Abstract:Objective:Most surgeons hold a pessimistic view at radical surgery for patients with advanced gallbladder carcinoma. The objective of this paper was to determine whether radical surgery is helpful for patients with advanced gallbadder carcinoma.Method:Radical surgery for 32 patients with advanced gallbladder carcinoma was performed. These patients include 7 cases subjected to liver segmentectomy, one case to hemiheptectomy, and 4 cases to resetion of hilar bile duct, 6 cases to combined resection of hepatic segments with hilar bile duct, and 14 cases to hepaticopancreatoduodenectomy (HPD). Skeletonization of hepatoduodenum ligament was performed in 26 cases. Results:All the operations were carried out successfully. Of the 17 cases who received the operations 12 months ago, 10 are still alive and 2 have lived more than 30 months. The longest survival time is 40 months. Of the 15 cases operated in last 12 months, 14 cases are still alive. Conclusion:It is possible to prolong the life or improve the life quality of patients with advanced gallbladder carcinoma by doing radical surgery more actively.
