Ultrasound-guided Percutaneous Nephrolithotomy in Obese Patients (body Mass Index≥35 Kg/m2) : Report of 35 Cases

Abstract:Objective:To evaluate the safety and efficacy of ultrasound-guided percutaneous nephrolithotomy in obese patients,and sum up the surgical techniques and experience.Method:The data of 35 cases from June 2014 to December 2016 undergoing ultrasound-guided percutaneous nephrolithotomy were analyzed retrospectively,including BMI,gender,laterality,staghorn,largest stone diameter and so on.Prior to surgery,the state of lung and heart function of all patients were evaluated.And the stone free rate was assessed by KUB postoperatively.Result:All 35 cases were performed successfully,among them,26 cases were done with 1 tract,7 cases with 2 tracts,and 2 cases with 3 tracts.The mean operating time was 81.6 mins,and the mean duration of nephrostomy tubes was 2.9 days.The mean postoperative hospitalization was 4.6 days,and the mean haemoglobin decrease was 13.1 g/L.The overall stone-free rate was 82.9%.There were six complications:two sepsis,one hyoxemia and three fevers.All complications were recovered with conservative treatment.Conclusion:Ultrasound-guided percutaneous nephrolithotomy in obese patients is safe and feasible,but it should be performed by experienced surgeons with good knowledge of ultrasound.
