grading System, there were 6 grade 0 patients, 68 gradeⅠpatients, 45 gradeⅡpatients and 2 gradeⅢpatients. Of them, 102 (84%) patients were satisfied with the therapeutic effects. Compared with the preoperative one, the mean postoperative HSS score was significantly increased (45.5±4.9 vs 75.6±15.1, P<0.05). Ideal effects were ob-served in OA patients with meniscus tear and (or) joint loose bodies, and the improvement rate of interlocking and flexion-extending limitation was 95.2%and 81.1%, respectively. <br> Conclusions:Arthroscopy is an ideal treatment for early stage knee OA with interlocking and (or) immobility caused by me-niscus tear and (or) joint loose bodies. However, clinical outcome of arthroscopy for OA without combined lesions is uncertain.
