Follow-up of the posterior cruciate ligament injury treated by arthroscopic reconstruction of ligament advanced reinforcement system

Abstract:AIM: To study the therapeutic results of posterior cruciate ligament (PCL) injury arthroscopic reconstruction with ligament advanced reinforcement system (LARS) artificial transplantation. METHODS: Fifteen cases with PCL injuries treated by LARS artificial ligament from August 2004 to January 2006 in Department of Orthopaedics, Changzheng Hospital were studied, they were all diagnosed as sports injury. With the informed consents, all the subjects received the PCL reconstruction or reinforcement operation by LARS artificial ligament (produced by France LARS Company, pattern number PPLY 110), which was mainly polyester fiber (polyethylene terephthalate) of high tenacity and good histocompatibility. Due to the specific weave, the collagen fiber and vascular endothelial cells grew in it. After normal use of antibiotics for 1 day via vein, the patients were assigned to passive activity with CPM machine, crutch walking at 3 days postoperation, and recovered to normal activity or physical exercise 2 months later. During the follow-ups of 1, 2, 6, 12, 24 months, the stability, activity ranged and pain site of knee joint were detected. Lysholm evaluation system was used to rank the function of knee joint, totally 100 points. Over 84 points as normal function, 66-84 points as acceptable function and less than 65 points as worse function. RESULTS: All 15 patients were involved in the result analysis without any drop.①Fifteen patients were followed up from 6 to 24 months (mean 11 months), including 10 cases for 6-12 months, 2 cases for 12-18 months, and 3 cases for 18-24 months. The unstable symptom of every knee was disappeared. The joint function was good with the flexion of 0°-(130±10.2)°. All the patients recovered to normal physical exercise at 2 months postoperation. No acute or chronic synovitis observed.②According to the Lysholm evaluation system, the preoperative scores was less than that of postoperative significantly (85.5±2.33, 45.2±1.23, t =9.547, P < 0.01). CONCLUSION: LARS artificial ligaments is featured at: ①being suitable for early rehabilitation postoperation and active exerciser.②accelerating the joint function.③presenting no complication and good histocompatibility.④the ideal graft for PCL injury reconstruction.
