Application of locking plate in the treatment of intertrochanteric femoral fracture

Abstract:Objective To compare the advantages and disadvantages of the treatment of intertrochanteric femoral fracture with the locking plate and proximal femoral nail antirotation ( PFNA ), and the influence on the prognosis of patients.Methods Retrospective analysis was performed on 341 cases of intertrochanteric femoral fracture from September 2009 to September 2015. According to the operation methods, the patients were divided into group A and B, the operation time, intraoperative fluoroscopy time, total blood loss, dominant blood loss, hidden blood loss, postoperative complications, loading time, Harris hip score and mortality were compared.Results Patients in group A were followed up for 13 - 50 months with an average of 21.3 months. The follow-up time of group B was 15 - 60 months ( mean: 22.2 months ). There was no significant difference between the 2 groups in operation time, dominant blood loss, postoperative complications, Harris hip function score and mortality. Mean total blood loss was ( 218 ± 20.5 ) ml in group A, which was higher than that of group B (143 ± 5.8 ) ml (P = 0.012 ); hidden blood loss: ( 123.6 ± 18.7 ) ml in group A was higher than that of group B ( 22.4 ± 3.5 ) ml (P = 0.002 ); intraoperative fluoroscopy time in group A ( 8.23 ± 1.5 ) h was higher than that of group B ( 4.31 ± 2.1 ) h (P = 0.031 ). The average load time after operation in group A was ( 14.2 ± 3.5 ) days, which was earlier than that of group B ( 40.3 ± 8.6 ) days (P = 0.014 ).Conclusions For the patients with femoral intertrochanteric fractures with closed reduction failure, locking plate is a good choice of internal fixing implants. For the patients with intertrochanteric fractures and closed reduction failure, locking plate is a choice of internal implant, with less blood loss and fewer intraoperative fluoroscopy times. There is no significant difference in the influence on the patients' prognosis with PFNA.
