Harvest and Clinical Application of Radial Artery in Coronary Artery Bypass Grafting

Abstract:Objective To review the harvesting and early clinical outcome of radial artery (RA) during coronary artery bypass grafting (CABG). Methods Twenty-two patients received CABG with the RA in whom the performance of the radial and ulnar arteries were checked preoperatively with the of quantified Allen's test to determine the safety of radial artery harvesting.The companied veins were harvested together with the artery.No touch method was used.Direct touching on the RA trunk was totally avoided.Papaverine solution of heparinized Ringer's solution was applied intraluminally into the free RA graft before it was stored in the same solution for later use.Twenty-two RAs were harvested and made to the posterior descending branches and the circum flex marginal branches and the anterior descending branches.All the proximal ends were anastomosed with the aorta.Calcium channel blockers were routinely used for 12 months postoperatively. Results Myocardial function was improved,and no perioperative myocardial infarction and pectoris angina occurred.No vascular spasm,bleeding and embolism during operation,and no ischemia phenomenon of left hand after operation were found. Conclusion The RA should beencouraged to be used as a routine graft in CABG.A “no-touch” harvesting technique and use of calcium-channel blockers are essential to the functioning of this conduit.
