Abstract:[Objective] To explore the clinical outcomes of Ilizarov technique combined with minimally invasive osteotomy in the proximal tibia and fibula for treating knee osteoarthritis with varus deformity.[Methods] A total of 36 patients,who were admitted to our hospital between January 2010 and December 2015 due to the presence of knee pain while walking,were retrospectively analyzed.These patients,consisted of 25 males and 11 females with a mean age of 58.5 years,manifested knee pain in a total of 39 knees including 19 left knees,14 right knees and 3 both knees.Based on the grading scale for osteoarthritis,these affected knee were rated as Grade 2 in 15 knees and Grade 3 in 24 knees.Preoperatively,the affected knees were evaluated and documented the parameters on average as follows:Visual Analogue Scale (VAS) score in 7,Hospital for Special Surgery (HSS) score in 58.4,the medial angle of the proximal tibia in 62.5°,as well as the tibial-femoral angle in 167.3°.All of the 39 knees were treated by minimally invasive osteotomy of the proximal tibia and fibula combined with gradual deformity correction by Ilizarov external fixation.[Results] The operation time ranged from 40 min to 95 min with an average of 55.0 min for the 36 patients (39 knees),by contrast,the time elapsed just in osteotomy and external fixation placement was 30 min to 60 min with a mean of 45.5 min.No serious complications such as neural or vascular injury occurred during the operation.Outpatient and telephone follow-ups were conducted in 35 patients (38 knees) for a mean of 34.5 months.The above mentioned parameters had significant improvements,such as the mean VAS score decreased to 2,the proximal tibia medial increased to 96°,and the tibia-femoral angle increased to 172.5° at weight-bearing position on AP view,additionally,the mean HSS score increased to 88.7 at the last follow-up.The treatment outcome was rated as the excellent in 30 knees,good in 6 knees,and fair in 2 knees,with an excellent and results rate of 94.7％ according to the Qin Sihe's evaluation standard for deformity correction.[Conclusions] The combined use of Ilizarov technique and proximal tibia and fibula osteotomy is an effective and minimally invasive method for treating knee osteoarthritis with reliable results.Since this method is easy to perform and can be adjusted on the coronal and sagittal planes,it is a suitable treatment for knee osteoarthritis patients with varying degrees of varus deformity.
